Obesity with developmental delay in infancy: a rare cause not to miss-pseudohypoparathyroidism.

Mild developmental delay in infants with obesity is often disregarded, attributing it to increased weight by parents and practitioners. We report an infant with obesity and gross motor delay after the first 6 months of age, seizures and obstructive sleep apnoea. The child was diagnosed with pseudohypoparathyroidism (PHP) and was medically managed with parenteral calcium, vitamin D and magnesium. Appropriate early interventions by developmental therapists addressing the developmental domains coordinated by developmental paediatricians and relevant drug and diet management corrected the delay. Hypocalcemic Tetany Transiently "Cured" by Pregnancy: A Case Report.

Hypocalcemia is an uncommon electrolyte abnormality. We evaluated a young woman with episodes of recurrent symptomatic hypocalcemia, which had started in her late teens. She was hypertensive and laboratory evaluation revealed elevated parathyroid hormone, elevated phosphate, and decreased renal phosphate excretion along with low ionized calcium levels. Her renin and aldosterone levels were elevated. While the response to treatment with calcium and 1,25(OH)2 dihydroxyvitamin D was modest, her laboratory values and symptoms improved significantly during pregnancy and then recurred post-delivery.

Pseudohypoparathyroidism: Challenges in Early Recognition and Diagnosis of a Rare Hereditary Disorder.

Pseudohypoparathyroidism (PHP) is an uncommon endocrine condition marked by an impaired response to parathyroid hormone (PTH), which results in biochemical abnormalities. Clinical manifestations can vary significantly, occasionally resembling other endocrine disorders. Genetic testing plays a critical role in distinguishing PHP from other conditions, as it enables precise diagnosis even when classical features are not initially present. We report the case of a male infant initially diagnosed with congenital hypothyroidism (CH) through neonatal screening and treated with levothyroxine. While gr

Germline-derived GNAS-Gsα variants associated with both gain-of-function and loss-of-function phenotypes.

OBJECTIVE: Heterozygous germline inactivating mutations in GNAS can cause hormonal resistance, while activating mutations, usually somatic, result in constitutive cyclic adenosine monophosphate (cAMP) stimulation. Recent research has described germline activating variants leading to nephrogenic syndrome of inappropriate antidiuresis (NSIAD). The present study aims to characterise 4 families with an unusual combination of symptoms indicative of loss of Gsα function and a tendency to hyponatraemia compatible with NSIAD. DESIGN: Clinical, genetic, structural, and functional characterization of G

Bone in Parathyroid Diseases Revisited: Evidence From Epidemiological, Surgical and New Drug Outcomes.

PTH-related disorders have a major impact on bone metabolism and skeletal properties because of the pivotal role of PTH in calcium and phosphate homeostasis and bone remodeling. Hyperparathyroidism is characterized by continuous exposure to excessive endogenous PTH, causing increased bone turnover in favor of bone resorption. Depending on the background of PTH overproduction, hyperparathyroidism is divided into primary, secondary, and tertiary hyperparathyroidism. The clinical presentation varies from deterioration of bone microarchitecture and decreased bone mineral density to profound bone i

Bidirectional disruption of GNAS transcripts causes broad methylation defects in pseudohypoparathyroidism type 1B.

Pseudohypoparathyroidism type 1B (PHP1B) is a multihormone resistance disorder caused by aberrant GNAS methylation. Characteristic epigenetic changes at GNAS differentially methylated regions (DMRs), i.e., NESP, AS1, AS2, XL, and A/B, are associated with specific structural defects in different autosomal dominant PHP1B (AD-PHP1B) subtypes. However, mechanisms underlying abnormal GNAS methylation remain incompletely defined, largely because viable PHP1B mouse models are lacking. Using lymphoblastoid cells and induced pluripotent stem cells, we show that various GNAS methylation patterns in PHP1

Atypical presentation of pseudohypoparathyroidism with absence of mutations in the GNAS gene: a case report.

BACKGROUND: This case report aimed to broaden the understanding of pseudohypoparathyroidism (PHP) manifestations when typical mutations in the GNAS gene are absent. The clinical, biochemical, and genetic investigations of a PHP case revealed diagnostic challenges and emphasized the importance of comprehensive genetic analysis for early diagnosis and appropriate management, as well as improved patient outcomes. CASE PRESENTATION: The case involved a 21-year-old man who has experienced recurrent limb convulsions and episodes of altered consciousness since the age of eight. Recent assessments du

Alpha-smooth muscle actin-expressing dermal sheath cells are a major cellular contributor to heterotopic subcutaneous ossifications in a mouse model of Albright hereditary osteodystrophy.

Heterotopic ossifications (HOs) are the pathologic process by which bone inappropriately forms outside of the skeletal system. Despite HOs being a persistent clinical problem in the general population, there are no definitive strategies for their prevention and treatment due to a limited understanding of the cellular and molecular mechanisms contributing to lesion development. One disease in which the development of heterotopic subcutaneous ossifications (SCOs) leads to morbidity is Albright hereditary osteodystrophy (AHO). Albright hereditary osteodystrophy is caused by heterozygous inactivat

Loss-of-function Gαs rare disease variants exert mutation-specific effects on GPCR signaling.

G protein-coupled receptors (GPCRs) are transmembrane detectors of extracellular signals that activate heterotrimeric G proteins to regulate intracellular responses. Because there are only 16 Gα proteins that can couple to GPCRs, variation in a single Gα can affect the function of numerous receptors. Here, we investigated two mutant forms of Gαs (L388R and E392K) that are associated with pseudohypoparathyroidism type Ic (PHPIc), a maternally inherited rare disease. Gαs is encoded by an imprinted gene, resulting in the mutant form of Gαs being the only version of the protein present in certain

Analysis of the prevalence and incidence of pseudohypoparathyroidism in Poland based on National Health Fund data with clinical presentation of own cases.

