Bilateral internal jugular vein (BIJV) sampling during surgery for primary hyperparathyroidism (PHPT) - scoping review of evidence and search for an optimal definition for lateralisation.

PURPOSE: Accurate localisation of hyperfunctioning parathyroid glands is crucial for successful parathyroid surgery. In patients with inconclusive imaging, intraoperative bilateral internal jugular venous sampling (BIJVS) has been reported; but its utility remains unclear. The purpose of the review is to evaluate published techniques and reported effectiveness of BIJVS in parathyroid surgery. METHODS: PubMed, Ovid and Cochrane databases were searched for articles on intraoperative BIJVS in parathyroid surgery. All original English language human studies reporting on lateralisation rates, diag

Is Parenteral Bisphosphonate Therapy Safe and Effective in the Management of Severe Primary Hyperparathyroidism Prior to Parathyroidectomy?

Primary hyperparathyroidism induced hypercalcaemic crisis is defined as an albumin- adjusted calcium ≥3.50 mmol/L, occurring in the presence of organ dysfunction. This endocrine emergency requires medical optimisation prior to urgent parathyroidectomy. However, few guidelines exist on the most appropriate course of treatment, including the use of preoperative bisphosphonates to control severe hypercalcaemia. Such caution is, in part, driven by concerns about aggravating postoperative hypocalcaemia in a cohort of patients already at heightened risk of developing hungry bone syndrome; a postoper

First-line [18F]F-choline PET/CT in primary hyperparathyroidism: a cost-effectiveness study from the diagnostic randomized APACH2 trial.

BACKGROUND: The randomized controlled multicentre APACH2 trial showed that first-line [18F]F-choline (FCH1) PET/CT is superior to [99mTc]Tc-sestaMIBI (MIBI1) SPECT/CT for referring patients with primary hyperparathyroidism (pHPT) to effective minimally invasive parathyroidectomy (MIP). The aim of this study was to weigh this clinical gain against the increase in imaging costs. METHODS: The effectiveness criterion per imaging strategy was the rate of normocalcemia at one month post true-positive first-line imaging-guided MIP. The medico-economic analysis, carried out from the payer's perspecti

10-year cardiovascular disease risk and its contributing factors in primary hyperparathyroidism.

OBJECTIVE: Surgery indications for primary hyperparathyroidism (PHPT) mostly concern kidney and bone outcome, but not cardiovascular disease (CVD). Age less than 50 and eGFR less than 60 ml/min/1.73m2 are two surgery indications. This study aims to evaluate factors influencing the 10-year CVD risk in PHPT patients. METHOD: 159 PHPT patients diagnosed between January 2024 - March 2025 were enrolled. The relationships between biochemical and echocardiographic parameters with 10-year CVD risk score calculated by PREVENT™ were analyzed. Univariate, multivariate, logistic regression and receive ope

Long-term outcomes and prognostic factors following microwave ablation vs surgical resection for primary hyperparathyroidism.

OBJECTIVE: To compare the long-term efficacy and safety of microwave ablation (MWA) vs surgical resection (SR) in patients with primary hyperparathyroidism (PHPT) over five years. MATERIALS AND METHODS: This retrospective study evaluated 172 consecutive patients with PHPT (88 underwent MWA, 84 underwent SR) from 2015 to 2025. The median follow-up was 70.7 months (interquartile range [IQR], 42.6-96.3) in the MWA group and 77.7 months (IQR, 56.5-113.8) in the SR group. Primary outcomes included first-treatment success rate (biochemical normalization after a single procedure without additional i

Cinacalcet therapy in symptomatic 11-year-old girl with familial hypocalciuric hypercalcemia type 3.

Familial hypocalciuric hypercalcemia (FHH) is a rare genetic disorder of calcium regulation. FHH type 3 (FHH3), caused by pathogenic variants in AP2S1, may present with clinically significant complications. Data on the treatment of this rare disorder is limited. CASE DESCRIPTION: We report an 11-year-old girl with a de novo heterozygous pathogenic variant in AP2S1 (NM_004069.6): c.44G>T p.(Arg15Leu). At the age of seven, our patient presented with hypercalcemia, hypocalciuria, and elevated parathyroid hormone (PTH), which were accompanied by low bone mineral density and persistent neurodevelop

Primary Hyperparathyroidism From Concurrent Parathyroid Hyperplasia and Ectopic Parathyroid Adenoma: A Case Report.

Primary hyperparathyroidism (PHPT), whether caused by an adenoma or hyperplasia, can be curative with parathyroidectomy. However, persistently elevated parathyroid hormone (PTH) despite parathyroidectomy suggests multigland disease. We present a case of concurrent single-gland parathyroid hyperplasia and an ectopic parathyroid adenoma in a 72-year-old woman with long-standing PHPT. Despite persistently elevated calcium and PTH levels, all diagnostic imaging was negative for parathyroid hyperplasia and adenoma. PTH remained elevated despite initial parathyroidectomy for hypercellular tissue con

Neuropsychiatric manifestations of primary hyperparathyroidism.

Neuropsychiatric manifestations occur in about 25% of patients with primary hyperparathyroidism (PHPT). Symptoms can range from depression, anxiety, fatigue or cognitive dysfunction which are commonly observed to more seldomly revealed states of mania, delirium or psychosis which warrant psychiatric intervention. The underlying pathophysiology is likely multifactorial, potentially explained by elevated parathormone and hypercalcemia, with subsequent direct and indirect effects on monoamine neurotransmission and neuroinflammation via monoamine oxidase, tyrosine hydroxylase, sodium-potassium ade

Exceedingly Hyper-Secreting Parathyroid Adenoma: A Literature Review and Case Series.

