INTRODUCTION AND IMPORTANCE: Struma ovarii is a rare monodermal ovarian teratoma composed predominantly of thyroid tissue, accounting for approximately 1% of ovarian neoplasms and 2%-3% of mature teratomas. Its coexistence with differentiated thyroid carcinoma - and particularly in the setting of normocalcemic primary hyperparathyroidism - has not been previously described.
CASE PRESENTATION: A 26-year-old woman presented with a painless 2 × 2 cm midline neck swelling. Ultrasound and fine-needle aspiration confirmed papillary thyroid carcinoma. Biochemistry revealed an elevated parathyroid ho

Calcium plays a fundamental role in cardiac electrophysiology and myocardial, contraction, and disturbances in calcium homeostasis may lead to significant electrical, and mechanical cardiac dysfunction. We report two clinically opposite but pathophysiologically related cases of calcium imbalance. The first case describes severe hypocalcemia presenting with long QT interval and acute heart failure due to hypocalcemic cardiomyopathy, with complete recovery of ventricular function after calcium correction. The second case describes a young patient with severe hypercalcemia due to primary hyperpar

This narrative review evaluates the current role of interventional radiology in the diagnosis and treatment of parathyroid disorders. In patients with biochemically confirmed hyperparathyroidism but inconclusive or discordant preoperative localization imaging, image-guided interventions can help establish a diagnosis that can alter management. This review outlines the current diagnostic and therapeutic modalities for the study of parathyroid pathology from the interventional radiologist's perspective, highlighting novel techniques and their growing clinical adoption. The combination of ultraso

Chronic kidney disease causes secondary hyperparathyroidism (SHPT) with hyperplastic enlarged parathyroid glands. Deep cervical spontaneous hemorrhage has been occasionally reported to be caused by rupture of an enlarged parathyroid gland under functionally uncontrolled SHPT. We encountered an unusual case of life-threatening spontaneous hemorrhage from a parathyroid gland in a patient undergoing hemodialysis. A 40-year-old man with end-stage renal disease received renal replacement therapy for 12 years. He had SHPT, which was functionally well controlled using multiple medications. The parath

BACKGROUND/AIM: Primary hyperparathyroidism (PHPT) is usually caused by a parathyroid adenoma. Parathyroid autotransplantation is commonly performed during thyroidectomy when gland perfusion is impaired. However, the development of adenomas in autotransplanted tissue is extremely rare. We present a case of PHPT caused by an adenoma arising from autotransplanted parathyroid tissue, which occurred 30 years after total thyroidectomy.
METHODS: A 60-year-old woman with a history of total thyroidectomy and parathyroid autotransplantation to the sternocleidomastoid muscle presented with hypercalcemi

Primary hyperparathyroidism (PHPT) is characterised by autonomous overproduction of parathyroid hormone (PTH), resulting in hypercalcemia and multisystem complications. Although routine calcium screening has reduced advanced skeletal disease in many regions, severe manifestations such as osteitis fibrosa cystica (OFC) still occur in resource-limited settings. We report a case of a woman in her 30s who presented with profound disability after 4-5 years of progressive bone pain, fatigue, proximal myopathy and eventual loss of independent ambulation, requiring her to drag herself on the ground. I

BACKGROUND: Mediastinal ectopic parathyroid adenoma (MEPA) presents significant difficulties in both preoperative localization and surgical management due to its deep mediastinal location and close relationship to critical surrounding structures. Traditional surgical approaches, such as median sternotomy or thoracotomy, are often necessary but are associated with considerable surgical morbidity. Currently, minimally invasive options for resecting deeply located mediastinal parathyroid lesions are limited and remain technically challenging.

BACKGROUND: Previous research has shown that cadmium-zinc-telluride (CZT) crystals, as semiconductor detectors, provide higher spatial and energy resolution than thallium-activated sodium iodide [NaI(Tl)] crystals. This study aimed to evaluate the extent to which a full-ring CZT single-photon emission computed tomography/computed tomography (SPECT/CT) system could improve the image quality of 99mTc-methoxyisobutylisonitrile (99mTc-MIBI) parathyroid imaging, as well as its potential to enhance diagnostic accuracy and optimize imaging protocols.

BACKGROUND: Parathyroid carcinoma (PC) is a rare and often misdiagnosed endocrine malignancy that can present with diverse clinical manifestations, complicating its detection and management.
CASE PRESENTATION: This case report presents a 59-year-old male diagnosed with parathyroid carcinoma (PC), initially misidentified as benign parathyroid hyperplasia and adenoma. Over the course of 4 years, the patient underwent three surgical interventions. Final surgery confirmed the definitive diagnosis of PC, which revealed vascular invasion. The patient exhibited unusual clinical manifestations, notab

OBJECTIVE: Eucalcemic postoperative parathyroid hormone elevation (PePTH) after curative parathyroidectomy for sporadic primary hyperparathyroidism is common, yet its temporal profile and clinical significance remain unclear. Transient hypoparathyroidism also occurs after surgery, but the immediate course of intact PTH (iPTH) recovery is poorly defined. We therefore characterized day-by-day postoperative calcium, phosphate, and iPTH trajectories through postoperative day (POD) 5 and evaluated preoperative predictors of PePTH at 6 months and 18-month biochemical outcomes.
METHODS: In this pros

