Delayed Onset Hypoparathyroidism Following Thyroidectomy: Case Report and Review of Literature.

Delayed postsurgical hypoparathyroidism (hypoPT) is a rare but important complication of thyroidectomy, often presenting years after the initial procedure. We report the case of a 48-year-old woman with a remote history of total thyroidectomy for Graves' disease, who presented with neuromuscular symptoms and severe hypocalcemia 22 years after surgery. Laboratory investigations revealed low calcium, low parathyroid hormone (PTH), and elevated phosphate, consistent with hypoPT. The patient was initially managed with intravenous calcium followed by alfacalcidol therapy. Literature review highligh

Transoral robotic thyroidectomy vs. Open thyroidectomy for thyroid cancer: A propensity Score-Matched analysis.

Transoral robotic thyroidectomy (TORT) offers a virtually scarless outcome but requires comparative validation studies against open thyroidectomy (OT). Patients who underwent thyroid surgery at our institution between January 2020 and October 2024 were enrolled. Through propensity score matching analysis, a final cohort of 322 adults with pathologically confirmed thyroid carcinoma was identified, comprising 161 patients in each group. We analyzed and compared clinical outcomes between the two surgical cohorts, along with patient-reported outcomes including Numerical Rating Scale (NRS) for pain

Successful transition from rhPTH(1-84) to palopegteriparatide in chronic hypoparathyroidism.

Hypoparathyroidism management has advanced in recent decades, particularly for patients unresponsive to conventional therapy. Several parathyroid hormone-based agents have been developed to better imitate physiologic hormone replacement. Recombinant human parathyroid hormone (1-84) [rhPTH(1-84), Natpar®] was approved as an adjunct to calcium and vitamin D for chronic hypoparathyroidism but was later withdrawn, with global production ceasing in 2024. Palopegteriparatide (Yorvipath®; TransCon PTH), a long-acting parathyroid hormone (1-34) prodrug recently approved, represents a new-generation re

A systematic review and meta-analysis: surgical outcomes comparing transoral robotic and open approaches to thyroidectomy.

Thyroid cancer incidence is rising, especially in young women. Conventional open thyroidectomy (OT) is effective but leaves a neck scar, affecting mental health. Transoral robotic thyroidectomy (TORT) offers scarless surgery, but its safety and completeness need evaluation. We systematically searched PubMed, Embase, Cochrane Library, and Web of Science for trials comparing TORT and OT. Outcomes included lymph node yield, complications (recurrent laryngeal nerve injury, hypoparathyroidism, hematoma, seroma, flap perforation), operation time, hospital stay, and postoperative pain. Autoimmune polyglandular syndrome type 1 with compound heterozygous AIRE gene pathogenic variants and stage 1 type 1 diabetes mellitus: case report and literature review of Chinese population.

BACKGROUND: Autoimmune polyendocrine syndrome type 1 (APS-1) is a rare monogenic autoimmune disorder caused by pathogenic variants in the AIRE gene, characterized by impaired central immune tolerance and multi-organ autoimmune damage. While relatively common in genetically isolated populations, genetically confirmed APS-1 cases remain exceptionally rare in Chinese individuals. To date, population-specific genotypic and phenotypic features of APS-1 in China have not been systematically summarized. CASE PRESENTATION: We report a 31-year-old female patient who presented with hypocalcemic convuls

Fahr's Syndrome in a Young Adult Male: A Case of Seizure and Cognitive Decline Secondary to Hypoparathyroidism.

Fahr's syndrome is a rare neurological condition characterized by bilateral intracranial calcifications secondary to metabolic or endocrine abnormalities, most commonly hypoparathyroidism. This condition is distinct from Fahr's disease (primary familial brain calcification), which is genetically inherited. A 30-year-old South Asian Pakistani man presented with sudden-onset generalized tonic-clonic seizures and progressive cognitive decline. Neuroimaging revealed bilateral, symmetrical calcifications in the basal ganglia, thalami, cerebellar dentate nuclei, periventricular regions, and centrum

Refractory hypocalcemia from combined autosomal dominant hypocalcemia type 2 and postsurgical hypoparathyroidism.

