Parathyroid-independent hypercalcemia as initial presentation of renal sarcoidosis with granulomatous interstitial nephritis: a case report.

BACKGROUND: Sarcoidosis is a multisystem granulomatous disorder predominantly affecting the lungs. Renal sarcoidosis often presents diagnostic challenges, PTH-independent hypercalcemia may be a clinical feature. CASE REPORT: A 48-year-old Asian female presented with persistent hypercalcemia (2.81 mmol/L), suppressed Parathyroid hormone PTH(1.24 pg/mL), and renal dysfunction (serum creatinine 2.02 mg/dL). Initial imaging showed mediastinal lymphadenopathy, 1.biopsy could not be performed as the size and location of the lymph nodes precluded safe conduct of the procedure, but bone marrow punctu

Cost-effectiveness of [¹¹C]Choline PET/CT as first-line imaging in primary hyperparathyroidism.

PURPOSE: We performed a cost-utility analysis, using prospectively gathered trial data, comparing two imaging strategies for localizing parathyroid adenomas in primary hyperparathyroidism (pHPT) to determine the most cost-effective approach. Additionally, we provide customizable open-source R-script enabling other centres to identify their optimal imaging strategy based on local diagnostic accuracy and cost data. METHODS: An evaluation of the diagnostic accuracy was performed for five imaging modalities: first-line cervical ultrasound (cUS) and [99mTc]Tc-methoxy isobutyl isonitrile-single-pho

Patient-Reported Outcome Measures Used in Primary Hyperparathyroidism: A Scoping Review.

BACKGROUND: Standardized measuring tools, such as patient-reported outcome measures (PROMs), are needed to sufficiently measure the effect of an intervention or to describe patterns of disease. The extent to which PROMs have been used in primary hyperparathyroidism (PHPT) research is undefined. We conducted this scoping review to delineate the current landscape of PROMs used in research involving patients with PHPT. METHODS: Six databases were systematically searched, and two independent reviewers identified all primary research articles that involved patients with PHPT and used PROMs. Charac

High normal parathyroid hormone and cardiovascular and total mortality: sex and age disparities in the ELSA-Brasil study.

BACKGROUND: Elevated parathyroid hormone (PTH) levels have been linked to higher cardiovascular (CV) and all-cause mortality in individuals with hyperparathyroidism or high-risk conditions (individuals with a mean age of 75-85 years and patients undergoing coronary angiography). However, it remains uncertain whether sex- and age-related differences influence these associations among normocalcemic and middle-aged adults from the general population. METHODS: We analyzed 4736 participants from the ELSA-Brasil study (mean age, 51.7 years; 55% females), followed for a median of 11.2 years. Baselin

The Fate of Normocalcemic Hyperparathyroidism: Insights from a 3-Year Multicenter Follow-Up.

Normocalcemic primary hyperparathyroidism is thought to be an early form of hypercalcemic primary hyperparathyroidism. However, some studies have shown progression to hypercalcemia, hypercalciuria or kidney stones, bone loss and fractures in a proportion of patients over time, whereas other studies have not observed such outcomes. The aim of this study was to investigate the clinicopathological features and natural history of the disease in a large patient population and to provide clinicians with strong evidence-based recommendations. This is a nationwide, multicenter, observational, retrospe

[Parapharyngeal salivary gland oncocytoma showing intense radiopharmaceutical accumulation identified during the evaluation of primary hyperparathyroidism by scintigraphy accompanied with SPECT/CT and its transoral endoscope-assisted removal].

Technetium-99m-MIBI (methoxy-isobutyl-isonitrile) is a radiopharmaceutical agent used in the evaluation of myocardial perfusion and parathyroid pathologies. MIBI scintigraphy is a commonly applied diagnostic method in the assessment of parathyroid adenomas for lesion localization and to exclude the presence of ectopic adenomas. However, tracer uptake may also occur in thyroid adenomas, thyroid carcinomas, and other lesions with high mitochondrial density. In our study, we present the case of a 79-year-old female patient. The patient had been under endocrinological care since 1997 due to osteop

Reevaluation of urinary calcium excretion as a criterion for surgery in primary hyperparathyroidism: a study on nephrolithiasis risk.

