Circulating α-Klotho and Multidimensional Aging and Frailty Outcomes: A Systematic Review and Meta-Analysis from the European Renal Association CKD-MBD Working Group.

Although α-Klotho has gained attention as a promising biomarker of aging, its association with frailty and broader aging-related outcomes beyond chronic kidney disease-mineral and bone disorder remains incompletely characterized. This systematic review and meta-analysis aimed to investigate the association between circulating α-Klotho levels and aging-related outcomes, including frailty, effects of physical activity and exercise interventions, body composition, cognitive and neuropsychiatric status, sarcopenia, and bone mineral density (BMD). A comprehensive literature search was performed acr

Eldecalcitol in older adults with kidney vulnerability: a narrative review of risk-based use, dose adjustment, and monitoring.

Eldecalcitol is an active vitamin D analogue used for osteoporosis in Japan. Its usual dose is 0.75 μg/day, but calcium-related adverse effects can become clinically important when reduced kidney reserve, frailty, low body weight, poor intake, dehydration risk, interacting medicines, or unreliable follow-up coexist. This narrative review synthesizes trial, post-marketing, pharmacovigilance, and real-world monitoring evidence to define a practical safety framework. Eldecalcitol reduces vertebral fractures compared with alfacalcidol, whereas hip-fracture protection has not been established; ther

Cost-effectiveness of preoperative parathyroid imaging in primary hyperparathyroidism: A synthesis of eight health-economic analyses.

PURPOSE: Preoperative localisation of hyperfunctioning parathyroid tissue underpins minimally invasive parathyroidectomy (MIP) in primary hyperparathyroidism (pHPT). Cervical ultrasound (cUS), [⁹⁹ᵐTc]Tc-sestamibi ([⁹⁹ᵐTc]Tc-MIBI) single-photon emission computed tomography/computed tomography (SPECT/CT), and choline positron emission tomography/computed tomography (PET/CT) differ in diagnostic accuracy and cost. Whether choline PET/CT should be reserved for second-line use or implemented first-line remains debated. We synthesised cost-effectiveness evidence from recent health-economic analyses

Systemic versus local regulation of vitamin D.

PURPOSE OF REVIEW: New clinical data and technical advances challenge the long-held belief that the kidney is the predominant organ for vitamin D synthesis and degradation. This review highlights recent discoveries in systemic and local vitamin D regulation and their potential to influence future development of novel treatment concepts. RECENT FINDINGS: New clinical studies of calcifediol supplementation in anephric patients and in chronic kidney disease provide evidence for significant extra-renal vitamin D production. The clinical findings are supported by recent laboratory-based studies th

Multisystem Sarcoidosis Presenting as Severe Hypercalcemic Crisis and Generalized Lymphadenopathy Mimicking Hematological Malignancy: A Case Report.

Sarcoidosis is a multisystem granulomatous disorder of unknown etiology characterized histologically by the formation of non-caseating granulomas in various organs and tissues. Typically, sarcoidosis is recognized by features such as dry cough, fever, erythema nodosum, and bilateral hilar lymphadenopathy. However, when patients present with severe hypercalcemia and generalized lymphadenopathy, sarcoidosis is usually overlooked, and we usually investigate for features of malignancy, bone metastasis, lymphoma, multiple myeloma, and primary hyperparathyroidism. We report the case of a 61-year-old

The impact of near-infrared autofluorescence and indocyanine green fluorescence on surgical management of primary hyperparathyroidism.

Surgery constitutes the only curative treatment of primary hyperparathyroidism (PHPT). The difficulty in localizing all hyperfunctioning parathyroid glands can lead to the persistence of the disease and require a surgical reintervention. Complementary intraoperative tools for the localization of hyperfunctioning parathyroid glands remain, however, limited. The objective of this review is to evaluate the role of near-infrared autofluorescence (NIRAF) and fluorescence with indocyanine green (ICG) injection in the surgery of PHPT. NIRAF allows real-time visualization of the parathyroid glands wit

Vitamin D Supplementation in Primary Hyperparathyroidism: Analysis of Benefits and Hazards.

