INTRODUCTION: Chronic kidney disease (CKD) has devastating effects on the skeletal system, resulting in an increased fracture risk. CKD results in the buildup of metabolites in the blood (often referred to as uremic toxins), and several uremic toxins have been linked to adverse skeletal outcomes. We have shown that dietary fiber inulin reduced uremic toxins and had a positive effect on bone. Here, we determine whether combining inulin with parathyroid hormone (PTH)-lowering therapies could improve skeletal properties beyond either therapy alone.
METHODS: An animal model of CKD-mineral bone di

The crosstalk between immune or alveolar epithelial cells and fibroblasts mediated by paracrine signaling molecules is associated with the pathogenesis of idiopathic pulmonary fibrosis (IPF). However, studies investigating the active involvement of soluble mediators derived from bronchial epithelial cells in fibroblast activation and the development of pulmonary fibrosis are limited. Reanalysis of bulk and single-cell RNA-sequencing data from human bronchus and IPF lung tissue revealed marked upregulation of parathyroid hormone-like hormone (PTHLH) in IPF lung tissue compared with normal tissu

Central Nervous System (CNS) injuries represent a major cause of long-term neurological disability and mortality worldwide, encompassing stroke, Traumatic Brain Injury (TBI), and Spinal Cord Injury (SCI). Despite distinct etiologies, these conditions share convergent secondary injury processes that critically shape disease progression and long-term outcomes. Alternative Splicing (AS), a unique post-transcriptional regulatory mechanism, has recently been recognized to play crucial roles in brain injury, to be dynamically activated following central nervous system (CNS) injury, and to contribute

Fibroblast growth factor 23 (FGF23) and α-Klotho play crucial roles in the pathogenesis of chronic kidney disease-mineral and bone disorder (CKD-MBD) due to their regulatory effects on phosphorus, calcium, parathyroid hormone, and calcitriol levels. Despite their importance, few studies have examined urinary concentrations of these compounds in feline chronic kidney disease (CKD), thereby limiting our understanding of their roles in disease progression. This study aimed to evaluate the urinary levels of FGF23 (uFGF23) and soluble α-Klotho (uKL) in cats at various stages of CKD, including acute

BACKGROUND: Primary hyperparathyroidism (pHPT) is infrequently associated with calcium-containing kidney stones, despite hypercalciuria. Parathyroidectomy (PTX) is the only curative treatment of the metabolic disorder and is indirectly regarded as prophylaxis of recurrence of stone formation. The aim of the systematic review was to evaluate the impact of PTX on stone recurrence and to identify possible predictors of recurrent stone formation.
METHODS: Following PRISMA guidelines, a systematic PubMed search was conducted through April 2025. Eligible studies including adults with successfully s

The Viennese Medical School was at the forefront of the clinical understanding of the relationship between parathyroid and bone metabolism. A century ago, F. Mandl (1925) and E. Gold (1927) described two patients clinically (osteitis fibrosa cystica) and biochemically (hypercalcemia, hypercalciuria) with a disease which Gold had already termed (primary) hyperparathyroidism (PHPT) in 1928. The two patients were successfully treated by removing one enlarged parathyroid gland each. After the clinical and biochemical diagnosis was confirmed, surgery was recommended. In 1933, Mandl summarized his r

The molecular mechanisms causing heritable disorders of hypo- and hyperphosphatemia involving the osteocyte-derived hormone fibroblast growth factor 23 (FGF23) and its co-receptor αKlotho (KL) have sprung new concepts underlying the endocrine control of phosphate and calcium (Ca), as well as the regulation of the active form of vitamin D, 1α,25-dihydroxy vitamin D3 (1,25D). These critical developments have proven important for the understanding and treatment of both rare, Mendelian diseases as well as for important implications for common disorders of blood phosphate excess and dysregulated 1,

ABSTRACT: PURPOSE: Primary hyperparathyroidism has been associated with subclinical neuromuscular dysfunction; however, the clinical correlates of muscle strength and morphology in PHPT remain undercharacterized. We hypothesized that muscle dysfunction in primary hyperparathyroidism is reversible after parathyroidectomy and that the reduction in serum calcium predicts early postoperative improvement.
ABSTRACT: DESIGN: Prospective observational study conducted at a tertiary care center.
ABSTRACT: METHODS: Fifty-three primary hyperparathyroidism patients and 53 age-, sex-, and body mass index-

