Iron deficiency anemia is commonly treated with intravenous iron when oral therapy is ineffective, poorly tolerated, or inadequately absorbed. Ferric carboxymaltose (FCM) is an effective intravenous iron formulation but is increasingly recognized to cause hypophosphatemia through fibroblast growth factor 23 (FGF23)-mediated renal phosphate wasting. Although FCM-induced hypophosphatemia is well described, concurrent clinically significant hypocalcemia is less commonly reported. We present the case of a 39-year-old woman with a history of Roux-en-Y gastric bypass, iron deficiency anemia, and vit

Parathyroid SPECT/CT is the standard examination for localization of hyperfunctioning parathyroid tissue before surgery for hyperparathyroidism. The effective dose (ED) delivered by CT comprises approximately one third of the radiation dose in this examination. Parathyroid SPECT/CT typically includes 1 CT covering the mediastinum because of the risk of ectopic adenomas. This study investigates whether the use of personalized CT (pCT) on the basis of SPECT images can reduce scan length and CT-derived ED, while maintaining the ability to detect ectopic adenomas, compared with full field-of-view

BACKGROUND: Multiparametric ultrasound (MPUS)-guided percutaneous microwave ablation (MWA) has emerged as a minimally invasive treatment option for primary hyperparathyroidism (PHPT), but pooled evidence on longitudinal biochemical outcomes and safety remains limited.
OBJECTIVE: This meta-analysis aimed to evaluate the efficacy and safety of percutaneous MWA performed under ultrasound guidance for PHPT.
METHODS: Following PRISMA guidelines, eligible cohort studies were retrieved from five electronic databases. Pooled cure rates, serial changes in intact parathyroid hormone (iPTH), serum calc

PURPOSE: Intraoperative parathyroid hormone (IOPTH) monitoring supports focused parathyroidectomy by confirming biochemical response after gland excision, but standard assays may be logistically burdensome. Whole-blood point-of-care (POC) assays offer faster turnaround but may have reduced sensitivity. We compared a whole-blood POC IOPTH assay (NBCL CONNECT) with a standard laboratory assay and evaluated candidate IOPTH criteria and sampling timepoints.
METHODS: We performed a single-centre retrospective cohort study of consecutive patients undergoing parathyroidectomy for primary hyperparath

Endomyocardial biopsy (EMB) is the gold standard for diagnosing post-heart transplant acute rejection (AR) but is invasive. Donor-derived cell-free DNA (dd-cfDNA) is a promising noninvasive biomarker, yet thresholds for Asian recipients remain undefined. This prospective study enrolled 80 patients, with 299 dd-cfDNA specimens analyzed via a novel Chinese-developed next-generation sequencing assay targeting 48 Indel sites. A reference cohort (n = 42) established post-transplant dd-cfDNA kinetics, while a biopsy cohort (n = 41, 18 acute cellular rejection [ACR], 9 antibody-mediated rejection [AM

Primary hyperparathyroidism (PHPT) is caused by the enlargement and overactivation of one or more of the parathyroid glands, via parathyroid adenomas or hyperplasia. This results in over-secretion of PTH hormone, which results in agonism of PTH1R and subsequent hypercalcemia. Blocking PTH action via an oral, small-molecule PTH1R antagonist may provide a convenient and improved therapy for PHPT. Herein, we report the discovery, optimization and characterization of a highly potent, bioavailable and efficacious series represented by 9c (hPTH1R IC50 = 5.2 nM), which based on pharmacokinetics and b

Dialysate calcium (DCa) and magnesium (DMg) and the organic acid used in the acid concentrate have a profound combined influence on intradialytic mineral balance and may influence vascular calcification progression and treatment-related symptoms. Whereas Kidney Disease: Improving Global Outcomes recommends a DCa between 1.25 (DCa 1.25) and 1.50 mmol/l (DCa 1.50), the optimal DCa for an individual patient remains uncertain. No consistent mortality difference between DCa 1.50 and 1.25 has been observed. However, DCa 1.50 may induce a positive calcium balance, raising concerns about vascular calc

OBJECTIVE: Circulating tumor human papillomavirus (HPV) DNA (ctHPV DNA) has shown clinical utility in advanced cervical cancer; however, its role in surgically resected disease remains unclear. We evaluated the detectability and clinicopathological associations of ctHPV DNA in preoperative plasma from patients with surgically resected cervical cancer using digital polymerase chain reaction (dPCR).
METHODS: We analyzed 49 surgically resected cervical cancer patients. HPV genotyping of formalin-fixed paraffin-embedded tumor tissues was performed using both PGMY09/11 consensus PCR and novel geno

CASE: A 17-year-old adolescent boy presented with a pathologic distal femur fracture and bilateral distal femoral metadiaphyseal lytic lesions. Multidisciplinary evaluation confirmed bilateral distal femoral brown tumors associated with primary hyperparathyroidism secondary to a parathyroid adenoma. Treatment included bone biopsy, curettage, grafting, and retrograde locked intramedullary nail, followed by definitive partial parathyroidectomy. The contralateral metadiaphyseal lesion was treated with corrective osteotomy and prophylactic intramedullary stabilization.
CONCLUSION: Primary hyperpa

