New insights into the pathogenesis of osteoporosis and the translational approach to personalized therapy.

Osteoporosis is increasingly recognized as a heterogeneous syndrome rather than a single disease, driven by diverse pathophysiological mechanisms. Subtypes-including postmenopausal estrogen deficiency, age-related bone loss, anorexia nervosa, and GLP-1 receptor agonist-associated weight loss-exhibit distinct bone turnover, fracture patterns, and therapeutic responses. Recent studies have established bone marrow adipose tissue (BMAT) as a critical regulator of the marrow microenvironment in postmenopausal and age-related bone loss. Weight loss of various origins-anorexia, GLP-1 agonists, calori

Type-specific detection and quantification of circulating tumor HPV DNA by digital PCR in surgically resected cervical cancer.

OBJECTIVE: Circulating tumor human papillomavirus (HPV) DNA (ctHPV DNA) has shown clinical utility in advanced cervical cancer; however, its role in surgically resected disease remains unclear. We evaluated the detectability and clinicopathological associations of ctHPV DNA in preoperative plasma from patients with surgically resected cervical cancer using digital polymerase chain reaction (dPCR). METHODS: We analyzed 49 surgically resected cervical cancer patients. HPV genotyping of formalin-fixed paraffin-embedded tumor tissues was performed using both PGMY09/11 consensus PCR and novel geno

Primary Hyperparathyroidism Presenting as a Distal Femur Pathologic Fracture in an Adolescent: A Case Report.

CASE: A 17-year-old adolescent boy presented with a pathologic distal femur fracture and bilateral distal femoral metadiaphyseal lytic lesions. Multidisciplinary evaluation confirmed bilateral distal femoral brown tumors associated with primary hyperparathyroidism secondary to a parathyroid adenoma. Treatment included bone biopsy, curettage, grafting, and retrograde locked intramedullary nail, followed by definitive partial parathyroidectomy. The contralateral metadiaphyseal lesion was treated with corrective osteotomy and prophylactic intramedullary stabilization. CONCLUSION: Primary hyperpa

Indocyanine green (ICG) guided thyroidectomy versus conventional thyroidectomy and the impact on parathyroid function preservation: an updated GRADE-assessed systematic review and meta-analysis.

BACKGROUND: Thyroidectomy carries a significant risk of parathyroid gland injury, with transient hypoparathyroidism occurring in 15-50% of cases and permanent hypoparathyroidism in up to 5%, resulting in lifelong morbidity. Near-infrared (NIR) autofluorescence imaging combined with indocyanine green (ICG) fluorescence angiography has emerged as an intraoperative adjunct for parathyroid identification and perfusion assessment, but its comparative efficacy over conventional white-light surgery remains to be comprehensively quantified. METHODS: A systematic search across five databases through J

FGF-23 is associated with increased risk of abdominal aortic calcification: A Mendelian randomization study with phosphate as a potential mediator.

Fibroblast growth factor 23 (FGF23) is involved in the regulation of phosphate metabolism and has been linked to vascular calcification. However, the genetic association between FGF23 and abdominal aortic calcification (AAC) remains unclear. We conducted a bidirectional two-sample Mendelian randomization (MR) study to evaluate the association between genetically predicted circulating FGF23 levels and AAC. Univariable MR was performed to assess the primary association, followed by multivariable MR adjusting for serum calcium, 25-hydroxyvitamin D, parathyroid hormone, and chronic kidney disease.

Sex differences in the clinical presentation and complications of primary hyperparathyroidism: insights from a single-center cohort study.

PURPOSE: Primary hyperparathyroidism (PHPT) predominantly affects women, particularly after menopause. Whether sex-related differences extend beyond epidemiology to influence the clinical expression of the disease remains incompletely understood. We investigated sex-related differences in the clinical phenotype of PHPT. METHODS: We retrospectively analyzed clinical, biochemical, imaging, and histopathological data from patients with PHPT referred to our Endocrinology Unit over a 5-year period. RESULTS: A total of 315 patients were included (64 men and 251 women; female-to-male ratio 3.9:1).

Indocyanine green fluorescence during total thyroidectomy: utility for predicting hypoparathyroidism and safety assessment.

PURPOSE: Parathyroid fluorescence with indocyanine green (ICG) might prevent and predict postoperative hypoparathyroidism (hypoPT) after total thyroidectomy (TT). We aimed to evaluate parathyroid gland vascularization with ICG during TT. Further, we aimed to evaluate safety of ICG fluorescence and to correlate changes in PTH with postoperative hypoPT after TT. METHODS: From May 2021 to June 2023, consecutive patients underwent TT and parathyroid fluorescence with ICG. Each visible parathyroid gland was scored 0, 1 or 2 depending on fluorescence intensity. Patients with at least one gland scor

Parathyroid Surgery for Primary Hyperparathyroidism in 2026.

Primary hyperparathyroidism (PHPT) is the most common parathyroid disease and one of the most common endocrine disorders. It results from autonomous overproduction of parathyroid hormone, which in the classic form of the disease causes hypercalcaemia. The ready access to biochemical testing in high-income countries has moved the disease spectrum from a classic symptomatic presentation to a biochemical anomaly that is asymptomatic or minimally symptomatic. Untreated PHPT, however, remains associated with significant consequences. This review summarizes up-to-date concepts in the diagnosis, imag

Surgery versus observation in patients 65 years or older with primary hyperparathyroidism: randomized clinical trial.

