The cross-cultural and chronological evolution of the kidney-bone relationship: from ancient medical insights to modern molecular endocrinology.

The kidney-bone axis constitutes a conserved bidirectional endocrine network that orchestrates skeletal remodeling and systemic mineral homeostasis. Its dysregulation underlies chronic kidney disease-mineral and bone disorder (CKD-MBD), renal osteodystrophy, secondary hyperparathyroidism, and associated vascular calcification, markedly elevating fracture and cardiovascular mortality risks. This review offers a unique cross-cultural and chronological synthesis of the kidney-bone relationship, bridging ancient medical insights with modern molecular endocrinology. Horizontally, we systematically

Single cell multi-omics guided hydrogel enables closed-loop therapy for renal osteodystrophy.

Fracture repair in renal osteodystrophy remains challenging because impaired bone quality compromises fixation stability, while current systemic therapies regulating bone metabolism do not sufficiently address the disease-specific mechanisms underlying defective repair. An integrated strategy combining early fracture stabilization with mechanism-guided local treatment is therefore needed. Here, integrated single-cell RNA sequencing and multi-omics analyses identify inflammatory macrophages as a key pathogenic population, characterized by glycolytic reprogramming driven by hypoxia-inducible fac

Hypophosphatemia after CAR-T cell therapy: a prospective study of inflammation, tubular injury and phosphate handling.

BACKGROUND: Hypophosphatemia is a frequent electrolyte abnormality after chimeric antigen receptor T-cell (CAR-T) therapy, but the clinical and biological factors associated with phosphate decline remain poorly defined. We prospectively evaluated the incidence, determinants, and potential biological associations of hypophosphatemia after CAR-T therapy. METHODS: We conducted a prospective study including 63 adults with B-cell lymphoma treated with CAR-T therapy between June 2020 and December 2023. Serum phosphate was measured daily during hospitalization. First-morning urine and blood samples

ADOPTION OF THE FIFTH WORKSHOP URINARY CALCIUM THRESHOLD IN ASYMPTOMATIC PRIMARY HYPERPARATHYROIDISM.

OBJECTIVE: The clinical presentation of sporadic primary hyperparathyroidism (PHPT) has shifted over the last decades towards an asymptomatic disease (aPHPT). International workshops have refined surgical criteria, and urinary calcium excretion (24h-UCa) has been among the most controversial parameters. Notably, 24h-UCa was the only modified criterion between the Fourth and Fifth Workshops. The threshold was lowered from a single cut-off of 400 mg/24h (Fourth Workshop) to gender-specific cut-offs of 300 mg/24h for males and 250 mg/24h for females (Fifth Workshop). METHODS: We retrospectively

Parathyroid Carcinoma: A Report of Two Cases.

Parathyroid carcinoma is a rare cause of hypercalcaemia, which can be potentially difficult to diagnose and treat. Since its clinical, biochemical, and radiological features overlap with those of parathyroid adenoma, diagnosis is often made only after histological review of the resected tissue. We report two cases managed at a United Kingdom district general hospital. The first case was of a 51-year-old man referred with persistent hoarseness and an incidental parathyroid lesion on imaging, in whom progressive hypercalcaemia led to excision and a histological diagnosis of parathyroid carcinoma

Progression of Secondary to Tertiary Hyperparathyroidism with a Coexisting Parathyroid Adenoma in Stage 3b Chronic Kidney Disease: A Case Report.

Tertiary hyperparathyroidism arises when prolonged secondary hyperparathyroidism progresses to autonomous parathyroid hormone secretion with persistent hypercalcemia, most often in chronic kidney disease. Diffuse or nodular multiglandular hyperplasia is the expected pathology; a discrete parathyroid adenoma arising within hyperplastic tissue is uncommon and may be overlooked when imaging is negative early in the disease course. A 60-year-old man with stage 3b chronic kidney disease and long-standing secondary hyperparathyroidism presented with recurrent hypercalcemia and severe vomiting. Labor

Why patients decline parathyroidectomy: A mixed-methods analysis of decision-making in primary hyperparathyroidism.

