Significance of GNAS mutations for morbid obesity in children.

BACKGROUND: Hereditary forms of obesity are characterized by early severe heterogeneous manifestations of the phenotype along with a rapid progression to morbid obesity, primary due to pathogenic variants of certain genes. Most forms are characterized by moderate to severe neuropsychic developmental delays, dysmorphic features and organ-specific developmental anomalies. RESULT: We searched for hereditary causes of morbid obesity in children by exome sequencing. As a result, we have identified 5 variants in the GNAS locus, two of which were identified for the first time: NM_000516.7(GNAS):c.20

Multidimensional Characterisation of Eating Behaviour in Genetic Obesity: A Systematic Review.

INTRODUCTION: Genetic forms of obesity, including monogenic (MO) and syndromic (SO) obesity, are characterised by severe, early-onset weight gain due to disrupted central regulation of body weight, typically involving key pathways such as the leptin-melanocortin axis. These alterations result in marked hyperphagia and complex eating behaviours, yet clinical characterisation remains limited. This review aimed to describe the multidimensional eating behaviour profiles across genetically confirmed obesity, explore their variability, and evaluate existing assessment tools to support early diagnosi

Late-Onset Pseudohypoparathyroidism: A Case Report.

Pseudohypoparathyroidism (PHP) is a rare endocrine disorder due to end-organ resistance to parathyroid hormone (PTH), resulting in hypocalcemia and hyperphosphatemia despite elevated PTH levels. It usually presents in children with distinctive features known as Albright Hereditary Osteodystrophy (AHO). We report a 33-year-old man who presented with wrist pain, muscle spasms, and tingling in his hands. Laboratory investigations revealed hypocalcemia, hyperphosphatemia, and elevated serum PTH, with normal serum magnesium, renal function, and vitamin D levels, suggestive of PTH resistance. The pa

Hypocalcemia Revisited: Thinking Outside the Box!

This case report details the presentation of a 62-year-old man with a background of hypothyroidism, hypertension, and chronic kidney disease (CKD) who experienced fatigue with acute-on-chronic hypocalcemia. Despite receiving oral calcium carbonate with vitamin D3 (cholecalciferol) supplementation in primary care, persistent hypocalcemia in the absence of significant renal impairment and a positive response to alfacalcidol (1α-hydroxycholecalciferol) prompted further investigation into genetic causes. A final diagnosis of pseudohypoparathyroidism type 1b (PHP1b) was confirmed through genetic st

Three-generation familial transmission of a GNAS variant: from gestational phenotype in the mother to evolving prenatal/neonatal phenotype in the offspring.

BACKGROUND: Disorders caused by GNAS gene variants, including pseudohypoparathyroidism (PHP) types 1a, 1b and 1c, and pseudo-pseudohypoparathyroidism (PPHP), present with complex imprinting and phenotypic variability. The gene's parent-of-origin expression pattern results in diverse clinical manifestations. CASE PRESENTATION: We report a three-generation family harboring a heterozygous missense variant in GNAS (c.305 C > T, p.Ala102Val), classified as "likely pathogenic". The proband, a 29-year-old primigravida, and her mother displayed phenotypic features of Albright hereditary osteodystroph

Pseudohypoparathyroidism Presenting With Recurrent Twitching: Challenges Making a Diagnosis in a Low-Resource Environment.

Pseudohypoparathyroidism (PHP) is a metabolic disorder that occurs due to target end-organ resistance to parathyroid hormone (PTH). It is a rare cause of severe symptomatic hypocalcemia as it characteristically manifests with high phosphate and low calcium. The clinical presentation, biochemical features, and severity vary from patient to patient leading to delay in diagnosis. Reduced awareness and lack of recognition of this rare clinical syndrome coupled with limited resources in rural health facilities also contribute to missed or late diagnosis. Reported here is a case of a 13-year-old gir

Basal Ganglia Calcification With Treatment-Resistant Hypocalcaemia and Persistent Psychosis in an 18-Year-Old: A Case Report.

Basal ganglia calcification (BGC) in young adults represents a pathological finding requiring systematic investigation, contrasting with incidental age-related calcification in elderly populations. We report an 18-year-old male patient who presented to the emergency department with a first episode of generalized tonic-clonic seizure with subsequent imaging revealing extensive bilateral BGC involving the basal ganglia, thalami, and cerebellum. His inpatient stay was complicated by treatment-resistant hypocalcaemia. A retrospective review revealed that hypocalcaemia had been documented eight mon

A Rare Familial Case of Pseudohypoparathyroidism Type 1b in Two Brothers Presenting With Recurrent Leg Cramps and Learning Difficulties.

We present a rare familial case of two brothers presenting with late-onset symptoms of posturing, cramps, and occasional falls. The brothers, who were initially under the pediatric team being investigated for rare thiamine-transporter-related genetic disorders, had significant symptomatic hypocalcemia and cerebral calcifications identified on brain imaging. They were both diagnosed with pseudohypoparathyroidism type 1b in their late teenage years and achieved stable calcium levels and symptom control after treatment titration. This case highlights the importance of identifying key hypocalcemia

Cerebral Calcification and Treatment-Resistant Seizures: A Rare Syndromic Presentation of Pseudohypoparathyroidism-A Case Report.

