Clinical significance of serum calcium-phosphorus product, parathyroid hormone, vitamin K2 in chronic kidney disease patients with coronary artery calcification.

Coronary artery calcification (CAC) is a major cardiovascular complication in chronic kidney disease (CKD) driven by disrupted calcium-phosphorus metabolism, secondary hyperparathyroidism, and vitamin K2 deficiency. Although CAC predicts cardiovascular events, the roles of calcium-phosphorus product (Ca × P), parathyroid hormone (PTH), and vitamin K2 in CAC progression and diagnosis remain underexplored. This study examines clinical and biochemical factors linked to CAC in CKD and evaluates the diagnostic potential of these biomarkers individually and in combination. This retrospective analysi

Metabolic Bone Disease in Captive Flying Foxes: A Conceptual Framework and Future Perspectives.

In Pteropus spp., metabolic bone disease has been consistently associated with fruit-based diets that are deficient in calcium, vitamin D precursors, and protein, as well as limited ultraviolet B (UVB) exposure, as reported in zoological surveys and clinical observations. Comparative mammalian physiology suggests that dysregulation of the endocrine axis involving parathyroid hormone (PTH), fibroblast growth factor 23 (FGF23), calcitonin, and calcitriol may contribute to disease development, although direct species-specific endocrine data in flying foxes remain scarce. This narrative review syn

2024 International expert consensus on ultrasound-guided thermal ablation for secondary and tertiary hyperparathyroidism.

Treatments for secondary and tertiary hyperparathyroidism (SHPT/THPT) remain significant challenges in patients with end-stage renal disease. Thermal ablation (TA) has emerged as a minimally invasive, safe, and effective alternative to surgical resection (SR). However, technical variations and a lack of standardization have limited its widespread adoption. To address these challenges, an international expert panel developed consensus recommendations using a modified Delphi method, integrated with a systematic literature review. As a result, sixteen recommendations were formulated, addressing d

Naturally Occurring Secondary Renal Hyperparathyroidism and Fibrous Osteodystrophy in a Pet Rabbit (Oryctolagus cuniculus).

Rabbits with end-stage kidney failure often develop hypercalcaemia and osteosclerosis. Secondary renal hyperparathyroidism and fibrous osteodystrophy can be experimentally induced in rabbits through nephrectomy and a low-calcium:high-phosphorus diet, but clinical case reports are rare. A thin, mixed-breed, neutered male rescue rabbit was brought to a veterinary clinic due to lameness of the right hind limb and gait difficulties. Radiographs showed severe osteodystrophy. Additionally, there was a displaced, ununited simple fracture in the right mid-femur and a healed simple fracture in the righ

Bone Health and Anti-Osteoporotic Medication Eligibility in Postmenopausal Women Undergoing Bariatric Surgery: The Impact of Age and Modifiable Risk Factors.

INTRODUCTION: Postoperative complications after metabolic and bariatric surgery (MBS) increase with age, yet data on bone health in older postmenopausal women remain limited. The 2022 European Calcified Tissue Society (ECTS) position statement recommend anti-osteoporotic medication (AOM) for patients with a T-score ≤ −2 and/or a fragility fracture within the past two years. This study evaluated the prevalence of AOM eligibility according to age (< 60 vs. ≥ 60 years) in postmenopausal women and identified associated risk factors. MATERIALS AND METHODS: We conducted a cross-sectional, observatio

The calcimimetic etelcalcetide attenuates pressure overload-induced cardiac hypertrophy in rats with and without chronic kidney disease.

INTRODUCTION: Calcimimetics such as etelcalcetide (ET) are used to manage secondary hyperparathyroidism patients with chronic kidney disease (CKD) on dialysis. While their cardiovascular benefits-including left ventricular hypertrophy (LVH) suppression-are recognized, the underlying mechanisms remain unclear. This study investigated how ET suppresses LVH using rat models. METHODS: LVH was induced via transverse aortic coarctation, and CKD by 5/6 nephrectomy. Rats were assigned to the sham, CKD, LVH, or CKD/LVH groups, with each pathological group further divided into vehicle- or ET-treated su

Effects of graded phosphate deficiency and vitamin D intervention on growth, bone metabolism, and mineralization in a rat model of neonatal-onset metabolic bone disease.

To investigate the dose-dependent effects of neonatal-onset phosphate deficiency on bone growth and mineralization and assess whether supraphysiological vitamin D3 or calcitriol can rescue skeletal defects. Newborn Sprague-Dawley rats were randomly assigned to 7 diets: phosphate-free (0P), low phosphate (1/2P), normal phosphate (NP), calcium-free (0Ca), phosphate/vitamin D-free (0P/D), and 0P/D supplemented with either supraphysiological vitamin D3 (0P/D + D3) or calcitriol (0P/D + calcitriol). Longitudinal radiographic assessments were performed before euthanasia at 6-8 wk. Serum analyses mea

Diagnostic accuracy of elastography in differentiating parathyroid lesions from cervical tissues: A systematic review and meta-analysis.