Progressive loss of kidney function in chronic kidney disease and end-stage kidney disease leads to distinct pathological changes in mineral and bone metabolism. Historically, the management of these patients focused on lowering their serum phosphate, parathyroid hormone, and normalizing serum calcium. However, persistently high morbidity and mortality of these patients from cardiovascular diseases and after bone fractures call for more specific therapies. Here, we summarize established and emerging therapies for managing the three main domains of mineral bone disorder in patients with advance

CONTEXT: Placental calcium (Ca2+) transport is essential for foetal bone mineralisation and development, as well as for Ca2+ homoeostasis. Rare mutations in transient receptor potential vanilloid (TRPV) 6 of the infant cause insufficient maternal-foetal Ca2+ transport through the placenta. Although there is a limited number of cases, phenotype analyses of these patients show skeletal deformities and hyperparathyroidism. In this study, we identified a case with a homozygous mutation in the TRPV6 channel that leads to the deletion of the Ca2+ pore.
PATIENT MEASUREMENTS: The neonate was born wit

Background/Objectives: Hypoparathyroidism (HPT) is a disorder caused by the insufficient production of parathyroid hormone (PTH). Its main features include decreased serum calcium, increased serum phosphorus, and abnormal bone modeling. In children, HPT is most commonly due to genetic disorders. Among rare genetic syndromes that can include HPT in their clinical spectrum is Kenny-Caffey syndrome (KCS) type 2. Conventional therapy for HPT primarily consists of oral calcium and active vitamin D metabolites. The major limitation of conventional therapy is hypercalciuria with an increased risk of

Background: Both primary hyperparathyroidism (PHPT) and chronic hypoparathyroidism (HypoPT) are associated with the onset and development of cardiovascular diseases (CVDs). In particular, PHPT is accompanied by the presence of elevated atherothrombotic risk, while the importance of traditional and new anthropometric indices to reflect the cardiovascular risk remains uncertain in this condition. This study aims to investigate whether novel and traditional anthropometric indices distinguish PHPT and whether these indices are correlated with atherothrombotic risk. Methods: A total of 40 subjects

BACKGROUND: The optimal extent of thyroidectomy remains controversial. Total thyroidectomy (TT) offers definitive disease control but it may raise the complications and completion thyroidectomy (CT) following initial lobectomy might lower the morbidity. This meta-analysis and systematic review compared TT and CT using endocrine and neurological outcomes in the postoperative period. METHODS: We searched databases of PubMed, Scopus, Embase, Web of Science and Google Scholar in accordance with PRISMA and until September 1, 2025. Primary Failure of Eruption (PFE) is a disorder characterized by aberrant tooth eruption, in which one or more teeth fail to follow the physiological eruptive pathway and remain partially or completely embedded within the bone or soft tissues. Although the etiopathogenesis of PFE is not yet fully elucidated, several contributing factors have been identified, including genetic alterations, hormonal disturbances, and systemic conditions. An expanding body of evidence points to the centrality of genetic determinants in the etiopathogenesis of PFE, supporting its occurrence in both syndromic conte

Background and Objectives: Hypocalcemia due to hypoparathyroidism (HypoPTH) is the most common complication following thyroid surgery, typically resulting from iatrogenic removal, tissue damage, or compromised vascularization of the parathyroid glands. Patients with persistent HypoPTH are at risk for long-term complications such as osteoporosis, cardiac dysfunction, and renal impairment. Lifelong regulation of calcium levels is therefore essential to prevent morbidity and mortality associated with these complications. In this study, we aimed to evaluate the functional engraftment efficacy of 3

A 12-year-old spayed female Korean shorthair cat was evaluated for mediastinal and cervical masses. A serum biochemistry analysis revealed hypercalcemia and hypophosphatemia, with normal parathyroid hormone (PTH) levels. Diagnostic imaging showed multicystic cervical and mediastinal masses with increased opacity in the left caudal lung field, suggestive of partial lung collapse. Abdominal imaging was unremarkable. Cytology of the mediastinal mass suggested an epithelial or neuroendocrine tumor. Hypercalcemia remained unresolved following bisphosphonate therapy; the cat was euthanized 290 days
