INTRODUCTION: Immune checkpoint inhibitors (ICIs) have revolutionised cancer treatment but are associated with agent-specific spectra of endocrine-related adverse events (irAEs). Data on prevalence of ICI-induced endocrinopathies in Indian populations remain limited. A study was conducted with an aim to determine the prevalence and spectrum of endocrinopathies resulting from ICI therapy in cancer patients at a tertiary care centre.
METHODS: This prospective cohort study included 100 adult cancer patients initiated on ICI therapy (anti-PD1 or PD-L1). Patients with pre-existing endocrinopathies

While atherothrombosis accounts for the majority of acute coronary syndrome (ACS) cases, non-atherosclerotic mechanisms like coronary embolism are important considerations in end-stage renal disease (ESRD). A 52-year-old woman with ESRD and tertiary hyperparathyroidism presented with myocardial infarction. Echocardiography identified mobile mitral valve masses. Angiography showed a left anterior descending artery occlusion, but intravascular ultrasound revealed minimal plaque burden. Histopathological analysis of aspirated material confirmed calcified necrotic debris rather than thrombus. Desp

OBJECTIVE: Nephrotoxicity of bisphosphonates limits their utility in the management of severe hypercalcemia among patients with primary hyperparathyroidism (PHPT) and impaired renal function. It remains unclear whether denosumab is an effective and safe therapeutic option for hypercalcemia in patients with PHPT. We aimed to compare the efficacy and safety of denosumab and zoledronic acid (ZA) in treating hypercalcemia in patients with PHPT.
METHODS: This single-center retrospective comparative study included ninety patients with PHPT and hypercalcemia. Forty-five were treated with ZA (4 mg in

Intrathyroid parathyroid adenomas can be difficult to detect on imaging and may be missed intraoperatively. 18F-fluorocholine positron emission tomography/computed tomography (FCH PET/CT) is a relatively new imaging modality used to evaluate patients with hyperparathyroidism preoperatively. We present the case of an intrathyroid parathyroid adenoma visualised on FCH PET/CT but not identified on the current conventional imaging modalities preoperatively, which subsequently underwent successful surgical removal. Trial Registration: ACTRN12625000509460.

Primary hyperparathyroidism (PHPT) caused by ectopic parathyroid adenomas in the superior mediastinum is rare and often leads to surgical failure. We report a case of severe PHPT in a 50-year-old female presenting with lower limb weakness and bone pain. Laboratory tests revealed marked hypercalcemia (serum calcium 3.48 mmol/L) and extremely elevated parathyroid hormone (PTH) levels (1920 pg/mL). Preoperative imaging, including neck ultrasound and 99mTc-MIBI SPECT/CT, precisely localized the ectopic adenoma to the superior mediastinum at the T1 vertebral level, posterior to the trachea, rather

BACKGROUND: Atypical parathyroid tumors (APTs) are diagnostically challenging neoplasms that display worrisome histopathological features but do not meet the criteria for carcinoma. Conventional biochemical and imaging parameters have limited discriminatory accuracy, and no validated preoperative biomarker exists to identify APT reliably. This study evaluated the diagnostic performance of the C-reactive protein-albumin-lymphocyte (CALLY) index as a composite immunonutritional marker for distinguishing APT from typical parathyroid adenomas (TPA).
METHODS: This retrospective observational study

Endocrinopathies are common chronic disorders in small-animal practice, and accurate estimation depends on standardized diagnostic criteria and denominator-defined populations. Brazilian epidemiological evidence is mostly drawn from hospital and referral settings, resulting in fragmented coverage, limited generalizability, and marked heterogeneity between studies. Conversely, VetCompass in the United Kingdom integrates standardized primary-care electronic records and supports denominator-based population inference. Searches were updated in January 2026 and covered PubMed, Web of Science, Scopu

Background: Given the limited and conflicting evidence regarding both the role of bisphosphonate (BP) use and the risk factors for postoperative hypocalcemia, as well as ongoing clinical hesitation toward preoperative BP use due to concerns about the occurrence of hypocalcemia, we aimed to investigate their association in patients with primary hyperparathyroidism (PHPT). Materials and Methods: A total of 73 patients undergoing parathyroidectomy were included, with 22 in the BP-positive (zoledronate) group and 51 in the BP-negative group. In BP-treated patients, biochemical values obtained afte

BACKGROUND: Hyperparathyroidism (HPT) is common in chronic kidney disease and may persist after kidney transplantation, but its prognostic significance remains uncertain because of heterogeneous definitions and variable timing of assessment.
METHODS: We performed a systematic review and meta-analysis of observational studies in adult kidney transplant recipients (KTRs) in accordance with PRISMA 2020 (PROSPERO CRD420261293712). PubMed/MEDLINE and Embase were searched from inception to 9 February 2026. Exposures included pre- and post-transplant parathyroid hormone (PTH)/Persistent HPT assessed

