Primary hyperparathyroidism (PHPT) is a metabolic disorder characterized by hypercalcemia with elevated or unsuppressed parathyroid hormone (PTH). It is rare in children but common in adults, particularly in women around the age of 50. In 85% of cases, PHPT is asymptomatic and is diagnosed following hypercalcemia and elevated PTH levels during routine examinations. PHPT occurring before the age of 25 and the normocalcemic phenotype are unusual situations that may delay diagnosis. These nonclassical forms expose patients to severe bone complications that can cause disability. Our objective is t

Traumatic Brain Injury (TBI) could lead to intracranial hemorrhage (ICH), which has now been identified as a major cause of death after trauma if it is not adequately diagnosed and properly treated within the first 24 hours. CT examination is widely preferred for urgent ICH diagnosis, which enables the fast identification and detection of ICH regions. However, the use of it requires the clinical interpretation by experts to identify the subtypes of ICH. Besides, it is unable to provide the details needed to conduct quantitative assessment, such as the volume and thickness of hemorrhagic lesion

Primary hyperparathyroidism (PHPT) is uncommon in the pediatric population and rarely presents with overt skeletal complications such as brown tumors. This case study describes a 16-year-old female who initially presented with abdominal pain and was subsequently found to have severe hypercalcemia (serum calcium 16.4 mg/dL) and markedly elevated parathyroid hormone (PTH 1700 pg/mL) and bilateral nephrolithiasis. There were no features suggestive of multiple endocrine neoplasias. A history of a previously diagnosed "non-ossifying fibroma" in the proximal tibia was later reinterpreted as a brown

BACKGROUND: Bone mineral density (BMD) measurement at the lumbar spine, femoral neck, total hip, and distal 1/3 radius is recommended for patients with primary hyperparathyroidism (PHPT). Osteoporosis at any site justifies surgery. The distal 1/3 radius is often neglected. This study evaluated the prevalence of distal 1/3 radius measurement, the proportion of osteoporosis at this site alone, and BMD changes 1 year post-parathyroidectomy in patients with PHPT.
METHODS: A total of 548 patients who underwent parathyroidectomy for PHPT between 2016 and 2024 in University Hospital Center of Nantes

IntroductionPrevious studies have demonstrated that C11 choline positron emission tomography/computed tomography (Choline-PET/CT) can localize abnormal parathyroid glands in the reoperative setting. This study assesses a large volume of patients with primary hyperparathyroidism (1HPT) who underwent Choline-PET/CT to confirm its utility in the setting of negative or equivocal conventional imaging.MethodsAll patients who underwent Choline-PET/CT for evaluation of 1HPT from July 2017 to July 2024 at a single institution were reviewed. Inclusion criteria were patients who underwent parathyroidecto

INTRODUCTION: The diagnosis of nephrocalcinosis (NC) is challenging due to difficulties in radiologic detection, clinical heterogeneity, and its broad etiological spectrum that includes both genetic and non-genetic causes. This study aimed to uncover, in a pioneering manner, the distinct NC etiologies in a Brazilian cohort according to age of onset.
METHODS: This retrospective cross-sectional study was based on 96 medical records of outpatients (66 adults and 30 children) with NC, who had followed a comprehensive investigation.
RESULTS: Distal renal tubular acidosis was the leading cause of

PURPOSE: Risk of vertebral fractures is increased in primary hyperparathyroidism (PHPT). Bone qualities affected by PHPT are not necessarily reflected in bone mineral density (BMD), requiring imaging modalities with capability to capture bone microstructure and geometry. Digital tomosynthesis (DTS) imaging is a method with such capability. Therefore, the objectives of this study were to determine (i) vertebral bone quality differences between women who have established PHPT and normal controls using DTS, and (ii) the extent to which DTS discriminates patients with PHPT from those without.

OBJECTIVE: Primary hyperparathyroidism (PHPT) has varied presentations whose successful surgical management demands integration of the expanding literature within a comprehensive decision-making framework. Our objective is to compare the workup and management of variant (i.e., normocalcemic hyperparathyroidism (NCPHPT) and normohormonal hyperparathyroidism (NHPHPT)) to classic hyperparathyroidism (CPHPT), taking into account localization studies, surgical considerations, utility of intraoperative parathyroid hormone (IOPTH), and surveillance to identify gaps in knowledge that limit management

BACKGROUND Hungry bone syndrome (HBS) is a severe, rapid, profound, prolonged, and sometimes fatal condition characterized by hypocalcemia, exacerbated by suppressed parathyroid hormone levels following parathyroidectomy in severe primary hyperparathyroidism (PHPT). PHPT is reported to affect 1% of the general population and its prevalence is increasing with age, with a prevalence rate of 2% among people over age 55 years. More than 80% of all cases of primary hyperparathyroidism are asymptomatic for most of the disease duration. CASE REPORT We described a rare case of primary hyperparathyroid

