Real-World Etelcalcetide Therapy for Secondary Hyperparathyroidism in Hemodialysis Patients with Inadequate Cinacalcet Response or Intolerance: Prospective Observational Study.

BACKGROUND: Cinacalcet is widely used for secondary hyperparathyroidism in hemodialysis (HD) patients. However, inadequate biochemical response, gastrointestinal intolerance, and pill burden may limit its use. Intravenous etelcalcetide offers an alternative in such cases. We aimed to provide real-world evidence on the effectiveness of etelcalcetide in reducing parathyroid hormone (PTH) levels and to assess the time to full response in HD patients with inadequate cinacalcet response or intolerance. METHODS: A prospective observational study was conducted among HD patients transitioned from cin

Beyond osteopoikilosis: a blended skeletal dysplasia associated with dual LEMD3 and ANO5 variants.

Osteopoikilosis is a rare autosomal dominant sclerosing bone dysplasia caused by pathogenic variants in LEMD3, which is typically asymptomatic and incidentally detected on radiographs. Gnathodiaphyseal dysplasia is an extremely rare skeletal disorder characterized by diaphyseal sclerosis, bone fragility, and pathological fractures. The coexistence of 2 genetically distinct skeletal dysplasias in a single individual is exceptionally uncommon. A 15-year-old female presented with progressive gait disturbance and a right hip intertrochanteric fragility fracture. Radiological evaluation demonstrate

[The 520th case: neck swelling, hypocalcemic convulsions, and spontaneous hemorrhage of a cystic parathyroid adenoma].

A 33-year-old man was admitted with a 4-day history of neck swelling and intermittent convulsions for 3 days. Physical examination revealed a soft, tender nodule measuring~4 cm×3 cm in the right lobe of the thyroid gland. Chvostek's and Trousseau's signs were positive. Laboratory tests revealed hypocalcemia, normal serum phosphorus and magnesium levels, elevated parathyroid hormone (PTH) levels, and reduced vitamin D levels. Bone turnover markers indicated active bone formation, and bone density testing revealed severe osteoporosis. Neck ultrasound and CT suggested a thyroid-origin mass; howev

Sagliker Syndrome: A Diagnostic Challenge.

Sagliker syndrome (SS), commonly referred to as "Uglifying human face Syndrome," is a rare but serious outcome of chronic kidney disease characterized by skeletal deformities, especially in the skull and jaws, due to uncontrolled secondary hyperparathyroidism (SHPT). The diagnosis is made through a combination of clinical symptoms, radiographic evidence (demonstrating bone deformities), and laboratory evaluations. Timely and effective treatment of SHPT is essential to prevent or lessen the onset of SS. Treatment includes managing phosphorus levels (through dietary changes, phosphate binders, a

The cross-cultural and chronological evolution of the kidney-bone relationship: from ancient medical insights to modern molecular endocrinology.

The kidney-bone axis constitutes a conserved bidirectional endocrine network that orchestrates skeletal remodeling and systemic mineral homeostasis. Its dysregulation underlies chronic kidney disease-mineral and bone disorder (CKD-MBD), renal osteodystrophy, secondary hyperparathyroidism, and associated vascular calcification, markedly elevating fracture and cardiovascular mortality risks. This review offers a unique cross-cultural and chronological synthesis of the kidney-bone relationship, bridging ancient medical insights with modern molecular endocrinology. Horizontally, we systematically

Single cell multi-omics guided hydrogel enables closed-loop therapy for renal osteodystrophy.

Fracture repair in renal osteodystrophy remains challenging because impaired bone quality compromises fixation stability, while current systemic therapies regulating bone metabolism do not sufficiently address the disease-specific mechanisms underlying defective repair. An integrated strategy combining early fracture stabilization with mechanism-guided local treatment is therefore needed. Here, integrated single-cell RNA sequencing and multi-omics analyses identify inflammatory macrophages as a key pathogenic population, characterized by glycolytic reprogramming driven by hypoxia-inducible fac

Hypophosphatemia after CAR-T cell therapy: a prospective study of inflammation, tubular injury and phosphate handling.