Background: Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome caused by fibroblast growth factor 23 (FGF-23)-secreting tumors, typically of mesenchymal origin, leading to renal phosphate wasting and severe bone demineralization and fragility fractures. Diagnosing TIO remains a significant clinical challenge, particularly when coexisting mineral metabolism disorders, such as hypercalcemic hyperparathyroidism, are masking its clinical presentation. Case Presentation: A 74-year-old woman with fragility fractures, generalized bone pain, and nephrolithiasis was initially diagnosed

Vitamin D is essential for calcium and phosphate homeostasis and skeletal development. Variants in the vitamin D receptor gene (VDR) cause vitamin D-dependent rickets type 2A (VDDR2A), characterized by end-organ resistance to 1,25-dihydroxyvitamin D (1,25(OH)2D). We describe a child of nonconsanguineous Middle Eastern parents who presented at age 9 months with poor linear growth and inadequate weight gain. Initial evaluation showed normocalcemia, hypophosphatemia, hyperparathyroidism, markedly elevated alkaline phosphatase, and radiographic rickets, leading to an initial diagnosis of hypophosp

BACKGROUND: Hypercalcaemic crisis (HCC) is a serious and potentially life-threatening complication of increased serum calcium concentration. It is most commonly due to primary hyperparathyroidism (PHPT), from a parathyroid hormone (PTH) secreting tumour. This paper looks at factors affecting outcomes of parathyroid surgeries in HCC patients.
METHODS: Data were collected retrospectively including demographics, biochemistry and histology results, details of localisation studies, time from referral to surgery, disease related admissions, length of hospital stay. Statistical analysis was performe

Purpose: Tertiary hyperparathyroidism is a frequent complication of end-stage kidney disease that may require parathyroidectomy when medical therapy fails. Recurrent hyperparathyroidism may occur after total parathyroidectomy with autotransplantation. This study assessed the safety, feasibility, and efficacy of percutaneous ultrasound-guided ablation for implanted parathyroid glands in patients with recurrent disease after total parathyroidectomy with autotransplantation. Materials and Methods: Eleven patients with recurrent hyperparathyroidism following total parathyroidectomy with autotransp

CASE: A 32-year-old man with end-stage renal disease (ESRD) from anti-neutrophil cytoplasmic antibody-associated vasculitis and tertiary hyperparathyroidism, presented with progressive right hip pain. A giant cell-rich lesion biopsy confirmed Brown tumor. Despite cinacalcet and vitamin D, hyperparathyroidism persisted, necessitating parathyroidectomy complicated by hungry bone syndrome and a pathological right hip fracture. Surgical fixation and prosthetic replacement were deferred due to high risk factors such as, ESRD and fracture pattern, as well as potential for postoperative implant failu

A 71-y-old woman with primary hyperparathyroidism underwent 99mTc-sestamibi SPECT/CT for preoperative localization. The study showed a left inferior parathyroid adenoma and, on extended field-of-view review, an intensely 99mTc-sestamibi-avid 1.5-cm lesion in the left breast. Diagnostic mammography and ultrasonography classified the lesion as Breast Imaging Reporting and Data System category 5, prompting a core biopsy that revealed Nottingham grade 3 invasive ductal carcinoma of no special type (human epidermal growth factor receptor 2 [HER2] overexpression [3+]), with a Ki-67 index of 80%. Parathyroid hormone (PTH), a well-established regulator of calcium homeostasis and bone remodeling, has been implicated in central nervous system (CNS) modulation beyond its classical peripheral endocrine functions. Utilizing multiple murine models of hyperparathyroidism, this study demonstrated that elevated PTH produces distinct behavioral phenotypes contingent upon disease etiology and progression. Administration of hPTH(1-34) elicited sexually dimorphic behavioral responses - enhanced locomotor activity in males and anxiety-like behavior in females - the latter being independent of estrus

OBJECTIVE: Primary hyperparathyroidism (PHPT) affects 1% of adults, but fewer than one-third of screen-eligible patients are tested. We developed and evaluated two new non-interruptive, clinician-facing alerts to increase appropriate PHPT screening.
MATERIALS AND METHODS: We designed process changes to facilitate guideline-informed patient identification via electronic phenotyping, laboratory test ordering, result interpretation, and follow-up. The computable phenotype was retrospectively validated. Clinician-facing alerts were built atop existing workflows for health maintenance (HM) and pen

Background: Osteoporosis/osteoporotic fractures are identified in both hypercalcemic (HC-HPT) and normocalcemic variant (NC-HPT) of primary hyperparathyroidism (HPT) at various rates. Objective: Noting the need of modern society to easily assess the osteoporotic fracture risk amid the diagnosis of HPT, we aimed to address this gap by analyzing the 10-year fracture risk assessment based on traditional FRAX (Fracture Risk Assessment Tool) model in comparison to the novel algorithm (FRAXplus), according to the adjustment for the presence of HPT, as well as for the use of lumbar bone mineral densi
