Mutational patterns in a large cohort of parathyroid carcinomas.

PURPOSE: A lack of targeted therapies make parathyroid carcinoma, a diagnostically challenging malignancy, difficult to treat. The rarity of this tumor type necessitates international collaboration to collect a sizable sample set for study. Prior studies have revealed the importance of driver mutations in the CDC73 gene and identified several putative drivers/aberrant pathways including PI3K/mTOR activation and CCND1 (cyclin D1) amplification. In this study, we sought to better understand the prevalence of putative oncogenic drivers in parathyroid carcinoma. METHODS: We subjected an expanded

The Effect of Prophylactic Protection of Inferior Parathyroid Gland Vein Blood Supply on Reducing Postoperative Hypoparathyroidism.

BACKGROUND: The objective of this study was to verify that a strategy involving prophylactic exposure of the inferior parathyroid glands and preservation of their venous blood flow-achieved through early identification and protection of their venous drainage-can enhance the success rate of maintaining glandular perfusion and effectively reduce the incidence of postoperative hypoparathyroidism. METHODS: This study included 600 patients who underwent total thyroidectomy with concomitant bilateral central neck dissection between November 2019 and December 2025. Patients were divided into two gro

Acromegaly as the Initial Presentation of Multiple Endocrine Neoplasia Type 1: A Case Report.

BACKGROUND Multiple endocrine neoplasia syndrome type 1 (MEN-1) is a rare autosomal-dominant disorder, caused by MEN-1 gene mutations, leading to tumors in multiple endocrine organs, classically the parathyroid glands, the anterior pituitary, and the pancreatic islet cells. Although primary hyperparathyroidism is usually the earliest manifestation, growth hormone-secreting pituitary adenomas rarely present as the initial clinical feature, potentially delaying recognition of the syndrome. CASE REPORT A 34-year-old woman presented with acromegalic features and hypercalcemia. Laboratory testing a

Case Report: A case of mediastinal parathyroid adenoma leading to hypercalcemic crisis complicated by acute kidney injury and postoperative hungry bone syndrome.

A 61-year-old man was admitted to the hospital with fatigue and hypercalcemia, with serum calcium levels as high as 6.16 mmol/L and parathyroid hormone (PTH) levels >5000 pg/mL, accompanied by progressively rising serum creatinine levels (150→266 μmol/L). A neck ultrasound revealed a small nodule in the left parathyroid region, but 99mTc-MIBI imaging identified a focal area of increased radiotracer uptake within the mediastinum. Following multidisciplinary consultation, a thoracoscopic mediastinal mass resection was performed, and pathology confirmed an ectopic parathyroid adenoma (3.2 × 2.8 ×

Disease-causing helix-8 mutations reprogram parathyroid hormone receptor-1 effector selectivity.

Helix-8 is an important determinant of G protein-coupled receptor (GPCR) effector selectivity, yet its functional relevance and contribution to human disease remain poorly understood. Two helix-8 mutations in parathyroid hormone receptor-1 (PTH1R), E4658.49K and E4698.53K, cause skeletal dysplasia, but their mechanistic effects on signaling were unclear. Using live-cell biosensors, structural modeling, and humanized knock-in mice, we show that these mutants reconfigure polar networks at the receptor's cytoplasmic face to reshape effector coupling. E465K shifts PTH1R signaling away from Gαs tow

Primary hyperparathyroidism in a 4-month-old German Shepherd mixed breed dog.

A 4-month-old female-intact German Shepherd mixed breed dog was evaluated for polyuria, polydipsia, oral discomfort, tachypnea, and pyrexia. Blood testing documented total and ionized hypercalcemia with an increased parathyroid hormone concentration. Computed tomography identified bone abnormalities of the mandibule and mild, diffuse pulmonary changes. Ultrasonography of the neck documented one mildly enlarged parathyroid gland that was surgically removed. Histopathology indicated diffuse chief cell hyperplasia. The ionized calcium concentration normalized within 7 days postoperatively.

Stage and age as distinct determinants of competing mortality in parathyroid carcinoma: a population-based analysis.

PURPOSE: Parathyroid carcinoma is a rare endocrine malignancy in which much of the long-term morbidity relates to chronic hypercalcemia. Conventional survival analyses treat deaths from causes other than the cancer as censoring events, which can distort estimates of cause-specific risk. We applied competing-risk methodology to characterize cause-specific mortality and to determine whether distinct clinical factors differentially influence cancer-specific and cardiovascular/renal mortality. METHODS: Patients with malignant parathyroid carcinoma (primary site C75.0) were identified from the SEE

Fine-needle-based localization techniques in parathyroid adenomas: a retrospective cohort study of histopathological safety and fibrosis patterns.

BACKGROUND: This study aimed to evaluate the association between fine-needle-based interventions widely used for preoperative and intraoperative localization of parathyroid adenomas and development of post-excision stromal fibrosis and other histopathological changes that may mimic malignancy. METHODS: A total of 164 patients who underwent parathyroidectomy for primary hyperparathyroidism were retrospectively evaluated. The patients were divided into three groups according to the number of ultrasound-guided fine-needle interventions: needle-naive (Group 0, n = 70), single-needle (Group 1, n =

[The 520th case: neck swelling, hypocalcemic convulsions, and spontaneous hemorrhage of a cystic parathyroid adenoma].

