Fine-needle-based localization techniques in parathyroid adenomas: a retrospective cohort study of histopathological safety and fibrosis patterns.

BACKGROUND: This study aimed to evaluate the association between fine-needle-based interventions widely used for preoperative and intraoperative localization of parathyroid adenomas and development of post-excision stromal fibrosis and other histopathological changes that may mimic malignancy. METHODS: A total of 164 patients who underwent parathyroidectomy for primary hyperparathyroidism were retrospectively evaluated. The patients were divided into three groups according to the number of ultrasound-guided fine-needle interventions: needle-naive (Group 0, n = 70), single-needle (Group 1, n =

Parathyroid Carcinoma: A Report of Two Cases.

Parathyroid carcinoma is a rare cause of hypercalcaemia, which can be potentially difficult to diagnose and treat. Since its clinical, biochemical, and radiological features overlap with those of parathyroid adenoma, diagnosis is often made only after histological review of the resected tissue. We report two cases managed at a United Kingdom district general hospital. The first case was of a 51-year-old man referred with persistent hoarseness and an incidental parathyroid lesion on imaging, in whom progressive hypercalcaemia led to excision and a histological diagnosis of parathyroid carcinoma

Severe Hypocalcemia and Hypophosphatemia Following Ferric Carboxymaltose in a Patient With Roux-en-Y Gastric Bypass.

Iron deficiency anemia is commonly treated with intravenous iron when oral therapy is ineffective, poorly tolerated, or inadequately absorbed. Ferric carboxymaltose (FCM) is an effective intravenous iron formulation but is increasingly recognized to cause hypophosphatemia through fibroblast growth factor 23 (FGF23)-mediated renal phosphate wasting. Although FCM-induced hypophosphatemia is well described, concurrent clinically significant hypocalcemia is less commonly reported. We present the case of a 39-year-old woman with a history of Roux-en-Y gastric bypass, iron deficiency anemia, and vit

Analysis of the clinical profile of multiple endocrine neoplasia syndromes and the association with other tumors.

OBJECTIVE: To examine the clinical profile of patients with MEN syndromes and possible associations with tumors less commonly identified in these conditions. METHODS: We present data from a chart review of 182 patients from our MEN registry. We identified 106 patients with MEN1 and 76 with MEN2 defined by clinical criteria. 45 patients with MEN1 and 69 with MEN2 were genetically confirmed cases. RESULTS: Among all patients with clinical MEN1, commonly associated tumors included parathyroid (89.6%), pituitary (61.3%), and gastroenteropancreatic neuroendocrine tumors (65.1%). Other tumors incl

Giant Cell Tumor of the Distal Ulna in an Elderly Patient on Hemodialysis: Diagnostic Challenge and Surgical Management.

Giant cell tumor of bone is a locally aggressive tumor that usually affects the epiphyseal region of long bones in young adults. Involvement of the distal ulna is rare, and the diagnosis in elderly patients with end-stage chronic kidney disease may be difficult because brown tumors related to secondary hyperparathyroidism can mimic giant cell tumor radiologically and histologically. We report the case of a 69-year-old woman with end-stage chronic kidney disease on hemodialysis who was admitted for a painful and progressively enlarging mass on the ulnar border of the left wrist that had been pr

The Economics of Intraoperative Parathyroid Hormone Monitoring for Parathyroid Surgery: A Scoping Review of Cost Analyses and Economic Evaluations.

OBJECTIVE: Intraoperative PTH (IOPTH) monitoring is recommended for primary hyperparathyroidism (PHPT), but questions surround its added clinical value and financial cost. We evaluated the methodological quality of economic evaluations investigating IOPTH monitoring in surgery for PHPT. METHODS: We searched several databases, including MEDLINE and EMBASE, from January 1990 to June 2026. All screening was performed in duplicate. We included studies that conducted economic evaluations of IOPTH monitoring. Risk of bias (ROB) and methodological quality were evaluated in duplicate using ECOBIAS an

Persistent Neonatal Hypocalcemia as an Early Clue to TCIRG1-Related Infantile Osteopetrosis in an Infant of a Diabetic Mother: A Case Report.

