In Vivo Engraftment and Functional Efficacy of a 3D-Bioprinted Human Parathyroid Equivalent.

Background and Objectives: Hypocalcemia due to hypoparathyroidism (HypoPTH) is the most common complication following thyroid surgery, typically resulting from iatrogenic removal, tissue damage, or compromised vascularization of the parathyroid glands. Patients with persistent HypoPTH are at risk for long-term complications such as osteoporosis, cardiac dysfunction, and renal impairment. Lifelong regulation of calcium levels is therefore essential to prevent morbidity and mortality associated with these complications. In this study, we aimed to evaluate the functional engraftment efficacy of 3

Different approaches of dissecting the recurrent laryngeal nerve during total thyroidectomy affect postoperative parathyroid function.

PURPOSE: This study aimed to compare the effects of proximal versus distal dissection of the recurrent laryngeal nerve (RLN) on the preservation of parathyroid function in patients undergoing total thyroidectomy for malignant thyroid tumors. METHODS: In this retrospective study, 209 patients were categorized into two groups based on the RLN exposure technique: a proximal approach (identification within 1 cm of the laryngeal entry point, n = 59) and a distal approach (identification within the tracheoesophageal groove, n = 150). Serum parathyroid hormone (PTH) and calcium levels were measured p

Hippocampal transcriptome profiling in a 22q11.2 deletion syndrome mouse model: comparison with human schizophrenia.

22q11.2 deletion syndrome (22q11.2DS) confers one of the highest genetic risks for schizophrenia, yet the molecular mechanisms remain incompletely understood. We performed comprehensive RNA sequencing of the dorsal hippocampus in Df1/+ mice, a 22q11.2DS model, integrating behavioral assessment and cross-species comparison with human schizophrenia postmortem data. Df1/+ mice exhibited selective contextual fear memory impairment without gross locomotor deficits. Transcriptomic analysis using integrated over-representation and gene set enrichment approaches revealed upregulation of synaptic signa

Non-Classical Presentations of Rare Hereditary Hypoparathyroidism: A Case Series of CASR, GNA11 and GATA3 Mutations.

BACKGROUND: Hereditary hypoparathyroidism (hypoPT) is a rare endocrine disorder caused by absent or insufficient parathyroid hormone (PTH) secretion. Genetic forms are uncommon and frequently underdiagnosed, particularly when clinical onset is atypical. PURPOSE: We present a case series of three patients with genetic hypoPT caused by CASR, GNA11 and GATA3 mutations, highlighting atypical clinical presentation, diagnostic challenges and management approaches. METHODS: Clinical data, biochemical results, and genetic findings were analysed in a case of three female patients with early-onset, no

Integrated multiomics profiling of amniotic fluid exosomes reveals dysregulated lipid and protein signatures in fetal 22q11.2 deletion syndrome.

BACKGROUND: Amniotic fluid exosomes (AF-Exos) are pivotal carriers of biological information during fetal development; however, their role in 22q11.2 Deletion Syndrome (22q11.2DS) remains unclear. To elucidate the molecular mechanisms underlying fetal anomalies in 22q11.2DS, this study performed a comprehensive multiomics analysis of AF-Exos obtained from 22q11.2DS fetuses (n = 5) and matched controls (n = 5). RESULTS: While exosomal morphology and size distribution remained unaltered, integrated lipidomic and proteomic profiling revealed profound molecular remodeling. Lipidomics analysis rev

Acute psychosis with convulsive episodes following total thyroidectomy induced by concurrent hypothyroidism and hypocalcemia: a case report.

BACKGROUND: Total thyroidectomy is a common surgical procedure, with postoperative anxiety and depression being recognized complications. However, the development of acute psychosis is a rare and severe manifestation. The etiology of such presentations is often complex, involving metabolic derangements that directly impact central nervous system function. We report a case of severe psychosis with convulsive features precipitated by the synergistic effects of two distinct postoperative complications: iatrogenic hypothyroidism and hypoparathyroidism-induced hypocalcemia. The unique monthly cycli

Management of patients with hypoparathyroidism: a Delphi consensus analysis.

PURPOSE: Chronic hypoparathyroidism is a rare endocrine disorder characterized by parathyroid hormone (PTH) deficiency and associated with substantial clinical and therapeutic challenges. Despite conventional therapy and the off-label use of teriparatide, important unmet needs persist regarding metabolic control, prevention of long-term complications, and health-related quality of life (HRQoL). Although recent international and national guidelines have updated recommendations on chronic hypoparathyroidism management, relevant uncertainties and heterogeneity remain in real-world practice, parti

Understanding hypoparathyroidism as a disease of broad functional deficiency.

Hypoparathyroidism (HypoPT) is a rare disease defined by undetectable, low, or inappropriately normal parathyroid hormone (PTH) concentrations in the presence of hypocalcemia. Calcium and active vitamin D are often used for the treatment of HypoPT but do not address the underlying lack of physiological PTH activity. In this review, we discuss how the chronic inadequacy of PTH leads to broad functional consequences. This functional deficiency results in a wide spectrum of symptoms, impaired functioning and quality of life, and significant economic and healthcare costs. We consider how kidney co

Approach to the patient: management of bone fragility in patients with chronic hypoparathyroidism.

