Utility of albumin-adjusted calcium and calcium‒phosphorus difference in screening for primary hyperparathyroidism.

PURPOSE: Primary hyperparathyroidism (PHPT) is often asymptomatic but increases the risk of osteoporosis, fractures, and cardiovascular mortality. We evaluated the effectiveness of albumin-adjusted calcium (aCa) formulas and the calcium–phosphorus (Ca–P) difference as prescreening tools for PHPT in health checkup populations. METHODS: We analyzed 5550 corresponding measurements of total calcium (tCa), serum albumin (Alb), and ionized calcium (iCa) to evaluate the consistency between iCa and tCa and between iCa and three aCa formulas (Payne, KDOQI-2, and James). Receiver operating characteristi

Serum Biomarker-Based Diagnostic Tools for Primary Hyperparathyroidism: A Systematic Review and Meta-Analysis with Implications for Primary Care.

Background: Hyperparathyroidism is a common endocrine disorder, and its diagnosis can be complex. Various indices based on blood biomarkers have been proposed to improve diagnostic accuracy. The objective of this systematic review was to analyze the diagnostic utility of different indices in primary hyperparathyroidism. Methods: A systematic review was performed with searches up to January 2026. Risk of bias was assessed, and a meta-analysis was conducted for indices with two or more studies, calculating sensitivity, specificity, and other accuracy measures. The certainty of the evidence was e

Recurrent Hyperparathyroidism Due to Hyperactive Ectopic Intrathymic Parathyroid Gland 20 Years After Parathyroidectomy.

Primary hyperparathyroidism is most commonly caused by a single adenoma, with multiglandular disease accounting for a smaller proportion of cases. A recurrent hyperparathyroidism decades after prior parathyroidectomy is uncommon and often presents with nonspecific multisystem symptoms that delay diagnosis. Ectopic parathyroid tissue, particularly in intrathymic or retrosternal locations, represents a challenging but important cause of persistent or recurrent disease. A 43‑year‑old woman with a history of three‑gland parathyroidectomy 20 years earlier presented with a year of progressive fatigu

Endocrine hypertension: the role of imaging in diagnosis and management.

Endocrine hypertension is an uncommon but treatable cause of secondary hypertension. It results from excessive hormone production by the endocrine glands or due to ectopic hormone production. The causes of abnormal hormonal production can be congenital or acquired. Specific syndromes can also predispose to the development of endocrine hypertension. Extensive catecholamine production can occur due to pheochromocytomas and paragangliomas (PPGLs). Excessive aldosterone secretion by the adrenal cortex commonly occurs due to idiopathic (bilateral) adrenal hyperplasia or aldosterone-producing adrena

Beyond Atrial Fibrillation: A Case of Thyrotoxicosis-Associated Ischemic Stroke in a Young Patient.

While it is presumed that thyrotoxicosis is a contributing factor for cardioembolic stroke secondary to atrial fibrillation (AF), cerebrovascular events in patients without cardiac arrhythmias remain exceptionally rare. We present the case of a 41-year-old African female patient with no traditional stroke risk factors who presented with sudden-onset left hemiplegia and dizziness (National Institutes of Health Stroke Scale (NIHSS) 9). Imaging revealed a right middle cerebral artery (MCA) territory infarct with hemorrhagic transformation and malignant edema (midline shift 3 mm), necessitating em

Intraoperative near-infrared autofluorescence in parathyroidectomy: associations with gland morphology, body mass index, and histopathology.

BACKGROUND: Identifying parathyroid glands during surgery is challenging. Recent studies suggest that near-infrared autofluorescence (NIRAF) detection can improve their recognition. However, the correlation between positive NIRAF detection and clinical features in parathyroidectomy remains unclear. This study aimed to explore correlation between positive NIRAF detection and clinical features in parathyroid surgery, aiming to improve the application of this technology. METHODS: Prospective data were collected from patients undergoing parathyroidectomy using handheld NIRAF devices. Data include

Modern advances in parathyroid surgery: A survey of North American surgeon associations.

BACKGROUND: Contemporary clinical guidelines recommend the use of intraoperative parathyroid hormone (IOPTH) when treating parathyroid disease, yet the degree to which parathyroid surgeons adhere to this, and other guideline recommendations remains unknown. To evaluate parathyroid surgeon practices when managing primary (PHPT), secondary (SHPT), and tertiary (THPT) hyperparathyroidism. METHODS: A cross-sectional electronic survey characterizing parathyroid surgery practices was conducted according to CHEERS and AAPOR guidance and distributed to four North American surgeon societies from Augus

Brown Tumor in a Patient With End-Stage Renal Disease: A Case Report of Secondary Hyperparathyroidism Complications.

The development of a brown tumor is a rare complication of secondary hyperparathyroidism, particularly in patients with chronic kidney disease. Despite advances in the treatment and management of secondary hyperparathyroidism, these tumors can still occur. We report the case of a 29-year-old man with end-stage renal disease treated with hemodialysis, who developed a brown tumor in the ribs. Despite medical treatment including calcium and vitamin D supplementation, and multiple parathyroidectomies, imaging revealed an expansive and sclerotic costal lesion, confirmed as a brown tumor. Diagnosis

Exploring multiple drug actions of a Ca2+-sensing receptor modulator: perspectives in alternative applications of cinacalcet.

