Primary hyperparathyroidism in pregnancy successfully treated with microwave ablation: a case report.

BACKGROUND: Primary hyperparathyroidism (PHPT) during pregnancy is rare, with an estimated prevalence of <1% among pregnant women. While often asymptomatic, it may lead to serious maternal complications (nephrolithiasis, pancreatitis, preeclampsia) and adverse fetal outcomes (growth restriction, miscarriage, preterm delivery, stillbirth). Surgical parathyroidectomy in the second trimester remains the gold standard treatment, but may be unfeasible due to gestational age, contraindications, or patient refusal. In such situations, minimally invasive techniques such as microwave ablation (MWA) hav

Identifying risk factors for postoperative hypocalcemia in primary hyperparathyroidism: 12-year experience in a tertiary hospital.

BACKGROUND: Hypocalcemia is the most common complication following parathyroidectomy (PTX) in patients with primary hyperparathyroidism (PHPT). With asymptomatic PHPT cases increasingly comprising a larger proportion of clinical presentations, the association between preoperative symptom status and postoperative hypocalcemia risk remains poorly defined. This study aimed to identify risk factors for hypocalcemia after PTX in symptomatic vs. asymptomatic PHPT patients to enable individualized calcium management protocols. METHODS: A retrospective analysis was performed on patients with primary

Safety and efficacy of ultrasound-guided microwave ablation in patients with primary hyperparathyroidism: a single-center retrospective study.

BACKGROUND: Ultrasound (US)-guided microwave ablation (MWA) for primary hyperparathyroidism (PHPT) is a relatively novel minimally invasive treatment. However, definitive evidence for the efficacy of thermal ablation in treating PHPT is not well characterized. This work aimed to evaluate the effectiveness and safety of US-guided MWA in patients with PHPT. METHODS: This retrospective study analyzed the data of patients diagnosed with PHPT who underwent US-guided MWA at Peking University First Hospital between October 2020 and October 2024. Serum levels of parathyroid hormone (PTH), calcium, an

Case Report: Nephrocalcinosis from pancreatic hypoplasia in HNF1B disease: a multigenerational expression with genetic confirmation in the youngest generation.

BACKGROUND: HNF1B-associated disease is a genetic disorder caused by heterozygous pathogenic variants in the HNF1B gene, leading to a variety of clinical phenotypes primarily affecting the kidneys, pancreas, liver, and genitourinary tract. First identified in 1997 as the cause of MODY5 (maturity-onset diabetes of the young, type 5), the disease spectrum has since expanded to include kidney disease, pancreatic hypoplasia, genital malformations, gout, hypomagnesemia, liver abnormalities, and primary hyperparathyroidism. Variable expressivity and limited penetrance complicate diagnosis, with some

Primary hyperparathyroidism-associated pancreatitis: poor prognosis, distinct serum calcium trajectories, and proactive therapeutic implications.

BACKGROUND: Primary hyperparathyroidism (PHPT) is an uncommon but clinically significant cause of acute pancreatitis (AP). Its clinical course and prognosis remain inadequately characterized compared to common AP etiologies. METHODS: A bibliometric analysis was performed to evaluate global research trends on PHPT-associated acute pancreatitis (PHPT-AP). We conducted a retrospective cohort study at Peking Union Medical College Hospital between 2000-2025. Clinical data were compared with patients with biliary and hypertriglyceridemia-induced AP. Subgroup analyses evaluated differences between P

Cost-Effectiveness of 18F-Choline PET/CT Versus MIBI SPECT/CT for Preoperative Localization in Primary Hyperparathyroidism in the Colombian Healthcare System.

INTRODUCTION: Primary hyperparathyroidism (PHPT) is a common endocrine disorder, most often caused by a single parathyroid adenoma, for which parathyroidectomy is the treatment of choice. Accurate preoperative localization is essential for focused surgery. In Colombia, technetium-99 m sestamibi SPECT/CT (MIBI-SPECT/CT) is the standard, though limited by low sensitivity. 18F-choline PET/CT offers superior accuracy but at a higher cost. The only randomized trial comparing both modalities is the APACH2 study (Quak et al.). METHODS: We developed a cost-effectiveness study using three decision-ana

A novel missense variant in the calcium-sensing receptor gene in a dog with hypercalcemia.