Primary hyperparathyroidism (PHPT) and vitamin D deficiency (VD) are two frequent medical conditions in the Western world. Purely by dint of their prevalence, these conditions are commonly comorbid. Moreover, they can interact with one another: VD may stimulate PTH secretion, while PHPT may lead to unregulated vitamin D activation from 25-hydroxyvitamin D (25OHD) to 1,25-dihydroxyvitamin D (1,25OH2D), thereby lowering 25OHD levels and further exacerbating VD. Thus, a real conundrum arises: should the careful clinician replete VD in PHPT or not? On the one hand, both conditions could lead to bo

Treatment modalities for primary hyperparathyroidism.

Treatment modalities for primary hyperparathyroidism must take account of the expected benefits and risks of each treatment envisaged, before choosing the definitive option to be proposed to the patient. In this section, a Foreword puts in perspective the difficulties involved in choosing the criteria for a particular treatment method. Treatments are then considered one after the other: surgery, local destruction and medical management. This section does not consider therapeutic indications.

Familial Hypocalciuric Hypercalcemia as a Misnomer: Limitations of 24-Hour Urine Calcium and Calcium:Creatinine Clearance Ratios for Identifying Germline Mutations in Patients with Primary Hyperparathyroidism.

BACKGROUND: Familial hypocalciuric hypercalcemia (FHH) can mimic primary hyperparathyroidism (PHPT), yet commonly used 24-hour urine calcium (UCa) and calcium:creatinine clearance ratio (CCCR) thresholds may not reliably identify patients with FHH before parathyroidectomy. STUDY DESIGN: We retrospectively reviewed a prospectively maintained single-institution database of adults undergoing initial or reoperative parathyroidectomy for presumed PHPT from January 2012 through December 2023. Patients required concurrent 24-hour urine calcium and creatinine measurements and serum calcium and creati

Severe Prolonged Hypocalcemia After Denosumab Administration in a Patient With Sleeve Gastrectomy: A Case Report.

Denosumab is a human monoclonal antibody that inhibits RANKL, thereby suppressing osteoclast-mediated bone resorption and increasing bone mass in patients with osteoporosis. Although generally well tolerated, denosumab is associated with hypocalcemia, particularly in individuals with impaired calcium and vitamin D homeostasis. We report the case of a 40-year-old woman with a history of sleeve gastrectomy who developed severe, symptomatic hypocalcemia following a single dose of denosumab despite receiving vitamin D supplementation. She presented with neuromuscular symptoms, QTc prolongation, pr

Calcium Disorders in Rheumatic Diseases: An Overlooked Problem With Major Clinical Implications.

Rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), spondyloarthritis, calcium pyrophosphate deposition disease, and the therapies used to treat them, particularly disease-modifying antirheumatic drugs, exemplify clinical contexts where calcium homeostasis has underappreciated relevance. Calcium balance is central to skeletal integrity, immune signalling, and multisystem function in rheumatic diseases, yet it is frequently overshadowed by inflammatory priorities. Despite growing evidence linking calcium disorders to morbidity, integration into routine rheumatology care remains limit

Comparison of Ultrasonographic and Computed Tomographic Features of Parathyroid Glands in Canine Primary Hyperparathyroidism.

A multicenter retrospective study compared ultrasonographic (US) and computed tomographic (CT) characteristics of parathyroid glands in 20 dogs with histopathological confirmation of primary hyperparathyroidism. Twenty-four glands were diseased (17 adenomas and 7 hyperplastic glands). Two blinded radiologists assessed US and CT studies in two readings. A similar total number of glands was visualized (US 59 and CT 63). Intermodality agreement for identification of the same individual glands was slight for presumed normal glands but perfect for diseased glands, with 23 of 24 diseased glands visu

Recurrent Primary Hyperparathyroidism Due to Adenoma Formation in Autotransplanted Parathyroid Tissue 28 Years After Parathyroidectomy in a Patient With Multiple Endocrine Neoplasia Type 2A.

Primary hyperparathyroidism (PHPT) is an established manifestation of multiple endocrine neoplasia type 2A (MEN2A). In patients requiring extensive parathyroid surgery, autotransplantation of parathyroid tissue may be performed to preserve parathyroid function. Recurrence due to adenoma formation within autotransplanted parathyroid tissue is exceptionally rare. We report a case of a 59-year-old male with genetically confirmed MEN2A caused by a heterozygous RET p.Cys634Tyr (C634Y) mutation who developed recurrent PHPT caused by adenoma formation within autotransplanted parathyroid tissue 28 yea

Ipsilateral Double Parathyroid Adenomas Causing Primary Hyperparathyroidism: A Rare Case.

