Delusion of Pregnancy: An Updated Systematic Review.

Delusion of pregnancy (DoP) is a rare psychopathological symptom characterised by the fixed, delusional conviction of being pregnant in the absence of objective evidence. The systematic review by Bera and Sarkar remains, to this day, the principal quantitative reference on the phenomenon, based on 84 cases published up to August 2014. The aim of the present work is to update this case series by including literature published between September 2014 and August 2026, and to analyse any clinical and aetiological differences that emerge relative to the original sample. Thirty-two new cases were ide

Predictive Factors for Diabetes Insipidus and Syndrome of Inappropriate Secretion of Antidiuretic Hormone After Transsphenoidal Surgery for Pituitary Adenoma.

Objective The objective of this study is to determine preoperative predictors of diabetes insipidus (DI) and syndrome of inappropriate secretion of antidiuretic hormone (SIADH) after transsphenoidal surgery (TSS) for pituitary adenomas. DI and SIADH are common complications associated with TSS. Methods This small unicentric retrospective study was conducted in a tertiary referral hospital in Saudi Arabia. The study enrolled 42 patients. We reviewed the medical records of all patients who underwent surgical resection of pituitary adenoma at our hospital between January 2010 and December 2022. T

Risk factors for idiopathic granulomatous mastitis: scoping review.

BACKGROUND: Idiopathic granulomatous mastitis (IGM) is a rare, benign, inflammatory breast condition. The diagnostic challenge can cause delays in diagnosis and treatment. The quality of evidence for the diagnosis and management of IGM is low. A scoping review of existing literature was conducted to determine risk factors for IGM, with the aim of informing clinical suspicion, earlier diagnosis, and treatment of this condition. METHODS: A scoping review of the MEDLINE, Embase, and Cochrane Central Register of Controlled Trials (CENTRAL) databases (2003-2023) was performed following PRISMA-ScR

Body Mass Index-Related Changes in Tibial Morphology: Considerations for Intramedullary Nailing.

Glypican-3 (GPC3) mRNA and protein are expressed in >80% of human hepatocellular carcinomas (HCC) but not in normal tissues except for placenta and fetal liver. The oncofetal antigen GPC3 is a glycosylphosphatidyl inositol-anchored membrane protein and may be secreted. It is a novel tumor marker for human HCC: GPC3 protein was present in sera from 40-50% of HCC patients, but was not detected in sera from patients with liver cirrhosis or chronic hepatitis, or in sera from healthy individuals. alpha-Fetoprotein (AFP) and PIVKA-II (protein induced by vitamin K absence or antagonist-II), are well

Desert Hedgehog mediates stem Leydig cell differentiation through Ptch2/Gli1/Sf1 signaling axis.

Desert Hedgehog (Dhh) mutations cause Leydig cell dysfunction, yet the mechanisms governing Leydig lineage commitment through Dhh-mediated receptor selectivity, transcriptional effector specificity, and steroidogenic coupling remain elusive. In this study, using CRISPR/Cas9-mediated gene knockout and stem Leydig cells (SLCs) transplantation, we identified a critical Dhh/Patched 2 (Ptch2)/Glioma-associated oncogene homolog 1 (Gli1)/steroidogenic factor 1 (Sf1) signaling axis essential for SLC differentiation in Nile tilapia (Oreochromis niloticus). Dhh deficiency resulted in defective adult Ley

Radiological and Functional Outcomes of Joshi External Stabilization System (JESS) in Proximal Tibia Fractures: A Case Series.

INTRODUCTION: External fixators have a limited role in the management of tibial plateau fractures (TPF). They are typically used as an initial management strategy. The Joshi External Stabilisation System (JESS) is a Kirschner wire-based fixator and has been used in TPF where plating may not be feasible due to poor skin conditions or delayed due to other soft tissue complications. This retrospective case series aimed to evaluate the radiological and functional outcomes of TPF managed using the JESS fixator. MATERIALS AND METHODS: Records of patients operated on with the JESS fixator for TPF be

Acute neurological Behçet's disease complicated by a syndrome of inappropriate secretion of antidiuretic hormone.