BACKGROUND: Hypercalcemia is most commonly caused by hyperparathyroidism, which is often due to the presence of a parathyroid adenoma and more rarely, a parathyroid carcinoma. Distinguishing between the two can be challenging when serum calcium and parathyroid hormone (PTH) levels are severely elevated. Carcinomas classically present with markedly increased values and more severe clinical symptoms. METHODS: Clinical data was obtained via retrospective chart review and literature review. RESULTS: Three cases of female patients with symptomatic hypercalcemia from primary hyperparathyroidism wi

The impact of hyperparathyroidism on skeletal muscle pathophysiology and physical function.

Primary hyperparathyroidism (PHPT), increasingly diagnosed in its asymptomatic form, is associated with clinically significant neuromuscular dysfunction. Growing evidence indicates that skeletal muscle is a direct target of parathyroid hormone (PTH), with chronic PTH excess impairing mitochondrial bioenergetics, promoting proteolysis, and altering muscle-bone-adipose endocrine crosstalk. Experimental studies confirm PTH receptor type 1 (PTHR1) expression in muscle fibers and satellite cells, while transcriptomic analyses of PHPT muscle reveal dysregulation of calcium signaling and oxidative me

Recurrent uterine adenofibroma revealed to be hyperparathyroidism-jaw tumor syndrome: Case report.

Uterine adenofibroma was previously classified as a benign mixed epithelial and mesenchymal tumor, but the WHO removed it from its fifth edition due to diagnostic overlap with low-grade adenosarcoma or benign polyps. We report a case of a 21-year-old woman with a recurrent uterine polypoid tumor initially diagnosed as adenofibroma on biopsy and resected multiple times. MRI revealed heterogeneous intrauterine masses with cystic changes, and PET-CT showed weak FDG uptake. Eventually, hysterectomy confirmed adenofibroma. She also had a prior diagnosis of primary hyperparathyroidism caused by para

Three Malagasy Cases of Severe Bone Complications Revealing Primary Hyperparathyroidism.

Primary hyperparathyroidism (PHPT) is a metabolic disorder characterized by hypercalcemia with elevated or unsuppressed parathyroid hormone (PTH). It is rare in children but common in adults, particularly in women around the age of 50. In 85% of cases, PHPT is asymptomatic and is diagnosed following hypercalcemia and elevated PTH levels during routine examinations. PHPT occurring before the age of 25 and the normocalcemic phenotype are unusual situations that may delay diagnosis. These nonclassical forms expose patients to severe bone complications that can cause disability. Our objective is t

Incidence, risk factors and survival outcomes of post-transplant tertiary hyperparathyroidism in kidney recipients.

BACKGROUND: Post-transplant tertiary hyperparathyroidism (PT-tHPT) is a well-recognized complication following kidney transplantation, characterized by persistent excessive secretion of parathyroid hormone (PTH) despite improved renal function. It is potentially associated with an increased risk of cardiovascular events, renal osteodystrophy, pathologic fractures, graft loss, and mortality. AIM: To evaluate the incidence, risk factors, and outcomes of PT-tHPT amongst kidney transplant recipients. METHODS: A total of 887 transplant recipients who underwent transplantation between 2000 and 202

Magnesium in chronic hemodialysis.

INTRODUCTION AND AIM: Patients treated with hemodialysis (HD) are at increased risk for all-cause and cardiovascular (CV) mortality even after adjustments for traditional CV risk factors. The authors identified lower plasma magnesium (pMg) as a risk factor for arrhythmias and sudden death in people treated with HD. The aim of this study was to determine the connection between dialysate Mg concentrations (dMg) and the clinical characteristics of patients on chronic HD. MATERIALS AND METHODS: An observational study including 103 chronic HD patients. Patients were divided into two groups based o

Second Chances or Setbacks: The Impact of Parathyroidectomy on Graft Failure Post-Kidney Transplantation.

BACKGROUND: Tertiary hyperparathyroidism is a common complication in kidney transplant (KT) recipients, often persisting despite successful graft function. However, the optimal treatment for improving long-term graft survival remains unclear. Parathyroidectomy (PTX) offers a definitive approach to restoring calcium homeostasis. This study aims to assess the effect of PTX on graft failure, the need for dialysis, and renal osteodystrophy in the 2 groups. STUDY DESIGN: A retrospective analysis of electronic medical records from a multi-institutional database (TriNetX) was completed. Patients dia

Primary Hyperparathyroidism With Brown Tumor-A Case Mistaken as Non-Ossifying Fibroma Resolved by Biochemical and Radiological Correlation.

Primary hyperparathyroidism (PHPT) is uncommon in the pediatric population and rarely presents with overt skeletal complications such as brown tumors. This case study describes a 16-year-old female who initially presented with abdominal pain and was subsequently found to have severe hypercalcemia (serum calcium 16.4 mg/dL) and markedly elevated parathyroid hormone (PTH 1700 pg/mL) and bilateral nephrolithiasis. There were no features suggestive of multiple endocrine neoplasias. A history of a previously diagnosed "non-ossifying fibroma" in the proximal tibia was later reinterpreted as a brown

Radius Bone Mineral Density Measurement Is Essential During Initial PHPT Workup: Results of a Retrospective Study on 400 Patients.

BACKGROUND: Bone mineral density (BMD) measurement at the lumbar spine, femoral neck, total hip, and distal 1/3 radius is recommended for patients with primary hyperparathyroidism (PHPT). Osteoporosis at any site justifies surgery. The distal 1/3 radius is often neglected. This study evaluated the prevalence of distal 1/3 radius measurement, the proportion of osteoporosis at this site alone, and BMD changes 1 year post-parathyroidectomy in patients with PHPT. METHODS: A total of 548 patients who underwent parathyroidectomy for PHPT between 2016 and 2024 in University Hospital Center of Nantes
