OBJECTIVE: To evaluate the efficacy and safety of a modified percutaneous ultrasound-guided ethanol ablation technique for the treatment of primary hyperparathyroidism in dogs.
METHODS: This prospective observational study was conducted at a single veterinary referral center. Dogs with persistent ionized hypercalcemia, normal/increased parathyroid hormone concentrations, normal/undetectable parathyroid hormone-related protein concentrations, and ultrasonographic identification of ≥ 1 presumed hyperfunctioning parathyroid nodule (≥ 2.5 mm) underwent percutaneous ultrasonographically guided eth

Primary hyperparathyroidism during pregnancy is rare but associated with substantial maternal and foetal morbidity, necessitating timely diagnosis and intervention. Intrathyroidal parathyroid adenomas account for as few as 0.7% of parathyroid adenomas, while cystic parathyroid adenomas represent an even smaller subset. The coexistence of both is exceedingly uncommon, with only nine cases reported and none previously described in pregnancy or using [18F]fluorocholine PET/CT for localisation. We describe a 37-year-old pregnant woman with PTH-dependent hypercalcaemia which persisted following ini

Parathyroid carcinoma is a rare form of cancer. We describe a 45-year-old man diagnosed with parathyroid carcinoma long after initial surgery. At age 24, the patient had elevated serum calcium and intact parathyroid hormone (PTH). Ultrasonography revealed a left lower parathyroid tumor, which led to the diagnosis of primary hyperparathyroidism. Parathyroidectomy was performed, and the pathological diagnosis was parathyroid adenoma. At age 32, hypercalcemia and elevated intact PTH recurred, and computed tomography revealed a right lung tumor with focal uptake on 99mTc-methoxyisobutylisonitrile

OBJECTIVE: Although skeletal fragility in primary hyperparathyroidism (pHPT) is well-recognized, data specific to lower limb fractures, the most disabling fractures, remain fragmented. This study aims to systematically synthesize evidence based on the existing literature to describe lower limb fractures in patients with pHPT, with emphasis on fracture characteristics and surgical management, and to perform a meta-analysis quantifying the risk of lower limb fracture in these patients.
METHODS: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 guidelines, we

Giant parathyroid adenomas (GPAs) are rare lesions that may clinically and radiologically mimic parathyroid carcinoma, particularly when associated with severe hypercalcemia. A 63-year-old male with hypertension and chronic kidney disease presented with symptomatic hypercalcemia and severe hyperparathyroidism. Laboratory evaluation revealed markedly elevated serum calcium and parathyroid hormone levels. Imaging demonstrated a large left-sided parathyroid mass suspicious for adenoma versus carcinoma. Following multidisciplinary evaluation, the patient underwent en bloc resection consisting of l

SUMMARY: An 11-year-old girl presented to the emergency department with hypophosphataemia. Subsequent investigations repeatedly demonstrated elevated serum parathyroid hormone (PTH) associated with normal serum total calcium, phosphate, vitamin D, and bone turnover markers. Imaging studies were initially interpreted as being consistent with the effects of hyperparathyroidism; however, follow-up review by specialists with paediatric expertise concluded otherwise. Further laboratory studies ultimately demonstrated that there was a false elevation in PTH, determined to be due to interference in t

Mineral and bone disease (MBD) is a frequent complication after solid organ transplantation. The rapid decline in bone mineral density (BMD) during the first year after kidney transplantation reflects complex interactions between immunosuppressive therapy, metabolic factors and pre-existing chronic kidney disease (CKD)-MBD. This narrative review summarizes the magnitude and time course of BMD loss after kidney transplantation, the underlying metabolic and pharmacologic mechanisms, and associated fracture risk. Diagnostic strategies including dual-energy X-ray absorptiometry (DXA), quantitative

INTRODUCTION: The potential effect of elevated parathyroid hormone (PTH) levels on nutritional status and sarcopenia in chronic kidney disease (CKD) patients undergoing hemodialysis (HD) has not yet been well established.
OBJECTIVE: The aim of this study was to evaluate the status of sarcopenia and nutritional aspects according to PTH levels in HD patients.
METHODS: This cross-sectional study evaluated consecutive patients attending three HD clinics in Northeast Brazil. Patients were stratified based on PTH levels: (1) 0-199 pg/mL, (2) 200-599 pg/mL, and (3) ≥ 600 pg/mL. Group 1 was excluded

INTRODUCTION: Chronic kidney disease (CKD) has devastating effects on the skeletal system, resulting in an increased fracture risk. CKD results in the buildup of metabolites in the blood (often referred to as uremic toxins), and several uremic toxins have been linked to adverse skeletal outcomes. We have shown that dietary fiber inulin reduced uremic toxins and had a positive effect on bone. Here, we determine whether combining inulin with parathyroid hormone (PTH)-lowering therapies could improve skeletal properties beyond either therapy alone.
METHODS: An animal model of CKD-mineral bone di

