[Infrequent application of intraoperative ultrasonography in urology].

Among the usual indications of intraoperative ultrasounds, we describe four infrequent applications that show how useful and powerful this technique could be for interventive urologists. The first case regards a 66 years old male who was affected by a renal metastasis from thoracic cage chondrosarcoma. The use of intraoperative ultrasounds permits to highlight atypical sonographic features of the secondary lesion that were not seen in preoperative radiologic exams and that are completely different from the usual renal lesions. The second case regards the treatment of prostatic abscess performe

PET and parathyroid L-[carbon-11]methionine accumulation in hyperparathyroidism.

UNLABELLED: The study was designed to characterize L-[methyl-11 C]methionine accumulation in abnormal parathyroid tissues of hyperparathyroidism (HPT). METHODS: Thirty-four patients with primary (n = 32) or secondary HPT were investigated with PET before primary or reoperative (n = 25) parathyroid surgery. Parathyroid 11C-methionine accumulation was analyzed for integrated uptake values in defined tissue volumes standardized for the injected dose and body weight (SUV), four contiguous pixels of maximal accumulation (SUVhs), SUV multiplied by area of region of interest (SUVr) and by the excise

Uremic leontiasis ossea: "bighead" disease in humans? Radiologic, clinical, and pathologic features.

PURPOSE: To describe the clinical, radiologic, and pathologic findings in patients with uremic leontiasis ossea (ULO). MATERIALS AND METHODS: Five patients with renal osteodystrophy developed marked hyperostosis of the facial and cranial bones. Radiologic studies included plain radiography of the skull (n = 5), computed tomography with three-dimensional reconstruction (n = 4), magnetic resonance imaging (n = 3), and fluorine-18 sodium fluoride positron emission tomography (PET) (n = 1). Specimens from bone biopsies (three patients) were examined. RESULTS: Skull and facial alterations were re

Fracturing osteoporosis after kidney transplantation--what are the options?

The patient who receives a renal graft does not have virgin bones. Renal transplantation must be performed in a patient with pre-existing bone disease, i.e. renal osteodystrophy, characterized by secondary hyperparathyroidism, calcitriol deficiency, phosphate retention, metabolic acidosis and possibly aluminium accumulation. These pathomechanisms are further modulated by factors such as duration of dialysis, type of dialysis, diet, control of serum phosphate, use phosphate-binding agents etc.

Effects of uremia on growth in children.

Growth failure is a major complication of uremia in infancy and childhood. The influence of the primary renal disease leading to uremic growth retardation in children, the contributing factors leading to growth failure, such as metabolic acidosis, renal osteodystrophy, hyperparathyroidism, nutrition-endocrine and developmental disorders, are reviewed to update nephrologists on the complex issue of growth failure in children with uremia. The collaboration between endocrinologists and nephrologists in treating children with growth retardation is highlighted by a recently completed National Insti

Predialysis calcitriol administration: effects on pre- and post-transplant renal osteodystrophy.

Twins with parallel loss of kidney function and moderate hyperparathyroid bone disease were participants in a double-blind study where twin A was given placebo and twin B calcitriol. After 8 months. A's bone disease had not improved, while B's bone had normalized. Thereafter, both received calcitriol until kidney transplantation 11 months later, when both had normal bone structure. Two years after transplantation, both twins had hyperparathyroid bone disease, but A had more pronounced changes. This report illustrates our findings in larger series: When started early in the course of renal fail

[Serum and urinary pyridinium cross-links in patients with predialysis chronic renal failure].

The concentrations of pyridinoline (Pyr), a novel marker of bone resorption, were simultaneously measured in both serum and urine samples by ion-paired high-performance liquid chromatography in 27 patients with predialysis chronic renal failure. The patients consisted of 19 males and 8 females, whose creatinine clearance (Ccr) ranged from 2.5 to 47.6 ml/min. The influence of residual renal function on serum Pyr and the clinical significance of serum and urinary Pyr were analyzed in patients with predialysis chronic renal failure. There were significant correlations between serum Pyr and serum

Case report: renal osteodystrophy in association with spinal stenosis in achondroplasia.

A 54 year old woman with achondroplasia presented with symptoms of spinal stenosis. Subsequent studies revealed end-stage renal disease with severe renal osteodystrophy that was characterised by radiographs and new bone biomarkers. We speculate that renal osteodystrophy accentuated spinal stenosis, which is a common feature of achondroplasia in adults.

Bone resorption and mRNA expression of IL-6 and IL-6 receptor in patients with renal osteodystrophy.

