[Parapharyngeal salivary gland oncocytoma showing intense radiopharmaceutical accumulation identified during the evaluation of primary hyperparathyroidism by scintigraphy accompanied with SPECT/CT and its transoral endoscope-assisted removal].

Technetium-99m-MIBI (methoxy-isobutyl-isonitrile) is a radiopharmaceutical agent used in the evaluation of myocardial perfusion and parathyroid pathologies. MIBI scintigraphy is a commonly applied diagnostic method in the assessment of parathyroid adenomas for lesion localization and to exclude the presence of ectopic adenomas. However, tracer uptake may also occur in thyroid adenomas, thyroid carcinomas, and other lesions with high mitochondrial density. In our study, we present the case of a 79-year-old female patient. The patient had been under endocrinological care since 1997 due to osteop

Age-specific differences in parathyroidectomy after implementation of guidelines for primary hyperparathyroidism.

BACKGROUND: Despite meeting guideline-based criteria for surgery, older adults with primary hyperparathyroidism (PHPT) undergo parathyroidectomy at lower rates. We aimed to evaluate age-specific differences in PHPT diagnosis, time to surgery, and surgical indications among patients offered parathyroidectomy. METHODS: Retrospective cohort study of adults with sporadic PHPT who underwent parathyroidectomy between 1/1/2017 and 12/31/2022 at a tertiary academic center was performed. Surgical indications were determined using the 2016 American Association of Endocrine Surgeons (AAES) and Fourth In

Reevaluation of urinary calcium excretion as a criterion for surgery in primary hyperparathyroidism: a study on nephrolithiasis risk.

The guideline for primary hyperparathyroidism (PHPT) recommends surgery for female patients with 24-hour urinary calcium levels >250 mg/day and male patients >300 mg/day, due to their risk for nephrolithiasis. The purpose of this study is to validate this criterion. We analyzed data from 595 patients undergoing surgery for PHPT. We investigated whether high urinary calcium excretion is associated with nephrolithiasis in PHPT patients, along with the presence of other factors. Nephrolithiasis was present in 224 patients and absent in 371. The 24-hour urinary calcium levels did not differ signif

Rare RET Variants in a Patient With MEN2A and Multiple Follicular-Derived Thyroid Tumors: A Case Report and Review of the Literature.

Multiple endocrine neoplasia (MEN) syndromes are a group of rare autosomal dominant hereditary disorders that are classified into 5 distinct types. MEN2A, the most common type of MEN2, is associated with mutations and variants of RET, which are continually being updated. However, studies regarding RET mutations/variants related to MEN2A in the Chinese population are scarce, and patients involving MEN2A along with other differentiated thyroid tumors are rarer. We report a 54-year-old Chinese woman diagnosed with MEN2A and multiple follicular-derived thyroid tumors. Ultrasonography and computed

Severe Hypercalcemia Following Pembrolizumab Therapy: A Case Report and A Literature Review.

INTRODUCTION/BACKGROUND: Immune checkpoint inhibitors (ICIs) play a central role in advanced cancer treatment, but they are associated with immune-related adverse events (irAEs) that include two cases of hypercalcemia induced by the programmed death-1 (PD-1) inhibitor, pembrolizumab. We report a unique case of a colon cancer patient treated with pembrolizumab who acutely developed life-threatening hypercalcemia. CASE PRESENTATION: This case presents a seventy-five-year-old woman suffering from colon adenocarcinoma with liver metastasis undergoing pembrolizumab therapy. Shortly after its secon

A novel homozygous inactivating mutation of the calcium-sensing receptor causing familial hypocalciuric hypercalcemia complicated with primary hyperparathyroidism due to parathyroid adenoma: A case report.

Familial hypocalciuric hypercalcemia is an autosomal dominant genetic disorder characterized by mild to moderate hypercalcemia, mild hypermagnesemia, and normal or inappropriately elevated parathyroid hormone levels. Familial hypocalciuric hypercalcemia 1 is the most prevalent form of familial hypocalciuric hypercalcemia and is typically caused by heterozygous loss-of-function mutations in the calcium-sensing receptor (CaSR) gene. Homozygous CaSR mutations are more commonly associated with neonatal severe primary hyperparathyroidism. We report the case of a female patient in her early 40s harb

Use and Implications of the Fracture Risk Assessment Tool in Primary Hyperparathyroidism.

IMPORTANCE: Although primary hyperparathyroidism (PHPT) increases the risk of osteoporotic fractures, estimation of fracture probability among patients with PHPT remains challenging. Use of the Fracture Risk Assessment Tool (FRAX) among these patients remains poorly characterized, and its utility in identifying patients who may experience fracture risk reduction following parathyroidectomy (PTX) remains unknown. OBJECTIVE: To examine the calibration of FRAX in adults with PHPT and to describe the association between estimated probability and differential fracture risk with PTX compared with n

A Simple Blood Count Marker for a Complex Diagnosis: Neutrophil-to-Lymphocyte Ratio in Atypical Parathyroid Adenomas.