Tumoral calcinosis (TC) is a rare complication of progressive kidney failure, characterized by massive depositions of calcium and phosphate into the periarticular soft tissue. Although generally associated with hyperparathyroidism and an elevated calcium-phosphorus product, there are few curative treatments for this condition. Although kidney transplantation typically leads to complete resolution of TC, the early posttransplant course may paradoxically mobilize soft tissue calcium into the general circulation, causing severe hypercalcemia and renal allograft injury. We report a rare case of ac

OBJECTIVE: Benign prostatic hyperplasia (BPH) and kidney stones are among the most common benign urologic conditions in males. A key risk factor for kidney stones is low fluid intake. Males with BPH frequently restrict fluid intake to manage lower urinary tract symptoms, potentially increasing their stone risk. We aimed to evaluate the incidence of kidney stones in males with symptomatic BPH compared with those without BPH.
METHODS: We conducted a nationwide retrospective cohort study using TriNetX, a database of deidentified electronic health records of over 118 million US patients covering

Cutaneous lichen amyloidosis (CLA) is a rare dermatological condition characterized by amyloid deposition in the skin, presenting as pruritic, hyperkeratotic papules. Although most cases are sporadic, CLA has been associated with multiple endocrine neoplasia type 2A (MEN2A), a hereditary syndrome caused by germline alterations in the RET proto-oncogene. In MEN2A, CLA is typically localized to the interscapular region and linked to RET codon 634 variants, whereas generalized forms are rare. We report a male patient with MEN2A and a generalized form of CLA that preceded the diagnosis of primary

Background/Objectives: The spectrum of chronic kidney disease-mineral and bone disorder (CKD-MBD) is changing and adynamic bone disease (ABD) is now believed to constitute the majority of CKD-MBD in the developed world. However, its prevalence and risk factors are poorly described in the literature. Its diagnosis requires bone biopsy, but biochemical criteria including parathyroid hormone (PTH) levels show good correlation. The aim of this study is to understand the prevalence of ABD in our patients with kidney failure (KF) on hemodialysis (HD), identify the risk factors for its development, a

BACKGROUND/OBJECTIVE: Primary hyperparathyroidism in pregnancy is rare, representing <1% of cases, but carries substantial maternal and fetal risks, including nephrolithiasis, pancreatitis, pre-eclampsia, miscarriage, and neonatal hypocalcemia. Diagnosis is obscured by physiologic reductions in calcium and limited imaging options. The objective of this report is to describe 2 cases of primary hyperparathyroidism in pregnancy where unique imaging modalities were utilized.

Parathyroid water-clear cell adenomas are rare causes of primary hyperparathyroidism, with fewer than 30 cases reported in the medical literature worldwide. Over the years, these tumors have been associated with delayed diagnosis and treatment, as well as challenges in the management of hyperparathyroidism. We report the case of a 36-year-old female with a medical history of stage IV chronic kidney disease who presented with a large parathyroid adenoma composed predominantly of clear cells. Her diagnosis was delayed due to negative preoperative imaging findings despite markedly elevated parath

BACKGROUND/OBJECTIVE: Multiple endocrine neoplasia (MEN) comprises a group of rare autosomal dominant genetic disorders characterized by the development of benign or malignant tumors in 2 or more endocrine glands, most commonly including the thyroid, parathyroid, pituitary, adrenal glands, and pancreatic neuroendocrine tissues. MEN4 is a recently recognized subtype that phenotypically overlaps with MEN1 and is caused by germline mutations in the cyclin-dependent kinase inhibitor 1B (CDKN1B) gene. However, due to the limited number of reported cases and the lack of standardized clinical guideli

For type I gastric neuroendocrine neoplasms (gNEN) larger than 1 cm endoscopic resection should be performed, whereas for tumors larger than 2 cm surgical resection should be carried out. Tumors between 1 and 2 cm in size are generally resected endoscopically following endosonography; however, surgery should be considered in the presence of risk factors such as G2/3 tumors, infiltration of the muscularis propria and vascular invasion. Type II gNENs are treated endoscopically or surgically in the same manner as type I tumors. It is essential to consider treatment of the underlying hypergastrine

CONTEXT: Primary hyperparathyroidism (PHPT) is a common endocrine disorder for which minimally invasive parathyroidectomy (MIP) is the standard surgical approach. Intraoperative parathyroid hormone (IOPTH) monitoring is commonly performed using second-generation (2G) parathyroid hormone (PTH) assay.
OBJECTIVE: To compare the performance of IOPTH central sampling peripheral sampling, and third-generation (3G) vs 2G assays in patients with PHPT.
METHODS: This is a prospective cohort study conducted at American University of Beirut Medical Center, a tertiary hospital in Beirut, Lebanon. The stu