BACKGROUND: Hypophosphatemia is a frequent electrolyte abnormality after chimeric antigen receptor T-cell (CAR-T) therapy, but the clinical and biological factors associated with phosphate decline remain poorly defined. We prospectively evaluated the incidence, determinants, and potential biological associations of hypophosphatemia after CAR-T therapy. METHODS: We conducted a prospective study including 63 adults with B-cell lymphoma treated with CAR-T therapy between June 2020 and December 2023. Serum phosphate was measured daily during hospitalization. First-morning urine and blood samples

Progression of Secondary to Tertiary Hyperparathyroidism with a Coexisting Parathyroid Adenoma in Stage 3b Chronic Kidney Disease: A Case Report.

Tertiary hyperparathyroidism arises when prolonged secondary hyperparathyroidism progresses to autonomous parathyroid hormone secretion with persistent hypercalcemia, most often in chronic kidney disease. Diffuse or nodular multiglandular hyperplasia is the expected pathology; a discrete parathyroid adenoma arising within hyperplastic tissue is uncommon and may be overlooked when imaging is negative early in the disease course. A 60-year-old man with stage 3b chronic kidney disease and long-standing secondary hyperparathyroidism presented with recurrent hypercalcemia and severe vomiting. Labor

Postoperative sensorineural hearing loss associated with prolonged hypocalcemia.

A 57-year-old woman with end-stage renal disease and secondary hyperparathyroidism underwent subtotal parathyroidectomy and developed profound postoperative hypocalcemia consistent with hungry bone syndrome, followed by acute sensorineural hearing loss on postoperative day 3. Calcium plays a critical role in cochlear hair cell transduction and auditory signaling, and disruptions in calcium homeostasis have been associated with hearing loss, although primarily in chronic or syndromic conditions rather than acute postoperative states. This case underscores the importance of aggressive calcium re

Severe Hypocalcemia and Hypophosphatemia Following Ferric Carboxymaltose in a Patient With Roux-en-Y Gastric Bypass.

Iron deficiency anemia is commonly treated with intravenous iron when oral therapy is ineffective, poorly tolerated, or inadequately absorbed. Ferric carboxymaltose (FCM) is an effective intravenous iron formulation but is increasingly recognized to cause hypophosphatemia through fibroblast growth factor 23 (FGF23)-mediated renal phosphate wasting. Although FCM-induced hypophosphatemia is well described, concurrent clinically significant hypocalcemia is less commonly reported. We present the case of a 39-year-old woman with a history of Roux-en-Y gastric bypass, iron deficiency anemia, and vit

The effects of dialysate acids and divalent ions on calcification and cardiovascular outcomes: old wisdom and new developments.

Dialysate calcium (DCa) and magnesium (DMg) and the organic acid used in the acid concentrate have a profound combined influence on intradialytic mineral balance and may influence vascular calcification progression and treatment-related symptoms. Whereas Kidney Disease: Improving Global Outcomes recommends a DCa between 1.25 (DCa 1.25) and 1.50 mmol/l (DCa 1.50), the optimal DCa for an individual patient remains uncertain. No consistent mortality difference between DCa 1.50 and 1.25 has been observed. However, DCa 1.50 may induce a positive calcium balance, raising concerns about vascular calc

Type-specific detection and quantification of circulating tumor HPV DNA by digital PCR in surgically resected cervical cancer.

OBJECTIVE: Circulating tumor human papillomavirus (HPV) DNA (ctHPV DNA) has shown clinical utility in advanced cervical cancer; however, its role in surgically resected disease remains unclear. We evaluated the detectability and clinicopathological associations of ctHPV DNA in preoperative plasma from patients with surgically resected cervical cancer using digital polymerase chain reaction (dPCR). METHODS: We analyzed 49 surgically resected cervical cancer patients. HPV genotyping of formalin-fixed paraffin-embedded tumor tissues was performed using both PGMY09/11 consensus PCR and novel geno

Primary Hyperparathyroidism Presenting as a Distal Femur Pathologic Fracture in an Adolescent: A Case Report.