INTRODUCTION: Preoperative differentiation of atypical parathyroid adenomas (APAs) from typical parathyroid adenomas (TPAs) is challenging because of their overlapping histopathological features. Simple biomarkers, such as the neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR), have been associated with tumor aggressiveness in various endocrine tumors; however, their role in APA remains unclear. METHODS: This retrospective cohort study included 149 patients who underwent parathyroidectomy for primary hyperparathyroidism between July 2020 and May 2024 at our institutio

RET Lys666Asn has a low rate of MEN2-related tumors but may be associated with pheochromocytoma.

PURPOSE: Multiple endocrine neoplasia type 2 (MEN2) is caused by germline pathogenic variants (PVs) in the RET proto-oncogene, leading to medullary thyroid carcinoma (MTC), pheochromocytoma, and primary hyperparathyroidism (PHPT). RET p.Lys666Asn is a rare PV, but its clinical significance remains incompletely understood. The purpose of this study is to expand the clinical understanding of the RET Lys666Asn variant. METHODS: Index patients carrying the RET Lys666Asn variant were identified through clinical genetic testing in two referral medical centers. Comprehensive clinical evaluation, bio

How has genetics changed the diagnosis and care of multiple endocrine neoplasia type 2? The merits and pitfalls of a genetic-based diagnostic and therapeutic approach.

The discovery of RET as the primary driver of hereditary medullary thyroid cancer in multiple endocrine neoplasia syndrome drastically changed the diagnosis, management, and prognosis of patients with this rare endocrine tumor. First, from a diagnostic viewpoint, RET testing within families became possible, ruling out unnecessary follow-up for negative patients and proposing an adapted surveillance protocol for positive patients. Second, large-scale epidemiological studies paved the way for early "prophylactic" thyroidectomy, rendering a historically fatal disease curable. RET identification a

Primary hyperparathyroidism presenting with pathological fractures mimicking malignancy: a case from Tanzania.

Primary hyperparathyroidism (PHPT) is commonly detected incidentally in high-income settings, but in resource-limited contexts it may still present with advanced skeletal disease. We report a 45-year-old woman with a seven-year history of progressive bone pain culminating in pathological fractures of the femoral neck and proximal humerus after minor trauma. Malignancy and myeloma were excluded by histology and hematologic work-up. Biochemistry showed severe hypercalcemia with strikingly elevated parathyroid hormone and alkaline phosphatase; imaging demonstrated diffuse osteopenia and localized

Treating microsatellite unstable metastatic parathyroid carcinoma with anti-PD1 therapy.

Parathyroid carcinoma (PC) is a rare endocrine malignancy that often clinically mimics primary hyperparathyroidism (PHPT) caused by parathyroid adenoma or hyperplasia. Complete resection of malignancy is the only curative treatment, and to date, there is no effective systemic therapy for advanced disease. A 60-year-old woman was referred for evaluation of hypercalcemia. Laboratory studies were consistent with PHPT. She underwent right superior parathyroidectomy, and pathology revealed PC with lymphovascular invasion. Postoperatively, calcium and parathyroid hormone levels normalized but recurr

Addition of Preoperative Ultrasound to 4DCT Increases Radiologist Confidence in Identifying Hyperfunctioning Parathyroid Glands.

BACKGROUND AND PURPOSE: 4D CT (4DCT) is a key imaging technique for preoperative localization of parathyroid adenomas in primary hyperparathyroidism. Ultrasound (US) is frequently added with the assumption that it enhances diagnostic performance. This study aimed to determine whether incorporating US into a 4DCT-based evaluation improves diagnostic accuracy and to assess its impact on radiologists' diagnostic confidence. MATERIALS AND METHODS: In this retrospective study, we reviewed 80 patients with primary hyperparathyroidism who underwent both 4DCT and US followed by curative parathyroidec

Serum Parathyroid Hormone and Long-Term Mortality in Hospitalized Patients: A Real-World Cohort Study.

Background: Parathyroid hormone (PTH) is closely linked to mineral metabolism, kidney function, inflammation, and cardiovascular disease. However, its clinical significance in unselected hospitalized populations remains insufficiently characterized. We aimed to evaluate the prevalence of elevated PTH and its association with long-term mortality in a real-world cohort of internal medicine inpatients. Methods: In this retrospective cohort study, electronic records of adults hospitalized in an internal medicine department in 2019 were reviewed. Patients with available in-hospital PTH measurements

Beyond Malignancy: Clinical Insights from Three Cases of Severe Hypercalcemia.

Severe hypercalcemia is a life-threatening condition requiring immediate treatment alongside a systematic evaluation to identify the underlying cause. Although malignancy is a common etiology among hospitalized patients, alternative causes must be considered to guide targeted therapy, as illustrated in these cases. The first case involved a 31-year-old postpartum woman with corrected calcium levels of 14.5 mg/dL and suppressed PTH. Hypercalcemia resolved after tapering and temporary cessation of breastfeeding, consistent with lactation-associated hypercalcemia that is likely PTHrP-mediated. Th
