Efficacy and Safety of Upacicalcet in Hemodialysis Patients with Secondary Hyperparathyroidism: Two Randomized Clinical Trials.

KEY POINTS: These 2 phase 3 trials assessed upacicalcet, an intravenous calcimimetic, in hemodialysis patients with secondary hyperparathyroidism. ≥30% reduction in intact parathyroid hormone levels occurred in 42% (Parathyroid Hormone [PTH] Attenuation Trial in Hemodialysis-1) and 44% (Parathyroid Hormone [PTH] Attenuation Trial in Hemodialysis-2) with upacicalcet versus 17% and 17%, respectively, with placebo (strata-adjusted; both P < 0.001). Asymptomatic hypocalcemia was more frequent with upacicalcet, and symptomatic hypocalcemia was infrequent; gastrointestinal events were similar to pla

A Two-Stage Prediction Strategy for Primary Hyperparathyroidism.

Background: Primary hyperparathyroidism parathyroid hormone (PTH) hypersecretion is difficult to recognise when calcium is normal and difficult to distinguish from secondary hyperparathyroidism once PTH is found to be elevated. These are separate problems arising at separate points in care. We developed and evaluated two interpretable models corresponding to these decision points using an internal-external validation design. Methods: We analysed 2788 adults undergoing PTH testing across three campuses of one hospital (development n = 1843; internal-external validation n = 445 and n = 500); tho

Gut-targeted interventions to complement PTH-lowering therapy.

Secondary hyperparathyroidism is a major contributor to skeletal fragility in chronic kidney disease, yet fracture risk remains high despite parathyroid hormone-lowering therapy. In a rat model of chronic kidney disease, Allen et al. showed that combining the prebiotic fiber inulin with a calcimimetic further suppresses elevated bone turnover while improving cortical bone architecture and mechanical properties. These findings highlight the potential of gut-targeted interventions to complement established parathyroid hormone-lowering strategies for improving skeletal health.

Exacerbation of Pre-existing Hypothyroidism Following Enarodustat Administration in a Patient on Peritoneal Dialysis: A Case Report.

Hypoxia-inducible factor prolyl hydroxylase (HIF-PH) inhibitors are widely used to treat renal anemia in patients with chronic kidney disease (CKD). While roxadustat, an HIF-PH inhibitor, is known to induce central hypothyroidism via thyroid hormone receptor-beta agonism, thyroid dysfunction associated with other HIF-PH inhibitors, such as enarodustat, remains unrecognized. We report the case of a 32-year-old male on peritoneal dialysis (PD) with underlying subclinical mild primary hypothyroidism and erythropoiesis-stimulating agent (ESA)-resistant renal anemia secondary to primary hyperparath

Near-infrared autofluorescence versus visual identification during parathyroidectomy: a systematic review and meta-analysis of surgical outcomes.

INTRODUCTION: Near-infrared autofluorescence (NIRAF) is an emerging adjunctive tool that facilitates intraoperative identification of parathyroid glands. Although it has shown promise in the setting of thyroidectomy, its role in parathyroidectomy for primary hyperparathyroidism (PHPT) remains unclear. METHODS: A systematic review was conducted according to PRISMA guidelines. Four databases were searched to identify studies comparing NIRAF with conventional visual identification of parathyroid glands during parathyroidectomy. The primary outcome was cure rates and secondary outcomes included o

Partial attenuated PTH suppression of calcimimetics may be due to the resulting hypocalcemia or a significant decrease in serum calcium levels within the normal reference range without adequate calcium supplementation.

Certain dialysis patients with secondary hyperparathyroidism (SHPT) show partial insensitivity to calcimimetics. These agents are allosteric activators of the calcium-sensing receptor (CaSR) and enhance their sensitivity to extracellular calcium, thereby inhibiting parathyroid hormone (PTH) secretion and eliciting a decrease in serum calcium levels. We hypothesize that the partial attenuated PTH suppression of calcimimetics may be due to the resulting hypocalcemia or a significant decrease in serum calcium levels within the normal range without adequate calcium supplementation. Effective suppr

Nutritional and Metabolic Trajectories up to 10 Years After Metabolic and Bariatric Surgery: A Tertiary-Centre Retrospective Longitudinal Cohort Study.