The cytokine interleukin-6 (IL-6) is a major cell regulatory factor that may play an important role in the bone remodeling of patients with renal failure. IL-6 exerts its action by binding to its receptor (IL-6R), which leads to transduction of a second messenger cascade within cells. In vitro as well as in vivo data point to IL-6 as an autocrine/paracrine factor in bone osteoclasts. Recently, bone cells from patients with Paget's disease were found to express IL-6 and IL-6R mRNA transcripts. However, in patients with renal bone disease, there is currently no in vivo evidence that osteoclasts

Does renal osteodystrophy develop and/or progress during the course of rhGH treatment?

Recent expansion of the use of recombinant growth hormone (GH) to non-GH-deficient patients demands close attention to possible complications in these patients, including the effects on bone. Recent studies on the use of GH in children with chronic renal failure (CRF) provide some early data. In several studies of GH in CRF no significant differences in radiographic osteodystrophy scores, serum calcium, phosphorus or parathyroid hormone (PTH) levels between treated and untreated groups have been found. Alkaline phosphatase increases transiently. The effect of ROD on growth response has not yet

Secondary hyperparathyroidism complicated by parathyromatosis.

Secondary hyperparathyroidism is a common complication of chronic renal disease. Clinical signs and symptoms tend to be severe and often are not controlled with medical measures. When medical therapy fails, parathyroidectomy becomes necessary. Recurrent hyperparathyroidism is not uncommon following surgery. One cause of surgical failure is parathyromatosis, which has been described as multiple nodules of hyperfunctioning parathyroid tissue scattered throughout the lower neck, superior mediastinum, or the arm if autotransplantation has been performed. Five cases of parathyromatosis in patients

[Alkaline phosphatase of bone origin in hemodialyzed patients. 110 assays].

OBJECTIVES: To evaluate the value of plasma bone-specific alkaline phosphatase (bAP) in the diagnosis and the prediction of the type of renal osteodystrophy in hemodialysis patients. To examine whether bAP correlated with the two classical biochemical markers of bone turnover in dialysis patients which are total alkaline phosphatases (tAP) and intact PTH (iPTH). METHODS: One hundred adult uremic patients undergoing regular hemodialysis therapy were included in the study. Plasma bAP was determined using a radioimmunometric assay. Intact PTH (1-84) was measured using a radioimmunometric assay,

[Management of renal osteopathy in patients with chronic dialysis].

During the past decade considerable progress has been made in the field of calcium metabolism associated with chronic renal failure. The main factor influencing the pathophysiology of calcium homeostasis is its dependence on parathyroid hormone and active vitamin D metabolites. Among several new diagnostic tools, and to establish the diagnosis of renal osteodystrophy, only the measurement of i-PTH activity has any clinical relevance. The main goal in treating renal osteodystrophy is the lowering of PTH activity by correcting hyperphosphatemia and administration of calcitriol given either orall

Bone loss of the radius in rheumatoid arthritis. Comparison between 34 patients and 40 controls.

We measured bone mineral density (BMD) in the radius by dual energy X-ray absorptiometry in 34 patients with rheumatoid arthritis (RA) and in 40 healthy controls. The BMD in RA patients in their fifties and sixties, but not in their forties and seventies, was significantly lower than that in the control subjects. The decrease in total radial BMD correlated with grip strength, RA activity and RA stage. The decrease in distal radial BMD correlated with RA activity, but not with grip strength. The levels of serum parathyroid hormone, alkaline phosphatase, and urinary hydroxyproline/creatinine wer

Age-related endocrine deficiencies and fractures of the proximal femur. II implications of vitamin D deficiency in the elderly.

In the United Kingdom, as many as 60% of institutionalized people who are not taking vitamin D supplements may be deficient. Both impaired mineralization and a hyperparathyroidism-related increase in bone turnover have been identified in the presence of vitamin D deficiency. Recent interventional data have confirmed the role of vitamin D deficiency in the pathogenesis of senile osteoporosis and indicated the need to maintain serum cholecalciferol levels within the normal range in elderly people.

Frequency of adynamic bone disease and aluminum storage in Italian uraemic patients--retrospective analysis of 1429 iliac crest biopsies.

BACKGROUND: Adynamic bone disease was initially attributed too aluminum intoxication in association with low circulating levels of parathyroid hormone. More recently adynamic bone disease has been described even in the absence of aluminum intoxication. PURPOSE OF THE STUDY: It was the purpose of this retrospective analysis of 1429 iliac crest biopsies sent to our laboratory from 1985 to 1994 by 41 Italian nephrology and dialysis centres to assess the frequency of adynamic bone disease and aluminum accumulation. METHODS: Adynamic bone disease was diagnosed by histological and histodynamic (te
