A PTH Value at 6 Hours Postsurgery Predicts the Diagnosis of Transient and Permanent Hypoparathyroidism.

CONTEXT: PTH+ levels after thyroid surgery are generally used to detect patients at risk of developing postoperative hypoparathyroidism. However, there is still a lack of consensus about the threshold value regarding its evaluation, the definition of gland function recovery, and the classification of hypoparathyroidism as permanent. OBJECTIVE: PTH levels (determined 6 hours after total thyroidectomy) could be effective for early prediction of the risk of postsurgical hypocalcemia and IV calcium requirements during hospitalization, comparing it with the predictive capacity of serum calcium lev

Exploring the mechanism of Yishen Huayu formula in intervening chronic kidney disease-mineral and bone disorder via the miR-340-5p/FGF-23/klotho signaling pathway.

BACKGROUND: Chronic kidney disease-mineral and bone disorder (CKD-MBD) is a severe complication of chronic kidney disease characterized by dysregulation of the FGF-23/Klotho axis. MicroRNA-340-5p (miR-340-5p) participates in this signaling pathway. Nevertheless, its functional role in CKD-MBD, as well as the underlying mechanism by which Yishen Huayu Formula (YSHYF) intervenes in this disease, remains unclear. METHODS: A rat model of CKD-MBD was established using nephrectomy combined with adenine administration and a high-phosphate diet. Indicators of renal function, renal histopathology, ren

Parathyroidectomy Restores Muscle Strength and Transcriptome in Individuals With Primary Hyperparathyroidism.

CONTEXT: Primary hyperparathyroidism leads to hypercalcemia and muscle dysfunction. Muscle weakness is associated with increased morbidity and mortality but is overlooked in surgical treatment guidelines. While parathyroidectomy is the only curative treatment, its effects on skeletal muscle strength and molecular remodeling remain underexplored. OBJECTIVE: Determining functional and molecular changes in skeletal muscle before and after parathyroidectomy. METHODS: A prospective observational study was conducted in the spring and fall of 2023. Patients underwent surgery at the Endocrine and Sa

Multimodal Imaging and Clinicopathologic Features of Parathyroid Lipoadenoma.

Parathyroid lipoadenoma (PLA) is a rare cause of primary hyperparathyroidism and is over 50% adipose tissue, which complicates preoperative localization. We aimed to describe clinical and imaging features of PLA in this case series. We retrospectively reviewed 4 patients with pathologically confirmed PLA and biochemical evidence of primary hyperparathyroidism. All patients exhibited elevated PTH and upper-normal to elevated calcium levels. PLAs demonstrated hyperechogenicity on ultrasonography, fat-equivalent attenuation on CT, and increased uptake on sestamibi SPECT/CT. Surgical excision resu

The dynamic changes and precise classification of parathyroid function within 1 year after thyroid cancer surgery.

BACKGROUND: Postoperative parathyroid dysfunction is a common complication of thyroid surgery. However, the temporal pattern of postoperative parathyroid function and standardized clinical interventions remains poorly defined. This study systematically characterizes the dynamic changes in postoperative parathyroid hormone (PTH) following thyroid cancer surgery and proposes a clinically applicable classification system with corresponding management strategies. MATERIALS AND METHODS: A total of 12 664 patients who underwent thyroid cancer surgery between 2012 and 2022 across three tertiary hosp

4D Parathyroid CT Using Photon-Counting Detector CT.

4D parathyroid CT is a multiphase protocol used in the setting of hyperparathyroidism to detect parathyroid adenomas, largely identified by their density patterns relative to the thyroid and enlarged nonlymphoid soft tissue in the central compartment. This technique has previously been described using traditional energy-integrating detector CT. We present our protocol using photon-counting detector CT as a means of using increased spatial and contrast resolution to help identify parathyroid adenomas.

Impact of Preoperative Calcium and Magnesium Supplementation on Quality of Life and Hypocalcemia Post-Thyroidectomy.

OBJECTIVE: Postoperative hypocalcemia and hypoparathyroidism are common complications after thyroidectomy, often impairing quality of life (QoL). This study investigates the impact of preoperative calcium and magnesium supplementation on postoperative QoL and hypocalcemia in patients undergoing total thyroidectomy for symptomatic nodular goitre or Graves' disease. METHODS: A total of 62 patients undergoing thyroidectomy for benign thyroid diseases were randomised into two groups. The intervention group (IG, n = 31) received 500 mg calcium carbonate thrice daily and 300 mg magnesium carbonate

Cardiovascular Involvement in Primary Hyperparathyroidism.

Increased serum calcium with elevated or nonsuppressed PTH levels is the biochemical hallmark of primary hyperparathyroidism (PHPT). A large body of evidence linked both hypercalcemia and high serum levels of PTH with cardiovascular diseases, due to their actions on cardiomyocytes, endothelial, pancreatic β, and fat cells. However, controversial epidemiological data exist regarding the higher prevalence of hypertension, arrhythmias, dyslipidemia, echocardiographic, and vascular parameter impairments in PHPT patients compared to controls. Different cardiovascular risk factors related to sex, ag

A novel ultrasound-based nomogram using contrast-enhanced and conventional ultrasound features to improve preoperative diagnosis of parathyroid adenomas versus cervical lymph nodes.