Parathyroid hormone is the main hormonal regulator of bone remodeling; thus, chronic hypoparathyroidism (HypoPT) leads to low bone turnover that might compromise bone strength, despite the normal or even high bone mineral density. Recent studies suggest that fracture risk might be increased in patients with HypoPT. Given that all available treatments for osteoporosis affect bone remodeling, management of bone fragility of any etiology in patients with HypoPT is challenging and lacks clinical evidence in terms of fracture risk reduction. In the present article, we describe the changes in bone r

Static Magnetic Field-Mediated Parathyroid Xenotransplantation Modulates Lymphocyte Migration: A Potential Immunosuppression-Free Long-Term Treatment for Hypoparathyroidism.

Static magnetic fields (SMFs) are underexplored as biophysical tools for transplant immunomodulation. This study investigated a 300 mT SMF as a non-pharmacological adjuvant to enhance graft survival in parathyroid xenotransplantation. Human parathyroid tissues were transplanted into Sprague-Dawley rats (n = 20) across four groups: control (G1), SMF-only (G2), transplantation-only (G3), and SMF-assisted transplantation (G4). Following 30-day continuous SMF exposure, functional and immunological assessments were performed. G4 achieved the highest systemic PTH recovery (p = 0.009) without alterin

Risk of bone overstimulation in long-term PTH(1-34) therapy for hypoparathyroidism.

OBJECTIVE: PTH(1-34), used off-label, effectively controls serum calcium in patients with chronic hypoparathyroidism inadequately managed by standard-of-care therapy. PTH(1-34) also strongly stimulates bone remodeling and often raises bone turnover markers above normal levels. This study aimed to visually assess the risk of excessive bone stimulation associated with prolonged PTH(1-34) replacement therapy. DESIGN: An observational study (N°20201026100318) was conducted in adults with chronic hypoparathyroidism treated with PTH(1-34) for over 2 years at 3 referral centers. METHODS: Patients u

Genome-wide tandem repeat expansions modify schizophrenia risk in the presence of a 22q11.2 deletion.

Schizophrenia develops in one in every four individuals with a pathogenic 22q11.2 deletion, yet the genetic modifiers influencing the manifestation of schizophrenia in this high-risk group remain incompletely understood. Here, we identify rare tandem repeat expansions (TREs) as significant contributors to schizophrenia risk in this population. Genome sequencing of 438 unrelated individuals with 22q11.2 deletions revealed a marked enrichment of rare genic TREs among those with schizophrenia, with effect sizes comparable to common polygenic risk. These TREs are disproportionately located in intr

Subjective health status, life performance and complications in chronic hypoparathyroidism - a German multicenter survey.

BACKGROUND: There is mounting evidence that conventional replacement strategies with calcium and vitamin D are insufficient to fully prevent complications of chronic hypoparathyroidism (HypoPT) in all patients. METHODS: To investigate the disease burden of chronic HypoPT, we performed a survey in 205 HypoPT patients (159 females; median age 55 years; median disease duration 16 years). Patients received a disease-specific questionnaire asking for subjective health status, comorbidities, disease-related emergencies and interference of HypoPT with daily and work life. Patients were further asses

Autofluorescence enhances intraoperative detection of parathyroid glands: a single-center comparative study.

INTRODUCTION: Permanent hypoparathyroidism after total thyroidectomy remains a major complication, occurring in close to 10% of cases and increasing risks of cardiac, renal, and metabolic morbidity. Near-infrared autofluorescence (NIRAF) imaging has emerged as a promising adjunct to improve parathyroid gland (PG) identification and preservation. This study evaluates the efficacy of NIRAF in enhancing intraoperative PG detection compared to conventional visual inspection. METHODS: This single-center retrospective cohort study included 349 patients undergoing total thyroidectomy. Outcomes were

Hypoparathyroidism: a brief historical overview for clinicians.

Hypoparathyroidism is a rare endocrine disorder characterized by deficient secretion of parathyroid hormone (PTH), resulting in hypocalcemia, hyperphosphatemia, and impaired mineral homeostasis. Although most cases are postsurgical, the disease encompasses a heterogeneous group of etiologies, including genetic, autoimmune, and infiltrative causes. For much of its history, hypoparathyroidism was considered unique among endocrine deficiencies in that it was not treated with hormone replacement, owing to limited understanding of parathyroid physiology and the absence of safe and effective PTH-bas

Population differences of chromosome 22q11.2 duplication structure predispose differentially to microdeletion and inversion.

Chromosome 22q11.2 microdeletion syndrome (22q11.2DS) is mediated by high-identity polymorphic low-copy repeats (LCRA-to-D) that have been challenging to sequence characterize. We sequence-resolved 135 chromosome 22q11.2 haplotypes from diverse humans and define 63 distinct structural configurations differing in size by 11-fold for LCRA. This diversity is driven by a 105 kbp segmental duplication flanked by 25 kbp inverted repeats that arose in the apes but expanded in humans ~1 million years ago. African LCRA haplotypes are significantly longer (p = 0.0047) and predicted to be more protective

Assessment of arterial stiffness and plasma FGF23 levels in patients with hypoparathyroidism.

INTRODUCTION: Patients with hypoparathyroidism are at a higher cardiovascular risk (CVR) due to mechanisms that are not yet well understood. In patients with renal failure, fibroblast growth factor 23 (FGF23) has been associated with an increased calcification and arterial stiffness (AS), leading to an increased CVR. In hypoparathyroidism-due to the situation of hyperphosphatemia-an increase in FGF23 is also probable. There is a paucity of research examining the relationship between FGF23 levels and AS in patients with hypoparathyroidism. The present study aims to assess AS, by measuring aorti