Infantile malignant osteopetrosis is a rare autosomal recessive skeletal disorder caused by impaired osteoclast-mediated bone resorption, resulting in diffuse osteosclerosis, bone marrow failure, neurologic complications, and metabolic disturbances. Early diagnosis remains challenging, particularly when initial manifestations overlap with common neonatal conditions. Persistent neonatal hypocalcemia may be an early but underrecognized clue. We report a Saudi male infant with genetically confirmed TCIRG1-associated osteopetrosis who presented with persistent neonatal hypocalcemia and secondary h

A novel ultrasound-based nomogram using contrast-enhanced and conventional ultrasound features to improve preoperative diagnosis of parathyroid adenomas versus cervical lymph nodes.

OBJECTIVES: Precise preoperative localization of parathyroid gland lesion is essential for guiding surgery in primary hyperparathyroidism (PHPT). The aim of our study was to investigate the contrast-enhanced ultrasound (CEUS) characteristics of parathyroid gland adenoma (PGA) and to evaluate whether PGA can be differentiated from central cervical lymph nodes (CCLN). METHODS: Fifty-four consecutive patients with PHPT were retrospectively enrolled and underwent preoperative imaging with high-resolution ultrasound (US) and CEUS, and underwent subsequent parathyroidectomy. Traumatic Brain Injury (TBI) could lead to intracranial hemorrhage (ICH), which has now been identified as a major cause of death after trauma if it is not adequately diagnosed and properly treated within the first 24 hours. CT examination is widely preferred for urgent ICH diagnosis, which enables the fast identification and detection of ICH regions. However, the use of it requires the clinical interpretation by experts to identify the subtypes of ICH. Besides, it is unable to provide the details needed to conduct quantitative assessment, such as the volume and thickness of hemorrhagic lesion

Radius Bone Mineral Density Measurement Is Essential During Initial PHPT Workup: Results of a Retrospective Study on 400 Patients.

BACKGROUND: Bone mineral density (BMD) measurement at the lumbar spine, femoral neck, total hip, and distal 1/3 radius is recommended for patients with primary hyperparathyroidism (PHPT). Osteoporosis at any site justifies surgery. The distal 1/3 radius is often neglected. This study evaluated the prevalence of distal 1/3 radius measurement, the proportion of osteoporosis at this site alone, and BMD changes 1 year post-parathyroidectomy in patients with PHPT. METHODS: A total of 548 patients who underwent parathyroidectomy for PHPT between 2016 and 2024 in University Hospital Center of Nantes

Millifluidic ion-selective optodes with integrated optical detection for colorimetric quantification of urinary calcium.

Measurement of urinary calcium concentration is essential for the diagnosis and management of various disorders related to calcium metabolism, including kidney stones, hypoparathyroidism, and hyperparathyroidism. Current testing protocols require patients to visit clinical laboratories or hospitals, and no at-home devices are currently approved for quantitative self-testing of urinary calcium. In this work, we report a low-cost and user-friendly colorimetric sensor for urinary calcium measurement. The sensor consists of a segment of sensing oil contained within a millifluidic channel, such as

H3-3A gene mutation analysis in giant cell tumor of bone and its histologic mimics: A single institutional study from India.

Though the diagnostic utility of H3.3G34W immunohistochemistry (IHC) for giant cell tumor of bone (GCTB) is well established, it is limited by a proportion of cases with variant H3-3A mutations [H3-3A: c.104G>T p. Nephrocalcinosis: unveiling renal tubulopathies in the genomic era.