INTRODUCTION: Less than one-third of patients diagnosed with primary hyperparathyroidism (PHPT) undergo parathyroidectomy. While disparities in referral have been well described, patient perspectives after endocrine surgery referral remain poorly understood. METHODS: We performed a mixed-methods study within a single academic health system, integrating qualitative patient interviews and chart review. Adults diagnosed with PHPT between 2021 and 2025 were identified, and those who did not undergo surgery were invited for a semi-structured interview to explore their decision-making. RESULTS: Am

How much is enough - surgical extent of initial parathyroidectomy and long-term outcomes in MEN1-related primary hyperparathyroidism.

CONTEXT: Multiple endocrine neoplasia type 1 (MEN1)-associated primary hyperparathyroidism (pHPT) is typically multiglandular, which contributes to both persistent and recurrent disease following surgery. The optimal extent of initial parathyroidectomy in MEN1 remains debated. OBJECTIVE: To evaluate long-term outcomes of subtotal parathyroidectomy (SPTX) and total parathyroidectomy with autotransplantation (TPTX-AT) versus less-than-subtotal parathyroidectomy (LSPTX) in MEN1-related pHPT, specifically regarding persistence, recurrence, permanent hypoparathyroidism, and need for reoperation.

Quality of Life Following Parathyroidectomy for Primary Hyperparathyroidism: Symptom Resolution Kinetics.

OBJECTIVE: Primary hyperparathyroidism has been associated with significant symptom burden and impaired quality of life (QoL), yet the trajectory of recovery, especially in reoperative patients is incompletely characterised. This study evaluated longitudinal QoL changes following parathyroidectomy, characterising symptom resolution kinetics and comparing outcomes between cohorts. A prospective longitudinal cohort study was conducted between July 2021 and September 2023 at a single high volume tertiary centre. Patients with biochemically confirmed primary hyperparathyroidism who underwent parat

Postoperative sensorineural hearing loss associated with prolonged hypocalcemia.

A 57-year-old woman with end-stage renal disease and secondary hyperparathyroidism underwent subtotal parathyroidectomy and developed profound postoperative hypocalcemia consistent with hungry bone syndrome, followed by acute sensorineural hearing loss on postoperative day 3. Calcium plays a critical role in cochlear hair cell transduction and auditory signaling, and disruptions in calcium homeostasis have been associated with hearing loss, although primarily in chronic or syndromic conditions rather than acute postoperative states. This case underscores the importance of aggressive calcium re

Severe Hypocalcemia and Hypophosphatemia Following Ferric Carboxymaltose in a Patient With Roux-en-Y Gastric Bypass.

Iron deficiency anemia is commonly treated with intravenous iron when oral therapy is ineffective, poorly tolerated, or inadequately absorbed. Ferric carboxymaltose (FCM) is an effective intravenous iron formulation but is increasingly recognized to cause hypophosphatemia through fibroblast growth factor 23 (FGF23)-mediated renal phosphate wasting. Although FCM-induced hypophosphatemia is well described, concurrent clinically significant hypocalcemia is less commonly reported. We present the case of a 39-year-old woman with a history of Roux-en-Y gastric bypass, iron deficiency anemia, and vit

Effect of Personalized Approach to CT Scan Length on Radiation Dose in Parathyroid SPECT/CT.

Parathyroid SPECT/CT is the standard examination for localization of hyperfunctioning parathyroid tissue before surgery for hyperparathyroidism. The effective dose (ED) delivered by CT comprises approximately one third of the radiation dose in this examination. Parathyroid SPECT/CT typically includes 1 CT covering the mediastinum because of the risk of ectopic adenomas. This study investigates whether the use of personalized CT (pCT) on the basis of SPECT images can reduce scan length and CT-derived ED, while maintaining the ability to detect ectopic adenomas, compared with full field-of-view

Percutaneous microwave ablation of hyperfunctioning parathyroid lesions under multiparametric ultrasound guidance for primary hyperparathyroidism: a systematic review and meta-analysis.

