INTRODUCTION: Infectious diseases can affect the endocrine system through multiple mechanisms, including direct cytopathic effects, immune dysregulation, inflammatory pathways, and in some cases the secretion of hormone-like substances by the pathogens themselves. While viral and bacterial infections represent the most frequent triggers, fungal and parasitic agents can also contribute, though less commonly.
METHODS: This narrative review, based on a search in PubMed and Google Scholar, examined both paediatric and adult studies, given the limited evidence available in children. The analysis f

INTRODUCTION AND IMPORTANCE: Meningitis is a significant global health threat, affecting millions and causing numerous deaths each year. Neisseria meningitides disproportionately affects resource-poor countries. Waterhouse-Friderichsen syndrome (WFS), a severe complication involving endotoxin-mediated adrenal damage, further worsens patient outcomes.
CASE PRESENTATION: A 35-year-old unvaccinated woman from Garbo, Somali, Ethiopia presented with a 24-hour history of fever, headache, and neck pain, progressing to altered mental status, right-sided weakness, and respiratory distress. Examination

Waterhouse-Friderichsen syndrome (WFS) represents a fulminant septic condition characterized by bilateral adrenal hemorrhage and rapid clinical deterioration. Although classically associated with meningococcal infection, fatal cases occur across multiple infectious etiologies and heterogeneous clinical contexts. In forensic practice, diagnosis frequently relies on post-mortem interpretation rather than clinical recognition, making consistent pathological assessment essential. A systematic review was conducted according to PRISMA guidelines to identify autopsy-confirmed fatal cases consistent w

Waterhouse-Friderichsen Syndrome (WFS) is a rare but life-threatening condition characterized by massive adrenal hemorrhage. WFS represents one of the features of the Overwhelming Post-Splenectomy Infection, which occurs any time after spleen removal and is recognized as the most serious complication in asplenic patients. We report a fatal case of WFS resulting from Streptococcus pneumoniae infection in a vaccinated and splenectomized patient. D.R., a 62-year-old man who had been splenectomized 20 years earlier following a traffic accident and had undergone Streptococcus Pneumoniae vaccination

BACKGROUND: Sepsis can lead to life-threatening complications such as hemophagocytic lymphohistiocytosis (HLH) and bilateral adrenal hemorrhage (BAH), commonly known as Waterhouse-Friderichsen syndrome. HLH involves severe inflammation and organ damage, often linked with infections and autoimmune diseases, while BAH leads to adrenal insufficiency, typically caused by Neisseria meningitidis, though Escherichia coli (E. coli) is a rare cause.
CASE PRESENTATION: A 27-year-old Japanese woman, with a history of diabetes mellitus and obesity, presented with a complicated urinary tract infection. Sh

BACKGROUND: Capnocytophaga canimorsus is a gram-negative bacterium found in the oral cavity of dogs and cats. It can cause severe infections, including sepsis, especially in immunocompromised individuals, though rare cases also occur in immunocompetent patients.
CASE PRESENTATION: A man in his seventies with hypertension developed fever, chills and abdominal pain after being scratched by his dog. He presented with severe abdominal pain, tachycardia, fever and elevated lactate levels. Initial CT scans did not reveal abdominal pathology, but septic shock was suspected. Despite treatment with br

Diagnosing sudden, unexplained death in healthy individuals is challenging. This case report details a rare case of fatal Waterhouse-Friderichsen Syndrome (WFS) in a previously healthy 27-year-old man. Post-mortem examination revealed vomit stains, widespread purpura, and haemorrhagic adrenal glands. Microbiological analysis confirmed a systemic infection with Capnocytophaga canimorsus. While C. canimorsus infections are uncommon, their association with WFS is exceedingly rare. To our knowledge, this is the first documented case of fatal C. canimorsus sepsis causing WFS in an immunocompetent p

OBJECTIVE: Eculizumab is a terminal complement C5 inhibitor widely used in the treatment of various complement-mediated diseases. Its safety information primarily comes from clinical trials and FDA labeling. This study aimed to systematically assess the real-world safety profile of Eculizumab based on the FDA Adverse Event Reporting System (FAERS) and to identify new adverse drug reactions (ADRs) that are not sufficiently highlighted in the product label.
METHODS: Adverse event reports related to Eculizumab from the FAERS database (2007 Q1-2025 Q2) were retrieved. Signal detection was perform

Adrenal haemorrhage (AH) is uncommon and often underrecognized and may complicate severe systemic stress, infection, or coagulopathy. Prompt diagnosis is essential to exclude adrenal insufficiency and adrenal neoplasms. Three cases of AH occurring in varied clinical settings describe diagnostic approaches, management, and outcomes and draw insights for clinicians. Case 1 is a patient in septic shock hospitalised in the ICU who developed bilateral AH (Waterhouse-Friderichsen syndrome). Case 2 is about a patient admitted for chest pain, fall, and suspected infective exacerbation of chronic obstr