OBJECTIVES: Precise preoperative localization of parathyroid gland lesion is essential for guiding surgery in primary hyperparathyroidism (PHPT). The aim of our study was to investigate the contrast-enhanced ultrasound (CEUS) characteristics of parathyroid gland adenoma (PGA) and to evaluate whether PGA can be differentiated from central cervical lymph nodes (CCLN). METHODS: Fifty-four consecutive patients with PHPT were retrospectively enrolled and underwent preoperative imaging with high-resolution ultrasound (US) and CEUS, and underwent subsequent parathyroidectomy. Aberrant Histone Methylation Profile in Patients with Sporadic Primary Hyperparathyroidism.

INTRODUCTION: Primary hyperparathyroidism (PHPT) is a systemic endocrine disorder characterized by elevated PTH levels and hypercalcemia. Gene-specific epigenetic modifications like histone methylation play a pivotal role in PHPT by altering expression of parathyroid-specific genes. However, global histone modifications in parathyroid tumors and their implications for tumor behavior remain underexplored. EXPERIMENTAL DESIGN: We performed comparative histone modification profiling in blood and tissue samples from sporadic parathyroid adenomas (PA; n = 30), atypical parathyroid tumors (APT; n =

Current Knowledge on Atypical Parathyroid Tumors and Emerging Strategies for Risk Stratification.

Atypical parathyroid tumors (APTs) are an uncommon subset of parathyroid neoplasms that carry substantial clinical relevance because of their histological and clinical resemblance to parathyroid cancer. Despite this importance, the diagnosis of APTs remains persistently challenging, even for experienced pathologists. Their histopathological features overlap extensively with those of parathyroid cancer, and at present, no specific immunohistochemical markers are available that can reliably distinguish between these entities. Moreover, the clinical manifestations of APTs are indistinguishable fr

Phosphorus Metabolism and Function in Ruminants: Current Knowledge.

Phosphorus (P) is a key macromineral essential for numerous physiological processes, including energy metabolism, skeletal mineralization, and cellular signaling. In dairy cattle, its homeostasis is tightly regulated by parathyroid hormone, calcitriol, and fibroblast growth factor 23 (FGF23). Phosphorus deficiency is common during the transition period and is associated with reduced milk yield, anemia, muscle weakness, and immunosuppression in severe or prolonged cases. This review summarizes the current understanding of phosphorus metabolism in ruminants, emphasizing differences from monogast

Impact of Parathyroidectomy on Vascular Stiffness: Exploring Pulse Wave Velocity and Renal Resistive Index.

CONTEXT: Primary hyperparathyroidism (PHPT), characterized by elevated parathyroid hormone (PTH) and hypercalcemia, has been implicated in vascular calcification and increased arterial stiffness. Carotid-femoral pulse wave velocity (PWV) is the gold-standard marker of large artery stiffness, while the Augmentation Index (AI), pulse pressure (PP), and Renal Resistive Index (RRI) offer insights into wave reflection and microvascular function. OBJECTIVE: This study aimed to assess the associations between PTH levels and these vascular parameters before and after parathyroidectomy (PTX). METHODS

Is Parenteral Bisphosphonate Therapy Safe and Effective in the Management of Severe Primary Hyperparathyroidism Prior to Parathyroidectomy?

Primary hyperparathyroidism induced hypercalcaemic crisis is defined as an albumin- adjusted calcium ≥3.50 mmol/L, occurring in the presence of organ dysfunction. This endocrine emergency requires medical optimisation prior to urgent parathyroidectomy. However, few guidelines exist on the most appropriate course of treatment, including the use of preoperative bisphosphonates to control severe hypercalcaemia. Such caution is, in part, driven by concerns about aggravating postoperative hypocalcaemia in a cohort of patients already at heightened risk of developing hungry bone syndrome; a postoper

First-line [18F]F-choline PET/CT in primary hyperparathyroidism: a cost-effectiveness study from the diagnostic randomized APACH2 trial.

BACKGROUND: The randomized controlled multicentre APACH2 trial showed that first-line [18F]F-choline (FCH1) PET/CT is superior to [99mTc]Tc-sestaMIBI (MIBI1) SPECT/CT for referring patients with primary hyperparathyroidism (pHPT) to effective minimally invasive parathyroidectomy (MIP). The aim of this study was to weigh this clinical gain against the increase in imaging costs. METHODS: The effectiveness criterion per imaging strategy was the rate of normocalcemia at one month post true-positive first-line imaging-guided MIP. 10-year cardiovascular disease risk and its contributing factors in primary hyperparathyroidism.

OBJECTIVE: Surgery indications for primary hyperparathyroidism (PHPT) mostly concern kidney and bone outcome, but not cardiovascular disease (CVD). Age less than 50 and eGFR less than 60 ml/min/1.73m2 are two surgery indications. This study aims to evaluate factors influencing the 10-year CVD risk in PHPT patients. METHOD: 159 PHPT patients diagnosed between January 2024 - March 2025 were enrolled. The relationships between biochemical and echocardiographic parameters with 10-year CVD risk score calculated by PREVENT™ were analyzed. Univariate, multivariate, logistic regression and receive ope