CASE: A 17-year-old adolescent boy presented with a pathologic distal femur fracture and bilateral distal femoral metadiaphyseal lytic lesions. Multidisciplinary evaluation confirmed bilateral distal femoral brown tumors associated with primary hyperparathyroidism secondary to a parathyroid adenoma. Treatment included bone biopsy, curettage, grafting, and retrograde locked intramedullary nail, followed by definitive partial parathyroidectomy. The contralateral metadiaphyseal lesion was treated with corrective osteotomy and prophylactic intramedullary stabilization. CONCLUSION: Primary hyperpa

Giant Cell Tumor of the Distal Ulna in an Elderly Patient on Hemodialysis: Diagnostic Challenge and Surgical Management.

Giant cell tumor of bone is a locally aggressive tumor that usually affects the epiphyseal region of long bones in young adults. Involvement of the distal ulna is rare, and the diagnosis in elderly patients with end-stage chronic kidney disease may be difficult because brown tumors related to secondary hyperparathyroidism can mimic giant cell tumor radiologically and histologically. We report the case of a 69-year-old woman with end-stage chronic kidney disease on hemodialysis who was admitted for a painful and progressively enlarging mass on the ulnar border of the left wrist that had been pr

Hazards of delay: Impact of parathyroidectomy timing on outcomes in secondary hyperparathyroidism of renal origin.

BACKGROUND: Surgical management is recommended in patients with secondary hyperparathyroidism when parathyroid hormone levels exceed 9 times the normal, and medical management has failed to control parathyroid hormone levels or associated complications. We aimed to examine the impact of the timing of parathyroidectomy on clinical outcomes in patients with secondary hyperparathyroidism and parathyroid hormone exceeding 9 times the normal. METHODS: In this retrospective cohort study using the TriNetX database, we included adults with a parathyroid hormone level of ≥600 pg/mL with concurrent sta

Liver transplantation-related osteoporosis and fragility fractures: pathogenic pathways, risk stratification and novel targeted therapy advances.

Liver transplantation (LT) serves as the definitive treatment for end-stage liver disease. Yet, osteoporosis (OP) and fragility fractures remain frequently overlooked skeletal complications that may impair the long-term prognosis of recipients. This review systematically summarizes recent evidence regarding epidemiological characteristics, multifactorial pathogenic pathways, risk stratification approaches, and advances in targeted interventions for LT-related bone disorders. Skeletal damage distributes across pre-transplant waiting, early post-transplant acute and long-term chronic phases; rap

Bone mineral density surveillance before and after parathyroidectomy: A provincial quality assurance retrospective cohort study.

BACKGROUND: Most patients with primary hyperparathyroidism have relatively mild disease with a negative impact on bone mineral density developing over years. Surgery to correct hyperparathyroidism has a protective effect on bone mineral density. The aim of this study was to evaluate the postoperative surveillance and outcomes of osteoporosis patients after parathyroidectomy. METHODS: Primary hyperparathyroidism patients who had undergone parathyroidectomy from 2020 to 2022 were identified using a prospective provincial operative reporting database. Follow-up data were extracted from a central

Surgery versus observation in patients 65 years or older with primary hyperparathyroidism: randomized clinical trial.

BACKGROUND: A substantial portion of surgically treated patients with primary hyperparathyroidism (pHPT) are older than 65 years. The indication for parathyroidectomy in older patients with pHPT and mild hypercalcaemia is unclear. Specifically, it is not known whether parathyroid surgery improves bone mineral density (BMD) in this age group. METHOD: Patients 65 years or older with pHPT, ionized calcium not exceeding 1.50 mmol/l, and a BMD Z-score above -2.5 standard deviations who were referred to the Department of Surgery, Lund University Hospital, were included in the study. Follow-up was p