The guideline for primary hyperparathyroidism (PHPT) recommends surgery for female patients with 24-hour urinary calcium levels >250 mg/day and male patients >300 mg/day, due to their risk for nephrolithiasis. The purpose of this study is to validate this criterion. We analyzed data from 595 patients undergoing surgery for PHPT. We investigated whether high urinary calcium excretion is associated with nephrolithiasis in PHPT patients, along with the presence of other factors. Nephrolithiasis was present in 224 patients and absent in 371. The 24-hour urinary calcium levels did not differ signif

Clinical manifestations of injured tracheal and esophageal branches of the recurrent laryngeal nerve following thyroid surgery.

PURPOSE: Tracheal branch (TB) and esophageal branch (EB) injuries of the recurrent laryngeal nerve (RLN) during thyroid surgery are underexplored. In this study, we examined postoperative clinical manifestations in these injuries. METHODS: Using a head-mounted 2.5× magnifying loupe, TBs and EBs were isolated to assess branching patterns and postoperative outcomes. RESULTS: TBs comprised 1–3 branches, most commonly two, whereas EBs comprised 1–2 branches, usually one. Clinical symptoms were more frequent after TB injury than in controls, with 81.6% of patients showing chest tightness or dyspnea

Selected updates in thyroid, parathyroid, and adrenal gland pathology.

Endocrine pathology is continuously advancing with with new classification systems and an increased understanding the underlying pathogenesis and genetic alterations, whether somatic or germline, in endocrine diseases. Many changes have recently occurred in the classification of thyroid carcinoma, the most complex of which involve the new category of "high-grade follicular cell derived non anaplastic thyroid carcinoma" which encompasses both poorly differentiated thyroid carcinomas and differentiated high-grade thyroid carcinomas. The word "hyperplasia" is no longer used with primary multiglan

A Randomized Trial Evaluating Intraoperative Ischemic Preconditioning of Parathyroid Glands During Total Thyroidectomy: A Signal for Earlier Parathyroid Function Recovery.

BACKGROUND: Ischemic preconditioning of parathyroid glands (IPCP) is biologically plausible but clinical evidence is limited. In this single-center randomized trial (ChiCTR2000039788), we compared IPCP versus control during total thyroidectomy with central neck dissection. METHODS: A total of 135 patients with differentiated thyroid carcinoma were analyzed (IPCP, n = 67; control, n = 68). IPCP consists of three cycles of 60-second occlusion followed by 60-second reperfusion of the ipsilateral superior and inferior thyroid arteries. The baseline characteristics, postoperative hypoparathyroidis

Severe Hypercalcemia Following Pembrolizumab Therapy: A Case Report and A Literature Review.

INTRODUCTION/BACKGROUND: Immune checkpoint inhibitors (ICIs) play a central role in advanced cancer treatment, but they are associated with immune-related adverse events (irAEs) that include two cases of hypercalcemia induced by the programmed death-1 (PD-1) inhibitor, pembrolizumab. We report a unique case of a colon cancer patient treated with pembrolizumab who acutely developed life-threatening hypercalcemia. CASE PRESENTATION: This case presents a seventy-five-year-old woman suffering from colon adenocarcinoma with liver metastasis undergoing pembrolizumab therapy. Shortly after its secon

A novel homozygous inactivating mutation of the calcium-sensing receptor causing familial hypocalciuric hypercalcemia complicated with primary hyperparathyroidism due to parathyroid adenoma: A case report.

Familial hypocalciuric hypercalcemia is an autosomal dominant genetic disorder characterized by mild to moderate hypercalcemia, mild hypermagnesemia, and normal or inappropriately elevated parathyroid hormone levels. Familial hypocalciuric hypercalcemia 1 is the most prevalent form of familial hypocalciuric hypercalcemia and is typically caused by heterozygous loss-of-function mutations in the calcium-sensing receptor (CaSR) gene. Homozygous CaSR mutations are more commonly associated with neonatal severe primary hyperparathyroidism. We report the case of a female patient in her early 40s harb

Use and Implications of the Fracture Risk Assessment Tool in Primary Hyperparathyroidism.