BACKGROUND: Multiparametric ultrasound (MPUS)-guided percutaneous microwave ablation (MWA) has emerged as a minimally invasive treatment option for primary hyperparathyroidism (PHPT), but pooled evidence on longitudinal biochemical outcomes and safety remains limited. OBJECTIVE: This meta-analysis aimed to evaluate the efficacy and safety of percutaneous MWA performed under ultrasound guidance for PHPT. METHODS: Following PRISMA guidelines, eligible cohort studies were retrieved from five electronic databases. Pooled cure rates, serial changes in intact parathyroid hormone (iPTH), serum calc

Whole-blood versus laboratory intraoperative parathyroid hormone assays and optimisation of intraoperative monitoring during parathyroidectomy: A retrospective cohort study.

PURPOSE: Intraoperative parathyroid hormone (IOPTH) monitoring supports focused parathyroidectomy by confirming biochemical response after gland excision, but standard assays may be logistically burdensome. Whole-blood point-of-care (POC) assays offer faster turnaround but may have reduced sensitivity. We compared a whole-blood POC IOPTH assay (NBCL CONNECT) with a standard laboratory assay and evaluated candidate IOPTH criteria and sampling timepoints. METHODS: We performed a single-centre retrospective cohort study of consecutive patients undergoing parathyroidectomy for primary hyperparath

Prospective evaluation of donor-derived cell-free DNA for noninvasive detection of acute rejection in an Asian heart transplant population.

Endomyocardial biopsy (EMB) is the gold standard for diagnosing post-heart transplant acute rejection (AR) but is invasive. Donor-derived cell-free DNA (dd-cfDNA) is a promising noninvasive biomarker, yet thresholds for Asian recipients remain undefined. This prospective study enrolled 80 patients, with 299 dd-cfDNA specimens analyzed via a novel Chinese-developed next-generation sequencing assay targeting 48 Indel sites. A reference cohort (n = 42) established post-transplant dd-cfDNA kinetics, while a biopsy cohort (n = 41, 18 acute cellular rejection [ACR], 9 antibody-mediated rejection [AM

Discovery and characterization of potent and efficacious small-molecule parathyroid hormone Receptor-1 (PTH1R) antagonists for the treatment of hypercalcemia.

Primary hyperparathyroidism (PHPT) is caused by the enlargement and overactivation of one or more of the parathyroid glands, via parathyroid adenomas or hyperplasia. This results in over-secretion of PTH hormone, which results in agonism of PTH1R and subsequent hypercalcemia. Blocking PTH action via an oral, small-molecule PTH1R antagonist may provide a convenient and improved therapy for PHPT. Herein, we report the discovery, optimization and characterization of a highly potent, bioavailable and efficacious series represented by 9c (hPTH1R IC50 = 5.2 nM), which based on pharmacokinetics and b

The effects of dialysate acids and divalent ions on calcification and cardiovascular outcomes: old wisdom and new developments.

Dialysate calcium (DCa) and magnesium (DMg) and the organic acid used in the acid concentrate have a profound combined influence on intradialytic mineral balance and may influence vascular calcification progression and treatment-related symptoms. Whereas Kidney Disease: Improving Global Outcomes recommends a DCa between 1.25 (DCa 1.25) and 1.50 mmol/l (DCa 1.50), the optimal DCa for an individual patient remains uncertain. No consistent mortality difference between DCa 1.50 and 1.25 has been observed. However, DCa 1.50 may induce a positive calcium balance, raising concerns about vascular calc

Type-specific detection and quantification of circulating tumor HPV DNA by digital PCR in surgically resected cervical cancer.

OBJECTIVE: Circulating tumor human papillomavirus (HPV) DNA (ctHPV DNA) has shown clinical utility in advanced cervical cancer; however, its role in surgically resected disease remains unclear. We evaluated the detectability and clinicopathological associations of ctHPV DNA in preoperative plasma from patients with surgically resected cervical cancer using digital polymerase chain reaction (dPCR). METHODS: We analyzed 49 surgically resected cervical cancer patients. HPV genotyping of formalin-fixed paraffin-embedded tumor tissues was performed using both PGMY09/11 consensus PCR and novel geno