Double parathyroid adenomas represent an uncommon but well-recognized subset of primary hyperparathyroidism. Awareness of this possibility is essential, as failure to identify multigland involvement preoperatively may result in intraoperative uncertainty, incomplete resection, and persistent disease. Despite advances in localization techniques, the sensitivity of commonly used imaging modalities - including ultrasonography, technetium-99m sestamibi scintigraphy, and four-dimensional computed tomography - remains limited in detecting double adenomas. We present the case of a patient with primar

Renal osteodystrophy in pediatric patients. A systematic review and meta-analysis.

Chronic kidney disease (CKD) in children is associated with significant comorbidities, including mineral and bone disorder (CKD-MBD). A key manifestation of CKD-MBD is renal osteodystrophy (ROD), diagnosed by bone biopsy followed by histomorphometric analysis. In pediatric patients, ROD can lead to bone deformities and impaired growth. However, the limited use of bone biopsy in this population hampers the understanding of ROD patterns and severity. This systematic review and meta-analysis aimed to evaluate histomorphometric findings in children with CKD. We screened 5082 abstracts published up

Etiological Spectrum, Clinical Characteristics, and Six-Month Treatment Outcomes of Refractory Rickets in Children: A Prospective Observational Cohort Study From Eastern India.

BACKGROUND: Refractory rickets comprises a heterogeneous group of inherited and acquired disorders characterized by persistent clinical, biochemical, and radiological evidence of rickets despite adequate vitamin D and calcium therapy. Owing to limited data from Eastern India, this study aimed to characterize the etiological spectrum of refractory rickets, compare the clinical, biochemical, and radiological characteristics across different etiologies, and evaluate treatment outcomes following six months of etiology-specific therapy. METHODS: This hospital-based prospective observational cohort

Acidic extracellular pH-induced PTH secretion in male mouse parathyroids is OGR1-dependent.

The role of extracellular acidity in regulating PTH secretion in cultured mouse parathyroid glands (PTGs) has not been studied to date, largely because of the technical difficulty of isolating mouse PTGs. We hypothesized that acidic extracellular pH directly stimulates PTH secretion through activation of a proton-sensing receptor, specifically ovarian cancer G protein-coupled receptor 1 (OGR1, also known as GPR68). To test this, we developed a method to reliably identify and isolate PTGs from male mice by administering 5-aminolevulinic acid, which induced selective fluorescence in these glands

Significantly elevated calcium levels and gallstone disease in a patient with familial hypocalciuric hypercalcaemia.

SUMMARY: Familial hypocalciuric hypercalcaemia (FHH) is a rare, benign genetic disorder of calcium homeostasis caused by inactivating variants in CaSR, GNA11, or AP2S1 genes. It is typically characterised by mild, asymptomatic hypercalcaemia and low urinary calcium excretion. Differentiation from primary hyperparathyroidism (PHPT) is essential, as misdiagnosis can lead to unnecessary surgery. We report a case of FHH type 1 (FHH1) in a man in his thirties presenting with a serum calcium level of 2.97 mmol/L, which is at the severe end of the FHH spectrum. Genetic testing revealed a heterozygous

Syndromic primary hyperparathyroidism.

Syndromic primary hyperparathyroidism has several features in common: younger age at diagnosis when compared with sporadic primary hyperparathyroidism, often synchronous or metachronous multi-glandular involvement, higher possibility of recurrence, association with other endocrine or extra-endocrine disorders, and suggestive family background with autosomal dominant inheritance. Hyperparathyroidism in multiple endocrine neoplasia type 1 is the most common syndromic hyperparathyroidism. It is often asymptomatic in adolescents and young adults, but may be responsible for recurrent lithiasis and/

Endocrine-related osteoporosis: the state of the art.

BACKGROUND: Endocrine-related secondary osteoporosis (ERSOP) comprises disorders in which bone fragility results from hormonal abnormalities other than age-related or menopausal hypogonadism. Its prevalence is underestimated, and timely recognition is essential because many forms improve or reverse when the underlying endocrine disturbance is corrected. OBJECTIVE: This Review highlights the evolving concepts, unmet needs, and research opportunities in ERSOP, with focus on hypercortisolism, primary aldosteronism, male hypogonadism, hypercalciuria, and FGF23-dependent phosphate-wasting disorder
