Giant Parathyroid Adenoma Masked by Severe Vitamin D Deficiency with Hypocalcemic Presentation: A Case Report.

INTRODUCTION: Giant parathyroid adenomas, defined as lesions weighing at least 3.5 grams, are rare and typically present with hypercalcemia due to excessive parathyroid hormone secretion. Severe vitamin D deficiency can alter this pattern by lowering serum calcium, which may obscure the diagnosis of primary hyperparathyroidism, especially in younger patients. This case is notable because a large parathyroid adenoma presented with hypocalcemia rather than hypercalcemia, creating an atypical biochemical profile that initially mimicked secondary hyperparathyroidism. CASE PRESENTATION: A 30-year-

Association between the Use of Calcimimetics and Vitamin D Receptor Activators and Fracture Risk in Patients Undergoing Maintenance Hemodialysis: A Cohort Study.

INTRODUCTION: Secondary hyperparathyroidism is a major complication of maintenance hemodialysis (MHD). Elevated serum parathyroid hormone levels increase fracture risk. We aimed to investigate the association between secondary hyperparathyroidism medication and fracture risk in patients undergoing MHD. METHODS: Patient data from a large medical claims database were used. Patients receiving MHD were divided into untreated, vitamin D receptor activator (VDRA), and calcimimetic groups. The outcome was the first hospitalization due to any fracture. First hospitalization for hip or vertebral fract

TRPV6-related intrauterine calciopenic rickets: a case report and literature review.

SUMMARY: We report a female Chinese neonate with antenatal rickets and secondary hyperparathyroidism who was found to have novel compound heterozygous TRPV6 variants (c.1160G>A and c.658C>T). She was treated with calcium, phosphate and vitamin D supplementation, achieving full biochemical and radiographic recovery by age two years. The review of all 14 reported cases confirms a consistent phenotype of neonatal secondary hyperparathyroidism with universal parathyroid hormone (PTH) elevation, frequent fractures and respiratory distress. Excellent outcomes are achieved with supplementation. The c

Accelerated bone loss increases osteoporosis and fracture risk in moderate to severe chronic kidney disease.

Chronic kidney disease-mineral and bone disorder (CKD-MBD) contributes to bone loss and fractures. Evidence on longitudinal changes in bone mineral density (BMD) in CKD remains limited. This retrospective cohort study evaluated the longitudinal decline in BMD across levels of kidney function and examined the associations between kidney function and the risk of osteoporosis and fractures. Patients with estimated glomerular filtration rate (eGFR) < 60 mL/min/1.73 m2 who underwent dual-energy X-ray absorptiometry between 2010 and 2023 were included. Associations between kidney function, longitudi

Diagnostic agreement of bone histomorphometry for renal bone disease.

INTRODUCTION: An iliac bone biopsy with semi-quantitative histomorphometry is the gold standard for evaluating renal osteodystrophy (ROD). However, lack of standardization of the bone biopsy procedure, sample handling, reading and reporting represent barriers to research synthesis and clinical collaboration. This study assessed diagnostic agreement across European laboratories performing histomorphometry for evaluation of ROD. METHODS: Five-millimeter transiliac biopsy slides from 5 cases were converted to digital images for analysis by experts from 7 laboratories. Parameters included quantit

Comparison of the effects of non-calcium and calcium-based phosphate binders on fibroblast growth factor-23 levels in patients with CKD-mineral and bone disorder: a meta-analysis of randomized controlled trials.

BACKGROUND: Phosphate binders are used to manage hyperphosphatemia in patients with chronic kidney disease-mineral and bone disorder (CKD-MBD) and may influence fibroblast growth factor 23 (FGF-23), a biomarker associated with disease progression and mortality. However, the comparative effects of calcium-based phosphate binders (CPBs) and non-calcium-based phosphate binders (NCPBs) on FGF-23 levels remain unclear. We conducted a meta-analysis of randomized controlled trials to compare the effects of NCPBs and CPBs on FGF-23 levels in patients with CKD-MBD with hyperphosphatemia. METHODS: A sys

Parathyroidectomy timing for persistent hyperparathyroidism in kidney transplant candidates and recipients: from secondary to tertiary disease: a literature review.