INTRODUCTION: The diagnosis of nephrocalcinosis (NC) is challenging due to difficulties in radiologic detection, clinical heterogeneity, and its broad etiological spectrum that includes both genetic and non-genetic causes. This study aimed to uncover, in a pioneering manner, the distinct NC etiologies in a Brazilian cohort according to age of onset. METHODS: This retrospective cross-sectional study was based on 96 medical records of outpatients (66 adults and 30 children) with NC, who had followed a comprehensive investigation. RESULTS: Distal renal tubular acidosis was the leading cause of

[18F]Fluorocholine PET/CT in a 15-year-old patient suggested HPT-JT syndrome with active cemento-ossifying fibroma.

BACKGROUND: Hyperparathyroidism-jaw tumor (HPT-JT) syndrome is a rare autosomal dominant disorder caused by CDC73 gene mutations, predisposing individuals to primary hyperparathyroidism (pHPT), cemento-ossifying fibromas, and other neoplastic conditions. [18F]Fluorocholine PET/CT has emerged as a tool for localizing hyperfunctioning parathyroid glands in pHPT, but its application in HPT-JT syndrome remains unreported. CASE PRESENTATION: We describe the case of a 15-year-old male presenting with severe hypercalcemia, increased PTH serum levels, and a history of cemento-ossifying fibroma remova

Persistent Hypercalcemia: Diagnostic Complexity With Multiglandular Hyperparathyroidism, Renal Cell Carcinoma, and Hereditary Tumor Features.

Persistent hypercalcemia after parathyroidectomy is a diagnostic challenge that requires careful evaluation beyond the parathyroid glands. We describe the case of a 76-year-old man with chronic kidney disease, resistant hypertension, and vestibular schwannoma who presented with weakness, constipation, and new, dark, raised lesions across his back. He reported undergoing parathyroidectomy one year earlier for presumed primary hyperparathyroidism, after which his hypercalcemia persisted. On admission, serum calcium was 13.1 mg/dL (reference = 8.5-10.5 mg/dL), and examination revealed numerous se

Unraveling the mechanistic links between blood pressure regulation and calcium-magnesium homeostasis: Insights into hypertension, hyperparathyroidism, and mineral disorders.

The systems regulating blood pressure and calcium-magnesium (Ca2+-Mg2+) homeostasis are increasingly recognized to have clinically relevant interactions, where alterations in one can lead to significant changes in the other. In this study, we developed a computational model integrating blood pressure regulation and Ca2+-Mg2+ homeostasis in a male rat. We simulated various conditions, including hypertension, Ca2+, Mg2+, and vitamin D3 deficiencies, and primary hyperparathyroidism. Simulations of hypertension, induced by various stimuli like increased renin or aldosterone secretion, demonstrated

Injection method alters diagnostic confidence and quantitative metrics in [18F]fluorocholine PET/CT for parathyroid adenoma localization.

BACKGROUND: Studies evaluating [18F]fluorocholine PET/CT for parathyroid adenoma localisation fail to report the injection method used. At our department, manual injection method (MIM) was initially used, before converting to an automated injection system (AIS). Afterwards, physicians observed increased retention of the radiotracer within the veins leading from the injection site, which had not been as present previously. The aim of this study was to evaluate whether the injection method had a clinical impact. METHODS: The study compared two groups consisting of 60 patients, where the injectio

A Systematic Review of Evidence, Misinterpretations, and the Urgent Need for Population-Specific Reference Standards Related to Vitamin D Deficiency in India: A Global Myth Imposed Locally?

India reports very high biochemical vitamin D deficiency when global cut-offs are applied, yet the corresponding disease burden appears low. Whether current thresholds are appropriate for Indian populations, therefore, remains uncertain. In this review, we aimed to systematically analyze the literature on vitamin D and non-skeletal outcomes and critically evaluate whether current deficiency thresholds are appropriate for India. We searched PubMed, Scopus, and the Cochrane databases (Jan 1, 2010, to Feb 29, 2024), focusing on randomized trials (RCTs), meta-analyses, and observational studies ad