INTRODUCTION: Pseudohypoparathyroidism (PHP) is a rare genetically determined disease with a wide range of symptoms related to target organs' resistance to parathyroid hormone (PTH). Lack or insufficient action of PTH on effector organs causes hypocalcemia and hyperphosphatemia. Some patients may have characteristic features of Albright hereditary osteodystrophy (AHO). METHODS: We estimated the prevalence and incidence of PHP using National Health Fund (NHF) data between 2013 and 2023. RESULTS: The prevalence was 0.62-0.64/100,000 person-years for Poland and 1.11-1.47/100,000 person-years fo

A Splice-Region Variant Causes an Atypical Presentation of GNAS Inactivation Disorder.

Pathogenic variants in GNAS can cause a wide range of diseases including pseudohypoparathyroidism, pseudopseudohypoparathyroidism, McCune-Albright syndrome, among others. The specific phenotypic features that may be seen are influenced by the variant type and location in the gene, whether it causes loss or gain of function, and whether it is germline or somatic in nature. The GNAS locus is imprinted, which also results in a parent-of-origin effect. Typically, germline loss of function variants on the maternal allele are associated with variable hormonal resistances, obesity, intrauterine growt

Multi-locus methylation analyses reveal GNAS methylation defects in three patients with the Beckwith-Wiedemann syndrome phenotype and no molecular defects in the 11p15.5 imprinted region.

BACKGROUND: Beckwith-Wiedemann syndrome (BWS) is a congenital imprinting disorder (ID) caused by molecular defects in the 11p15.5 imprinted region, such as hypomethylation of the KCNQ1OT1:TSS-differentially methylated region (KCNQ1OT1-DMR) and hypermethylation of the H19/IGF2:IG-DMR, and maternal CDKN1C pathogenic variants, with various clinical characteristics, including overgrowth and macroglossia. Recently, the concept of Beckwith-Wiedemann spectrum (BWSp) and a clinical scoring system for BWS have been proposed, and cases with four or more points are diagnosed with classic BWS, and 20% of

Natural History of Hyperphagia in Patients with Pseudohypoparathyroidism.

Background/Objectives: Pseudohypoparathyroidism (PHP) is a group of genetic disorders characterized by end-organ resistance to multiple hormones, short stature, brachydactyly, subcutaneous ossifications, obesity, and developmental delays. The tissue specific imprinting of GNAS in the hypothalamus may lead to different eating behavior phenotypes in maternally inherited (PHP1A, PHP1B) vs. paternally inherited (PPHP) variants. In this exploratory study, we aimed to evaluate differences in eating behaviors in a cohort of patients with PHP1A, PPHP and PHP1B. Methods: Assessments included caregiver-

Pseudohypoparathyroidism type 1B mimicking gitelman syndrome: diagnostic pitfalls and molecular insights.

OBJECTIVE: Pseudohypoparathyroidism type 1B (PHP1B), caused by abnormal methylation of the GNAS gene leading to parathyroid hormone (PTH) resistance, lacks Albright hereditary osteodystrophy features and is often misdiagnosed. PHP1B and Gitelman syndrome (GS) share overlapping features, including hypokalemia, hypocalcemia, hypomagnesemia, and metabolic alkalosis, posing challenges in clinical differentiation. This study aimed to explore the clinical characteristics, diagnostic strategies, and therapeutic responses of PHP1B presenting with hypokalemia and to explicitly address the diagnostic ch

Human diseases caused by homozygous PTH1R mutations.

The parathyroid hormone receptor type 1 (PTH1R) is a G protein-coupled receptor that mediates the actions of parathyroid hormone (PTH) in the regulation of blood calcium levels, as well as PTH-related protein (PTHrP) in the regulation of skeletal development. Severe loss-of-function homozygous mutations in PTH1R are incompatible with life as in Blomstrand's lethal chondrodysplasia, characterized by accelerated growth plate ossification. More recently, homozygous mutations located in the transmembrane helices, extracellular domains and C-tail of the PTH1R were identified in patients with milder

Six cases of ectopic cutaneous ossification associated with GNAS gene variants.

The GNAS gene gives rise to stimulatory G protein Gs-alpha (Gsα), a pivotal transducer of hormonal signalling pathways. Variants inherited maternally lead to reduced Gsα activity in certain tissues, resulting in resistance to multiple hormones and manifesting clinically as pseudohypoparathyroidism (PHP). In contrast, paternal transmission is linked to pseudopseudohypoparathyroidism (PPHP), which is characterized by extensive ectopic cutaneous ossification but lacks endocrine resistance. Thus, the phenotypic spectrum arising from GNAS gene variants is profoundly influenced by the parent-of-orig

Imprinting and skeletal disorders: lessons from pseudohypoparathyroidism and related disorders.

Pseudohypoparathyroidism (PHP) was first described as a syndrome characterized by PTH resistance combined with skeletal abnormalities known as Albright's hereditary osteodystrophy (AHO). Studies have since focused on genetic or epigenetic alterations underlying PHP and related disorders. The α-subunit of the stimulatory G protein (Gsα) mediates the signaling of G protein-coupled receptors that stimulate cAMP generation. The Gsα-cAMP cascade is pivotal for human skeletal growth, as evidenced by pathogenic mutations converging on this signaling pathway in a spectrum of skeletal dysplasias that o