BACKGROUND: Accurate identification of parathyroid lesions is essential for diagnosis and management, yet conventional imaging techniques, including ultrasound and ^99mTc-sestamibi scintigraphy, can be discordant with the reference standard. PURPOSE: To evaluate the diagnostic accuracy of elastography in differentiating parathyroid lesions from surrounding cervical tissues. METHODS: PubMed, Embase, and Scopus were searched through July 2025. Studies assessing elastography in patients with suspected or confirmed parathyroid lesions-including adenomas, hyperplasia, and carcinoma-and reporting

Exceptionally high lumbar spine DXA T-score in a nonagenarian: an artefact of degenerative spinal changes and vascular calcification.

Dual-energy X-ray absorptiometry (DXA) is the gold standard for diagnosing osteoporosis, but degenerative spinal changes and vascular calcifications can result in artefactual overestimation of bone mineral density (BMD) (3-5). Extreme elevations in T-scores in the absence of metabolic bone disease are exceptionally uncommon. We report a 95-year-old male with multiple chronic comorbidities who presented with poor oral intake and a urinary tract infection. DXA revealed a lumbar spine T-score of + 11.0. Laboratory and imaging evaluation showed no evidence of osteopetrosis, Paget's disease, second

[Switching patients from cinacalcet to etelcalcetide: evaluation of predictors of efficacy in a retrospective cohort study].

BACKGROUND: The prevalence of SHPT depends on control strategy and target level of PTH. There is insufficient data for a justified choice of goals and strategy for the SHPT correction. AIM: A retrospective, multicenter, cohort study was conducted to assess the ethelcalcetide efficacy after six months in real world practice. The study sought to identify whether MBD-CKD characteristics are predictive of achieving the goals. MATERIALS AND METHODS: A study included 302 patients in 20 dialysis units in St. Petersburg. The primary endpoint was the proportion of patients who reached the target of P

Correlation of carotid artery intima-media thickness with parameters of mineral bone disorder in patients with chronic kidney disease.

Background Chronic kidney disease (CKD) is associated with accelerated cardiovascular (CV) risk, particularly due to subclinical atherosclerosis. Carotid intima-media thickness (CIMT) is a non-invasive marker of early athero-sclerotic changes. Altered mineral metabolism in CKD, also known as CKD-mineral bone disorder (CKD-MBD), may contribute to vascular damage. We evaluated the association between CIMT and mineral metabolism markers in people with diabetes who had pre-dialysis CKD. Methods A cross-sectional observational study was done in 110 adults with diabetes and pre-dialysis CKD. Biochem

Prevalence of Hypercalcemia in Patients With Pulmonary Tuberculosis and Impact of Treatment.

Background and aim Hypercalcemia is a recognized metabolic complication in patients with pulmonary tuberculosis (TB), attributed to extrarenal synthesis of 1,25-dihydroxyvitamin D (1,25(OH)₂D or calcitriol) by activated macrophages within granulomatous lesions. Biochemical characterization, including parathyroid hormone (PTH) measurement, is essential for excluding alternative etiologies. Serum calcitriol measurement is critical for directly confirming the postulated mechanism. The aim of the study was to determine the prevalence, severity pattern, biochemical profile, including serum calcitri

Correlations of Parathormone and Biochemical Parameters in Chronic Kidney Disease.

INTRODUCTION: Hyperparathyroidism due to chronic kidney disease (CKD) is a common complication characterized by elevated parathyroid hormone (PTH) levels secondary to derangements in the homeostasis of calcium and phosphorus. The aim of our study was to figure out PTH, calcium, phosphorus, and magnesium levels in CKD and the possible correlations between them and the stages of CKD. MATERIALS AND METHODS: We performed a prospective study including 217 outpatients with levels of serum creatinine out of reference range from February 2023 to July 2024. We calculated the glomerular filtration rate

Parathyroid carcinoma as an overlooked etiology of osteoporosis in postmenopausal women: a case report.

Parathyroid carcinoma (PC), an extremely rare endocrine malignancy, disrupts calciumphosphorus homeostasis and lead to musculoskeletal system disorders including osteoporosis, bone pain and pathological fractures. For postmenopausal women, osteoporosis is a common disease. Therefore, secondary osteoporosis is often overlooked in this demographic. We report a 54-year-old woman presenting to Orthopedics Department due to arthralgia diagnosed with severe postmenopausal osteoporosis (PMO). After pharmacotherapy, the patient's symptoms did not show significant improvement. Subsequent endocrine eval

Drug-Induced Osteoporosis.

The administration of various medications can induce bone loss as an adverse effect and may result in drug-induced osteoporosis, an important and clinically relevant form of secondary osteoporosis associated with an increased fracture risk. This review summarizes the skeletal effects of selected commonly used drugs with respect to bone metabolism, bone mineral density, and fracture outcomes. Medications may exert direct effects on osteoblasts and/or osteoclasts, leading to impaired bone remodeling and reduced bone mass. Alternatively, indirect mechanisms may contribute to skeletal damage, incl

Quality of Life After Parathyroidectomy in Patients With End-Stage Renal Disease: A Systematic Review.

INTRODUCTION: Secondary hyperparathyroidism (SHPT) is a common complication of end-stage renal disease (ESRD), with parathyroid hormone (PTH) overproduction leading to symptoms such as bone pain, pruritus, and fatigue. These symptoms diminish the already compromised quality of life (QoL) in ESRD patients. Parathyroidectomy (PTX) is offered to those who fail medical management, but its association with QoL is not addressed in treatment guidelines. This systematic review examines QoL in SHPT patients after PTX. METHODS: A systematic review was conducted using Preferred Reporting Items for Syste