A 33-year-old man was admitted with a 4-day history of neck swelling and intermittent convulsions for 3 days. Physical examination revealed a soft, tender nodule measuring~4 cm×3 cm in the right lobe of the thyroid gland. Chvostek's and Trousseau's signs were positive. Laboratory tests revealed hypocalcemia, normal serum phosphorus and magnesium levels, elevated parathyroid hormone (PTH) levels, and reduced vitamin D levels. Bone turnover markers indicated active bone formation, and bone density testing revealed severe osteoporosis. Neck ultrasound and CT suggested a thyroid-origin mass; howev

The effect of routine intraoperative neuromonitoring on thyroidectomy complications: A propensity score-matched retrospective cohort study.

Intraoperative neuromonitoring (IONM) is commonly used during thyroid surgery to facilitate recurrent laryngeal nerve (RLN) identification and potentially reduce related postoperative complications. However, the efficacy of routine IONM in decreasing complication rates remains debated. In this study, we investigated the impact of IONM on postoperative complications after thyroidectomy. This retrospective single-center study included 214 thyroidectomy operations performed between 2021 and 2024. Patients were divided into 2 groups: Group 1 (n = 107) underwent surgery with routine IONM, and Group

The cross-cultural and chronological evolution of the kidney-bone relationship: from ancient medical insights to modern molecular endocrinology.

The kidney-bone axis constitutes a conserved bidirectional endocrine network that orchestrates skeletal remodeling and systemic mineral homeostasis. Its dysregulation underlies chronic kidney disease-mineral and bone disorder (CKD-MBD), renal osteodystrophy, secondary hyperparathyroidism, and associated vascular calcification, markedly elevating fracture and cardiovascular mortality risks. This review offers a unique cross-cultural and chronological synthesis of the kidney-bone relationship, bridging ancient medical insights with modern molecular endocrinology. Horizontally, we systematically

Single cell multi-omics guided hydrogel enables closed-loop therapy for renal osteodystrophy.

Fracture repair in renal osteodystrophy remains challenging because impaired bone quality compromises fixation stability, while current systemic therapies regulating bone metabolism do not sufficiently address the disease-specific mechanisms underlying defective repair. An integrated strategy combining early fracture stabilization with mechanism-guided local treatment is therefore needed. Here, integrated single-cell RNA sequencing and multi-omics analyses identify inflammatory macrophages as a key pathogenic population, characterized by glycolytic reprogramming driven by hypoxia-inducible fac

Progression of Secondary to Tertiary Hyperparathyroidism with a Coexisting Parathyroid Adenoma in Stage 3b Chronic Kidney Disease: A Case Report.

Tertiary hyperparathyroidism arises when prolonged secondary hyperparathyroidism progresses to autonomous parathyroid hormone secretion with persistent hypercalcemia, most often in chronic kidney disease. Diffuse or nodular multiglandular hyperplasia is the expected pathology; a discrete parathyroid adenoma arising within hyperplastic tissue is uncommon and may be overlooked when imaging is negative early in the disease course. A 60-year-old man with stage 3b chronic kidney disease and long-standing secondary hyperparathyroidism presented with recurrent hypercalcemia and severe vomiting. Labor

Cardiovascular Morbidity and Mortality in Chronic Hypoparathyroidism: A Systematic Review and Meta-Analysis.

Chronic hypoparathyroidism (HypoPT) may increase cardiovascular risk, yet current care rarely includes structured cardiovascular assessment. This systematic review and meta‑analysis aimed to quantify the association of chronic HypoPT with major cardiovascular events. Electronic databases and registries were searched from their inception through 10 June 2026 to identify controlled cohort studies of adults with chronic HypoPT and non-HypoPT comparators that reported cardiovascular outcomes. Random‑effects meta‑analyses (for hazard ratios [HRs]) were conducted for the primary and secondary outcom

Negative symptom dimensions link cognitive impairment to global and social functioning in 22q11.2 deletion syndrome.

BACKGROUND: 22q11.2 deletion syndrome (22q11DS) confers a 20-30% lifetime risk of schizophrenia, with negative symptoms associated with poorer outcomes and higher psychosis risk. High rates of neurocognitive deficits, autism spectrum traits (ASD), and attention-deficit/hyperactivity disorder (ADHD) may further complicate symptom profiles and functional outcomes. We examined relationships among positive and negative symptom severity, ASD traits, ADHD symptoms, and global functioning in 22q11DS. METHODS: Individuals with 22q11DS (n = 38) and healthy comparison subjects (n = 34) completed clinic

Late-onset atypical pseudohypoparathyroidism type 1A with intracranial calcification and epilepsy associated with a novel GNAS variant: a case report.

Pseudohypoparathyroidism (PHP) type 1A is a rare inherited disorder characterized by resistance to parathyroid hormone and, typically, features of Albright hereditary osteodystrophy. We report a late-onset case in a 59-year-old woman who presented with cognitive decline, behavioral and psychiatric symptoms, gait instability, and seizure-like episodes. Laboratory tests showed hypocalcemia, hyperphosphatemia, and elevated parathyroid hormone levels. Brain imaging revealed symmetrical calcifications in the bilateral basal ganglia, thalami, and periventricular regions, while electroencephalography