BACKGROUND/OBJECTIVES: Metabolic and bariatric surgery (MBS) is the most effective durable treatment for severe obesity but predisposes to micronutrient deficiency. Long-term, broad-panel biochemical data are scarce. We examined nutritional and metabolic trajectories over up to 10 years and compared sleeve gastrectomy (SG) with Roux-en-Y gastric bypass (RYGB). METHODS: Retrospective single-centre cohort undergoing MBS (NHS Lothian, 2011-2025). Twenty-five biochemical markers were captured at baseline, 6 months, 1, 5 and 10 years, and analysed using linear mixed-effects models adjusted for pro

Robot-Assisted Radical Prostatectomy in Kidney Transplant Recipients: an Updated Systematic Review and Surgical Perspective>.

OBJECTIVES: Persistent hyperparathyroidism and calcitriol deficiency coexist in most long -term kidney transplant recipients, yet the combined skeletal effects of these presentations have not been formally quantified. MATERIALS AND METHODS: In this cross -sectional analytical study at People's Hospital 115 (Ho Chi Minh City, Vietnam ), we studied 135 kidney transplant recipients (mean age 51.0 ± 11.3 years; 66.7 % male; mean time posttransplant 71.4 ± 28.1 months ) who underwent simultaneous measurement of serum intact parathyroid hormone, 1,25 -dihydroxyvitamin D, and dual -energy x -ray abs

Revisiting Cinacalcet HCl Hepatic Disposition: Insights from Isolated Perfused Rat Liver.

INTRODUCTION: Hepatic drug handling determines systemic exposure, yet the balance between tissue uptake, intracellular binding, and metabolic clearance is difficult to dissect in vivo. Cinacalcet hydrochloride (CINA), a calcimimetic used to manage secondary hyperparathyroidism, has low oral bioavailability and is widely described as a high-extraction drug subject to extensive hepatic metabolism. Whether tissue sequestration rather than biotransformation underlies its rapid hepatic removal has not been tested directly in an intact ex vivo liver model. METHODS: We investigated the hepatic clear

Assessment of health literacy across types of hyperparathyroidism.

BACKGROUND: Health literacy is the ability to obtain, process, and understand basic health information necessary to make informed decisions. Low health literacy has been associated with delays in diagnosis and treatment, reduced adherence, and worse clinical outcomes. This study aimed to assess health literacy among individuals with hyperparathyroidism. METHODS: The National Institutes of Health All of Us Research database was used to evaluate health literacy in adults with primary, renal secondary, nonrenal secondary, and tertiary hyperparathyroidism. Health literacy was assessed using 3 que

Vitamin D-induced hypercalcemia and nephrolithiasis in a 58-year-old woman with biochemical findings suggestive of 24-hydroxylase deficiency: a case report.

BACKGROUND: Hypercalcemia is most often caused by primary hyperparathyroidism and malignancies. Non-parathyroid hormone (PTH)-dependent hypercalcemia can be caused by abnormal vitamin D metabolism related to excess production from granulomatous disease, excess intake or defects in its degradation. Disturbance in vitamin D metabolism can lead to increased vitamin D sensitivity which can cause hypervitaminosis D at doses otherwise considered safe. Decreased 24-hydroxylase activity due to CYP24A1 mutation leads to increase in 1,25-dihydroxyvitamin D [1,25(OH)2D] and decreases in vitamin D degrada

Preventive strategies for postoperative hypocalcemia and hungry bone syndrome following parathyroidectomy: a systematic review and pairwise meta-analysis.