Childhood is a long period extending up to the age of 18 years. Childhood encompasses different developmental stages; each stage has specific characteristics. This 5-year study included 244 autopsied children who died unexpectedly due to natural causes. This study was conducted in the forensic pathology unit of the Egyptian Forensic Medicine Authority (EFMA). Pathological causes of death were diagnosed in 181 cases, representing nearly three-quarters (74.2 %) of cases. Males represented 60.8 % of these cases. More than half (51.4 %) of deaths due to natural disease occurred within the first mo

Waterhouse-Friderichsen syndrome (WFS) is a rare but life-threatening complication associated with acute hemorrhagic necrosis of the adrenal glands, primarily linked to meningococcal infection. This report details the case of a 62-year-old female with HIV/AIDS and substance misuse who presented with ventricular tachycardia and hemodynamic instability. Subsequent evaluation revealed WFS in the context of disseminated meningococcal infection. The case highlights the diversity of WFS manifestations and the diagnostic challenges, particularly in patients with comorbidities. Managing WFS involves a

The Waterhouse-Friderichsen syndrome represents a critical condition characterized by a septic shock associated with a disseminated intravascular coagulation causing the plugging of the microvascular network virtually all organs and systems, including the skin, the kidneys, the liver, and adrenal glands; the mortality rate is elevated, and survivors often must undergo multiple limb amputations. Here, we describe the uncommon case of an asplenic patient who developed this syndrome after a superficial wound caused by a dog bite causing an initial infection due to Capnocytophaga canimorsus that i

Febrile seizures, which are relatively common in young children, are often triggered by an infection and resolve quickly. Prompt presentation to a pediatric department is mandatory after any first seizure and every time for children ≤ 12 months. Central nervous system (CNS) diseases in childhood are able to cause seizures or other neurological disorders. Even the slightest suspicion of a seizure with CNS involvement must be promptly treated. In case of doubt, both an antiviral and an antibacterial treatment are started in parallel, which can be stopped after detecting the pathogen. Lumbar punc

Waterhouse-Friderichsen syndrome and central diabetes insipidus are uncommon but potentially fatal endocrine and metabolic diseases. Waterhouse-Friderichsen syndrome is defined as adrenal insufficiency caused by adrenal hemorrhage, which is typically bilateral and most frequently due to meningococcal infection. It is usually diagnosed by necropsy. Central diabetes insipidus in children is often caused by trauma, intracranial lesions, autoimmune diseases, and infections. In addition, it can be caused by mutations in the AVP-NPII gene, although this occurs typically later in childhood rather tha

Waterhouse-Friderichsen syndrome is a rare but potentially fatal disorder of the adrenal gland characterized by bilateral adrenal hemorrhage. It is classically a result of meningococcal sepsis and presents acutely with features of shock, petechial rashes, abdominal pain, and non-specific symptoms such as headache, fatigue, and vomiting. Treatment consists of fluid resuscitation, corticosteroid replacement, and possibly surgery. The prognosis is poor despite treatment. This chapter will review the etiology, pathogenesis, clinical features, and management of the disease.

BACKGROUND Waterhouse-Friderichsen syndrome, also known as acute adrenal insufficiency due to adrenal gland hemorrhage, is an uncommon and frequently fatal condition classically presenting with fever, shock, rash, and coagulopathy. Although most often associated with Meningococcemia, many other etiologies have been implicated, including reports of Staphylococcus aureus infection on autopsy examinations. This report details an adult intravenous drug user with adrenal hemorrhage associated with methicillin-resistant Staphylococcus aureus (MRSA) bacteremia. CASE REPORT A 58-year-old man with a hi

BACKGROUND: Capnocytophaga canimorsus, a Gram-negative rod, belongs to the Flavobacteriaceae family and colonizes the oropharynx of dogs and cats. Infections with C. canimorsus are rare and can induce a systemic infection with a severe course of the disease. So far, only five case reports of C. canimorsus infections associated with Waterhouse-Friderichsen Syndrome (WFS) have been reported with only two of the patients having a history of splenectomy.
CASE PRESENTATION: Here, we report a fatal case of WFS due to C. canimorsus bacteremia and mycetal superinfection in a 61-year-old female asplen

We report three cases of Waterhouse-Friderichsen syndrome (WFS) that were confirmed during forensic autopsies. Case 1 involved a man in his 50s post-splenectomy. Bacteriological examination revealed Streptococcus pneumoniae (S. pneumonia). The patient was considered to have died of asphyxiation after aspirating vomit. Case 2 involved a man in his 40s. Bacteriological examination again revealed S. pneumoniae. Histopathological examination showed hypoplasia of the spleen. This patient was considered to have died of multiple-organ failure due to sepsis, disseminated intravascular coagulation, and