A 49-year-old female with a history of chronic hypocalcemia had serum calcium concentrations that ranged between 6.4 and 8.5 mg/dL (nl, 8.6-10.3 mg/dL) with inappropriately low to low-normal parathyroid hormone (PTH) concentrations. She did not report symptoms of hypocalcemia. A thyroid nodule was discovered, and the patient underwent total thyroidectomy for papillary thyroid cancer. Parathyroid autotransplantation was not performed since parathyroid glands were not identified during the surgery. After total thyroidectomy, the patient developed a more severe symptomatic, refractory hypocalcemi

Development of a Novel Immunoprotective Culture System for Parathyroid Allografts: Utilizing Static Magnetic Fields to Modulate Lymphocyte Migration.

Parathyroid allotransplantation is a promising treatment for hypoparathyroidism, yet immune rejection and fibrosis remain significant barriers. This study evaluates a novel immunoprotective culture system utilizing a moderate-intensity static magnetic field (SMF) to modulate lymphocyte migration without compromising graft functionality. Human parathyroid cells were encapsulated and divided into 10 experimental groups, co-cultured with Jurkat T-lymphocytes, and either exposed to SMF or maintained as controls. Over 72 h, we analyzed parathormone (PTH) secretion, cell viability (via proliferation

Reflections on prevention and treatment of post-thyroidectomy hypoparathyroidism: current management approaches and future prospects.

Postoperative hypoparathyroidism (hypoPT) is one of the predominant and severe complications of thyroid surgery. This condition manifests as suboptimal circulatory levels of parathyroid hormone (PTH), engendering multifaceted systemic perturbations. This comprehensive review elucidates the underlying pathophysiological mechanisms and clinical presentations associated with hypoparathyroidism. Moreover, methodologies for the precise recognition and preservation (naked eye, near-infrared autofluorescence, indocyanine green, carbon nanoparticles, and mitoxantrone hydrochloride) of the parathyroid

Vitamin D Supplementation to Prevent Post-Thyroidectomy Hypocalcemia.

OBJECTIVE: Hypocalcemia is the most common complication following thyroidectomy. Previous studies yielded inconsistent results on whether vitamin D3 prevents postoperative hypocalcemia and were conducted in non-European countries with different dietary habits and baseline vitamin D levels. Therefore, we explored the effect of preoperative vitamin D3 on post-thyroidectomy hypocalcemia in a Dutch cohort. METHODS: Patients undergoing thyroidectomy between 2023 and 2025 received 100,000 IU vitamin D3 1 week before surgery ("vitamin D group") and were compared to a historical cohort (2019-2022) wi

Learning curve in autofluorescence-guided thyroid surgery.

BACKGROUND: Near-infrared autofluorescence (NIRAF) has been introduced as an adjunct to improve parathyroid gland (PG) identification during thyroid surgery. The extent to which its intraoperative application is influenced by a learning curve remains unclear. This study aimed to evaluate and quantify a potential learning curve in NIRAF-guided thyroid surgery. MATERIALS AND METHODS: We conducted a retrospective cohort study including all consecutive patients undergoing image-based NIRAF-guided thyroid surgery between 2021 and 2023 at two Danish ENT departments. After each procedure, all surgeo

Therapeutic central lymph node dissection in papillary thyroid cancer: what is the optimal extent in unilateral nodal disease?

BACKGROUND: The optimal extent of central lymph node dissection (CLND) in patients with unilateral papillary thyroid cancer (PTC) and clinical nodal involvement (cN1a) confined to the ipsilateral central compartment remains controversial due to its potential risks, including permanent hypoparathyroidism and recurrent laryngeal nerve palsy. This study aimed to assess the prevalence of contralateral central neck recurrence following omission of contralateral prophylactic CLND. METHODS: A retrospective, single-center study was conducted, including patients with unilateral cN1a PTC who underwent i

Papillary Thyroid Carcinoma in the Era of De-Escalation: Toward Personalized and Less Aggressive Management.