BACKGROUND: Parathyroid carcinoma (PC) is a rare, slow-growing but locally aggressive endocrine malignant tumor. Its clinical diagnosis is difficult, especially intrathyroid parathyroid carcinoma with atypical or asymptomatic symptoms. It is difficult to make a clear diagnosis before and during surgery.
CASE DESCRIPTION: A 27-year-old female with elevated parathyroid hormone for 1 month showed a hypoechoic nodule immediately behind the middle of the left lobe of the thyroid gland. Laboratory work showed mild elevations in blood calcium and parathyroid hormone. She underwent parathyroid adenom

BACKGROUND: Evidence on sex-related differences in chronic kidney disease (CKD)-related complications is limited and often based on cross-sectional studies. This study longitudinally examines clinical and biochemical parameters of CKD complications, focusing on differences between men and women.
METHODS: We performed a retrospective study within the Chronic Renal Insufficiency Cohort (CRIC) Study cohort, including adults with baseline estimated glomerular filtration rate (eGFR) 20-70 mL/min/1.73 m² and at least one serum creatinine measurement. Demographic, clinical, laboratory and therapeuti

BACKGROUND: Primary hyperparathyroidism (pHPT) is infrequently associated with calcium-containing kidney stones, despite hypercalciuria. Parathyroidectomy (PTX) is the only curative treatment of the metabolic disorder and is indirectly regarded as prophylaxis of recurrence of stone formation. The aim of the systematic review was to evaluate the impact of PTX on stone recurrence and to identify possible predictors of recurrent stone formation.
METHODS: Following PRISMA guidelines, a systematic PubMed search was conducted through April 2025. Eligible studies including adults with successfully s

The Viennese Medical School was at the forefront of the clinical understanding of the relationship between parathyroid and bone metabolism. A century ago, F. Mandl (1925) and E. Gold (1927) described two patients clinically (osteitis fibrosa cystica) and biochemically (hypercalcemia, hypercalciuria) with a disease which Gold had already termed (primary) hyperparathyroidism (PHPT) in 1928. The two patients were successfully treated by removing one enlarged parathyroid gland each. After the clinical and biochemical diagnosis was confirmed, surgery was recommended. In 1933, Mandl summarized his r

ABSTRACT: PURPOSE: Primary hyperparathyroidism has been associated with subclinical neuromuscular dysfunction; however, the clinical correlates of muscle strength and morphology in PHPT remain undercharacterized. We hypothesized that muscle dysfunction in primary hyperparathyroidism is reversible after parathyroidectomy and that the reduction in serum calcium predicts early postoperative improvement.
ABSTRACT: DESIGN: Prospective observational study conducted at a tertiary care center.
ABSTRACT: METHODS: Fifty-three primary hyperparathyroidism patients and 53 age-, sex-, and body mass index-

CONTEXT: Placental calcium (Ca2+) transport is essential for foetal bone mineralisation and development, as well as for Ca2+ homoeostasis. Rare mutations in transient receptor potential vanilloid (TRPV) 6 of the infant cause insufficient maternal-foetal Ca2+ transport through the placenta. Although there is a limited number of cases, phenotype analyses of these patients show skeletal deformities and hyperparathyroidism. In this study, we identified a case with a homozygous mutation in the TRPV6 channel that leads to the deletion of the Ca2+ pore.
PATIENT MEASUREMENTS: The neonate was born wit

Background: Both primary hyperparathyroidism (PHPT) and chronic hypoparathyroidism (HypoPT) are associated with the onset and development of cardiovascular diseases (CVDs). In particular, PHPT is accompanied by the presence of elevated atherothrombotic risk, while the importance of traditional and new anthropometric indices to reflect the cardiovascular risk remains uncertain in this condition. This study aims to investigate whether novel and traditional anthropometric indices distinguish PHPT and whether these indices are correlated with atherothrombotic risk. Methods: A total of 40 subjects

BACKGROUND: Hyperparathyroidism (HPT) is a frequent complication in chronic kidney disease and may persist in 20%-50% of cases 1 year after kidney transplantation (KT). HPT contributes to long-term allograft dysfunction and increased risk of fractures. There is scant data on the prevalence of HPT in kidney transplant recipients (KTRs) in Saudi Arabia. The aim of this study is to evaluate the prevalence of this complication and to identify its risk factors in a single center in Saudi Arabia.
SUBJECTS AND METHODS: In a retrospective study, data of 204 KTRs who underwent KT between January 2017

PURPOSE: To characterize the clinical and genetic features of Chinese FHH type 1 (FHH1) patients and evaluate biochemical discriminators between FHH1 and sporadic primary hyperparathyroidism (s-PHPT). METHODS: This retrospective study included 11 genetically confirmed FHH1 and 55 s-PHPT patients. Their clinical, biochemical, and imaging data were compared. Genetic testing was conducted via targeted next-generation or whole-exome sequencing. The key discriminators were assessed by receiver operating characteristic (ROC) analysis. RESULTS: Among the 11 FHH1 patients, 63.6% were females and 63.6%
