Right Aortic Arch With Bilateral Ductus Arteriosus, Aberrant Left Subclavian Artery, Interrupted Left Carotid Artery.

We show an exceptional case of a right aortic arch (RAA) in a patient with DiGeorge syndrome, breaking the rule that the first epiaortic vessel courses to the contralateral side of the aortic arch. This is a RAA with an aberrant left subclavian artery (ALSA) and an isolated left carotid artery (ILCA) arising from the left pulmonary artery (LPA) via a left anterior ductus arteriosus (LADA), along with bilateral ductus arteriosus. This unique case highlights educational pitfalls in using a simple rule to define arch sidedness. Although challenging, echocardiographic diagnosis of such vascular an

22q11.2 Deletion Syndrome in Offspring Conceived via Assisted Reproductive Technology Versus Spontaneously.

Background/Objectives: The majority of chromosome 22q11.2 deletions are de novo, resulting from meiotic non-allelic homologous recombination (NAHR). While 22q11.2 deletion syndrome (22q11.2DS)-associated phenotypes are well characterized, risk factors leading to NAHR are poorly understood, including the possible relationship with assisted reproductive technology (ART). Here we examined the prevalence of ART conceptions and medical comorbidities in patients with 22q11.2DS vs. spontaneously conceived (SC) patients with 22q11.2DS. Methods: Retrospective analysis, under IRB approval, of medical re

Calcium and vitamin D reduce hypoparathyroidism and hospital stay after thyroidectomy: A randomized controlled trial.

BACKGROUND: Transient hypoparathyroidism is a common complication after total thyroidectomy, prolonging hospitalization and necessitating calcium and/or vitamin D replacement. Evidence from prospective randomized trials on preventive supplementation remains limited. This study aimed to evaluate whether prophylactic calcium and vitamin D supplementation reduces transient hypoparathyroidism and shortens hospital stay after total thyroidectomy. METHODS: In this single-center, prospective, randomized, controlled, open-label trial, 600 patients undergoing total thyroidectomy without central neck d

Early rebalancing of neuroinflammatory cascades lastingly rescues prefrontal deficits in a 22q11.2ds model.

Cognitive deficits, a characteristic feature of neuropsychiatric disorders, reflect perturbed activity in neuronal networks. Increasing evidence has linked neuroinflammation to impaired neuronal activity and resulting cognitive dysfunction, yet the underlying cellular mechanisms and developmental dynamics remain largely unclear. Here, we address this knowledge gap by investigating the Df(16)A mouse model of human 22q11.2 microdeletions, a prevalent chromosomal abnormality associated with an increased incidence of neuropsychiatric disorders. During early postnatal development, Df(16)A+/- mice s

[Prenatal diagnosis of 22q11.2 microduplication syndrome in a three-generation family: Clinical-genetic characteristics and literature review].

OBJECTIVE: To explore the genetic etiology for a pregnant woman with a history of multiple adverse pregnancies and assess the phenotype-genotype correlation of 22q11.2 microduplication syndrome in her family. METHODS: Amniotic fluid sample was taken from a pregnant woman for whom non-invasive prenatal screening indicated chromosome 22 abnormalities in the fetus. Peripheral blood samples from the woman, her brother and parents were collected for high-throughput low-depth whole genome sequencing (CNV-seq). A pedigree traceability analysis of the results was conducted in conjunction with analysi

Palopegteriparatide for Adults with Chronic Hypoparathyroidism: Skeletal Dynamics Through 3 yr of the Phase 2 paTH Forward Trial.

Hypoparathyroidism is an endocrine disease caused by insufficient levels of parathyroid hormone (PTH), which acts directly on bone and kidney and indirectly on the intestine to regulate calcium and phosphate balance. In clinical trials, palopegteriparatide (TransCon PTH) treatment enabled independence from conventional therapy (no active vitamin D, ≤500 mg/d calcium) and maintained serum biochemistries within normal ranges. The current analyses describe patterns of change in bone mineral density (BMD), serum bone turnover markers, and serum and urine calcium in adults with chronic hypoparathyr

Epidemiology of chronic hypoparathyroidism in Italy: a systematic review and meta-analysis.