BACKGROUND: A substantial portion of surgically treated patients with primary hyperparathyroidism (pHPT) are older than 65 years. The indication for parathyroidectomy in older patients with pHPT and mild hypercalcaemia is unclear. Specifically, it is not known whether parathyroid surgery improves bone mineral density (BMD) in this age group. METHOD: Patients 65 years or older with pHPT, ionized calcium not exceeding 1.50 mmol/l, and a BMD Z-score above -2.5 standard deviations who were referred to the Department of Surgery, Lund University Hospital, were included in the study. Follow-up was p

22q11.2 Deletion Syndrome: A Comprehensive Review for Pediatricians From Diagnosis to Adult Care Transition.

Affecting ~1 in 4000 to 6000 live births, 22q11.2 deletion syndrome (22q11.2DS) is the most common chromosomal microdeletion syndrome. It results from a 1.5 to 3 Mb heterozygous deletion at chromosome 22q11.2 and is associated with a broad spectrum of clinical manifestations, including cardiac defects, palatal abnormalities, thymic hypoplasia, developmental delays, and neuropsychiatric symptoms. Multiple techniques such as chromosomal microarray and sequencing techniques such as fluorescence in situ hybridization help in the diagnosis of 22q11.2DS. A multidisciplinary approach to management is

Autosomal recessive HOXA3 deficiency causes congenital athymia and laryngeal malformation.

BACKGROUND: Approximately 10% of patients with Severe Combined Immunodeficiency (SCID) phenotype lack a known genetic cause. In particular, the molecular basis of thymic defects is poorly understood. Homeobox (HOX) genes encode conserved transcription factors that control spatial body development. The function of human HOXA3 is undefined. OBJECTIVE: We characterize the first human with HOXA3 deficiency and investigate the role of HOXA3 in immune system development. METHODS: We performed clinical and immunological phenotyping, as well as genetic sequencing. A homozygous nonsense variant in HO

Ethnic-Racial Identity and Alcohol Use Moderated by Family Factors among Diverse Emerging Adults.

Defining biomarkers that predict therapeutic effects and adverse events is a crucial mandate to guide patient selection for personalized cancer treatments. In the present study, we applied a pharmacometabolomics approach to identify biomarkers potentially associated with pathological complete response to trastuzumab-paclitaxel neoadjuvant therapy in HER-2 positive breast cancer patients. Based on histological response the 34 patients enrolled in the study were subdivided into two groups: good responders (n = 15) and poor responders (n = 19). The pre-treatment serum targeted metabolomics profil

Endocrine Hypertension in Children and Adults: Beyond Primary Aldosteronism.

Endocrine hypertension refers to elevated blood pressure resulting from an identifiable hormonal cause that is potentially treatable. Endocrine hypertension occurs in ∼30% of young adults with hypertension (18-40 years), in 30-50% of patients with resistant hypertension, most commonly due to primary aldosteronism, and can occur in a small yet clinically significant subset of children with hypertension. Other causes of endocrine hypertension include excess mineralocorticoid production, cortisol excess or impaired cortisol metabolism, disorders of the mineralocorticoid receptor or epithelial sod

High-Resolution Quantification of Two-Way Nanobody Synergy Using Automated Liquid Handling and Computational Modeling.

Evaluating single-domain antibody cooperativity is essential for developing potent, escape-resistant antiviral biologics. Here, we present a protocol that reproducibly quantifies functional synergy between neutralizing nanobody pairs in standardized viral infectivity assays. Controlled automated liquid handling prepares two-dimensional concentration matrices, minimizing pipetting variance and systematic error. Neutralization data are fitted using quantitative models that independently estimate potency, cooperativity, and efficacy to distinguish additive, synergistic, and antagonistic effects b

Aperiodic EEG spectral features as consistent indicators of Parkinson's disease: a machine learning approach utilizing fitting oscillations and one-over-F decomposition.

A major challenge in Parkinson's disease (PD) research is the identification of non-invasive neurophysiological markers that can distinguish PD patients from healthy controls while providing interpretable information about cortical dysfunction. This study investigates whether aperiodic EEG spectral parameters extracted using Fitting Oscillations and One-Over-F decomposition can serve as candidate indicators for exploratory PD classification and medication-state analysis. Resting-state EEG data from 15 PD patients and 16 healthy controls were analyzed. For PD participants, both medicated and un

Nonconsensual sexting experiences are associated with bullying and online gossiping victimization among early adolescents in Nicaragua.

BACKGROUND: Nonconsensual sexting experiences are a form of image-based sexual abuse that can have serious consequences for the victim. Prior work has found that the forwarding of sexts can happen in negative peer contexts, such as bullying and online gossiping victimization. This study aims to examine the associations between: (1) two distinct types of nonconsensual sexting experiences (i.e. receiving unsolicited forwarded sexts and having one's own sexts forwarded without consent), and (2) bullying victimization and online gossiping victimization among early adolescents in Nicaragua, whose e

Giant Intrathyroidal Parathyroid Carcinoma Presenting as an Asymptomatic Hypercalcemia: A Case Report and Literature Review.

Parathyroid carcinoma (PC) is a rare endocrine malignancy characterized by uncontrolled secretion of parathyroid hormone (PTH). This article presents a case of a giant PC that was entirely located within the thyroid gland and was initially misdiagnosed as a thyroid nodule. Our case is noteworthy for the absence of clinical symptoms and the markedly elevated serum calcium and PTH levels. We provide a detailed description of the patient's imaging findings and systematically review the diagnostic and therapeutic modalities of PC.