Giant cell tumor of bone is a locally aggressive tumor that usually affects the epiphyseal region of long bones in young adults. Involvement of the distal ulna is rare, and the diagnosis in elderly patients with end-stage chronic kidney disease may be difficult because brown tumors related to secondary hyperparathyroidism can mimic giant cell tumor radiologically and histologically. We report the case of a 69-year-old woman with end-stage chronic kidney disease on hemodialysis who was admitted for a painful and progressively enlarging mass on the ulnar border of the left wrist that had been pr

BACKGROUND: Surgical management is recommended in patients with secondary hyperparathyroidism when parathyroid hormone levels exceed 9 times the normal, and medical management has failed to control parathyroid hormone levels or associated complications. We aimed to examine the impact of the timing of parathyroidectomy on clinical outcomes in patients with secondary hyperparathyroidism and parathyroid hormone exceeding 9 times the normal.
METHODS: In this retrospective cohort study using the TriNetX database, we included adults with a parathyroid hormone level of ≥600 pg/mL with concurrent sta

X-linked dominant hypophosphatemic rickets (XLH) is a rare genetic disorder caused by mutations in the PHEX gene and characterized by chronic hypophosphatemia due to dysregulation of fibroblast growth factor 23 (FGF23). This condition leads to bone deformities, growth impairment, and chronic pain. Conventional treatment with oral phosphate and calcitriol often has limited efficacy and is associated with adverse effects such as hyperparathyroidism and nephrocalcinosis. This case report describes the therapeutic outcomes of three pediatric patients with XLH treated with burosumab, a monoclonal a

Liver transplantation (LT) serves as the definitive treatment for end-stage liver disease. Yet, osteoporosis (OP) and fragility fractures remain frequently overlooked skeletal complications that may impair the long-term prognosis of recipients. This review systematically summarizes recent evidence regarding epidemiological characteristics, multifactorial pathogenic pathways, risk stratification approaches, and advances in targeted interventions for LT-related bone disorders. Skeletal damage distributes across pre-transplant waiting, early post-transplant acute and long-term chronic phases; rap

PURPOSE: Primary hyperparathyroidism (PHPT) predominantly affects women, particularly after menopause. Whether sex-related differences extend beyond epidemiology to influence the clinical expression of the disease remains incompletely understood. We investigated sex-related differences in the clinical phenotype of PHPT.
METHODS: We retrospectively analyzed clinical, biochemical, imaging, and histopathological data from patients with PHPT referred to our Endocrinology Unit over a 5-year period.
RESULTS: A total of 315 patients were included (64 men and 251 women; female-to-male ratio 3.9:1).

Primary hyperparathyroidism (PHPT) is the most common parathyroid disease and one of the most common endocrine disorders. It results from autonomous overproduction of parathyroid hormone, which in the classic form of the disease causes hypercalcaemia. The ready access to biochemical testing in high-income countries has moved the disease spectrum from a classic symptomatic presentation to a biochemical anomaly that is asymptomatic or minimally symptomatic. Untreated PHPT, however, remains associated with significant consequences. This review summarizes up-to-date concepts in the diagnosis, imag

OBJECTIVE: Accurate preoperative localization of the affected parathyroid gland is important for focused parathyroidectomy in primary hyperparathyroidism, yet no single imaging modality is uniformly reliable. This study evaluated the incremental localization benefit of combining ultrasonography (US), 99mTc-sestamibi scintigraphy (MIBI), and four-dimensional computed tomography (4D-CT) and assessed agreement between imaging-measured and histopathological adenoma size.
METHODS: This retrospective, single-center study included 118 patients with histopathologically confirmed parathyroid adenoma w

BACKGROUND: Most patients with primary hyperparathyroidism have relatively mild disease with a negative impact on bone mineral density developing over years. Surgery to correct hyperparathyroidism has a protective effect on bone mineral density. The aim of this study was to evaluate the postoperative surveillance and outcomes of osteoporosis patients after parathyroidectomy.
METHODS: Primary hyperparathyroidism patients who had undergone parathyroidectomy from 2020 to 2022 were identified using a prospective provincial operative reporting database. Follow-up data were extracted from a central

BACKGROUND: A substantial portion of surgically treated patients with primary hyperparathyroidism (pHPT) are older than 65 years. The indication for parathyroidectomy in older patients with pHPT and mild hypercalcaemia is unclear. Specifically, it is not known whether parathyroid surgery improves bone mineral density (BMD) in this age group.
METHOD: Patients 65 years or older with pHPT, ionized calcium not exceeding 1.50 mmol/l, and a BMD Z-score above -2.5 standard deviations who were referred to the Department of Surgery, Lund University Hospital, were included in the study. Follow-up was p