BACKGROUND AND OBJECTIVE: Secondary hyperparathyroidism (SHPT) in patients with chronic kidney disease (CKD) is a major challenge in the context of patients awaiting renal transplantation. Optimal timing of parathyroidectomy (PTX) for these patients remains controversial, particularly to prevent tertiary hyperparathyroidism (THPT) and minimize postoperative complications such as hungry bone syndrome (HBS). The aim of this study is to evaluate current evidence on optimal timing of PTX in renal transplant candidates. METHODS: To review the current evidence regarding the optimal timing of PTX in

Effect of dialysate buffer practices on serum parathyroid hormone concentrations in real-life french patients receiving hemodialysis.

BACKGROUND: Dialysate composition is crucial for managing secondary hyperparathyroidism (SHPT) in patients on hemodialysis. Acetate-based dialysates elevate serum acetate and trigger adverse effects, while citrate and hydrochloric acid formulations might offer distinct clinical benefits. METHODS: In this longitudinal observational real-life study, 876 adult patients on hemodialysis were classified by acid component of the dialysate (acetic acid, citric acid, or hydrochloric acid). Monthly pre-dialysis parathyroid hormone (PTH) and bone-mineral biomarkers were measured. A linear mixed model (L

Clinical and Biochemical Correlates of Parathyroid Gland Burden in Patients Undergoing Parathyroidectomy for Secondary Hyperparathyroidism: A Retrospective Observational Study.

Background/Objectives: Secondary hyperparathyroidism (SHPT) is a common and clinically significant complication of advanced chronic kidney disease and may require surgical intervention when medical therapy fails. This study aimed to evaluate the association between parathyroid gland burden, defined by gland number and size parameters, and biochemical disease severity in patients undergoing parathyroidectomy for SHPT, and to assess the concordance between preoperative imaging findings and intraoperative observations. Although parathyroid gland enlargement is a hallmark of SHPT, the clinical rel

Construction of a CKD-MBD osteoporosis animal model using adenine combined with high-phosphorus diet at different time intervals and gradient concentrations.

Recent advancements have led to the establishment of various animal models for chronic kidney disease-mineral and bone disorder (CKD-MBD), including 5/6 nephrectomy and drug induction. However, these models predominantly focus on kidney and vascular pathology, often neglecting skeletal lesions and exhibiting high mortality rates and unclear osteoporosis. Phosphorus plays a crucial role in bone metabolism, and prolonged high phosphorus intake can disrupt normal bone function. Our study introduces a novel CKD-MBD osteoporosis model using mice fed with adenine at three concentrations, combined wi

High prevalence of sarcopenia, osteoporosis and osteosarcopenia in patients with chronic kidney disease compared with healthy controls.

UNLABELLED: Brief rationale: The impact of non-dialysis chronic kidney disease (CKD) on combined muscle and bone decline remains insufficiently described. MAIN RESULT: Patients with CKD showed markedly higher rates of sarcopenia, osteoporosis, and osteosarcopenia than matched healthy controls. Significance of the paper: These findings support systematic evaluation of muscle and bone health in CKD management. PURPOSE: Chronic kidney disease (CKD) negatively affects bone and muscle quality. The combination of sarcopenia and osteoporosis, osteosarcopenia, is associated with an increased risk of

Biomarkers of phosphate homeostasis and metabolic bone disease of prematurity: impact of early nutrition and regulation by vitamin D, iPTH, PTHrP, and FGF23.

BACKGROUND: Global practices for supplementing calcium, phosphate, and vitamin D in early life, as well as for screening for metabolic bone disease of prematurity (MBDP), remain inconsistent. Furthermore, researchers have not yet established gestational age (GA)-specific reference intervals for phosphate homeostasis biomarkers. This study aimed to assess phosphate homeostasis parameters and MBDP risk factors under local neonatal nutritional practices and to characterize the serial levels of key hormones involved in phosphate metabolism, including 1,25-dihydroxyvitamin D (1,25(OH)2D), intact pa
