Familial hypocalciuric hypercalcemia (FHH) is a rare autosomal dominant disorder of calcium homeostasis characterized by mild hypercalcemia, non-suppressed parathyroid hormone (PTH), and hypocalciuria. FHH type 2 (FHH2), caused by mutations in the GNA11 gene, is particularly rare. Differentiating FHH from primary hyperparathyroidism (PHPT) is crucial to avoid unnecessary surgical intervention. We report a 63-year-old male with persistent hypercalcemia and inappropriately normal PTH levels. Biochemical evaluation revealed marked hypocalciuria with a fractional excretion of calcium of 0.0039. Im

BACKGROUND: Distinguishing benign from high-risk parathyroid lesions [atypical parathyroid tumor (APT) and parathyroid carcinoma (PC)] preoperatively is crucial for surgical strategy, yet remains difficult in tertiary referral centers confronting an enriched risk profile. This study sought to develop and validate a practical nomogram for individualized risk quantification in this specific clinical context.
METHODS: We conducted a single-center, retrospective cohort study of patients with biochemically active solitary parathyroid lesions who underwent surgery between January 2009 and May 2024

BACKGROUND: Ectopic mediastinal parathyroid adenomas are a rare cause of persistent primary hyperparathyroidism and present operative challenges due to proximity to major mediastinal structures. Robotic trans-subxiphoid resection using multi-port platforms has been described; however, experience with a true uniportal single-port robotic approach remains limited.
METHODS: We report a case series of two patients with biochemically confirmed primary hyperparathyroidism and imaging-localized anterior mediastinal parathyroid adenomas who underwent excision using a single-port subxiphoid robotic te

BACKGROUND AND OBJECTIVE: Secondary hyperparathyroidism (SHPT) in patients with chronic kidney disease (CKD) is a major challenge in the context of patients awaiting renal transplantation. Optimal timing of parathyroidectomy (PTX) for these patients remains controversial, particularly to prevent tertiary hyperparathyroidism (THPT) and minimize postoperative complications such as hungry bone syndrome (HBS). The aim of this study is to evaluate current evidence on optimal timing of PTX in renal transplant candidates.
METHODS: To review the current evidence regarding the optimal timing of PTX in

Central giant cell granuloma (CGCG) is an uncommon benign intraosseous lesion of the jaws that typically affects the anterior mandible in young females. Aggressive variants may demonstrate rapid growth, cortical expansion, pain, root resorption, and recurrence. This report presents an unusual case of aggressive CGCG involving the posterior maxilla in a 20-year-old female patient who presented with progressive swelling and facial asymmetry. Radiographic evaluation revealed a well-defined expansile lytic lesion involving the right maxilla and maxillary sinus. Differential diagnoses included aden

BACKGROUND: Dialysate composition is crucial for managing secondary hyperparathyroidism (SHPT) in patients on hemodialysis. Acetate-based dialysates elevate serum acetate and trigger adverse effects, while citrate and hydrochloric acid formulations might offer distinct clinical benefits.
METHODS: In this longitudinal observational real-life study, 876 adult patients on hemodialysis were classified by acid component of the dialysate (acetic acid, citric acid, or hydrochloric acid). Monthly pre-dialysis parathyroid hormone (PTH) and bone-mineral biomarkers were measured. A linear mixed model (L

BACKGROUND: Primary hyperparathyroidism (PHPT) is associated with neurological damage, which tends to improve after parathyroidectomy (PTX). Oxidative stress might play a role in these pathological changes. This study aimed to investigate oxidative stress and neuronal damage markers following PTX in patients with PHPT.
METHODS: The study involved 36 patients with PHPT who underwent surgical treatment. Markers for oxidative stress, including serum total antioxidant status (TAS), total oxidative status (TOS), oxidative stress index (OSI), 7-ketocholesterol (7-KC) and cholestan-3β,5α,6β-triol (C

An 18-year-old man presented with hypercalcemia and suspected primary hyperparathyroidism. Initial complete thyroid ultrasound revealed no significant abnormalities. However, ectopic parathyroid hyperfunction could not be excluded. Subsequent 18 F-fluoromethylcholine ( 18 F-FCH) PET/CT demonstrated a focus of increased tracer uptake in the anterior superior mediastinum, suggestive of an ectopic parathyroid adenoma. Guided by imaging localization, the patient underwent minimally invasive excision. Postoperative pathologic biopsy confirmed the lesion as an ectopic parathyroid adenoma. This case