IMPORTANCE: Although primary hyperparathyroidism (PHPT) increases the risk of osteoporotic fractures, estimation of fracture probability among patients with PHPT remains challenging. Use of the Fracture Risk Assessment Tool (FRAX) among these patients remains poorly characterized, and its utility in identifying patients who may experience fracture risk reduction following parathyroidectomy (PTX) remains unknown. OBJECTIVE: To examine the calibration of FRAX in adults with PHPT and to describe the association between estimated probability and differential fracture risk with PTX compared with n

A Simple Blood Count Marker for a Complex Diagnosis: Neutrophil-to-Lymphocyte Ratio in Atypical Parathyroid Adenomas.

INTRODUCTION: Preoperative differentiation of atypical parathyroid adenomas (APAs) from typical parathyroid adenomas (TPAs) is challenging because of their overlapping histopathological features. Simple biomarkers, such as the neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR), have been associated with tumor aggressiveness in various endocrine tumors; however, their role in APA remains unclear. METHODS: This retrospective cohort study included 149 patients who underwent parathyroidectomy for primary hyperparathyroidism between July 2020 and May 2024 at our institutio

Addition of Preoperative Ultrasound to 4DCT Increases Radiologist Confidence in Identifying Hyperfunctioning Parathyroid Glands.

BACKGROUND AND PURPOSE: 4D CT (4DCT) is a key imaging technique for preoperative localization of parathyroid adenomas in primary hyperparathyroidism. Ultrasound (US) is frequently added with the assumption that it enhances diagnostic performance. This study aimed to determine whether incorporating US into a 4DCT-based evaluation improves diagnostic accuracy and to assess its impact on radiologists' diagnostic confidence. MATERIALS AND METHODS: In this retrospective study, we reviewed 80 patients with primary hyperparathyroidism who underwent both 4DCT and US followed by curative parathyroidec

Spontaneous hemorrhage from a functionally well-controlled, non-enlarged but hyperplastic parathyroid gland due to secondary hyperparathyroidism: a case report.

Chronic kidney disease causes secondary hyperparathyroidism (SHPT) with hyperplastic enlarged parathyroid glands. Deep cervical spontaneous hemorrhage has been occasionally reported to be caused by rupture of an enlarged parathyroid gland under functionally uncontrolled SHPT. We encountered an unusual case of life-threatening spontaneous hemorrhage from a parathyroid gland in a patient undergoing hemodialysis. A 40-year-old man with end-stage renal disease received renal replacement therapy for 12 years. He had SHPT, which was functionally well controlled using multiple medications. The parath

[Endocrine emergencies].

An endocrine crisis is a hormonal disorder that leads to death if left untreated. The fundamental importance of endocrine regulatory circuits for homeostasis is also evident in critical illnesses-as a consequence of another disease or as a cause. In fact, the clinical presentation of a crisis is often nonspecific and therefore differs from the classical presentation of endocrine disorders. This review summarizes intensive care aspects of crises due to glucocorticoid deficiency (adrenal crisis) or excess (Cushing's crisis), excess thyroid hormone (thyroid storm), and hypercalcemic crisis (parat

Early Postoperative Intact Parathyroid Hormone Dynamics After Curative Parathyroidectomy for Sporadic Primary Hyperparathyroidism: Trajectories, Predictors of Eucalcemic PTH Elevation, and 18-Month Outcomes.

OBJECTIVE: Eucalcemic postoperative parathyroid hormone elevation (PePTH) after curative parathyroidectomy for sporadic primary hyperparathyroidism is common, yet its temporal profile and clinical significance remain unclear. Transient hypoparathyroidism also occurs after surgery, but the immediate course of intact PTH (iPTH) recovery is poorly defined. We therefore characterized day-by-day postoperative calcium, phosphate, and iPTH trajectories through postoperative day (POD) 5 and evaluated preoperative predictors of PePTH at 6 months and 18-month biochemical outcomes. METHODS: In this pros

Preoperative CALLY Index for Identifying Atypical Parathyroid Tumors.

BACKGROUND: Atypical parathyroid tumors (APTs) are diagnostically challenging neoplasms that display worrisome histopathological features but do not meet the criteria for carcinoma. Conventional biochemical and imaging parameters have limited discriminatory accuracy, and no validated preoperative biomarker exists to identify APT reliably. This study evaluated the diagnostic performance of the C-reactive protein-albumin-lymphocyte (CALLY) index as a composite immunonutritional marker for distinguishing APT from typical parathyroid adenomas (TPA). METHODS: This retrospective observational study
