Tooth mobility revealing a brown tumor associated with normocalcemic hyperparathyroidism: case report and literature review.

BACKGROUND: Brown tumors are rare non-neoplastic bone lesions associated with hyperparathyroidism. They are uncommon in the jaws and are usually linked to primary or secondary hyperparathyroidism, while tertiary hyperparathyroidism is rarely reported, particularly in normocalcemic patients. This report aims to present a mandibular brown tumor in a patient with long-standing renal disease and normocalcemia, and to discuss the diagnostic challenges between secondary and tertiary hyperparathyroidism. CASE PRESENTATION: A 41-year-old male on chronic hemodialysis with a history of parathyroidectomy

Effect of Exercise on Parathyroid Hormone Levels in Dialysis Patients: A Systematic Review and Meta-Analysis.

INTRODUCTION: Exercise has been increasingly recognized as a safe and effective adjunct therapy for improving physiological and biochemical outcomes in dialysis patients. However, its specific impact on parathyroid hormone (PTH) regulation remains uncertain. This systematic review and meta-analysis aimed to determine the effect of exercise interventions on PTH levels in adults undergoing dialysis. METHODS: A systematic search of PubMed, Scopus, Embase, Web of Science, and Google Scholar databases was conducted up to October 31, 2025, without language or date restrictions. Eligible interventio

Hungry bone syndrome after parathyroidectomy in secondary and tertiary hyperparathyroidism: a retrospective cohort study.

PURPOSE: Hungry bone syndrome (HBS) is a common and critical postoperative complication in patients undergoing parathyroidectomy (PTX) for secondary hyperparathyroidism (SHPT) and tertiary hyperparathyroidism (THPT). We aimed to identify clinical predictors of HBS and assess its impact on bone mineral density (BMD) after PTX. METHODS: We retrospectively analyzed data of patients with SHPT and THPT who underwent PTX at Asan Medical Center (2010-2022). Clinical characteristics, including biochemical markers and BMD, were investigated. HBS was defined as profound hypocalcemia of less than 8.4 mg

The Endocrine Giants of Chronic Kidney Disease.

Chronic Kidney Disease (CKD) is increasingly being recognized as an endo-metabolic disorder in which a cascade of hormonal and electrolyte imbalances impacts the morbidity and mortality. We propose six "endocrine giants" of CKD- anaemia, secondary hyperparathyroidism, hypovitaminosis D, hypogonadism, hyperkalaemia, and hyponatraemia- each exerting a detrimental effect on cardiovascular, skeletal, metabolic, and neurological health. Their early recognition and prompt management can attenuate downstream cardiometabolic sequelae. This "endocrine giants" framework provides an integrative approach

Secondary Hyperparathyroidism in Post-Bariatric Population: A Descriptive Analysis.

BACKGROUND: Secondary hyperparathyroidism (SHPT) is a known metabolic disturbance following bariatric surgery (BS), often driven by alterations in calcium, phosphorus, and vitamin D metabolism. Despite its recognition, the prevalence of SHPT in the Middle East and Northern Africa region remain unclear. This study aims to describe the prevalence of SHPT in a cohort of post-bariatric surgery patients of the region. METHODS: We conducted a retrospective analysis of patients who underwent primary laparoscopic sleeve gastrectomy (LSG) or Roux-en-Y gastric bypass (RYGB) at our institution between Ja

Risk factors for adverse outcomes in first-time renal parathyroid surgery: an analysis of the United Kingdom Registry of Endocrine and Thyroid Surgeons.

BACKGROUND: Recommendations for surgery in patients with renal hyperparathyroidism lack evidence. This study analysed outcomes of first-time renal parathyroidectomy in United Kingdom Registry of Endocrine and Thyroid Surgery (UKRETS) to establish risk factors for adverse outcomes. METHODS: Data from 2005 to 2020 was analysed. Entries with missing data, age <15 and >85 years, and length of stay (LOS) >28 days were excluded. Outcome variables included temporary hypocalcaemia, persistent disease, re-operation for bleeding, general complications, readmission and LOS. Persistent disease was only a

Modern advances in parathyroid surgery: A survey of North American surgeon associations.

BACKGROUND: Contemporary clinical guidelines recommend the use of intraoperative parathyroid hormone (IOPTH) when treating parathyroid disease, yet the degree to which parathyroid surgeons adhere to this, and other guideline recommendations remains unknown. To evaluate parathyroid surgeon practices when managing primary (PHPT), secondary (SHPT), and tertiary (THPT) hyperparathyroidism. METHODS: A cross-sectional electronic survey characterizing parathyroid surgery practices was conducted according to CHEERS and AAPOR guidance and distributed to four North American surgeon societies from Augus

Band keratopathy and conjunctival calcification in end-stage kidney disease: epidemiology, pathophysiology and clinical management.

Conjunctival and corneal calcification, particularly its severe manifestation, band keratopathy, represents a prevalent yet frequently overlooked form of metastatic calcification in end-stage kidney disease (ESKD). This review integrates current evidence on the epidemiology, pathophysiology and interdisciplinary management of uraemic ocular calcification. The pathogenesis is conceptualized through a 'two-hit' model: the systemic 'first hit' involves the specific milieu of chronic kidney disease-mineral and bone disorder driven by hyperphosphataemia, calcium load, secondary hyperparathyroidism

The Janus face of parathyroid hormone in renal osteodystrophy.