Tumoral calcinosis is a rare pathology characterized by extraarticular soft tissue calcium deposition, commonly near large joints. Tumoral calcinosis may be idiopathic or associated with underlying disorders, such as end-stage renal disease (ESRD), sarcoidosis, and hyperparathyroidism. The cervical spine is a rare location for tumoral calcinosis, with only 11 cases of uremic tumoral calcinosis previously reported in the literature. We report a case of cervical tumoral calcinosis in a 46-year-old man with end-stage renal disease on peritoneal dialysis. He presented with unilateral extremity wea

INTRODUCTION AND IMPORTANCE: Preservation of the parathyroid glands during thyroid surgery is crucial to prevent postoperative hypocalcemia. Parathyroid glands exhibit intrinsic autofluorescence with an emission peak around 820 nm, and near-infrared imaging has been applied for their intraoperative identification. However, its feasibility in endoscopic thyroid surgery remains unclear, particularly when using a small-diameter endoscope. Here, we report two cases in which parathyroid autofluorescence was successfully visualized during endoscopic surgery using a 5-mm endoscope.

The widespread adoption of prostate-specific membrane antigen (PSMA)-targeted PET/CT has profoundly reshaped molecular imaging in prostate cancer and has substantially reduced the routine use of radiolabeled choline tracers. However, the transition from choline to PSMA imaging should not be interpreted simply as the replacement of one radiopharmaceutical by another, but rather as part of a broader evolution from metabolism-based imaging toward receptor-targeted and biology-driven imaging strategies. This narrative review critically reassesses the residual and emerging role of choline PET/CT in

BACKGROUND/AIMS: Multiple epidemiological studies in different ethnic groups demonstrate diverse results about the prevalence of chondrocalcinosis (CC), yet no definitive data are available concerning its frequency in most countries. Furthermore, the prevalence of CC in the Turkish population is not known to date. The aim of this study was to investigate the frequency of radiographic CC in the knee, wrist, or both joints in individuals aged ≥50 years from the hospital records of the last 5 years.
MATERIALS AND METHODS: In this retrospective study, radiographs of the knee and/or wrist taken at

This is a classic and complete representation of the multiple endocrine neoplasia type 2A (MEN2A) syndrome, involving primary hyperparathyroidism, medullary thyroid carcinoma, and pheochromocytomas. This case is particularly noteworthy for the recurrent, contralateral presentation of the pheochromocytoma, which emerged years after the initial surgical management. Recurrent and contralateral pheochromocytomas present unique anesthetic, surgical, and postoperative problems due to excess catecholamines and severe, potentially life-threatening hemodynamic instability in the perioperative phase. Hypercalcemia is commonly caused by hyperparathyroidism and malignancy. Medication-induced hypercalcemia is less common. Tirzepatide is a dual glucagon-like peptide-1/glucose-dependent insulinotropic polypeptide receptor agonist (GLP-1/GIP RA) used to aid weight loss and glycemic control. It is associated with gastrointestinal side effects, but its effect on calcium homeostasis has not been well reported. We report the case of a 75-year-old woman with a history of type 2 diabetes, hypertension, obesity, chronic kidney disease, and post-surgical hypothyroidism and hypoparathyroidism, who presen

Hyperparathyroidism is a common cause of persistent hypercalcemia and is typically caused by a single parathyroid adenoma. Prompt treatment of hyperparathyroidism is essential to minimize impacts on various body systems such as the skeletal, renal, and cardiovascular systems. Hyperparathyroidism is typically cured by surgical removal of the adenoma, but persistent disease may occur in the presence of multiglandular hyperplasia or remaining adenoma. Further contributing to persistent disease are ectopic parathyroid glands. In this case a 51-year-old male was incidentally found to have hypercalc

PURPOSE OF REVIEW: We summarize and discuss evidence on management of chronic hypoparathyroidism (HypoPT) and primary hyperparathyroidism (PHPT) in pregnancy and lactation to provide guidance for clinical care.
RECENT FINDINGS: Women with HypoPT are at increased risk of certain pregnancy complications, though most pregnancies are uncomplicated. Conventional therapy with activated vitamin D and calcium is continued throughout pregnancy, but there are unpredictable changes in the required dosages warranting regular surveillance during pregnancy (e.g., all 3 to 4 weeks). Data on parathyroid horm