BACKGROUND: Hypercalcemia is an electrolyte imbalance with multiple etiologies, the most common being primary hyperparathyroidism (PHPT). The diagnostic workup of PHPT is relatively straightforward, as elevated serum calcium levels in the presence of elevated or inappropriately normal parathyroid hormone (PTH) immediately raise the suspicion for PHPT. However, this typical presentation can be unhinged by the presence of severe vitamin D deficiency, which may mask hypercalcemia. Furthermore, vitamin D deficiency itself can contribute to PTH increase.
CASE PRESENTATION: We report the case of a

Sporadic primary hyperparathyroidism is most commonly caused by a single parathyroid adenoma. Traditionally, bilateral neck exploration with assessment of all four glands was the gold standard, achieving cure rates up to 98%. This approach has largely been replaced by limited exploration (LE), in which a prelocalized hyperfunctioning gland is selectively removed using a small open, video-assisted, or endoscopic approach.Successful LE relies on accurate preoperative localization of single-gland disease. First-line imaging consists of high-resolution ultrasound combined with 99mTc-sestamibi scin

Primary hyperparathyroidism in patients with tuberous sclerosis (TS) is extremely rare: only three cases have been reported previously, two of them in pediatric patients. An 11-year-old male patient with a clinical and molecular diagnosis of TS presented with mild symptomatic hypercalcemia in routine laboratory tests. Biochemical evaluation confirmed hypercalcemia with hypophosphatemia and inadequate parathyroid hormone levels, with radiological signs of hyperparathyroidism. Ultrasound demostrated hyperplasia of the left superior parathyroid gland, and a sestamibi and technetium-99m scintigram

BACKGROUND: Hyperparathyroidism-jaw tumor (HPT-JT) syndrome is a rare autosomal dominant disorder caused by CDC73 gene mutations, predisposing individuals to primary hyperparathyroidism (pHPT), cemento-ossifying fibromas, and other neoplastic conditions. [18F]Fluorocholine PET/CT has emerged as a tool for localizing hyperfunctioning parathyroid glands in pHPT, but its application in HPT-JT syndrome remains unreported.
CASE PRESENTATION: We describe the case of a 15-year-old male presenting with severe hypercalcemia, increased PTH serum levels, and a history of cemento-ossifying fibroma remova

BACKGROUND: In patients with primary hyperparathyroidism (pHPT), the importance of oxyphil cells for hormonal activity is uncertain.
METHODS: This retrospective observational study analyzed the histopathological findings of postoperative specimens from patients with pHPT (single gland disease). The indication for surgery was based on increased plasma intact parathyroid hormone (iPTH) concentrations and the presence of symptoms typical of pHPT. To quantify approximate oxyphil cell content (OCC), we determined the percentage of oxyphil cells observed in each specimen. To quantify the approximat

Persistent hypercalcemia after parathyroidectomy is a diagnostic challenge that requires careful evaluation beyond the parathyroid glands. We describe the case of a 76-year-old man with chronic kidney disease, resistant hypertension, and vestibular schwannoma who presented with weakness, constipation, and new, dark, raised lesions across his back. He reported undergoing parathyroidectomy one year earlier for presumed primary hyperparathyroidism, after which his hypercalcemia persisted. On admission, serum calcium was 13.1 mg/dL (reference = 8.5-10.5 mg/dL), and examination revealed numerous se

The systems regulating blood pressure and calcium-magnesium (Ca2+-Mg2+) homeostasis are increasingly recognized to have clinically relevant interactions, where alterations in one can lead to significant changes in the other. In this study, we developed a computational model integrating blood pressure regulation and Ca2+-Mg2+ homeostasis in a male rat. We simulated various conditions, including hypertension, Ca2+, Mg2+, and vitamin D3 deficiencies, and primary hyperparathyroidism. Simulations of hypertension, induced by various stimuli like increased renin or aldosterone secretion, demonstrated

BACKGROUND: Primary hyperparathyroidism (PHPT) during pregnancy is rare, with an estimated prevalence of <1% among pregnant women. While often asymptomatic, it may lead to serious maternal complications (nephrolithiasis, pancreatitis, preeclampsia) and adverse fetal outcomes (growth restriction, miscarriage, preterm delivery, stillbirth). Surgical parathyroidectomy in the second trimester remains the gold standard treatment, but may be unfeasible due to gestational age, contraindications, or patient refusal. In such situations, minimally invasive techniques such as microwave ablation (MWA) hav

BACKGROUND: Hypocalcemia is the most common complication following parathyroidectomy (PTX) in patients with primary hyperparathyroidism (PHPT). With asymptomatic PHPT cases increasingly comprising a larger proportion of clinical presentations, the association between preoperative symptom status and postoperative hypocalcemia risk remains poorly defined. This study aimed to identify risk factors for hypocalcemia after PTX in symptomatic vs. asymptomatic PHPT patients to enable individualized calcium management protocols.
METHODS: A retrospective analysis was performed on patients with primary