BACKGROUND/OBJECTIVE: Pseudohypoparathyroidism (PHP) has a varied presentation, ranging from biochemically detected hypocalcemia in asymptomatic individuals to treatment-refractory seizures. Features may develop over time. Hypocalcemia with recurrent seizures in adolescents indicates a cause other than a primary central nervous system cause, rather than hypoparathyroidism and pseudohypoparathyroidism being the endocrinological causes. CASE REPORT: We present a case of an adolescent girl with difficult-to-treat seizures and dystonic body movement whose diagnostic workup confirmed PHP. Phenotyp

PLAIN ENGLISH SUMMARY: Parents of children with physical disabilities do a lot to support their child in daily life. In doing this they are faced with many challenges. These parents have a wide range of unmet needs, especially for information, on different topics. It is sometimes hard for them to get the right information at the right moment, and to ask the right questions to physicians and other healthcare professionals. In order to develop a digital tool to help parents formulate questions and find information, we thought it would be crucial to work together in a process of co-creation with

Metabolic Implications of PCOS.

BACKGROUND: In children, necrotizing pneumonia (NP) is an uncommon, severe complication of pneumonia. It is characterized by destruction of the underlying lung parenchyma resulting in multiple small, thin-walled cavities and is often accompanied by empyema and bronchopleural fistulae. REVIEW: NP in children was first reported in children in 1994, and since then there has been a gradual increase in cases, which is partially explained by greater physician awareness and use of contrast computed tomography (CT) scans, and by temporal changes in circulating respiratory pathogens and antibiotic pre

Interpreting epigenetic causes of recurrent hypokalemia and seizures: Gitelman syndrome co-exist with pseudohypoparathyroidism type 1B.

We describe a unique case of 27-year-old male with Gitelman syndrome (GS) co-exist with pseudohypoparathyroidism type 1B (PHP1B). The patient presented with a 5-year history of seizures, tetany, and numbness of the extremities. Further examinations showed recurrent hypokalemia, inappropriate kaliuresis, hypocalcemia, hyperphosphatemia, and elevated PTH levels. A novel variant of autosomal recessive GS (p.Val287Met SLC12A3) and a novel 492.3Kb deletion containing the whole of STX16, were discovered by a whole-exome sequencing. Following the diagnosis, calcitriol, calcium, and potassium suppleme

GNAS AS2 methylation status enables mechanism-based categorization of pseudohypoparathyroidism type 1B.

Pseudohypoparathyroidism type 1B (PHP1B) results from aberrant genomic imprinting at the GNAS gene. Defining the underlying genetic cause in new patients is challenging because various genetic alterations (e.g., deletions, insertions) within the GNAS genomic region, including the neighboring STX16 gene, can cause PHP1B, and the genotype-epigenotype correlation has not been clearly established. Here, by analyzing patients with PHP1B with a wide variety of genotypes and epigenotypes, we identified a GNAS differentially methylated region (DMR) of distinct diagnostic value. This region, GNAS AS2,

Diagnosis and approach of pseudohypoparathyroidism type 1A and related disorders during long term follow-up: a case report.

OBJECTIVES: Pseudohypoparathyroidism type 1A (PHP1A) encompasses the association of resistance to multiple hormones, features of Albright hereditary osteodystrophy and decreased Gsα activity. Little is known about the early signs of PHP1A, with a delay in diagnosis. We report two PHP1A cases and their clinical and biochemical findings during a 20-year follow-up. CASE PRESENTATION: Clinical suspicion was based on obesity, TSH resistance and ectopic ossifications which appeared several months before PTH resistance, at almost 3 years of age. Treatment with levothyroxine, calcitriol and calcium w

Novel 4.18 Mb deletion resulting in 2q37 microdeletion syndrome combined with PTH resistance found in one Chinese patient.

BACKGROUND: 2q37 microdeletion syndrome is a rare clinical condition characterized by a series of physical abnormalities. Its Albright hereditary osteodystrophy (AHO)-like manifestations and possible complication of biochemical abnormalities indicating PTH resistance greatly increased the likelihood of misdiagnosis with classic pseudohypoparathyroidism (PHP) caused by GNAS mutation or methylation alteration, even though there have only been six reports of such clinical occasions. PURPOSE: to investigate the underlying genetic defect in a male patient presenting hypocalcemia, elevated PTH and

Epileptic seizures and abnormal tooth development as primary presentation of pseudohypoparathyroidism type 1B.

Pseudohypoparathyroidism (PHP) is a rare genetic disorder characterised by a non-functioning PTH. Usually, the diagnosis is made following (symptomatic) hypocalcaemia. We describe a case in which epileptic seizures and abnormalities in dental development were the main clinical manifestation of PHP type 1B. This case demonstrates the importance of screening for hypocalcaemia in patients with de novo epileptic seizures. In addition, antiepileptic medications themselves may interfere with calcium-phosphate metabolism, causing or aggravating a hypocalcaemia as well. By correcting the calcium level