BACKGROUND: Postoperative hypocalcemia and hungry bone syndrome (HBS) remain clinically important after parathyroidectomy, particularly in renal hyperparathyroidism. Preventive drug regimens, calcium-infusion protocols, dialysis-related measures, and bundled pathways are used, but direct head-to-head comparisons between active strategies are largely absent. We therefore mapped the available strategies and determined which clinically comparable direct comparisons could support pairwise meta-analysis. METHODS: The review was registered in PROSPERO (CRD420261373430). PubMed/MEDLINE, Embase, Web

Real-World Etelcalcetide Therapy for Secondary Hyperparathyroidism in Hemodialysis Patients with Inadequate Cinacalcet Response or Intolerance: Prospective Observational Study.

BACKGROUND: Cinacalcet is widely used for secondary hyperparathyroidism in hemodialysis (HD) patients. However, inadequate biochemical response, gastrointestinal intolerance, and pill burden may limit its use. Intravenous etelcalcetide offers an alternative in such cases. We aimed to provide real-world evidence on the effectiveness of etelcalcetide in reducing parathyroid hormone (PTH) levels and to assess the time to full response in HD patients with inadequate cinacalcet response or intolerance. METHODS: A prospective observational study was conducted among HD patients transitioned from cin

Beyond osteopoikilosis: a blended skeletal dysplasia associated with dual LEMD3 and ANO5 variants.

Osteopoikilosis is a rare autosomal dominant sclerosing bone dysplasia caused by pathogenic variants in LEMD3, which is typically asymptomatic and incidentally detected on radiographs. Gnathodiaphyseal dysplasia is an extremely rare skeletal disorder characterized by diaphyseal sclerosis, bone fragility, and pathological fractures. The coexistence of 2 genetically distinct skeletal dysplasias in a single individual is exceptionally uncommon. A 15-year-old female presented with progressive gait disturbance and a right hip intertrochanteric fragility fracture. Radiological evaluation demonstrate

[The 520th case: neck swelling, hypocalcemic convulsions, and spontaneous hemorrhage of a cystic parathyroid adenoma].

A 33-year-old man was admitted with a 4-day history of neck swelling and intermittent convulsions for 3 days. Physical examination revealed a soft, tender nodule measuring~4 cm×3 cm in the right lobe of the thyroid gland. Chvostek's and Trousseau's signs were positive. Laboratory tests revealed hypocalcemia, normal serum phosphorus and magnesium levels, elevated parathyroid hormone (PTH) levels, and reduced vitamin D levels. Bone turnover markers indicated active bone formation, and bone density testing revealed severe osteoporosis. Neck ultrasound and CT suggested a thyroid-origin mass; howev

Sagliker Syndrome: A Diagnostic Challenge.

Sagliker syndrome (SS), commonly referred to as "Uglifying human face Syndrome," is a rare but serious outcome of chronic kidney disease characterized by skeletal deformities, especially in the skull and jaws, due to uncontrolled secondary hyperparathyroidism (SHPT). The diagnosis is made through a combination of clinical symptoms, radiographic evidence (demonstrating bone deformities), and laboratory evaluations. Timely and effective treatment of SHPT is essential to prevent or lessen the onset of SS. Treatment includes managing phosphorus levels (through dietary changes, phosphate binders, a

The cross-cultural and chronological evolution of the kidney-bone relationship: from ancient medical insights to modern molecular endocrinology.

The kidney-bone axis constitutes a conserved bidirectional endocrine network that orchestrates skeletal remodeling and systemic mineral homeostasis. Its dysregulation underlies chronic kidney disease-mineral and bone disorder (CKD-MBD), renal osteodystrophy, secondary hyperparathyroidism, and associated vascular calcification, markedly elevating fracture and cardiovascular mortality risks. This review offers a unique cross-cultural and chronological synthesis of the kidney-bone relationship, bridging ancient medical insights with modern molecular endocrinology. Horizontally, we systematically