The Ca2+-sensing receptor (CaSR), a G-protein-coupled receptor activated by raised extracellular Ca2+ concentration, has been well studied for its role in regulating parathyroid hormone (PTH) secretion. As a calcimimetic and positive allosteric modulator of CaSR, cinacalcet activates parathyroid cell CaSR by increasing the latter's sensitivity to Ca2+, causing a reduction in PTH secretion and thus serum Ca2+ level. Cinacalcet is clinically used to treat secondary hyperparathyroidism in chronic kidney disease patients and hypercalcemia in parathyroid cancer patients. Recently, cinacalcet has be

Papillary Thyroid Microcarcinoma: Diagnostic and Histopathological Insights From a Case Report.

The coexistence of primary hyperparathyroidism and thyroid nodular disease is frequently encountered in clinical practice, and the simultaneous presence of primary hyperparathyroidism and papillary thyroid microcarcinoma is clinically significant and may pose diagnostic and therapeutic challenges. A 63-year-old postmenopausal woman presented with a short history of painless left-sided neck swelling. Initial ultrasonography revealed a bulky left thyroid lobe with a dominant isoechoic nodule, and fine-needle aspiration cytology (FNAC) categorised the lesion as Bethesda Category IV, suspicious fo

Autofluorescence can aid in differentiating normal from pathologic parathyroid glands during parathyroidectomy.

BACKGROUND: Several studies suggest that during parathyroidectomy near-infrared autofluorescence may be able to help differentiate hyperfunctional from normal parathyroid glands. However, to date, no clear metric for employing autofluorescence in this manner in real-time during surgery has been described. This study investigated whether a specific measure of autofluorescence heterogeneity can aid discrimination of pathologic from normal parathyroid glands. METHODS: In patients undergoing curative parathyroidectomy for classic or normohormonal primary hyperparathyroidism, a near-infrared autof

Beyond Painful Bones and Renal Stones: Acute Crystal Arthropathy of Dual Aetiology May Be an Unforeseen Complication Following Parathyroid Surgery.

Primary hyperparathyroidism (PHPT), one of the most common endocrine conditions, has myriad manifestations, including nephrolithiasis, osteoporosis, neuropsychiatric complications and fatigue. The treatment of choice is focused parathyroidectomy when the lesion is localised. Common complications following parathyroidectomy include hypoparathyroidism and hungry bone syndrome. Acute crystal arthropathy due to gout and pseudogout following parathyroid surgery is scarcely reported. We report the case of a 47-year-old man who developed painful swelling of bilateral ankle joints following parathyroi

Preoperative imaging strategies to optimize surgical treatment in primary hyperparathyroidism (pHPT).

PURPOSE: Accurate preoperative localization of parathyroid adenomas is essential to enable targeted and minimally invasive surgery in patients with primary hyperparathyroidism (pHPT). A variety of imaging techniques are available for this purpose. In routine clinical practice, cervical ultrasound (US) combined with scintigraphy is most commonly used as the initial diagnostic approach. However, the optimal strategy or combination of imaging modalities for reliable localization prior to surgery and its relevance for postoperative outcomes remains a matter of ongoing discussion. The aim of this s

Textbook outcome in surgery for primary hyperparathyroidism: first steps toward a composite quality metric.

Textbook Outcome (TO) is a composite quality metric reflecting the ideal postoperative course through the simultaneous achievement of multiple desirable endpoints. Although widely explored in other surgical specialties, its application in endocrine surgery remains limited, and has not been specifically assessed in primary parathyroidectomy for sporadic primary hyperparathyroidism. A retrospective cohort study was conducted including 100 consecutive patients who underwent parathyroidectomy for sporadic primary hyperparathyroidism (PHPT) between January 2019 and February 2023 at a tertiary endoc

Parathyroid Hormone Related Peptide (PTHrP) Revisited: A Comparison of Levels in Malignant and Non-malignant Conditions.

Elevated parathyroid hormone-related peptide (PTHrP) levels are often presumed to indicate malignancy-associated hypercalcemia, yet PTHrP may be elevated in nonmalignant conditions. We compared PTHrP levels in patients with known malignancies with patients with elevations of PTHrP who had defined nonmalignant diagnoses to determine which levels of PTHrP were more indicative of malignancy. A retrospective review of electronic medical records in a large midwestern hospital system was conducted using search terms hypercalcemia with elevated PTHrP (> 2.3 pmol/L (men) and > 3.4 pmol/L (women). Accelerated bone loss increases osteoporosis and fracture risk in moderate to severe chronic kidney disease.

Chronic kidney disease-mineral and bone disorder (CKD-MBD) contributes to bone loss and fractures. Evidence on longitudinal changes in bone mineral density (BMD) in CKD remains limited. This retrospective cohort study evaluated the longitudinal decline in BMD across levels of kidney function and examined the associations between kidney function and the risk of osteoporosis and fractures. Patients with estimated glomerular filtration rate (eGFR) < 60 mL/min/1.73 m2 who underwent dual-energy X-ray absorptiometry between 2010 and 2023 were included.