A 5-year-old female spayed Border Collie cross was evaluated for a 3-year history of hypercalcemia. Abnormalities were not detected on physical examination. Parathyroid hormone (PTH) concentration was at the upper end of the reference interval, but diagnostic imaging did not identify any enlarged or ectopic parathyroid tissue. Hypercalcemia failed to resolve with unilateral parathyroidectomy, or administration of bisphosphonates, and calcimimetics. Despite hypercalcemia, urine calcium excretion was inappropriately low, consistent with Familial Hypocalciuric Hypercalcemia described in humans wi

Postparathyroidectomy calciphylaxis in a patient with primary hyperparathyroidism: a rare complication.

Calciphylaxis, a rare and life-threatening condition involving calcification of small to medium-sized arterioles in subcutaneous tissue, leads to ischemia and necrosis. While typically seen in chronic kidney disease (CKD), a small proportion of cases can present without CKD, referred to as nonuremic calciphylaxis (NUC). NUC associated with primary hyperparathyroidism is rare with limited evidence-based management. A 41-year-old woman with no prior medical history presented with cardiac arrest due to severe hypercalcemia of 17.87 mg/dL (4.46 mmol/L) (normal reference range, 8.6-10.3 mg/dL; 2.2-

Multiple endocrine neoplasia type 1 with MEN1 c.652C>T (p.Arg218Trp): variant reclassified to likely pathogenic.

Multiple endocrine neoplasia type 1 (MEN1) syndrome is the most common inherited cause of familial primary hyperparathyroidism. Diagnosis can be clinical, with at least 2 MEN1-associated tumors, or with 1 tumor plus a family history, or through genetic testing. A 31-year-old woman presented with hypercalcemia due to primary hyperparathyroidism. A 4-dimensional computed tomography (4D CT) and ultrasonography (US) of neck revealed 1 right-sided and 1 left-sided lesion consistent with parathyroid adenomas. Given her young age and a maternal history of primary hyperparathyroidism genetic testing w

Vitamin D Pretreatment to Prevent the Risk of Postoperative Hypocalcemic Complications After Parathyroidectomy in Primary Hyperparathyroidism: A Systematic Review and Meta-Analysis.

BACKGROUND: Parathyroidectomy is the treatment for primary hyperparathyroidism, yet postoperative hypocalcemia and hungry bone syndrome remain common. Vitamin D deficiency has been suggested as a modifiable risk factor, but evidence supporting preoperative supplementation is inconsistent. METHODS: Following PRISMA guidelines, we systematically searched five databases (up to January 2025) for studies evaluating vitamin D supplementation before parathyroidectomy in primary hyperparathyroidism. Data were extracted in duplicate, risk of bias assessed, and random-effects meta-analyses performed.

Primary Hyperparathyroidism in the Pediatric Population: Surgical Considerations and Outcomes: A Narrative Review.

Primary hyperparathyroidism (PHPT) in the pediatric population is a rare but clinically important endocrine disorder that poses significant diagnostic and therapeutic challenges. In contrast to adult PHPT, which is often detected incidentally, pediatric patients are frequently symptomatic at diagnosis, with manifestations reflecting prolonged exposure to hypercalcemia and elevated parathyroid hormone levels. Neonatal forms, particularly neonatal severe hyperparathyroidism, represent life-threatening conditions requiring prompt biochemical recognition and urgent intervention. The heterogeneity

Association of Intraoperative Parathyroid Hormone Decline with Early Postoperative Hypocalcemia: A Single-Center Retrospective Study.

Background/Objectives: Postoperative early hypocalcemia (PEH) is a key postoperative issue after parathyroidectomy in primary hyperparathyroidism. It often leads to long-lasting hypocalcemia, requiring more calcium and active vitamin D supplements. This study aimed to determine whether the extent of intraoperative parathyroid hormone (PTH) decline, measured 15 min after parathyroid tumor excision, could serve as a reliable intraoperative rule-out marker for PEH. Methods: We conducted a retrospective review of 88 adult patients who underwent surgical intervention for a solitary parathyroid tumo

Effects of Gender, Menopause, Vitamin D Status, and Tumor Parathyroid Cell Activity on Serum Phosphate Levels in a Large Cohort of Patients with Sporadic Hypercalcemic Primary Hyperparathyroidism.

Diagnosis of primary hyperparathyroidism (PHPT) relies on the detection of hypercalcemia and increased circulating parathormone (PTH) levels. However, the disease induces a deep deregulation of phosphate metabolism. A total of 960 PHPT patients (848 females, 112 males) were retrospectively enrolled; biochemical and clinical data were collected at PHPT diagnosis. At variance with previous studies, hypophosphatemia was diagnosed using sex- and age-specific serum phosphate reference range. Reduced serum phosphate levels were detectable in 49% of PHPT males and 55% of PHPT females. Moderate hypoph