PURPOSE: Hypoparathyroidism (HPT) is an endocrine disease caused by insufficient levels of parathyroid hormone (PTH). PTH is the primary regulator of the calcium/phosphate balance, acting directly on bone and kidney, and indirectly on the intestine. Regarding the epidemiology of chronic HPT, there are limited data in Italy. This study aimed to establish incidence and prevalence of chronic HPT in Italy. METHODS: A systematic literature review was conducted to identify the current knowledge regarding the epidemiology of chronic HPT in Italy, and a random effect meta-analysis was performed to est

Clinical and genetic characterization of patients with Digeorge syndrome: a single-center, first report from Sudan.

BACKGROUND: DiGeorge syndrome (DGS; OMIM #188400), also known as 22q11.2 deletion syndrome, is characterized by cardiac defects, abnormal facial features, thymic hypoplasia, cleft palate, and hypocalcemia. The syndrome is ranked as the second most common chromosomal change after Down’s syndrome, accounting for 1 in 2000 newborns. DGS syndrome is typically diagnosed through CGH and/or FISH analysis in developed countries. However, in low-resource healthcare settings, diagnosis often relies primarily on clinical manifestations due to limited access to genetic testing. The present study, the firs

Severe hypomagnesemia and secondary hypocalcemia associated with vonoprazan in a patient with type 2 diabetes.

Hypomagnesemia is a well-known complication of long-term proton pump inhibitor usage; the stronger acid-suppressive effect of potassium-competitive acid blockers (P-CABs) may increase the risk of hypomagnesemia to a similar or greater extent. However, reports of P-CAB-associated hypomagnesemia are scarce. A 78-year-old Japanese man with a history of type 2 diabetes and myocardial infarction, who was receiving low-dose aspirin and long-term vonoprazan, presented with epigastric discomfort. Laboratory testing revealed significant hypocalcemia, hypomagnesemia, and low intact parathyroid hormone l

[A case report of severe hypocalcemia secondary to Fahr's syndrome].

Fahr's syndrome is a rare neurological disease characterized by bilateral intracranial calcifications associated with disturbances in calcium-phosphate metabolism, most commonly due to hypoparathyroidism. We report a 56-year-old man presenting with generalized seizures and behavioral changes. Laboratory evaluation revealed severe hypocalcemia, hyperphosphatemia, and low parathyroid hormone levels. Brain computed tomography demonstrated symmetrical calcifications involving the basal ganglia and other subcortical structures, consistent with Fahr's syndrome. Diagnosis was established through the

Primary hyperparathyroidism in a child with tuberous sclerosis.

Primary hyperparathyroidism in patients with tuberous sclerosis (TS) is extremely rare: only three cases have been reported previously, two of them in pediatric patients. An 11-year-old male patient with a clinical and molecular diagnosis of TS presented with mild symptomatic hypercalcemia in routine laboratory tests. Biochemical evaluation confirmed hypercalcemia with hypophosphatemia and inadequate parathyroid hormone levels, with radiological signs of hyperparathyroidism. Ultrasound demostrated hyperplasia of the left superior parathyroid gland, and a sestamibi and technetium-99m scintigram

Gene dosage effects of 22q11.2 copy number variants on in-vivo measures of white matter axonal density and dispersion.

22q11.2 deletion (22qDel) and duplication (22qDup) carriers have an increased risk of neurodevelopmental disorders and exhibit altered brain structure, including white matter microstructure. However, the underlying cellular architecture and age-related changes contributing to these white matter alterations remain poorly understood. Neurite orientation dispersion and density imaging (NODDI) was used on mixed cross-sectional and longitudinal data to examine group differences and age-related trajectories in measures of axonal density (i.e., intracellular volume fraction; ICVF), axonal orientation