This editorial comment discusses novel insights into renal osteodystrophy from a parallel publication, highlighting that uremia per se independently impairs bone cell viability and induces skeletal parathyroid hormone resistance. Using multimodal bone analyses, Wagner et al. show that parathyroid hormone exerts compartment-specific effects: protective for trabecular osteocytes yet detrimental to cortical integrity via increased porosity and by bone fibrosis and increased resorption. These findings redefine optimal parathyroid hormone targets and emphasize addressing uremic toxicity to reduce f

Brown Tumor in a Patient With End-Stage Renal Disease: A Case Report of Secondary Hyperparathyroidism Complications.

The development of a brown tumor is a rare complication of secondary hyperparathyroidism, particularly in patients with chronic kidney disease. Despite advances in the treatment and management of secondary hyperparathyroidism, these tumors can still occur. We report the case of a 29-year-old man with end-stage renal disease treated with hemodialysis, who developed a brown tumor in the ribs. Despite medical treatment including calcium and vitamin D supplementation, and multiple parathyroidectomies, imaging revealed an expansive and sclerotic costal lesion, confirmed as a brown tumor. Diagnosis

X-linked hypophosphatemia in the presence of a CLDN16 variant: implications for renal handling and disease severity.

X-linked hypophosphatemia (XLH), a rare genetic disorder caused by PHEX mutations, leads to fibroblast growth factor 23-mediated phosphate wasting, hypophosphatemia, and impaired bone mineralization. We report a 28-year-old woman with XLH carrying a heterozygous PHEX c.1080-1G>A splice-site mutation and a CLDN16 c.165_166delinsC mutation. Diagnosed at 18 months of age, she received long-term phosphate and active vitamin D metabolites, which resulted in secondary hyperparathyroidism, vertebral fractures, and medullary nephrocalcinosis requiring subtotal parathyroidectomy. Although CLDN16 mutati

Endocrine disorders linked to chronic kidney disease: Mechanisms and clinical implications.

Chronic kidney disease (CKD) is a progressive condition that disrupts multiple endocrine axes, contributing to a broad range of hormonal imbalances, including mineral and bone disorder, gonadal dysfunction, thyroid abnormalities, insulin resistance, and elevated prolactin levels. These disturbances not only exacerbate comorbidities such as cardiovascular disease, anemia, diabetes, and sarcopenia, but may also accelerate the decline of renal function. This article aims to provide a comprehensive overview of the pathophysiology, clinical impact, and management strategies for endocrine disorders

Exploring multiple drug actions of a Ca2+-sensing receptor modulator: perspectives in alternative applications of cinacalcet.

The Ca2+-sensing receptor (CaSR), a G-protein-coupled receptor activated by raised extracellular Ca2+ concentration, has been well studied for its role in regulating parathyroid hormone (PTH) secretion. As a calcimimetic and positive allosteric modulator of CaSR, cinacalcet activates parathyroid cell CaSR by increasing the latter's sensitivity to Ca2+, causing a reduction in PTH secretion and thus serum Ca2+ level. Cinacalcet is clinically used to treat secondary hyperparathyroidism in chronic kidney disease patients and hypercalcemia in parathyroid cancer patients. Recently, cinacalcet has be

Intraoperative Parathyroid Hormone in Minimally Invasive Parathyroidectomy (MIP) for Primary Hyperparathyroidism With Concordant Localisation.

OBJECTIVE: There is increasing use of intraoperative parathyroid hormone (ioPTH) in minimally invasive parathyroidectomy (MIP). While ioPTH has improved cure rate, there is little evidence to suggest effectiveness in primary hyperparathyroidism (PHP) with concordant preoperative localisation. This study aims to determine if ioPTH improves cure rates in such cases. DATA SOURCES: A search of PubMed, Embase, and Cochrane databases identified studies that compared MIP with and without ioPTH, in patients with concordant preoperative imaging of ultrasonography and technetium-99m sestamibi (MIBI) sc

Differences in Endocrine Manifestations in HIV-Infected Subjects on Anti-Retroviral Therapy (ART) Versus not on ART.

INTRODUCTION: Few studies have examined endocrine dysfunction associated with anti-retroviral therapy (ART) for human immunodeficiency virus (HIV) infection. The aim is to assess various endocrine changes, including vitamin D metabolism and glycaemic status, of subjects on ART versus not on ART (non-ART) and to assess the relationship with CD4 + T helper cell count. METHODS: Ninety-three HIV-positive subjects were selected and underwent thyroid function tests (thyroid stimulating hormone and free thyroxine). Fasting plasma glucose, glycated haemoglobin, Cortisol, ACTH, total testosterone (mal

Tissue-specific adaptive responses of the vitamin D3 auto-/paracrine system to vitamin D3 deficiency in bone and brain.

OBJECTIVE: We hypothesized that vitamin D3 (VD3) coordinates a complex functional interplay between cerebral homeostasis and skeletal integrity by modulating mineral metabolism and osteotropic signaling. This neuroskeletal axis is likely mediated via a feedback loop involving fibroblast growth factor 23 (FGF23), a key regulator within both osseous and cerebral tissues. In this context, our study aimed to characterize tissue-specific adaptive responses by analyzing correlations between components of the vitamin D3 system and FGF23 signaling under physiological and VD3-deficient conditions.