Background: Papillary thyroid carcinoma (PTC) is the most common type of thyroid cancer and is generally associated with an excellent prognosis. Historically, treatment strategies were uniform and frequently aggressive, including total thyroidectomy and routine radioiodine ablation, even in low-risk cases. Current Perspective: Over the past decade, the management of PTC has shifted toward a de-escalation paradigm. This transition is driven by high evidence showing that the majority of PTCs follow an indolent course, with low recurrence and mortality rates. As a result, there is increasing emph

Topical Sodium Thiosulfate for the Treatment of Calciphylaxis: Case Report.

Calciphylaxis is a rare syndrome of microvascular calcification and skin infarction, occurring most commonly in patients with kidney failure. Calciphylaxis carries a poor prognosis, and current treatment involves mitigating risk factors, wound care, and medical therapies such as intravenous sodium thiosulfate for calcium chelation. We present an innovative treatment approach of topical sodium thiosulfate soaked gauze successfully used to treat calciphylaxis of a 58-year-old female with renal transplant. She presented with a tender bruise over the posterior right calf, in the context of good gr

Rethinking prophylactic central neck dissection in clinically node-negative papillary thyroid cancer.

Papillary thyroid cancer is associated with a high rate of lymph node metastases, mainly in the form of small metastases in the central compartment. Even when present, these metastases are most often undetected by preoperative ultrasound (clinically node-negative or cN0) and only identified by systematically performing a prophylactic central neck dissection. However, even in the absence of prophylactic central neck dissection to remove these metastases, clinically detected recurrence in lymph nodes occurs in fewer than 10% of patients, which explains the controversy surrounding the procedure.

Juvenile Nasopharyngeal Angiofibroma in an Adult Patient: A Rare Presentation with Fahr Syndrome and Multiple Comorbidities-A Case Report and Literature Review.

Background and Clinical Significance: Juvenile nasopharyngeal angiofibroma (JNA) is a benign but locally aggressive vascular tumor, classically affecting adolescent males. Diagnosis in adulthood is exceptionally uncommon and may mimic other vascular or malignant nasopharyngeal lesions. This patient also had chronic hypocalcemia with Fahr-like intracranial calcifications secondary to long-standing postoperative hypoparathyroidism after thyroid carcinoma treatment. To our knowledge, this coexistence has not been previously reported. Case Presentation: A 34-year-old Caucasian male with papillary

Prophylactic Central Lymph Node Dissection Versus Thyroidectomy Alone in Clinically Node-Negative Thyroid Cancer: A Systematic Review and Meta-Analysis.

BackgroundThe role of prophylactic central neck dissection (pCND) in clinically node-negative (cN0) thyroid cancer patients remains debated. Although pCND may enhance staging and reduce recurrence, it also carries risks such as hypoparathyroidism and laryngeal nerve injury. This meta-analysis of randomized controlled trials (RCTs) aims to provide more robust evidence regarding the benefits and risks of pCND.MethodsA systematic review and meta-analysis were conducted following PRISMA guidelines. Eligible RCTs comparing thyroidectomy with and without pCND were identified through searches of PubM

22q11.2 duplication syndrome and LZTR1-related Noonan syndrome type 10 overlapping phenotypes in a dual diagnosis - fetal hydrops: a case report.

BACKGROUND: Nonimmune factors are the most common cause of fetal hydrops, which are characterized by the accumulation of pathological fluid due to various causes. The etiology of this disease is complex, with a relatively high mortality rate, and the prognosis largely depends on the underlying cause. Notably, fetal hydrops attributable to genetic causes, such as chromosomal abnormalities or gene mutations, are typically associated with a poor prognosis.