We report a rare case of acute neurological Behçet's disease (neuro-BD) diagnosed concurrently with systemic Behçet's disease (BD), classified as definite BD, and complicated by syndrome of inappropriate antidiuretic hormone secretion (SIADH). A man in his 60s presented with fever, headache and mucocutaneous lesions including oral aphthae, folliculitis-like rashes, genital ulcers and hyponatraemia. He was diagnosed with BD based on mucocutaneous findings. Brain MRI showed no abnormalities, and cerebrospinal fluid analysis revealed aseptic meningitis, supporting the diagnosis of acute neuro-BD.

Correlation Between Endocrine and Other Clinical Factors with Peripapillary Retinal Nerve Fiber Layer Thickness After Surgical Treatment of Pediatric Craniopharyngioma.

Background: Visual dysfunction resulting from damage to the optic nerve and retinal neurons represents a significant concern in the postoperative management of childhood-onset craniopharyngioma (CP) survivors. The study aims to evaluate the influence of clinical parameters assessed in patients before and after neurosurgery of CP on peripapillary retinal nerve fiber layer (RNFL) thickness results, using optical coherence tomography (OCT) as early markers of compressive neuropathy. Methods: This study retrospectively examined 73 eyes from 38 individuals diagnosed with CP and 64 eyes from 32 heal

An Uncommon Link: Syndrome of Inappropriate Antidiuretic Hormone Secretion (SIADH) in the Setting of Myeloproliferative Neoplasms.

Syndrome of inappropriate antidiuretic hormone secretion (SIADH) is a well-known cause of hyponatremia, frequently associated with small-cell lung carcinoma. Its association with myeloproliferative neoplasms (MPNs), however, is rare and underreported. We describe the case of an 81-year-old female with a history of chronic hyponatremia attributed to SIADH, requiring multiple hospitalizations. During her hospitalizations, she would be on fluid restriction and urea tablets. Her medical history included paroxysmal atrial fibrillation, a stable pulmonary nodule, and seropositive rheumatoid arthriti

Multiple endocrine neoplasia with an atypical clinical course and a MEN1 gene variant of uncertain pathogenicity: A case report.

RATIONALE: Multiple endocrine neoplasia type 1 (MEN1) is characterized by the coexistence of multiple endocrine tumors, most commonly due to autosomal dominant mutations in the tumor suppressor gene MEN1. Because this neoplastic disease is rare, diagnosis is often challenging. Some patients remain undiagnosed because of absent or nonspecific symptoms, while others may die before a correct diagnosis is established. PATIENT CONCERNS: An 88-year-old man presenting with progressive dementia and heat stroke was admitted to our hospital. Close examinations revealed syndrome of inappropriate secreti

Beyond the Classic: Atypical Dengue Presentations.

Dengue is an arthropod-borne viral infection caused by the dengue virus and spread by the vector Aedes mosquitoes. The clinical spectrum of dengue includes undifferentiated fever, dengue fever, dengue hemorrhagic fever, dengue shock syndrome, and expanded dengue syndrome (EDS). The patients may present with atypical symptoms and involve organs like the liver, kidney, brain, heart, bone marrow, and central nervous system. Three atypical dengue cases belonging to the EDS that had presented to the Emergency Department of a tertiary care center in Kerala, South India, are described in the case ser

Recurrent Acute-on-Chronic Hyponatraemia: A Diagnostic Odyssey Involving Drug-Induced Syndrome of Inappropriate Antidiuretic Hormone Secretion (SIADH), Chronic Cannabis Use, and Occult Lung Adenocarcinoma.

Hyponatraemia is a common clinical presentation and one of the most frequently encountered electrolyte abnormalities in hospitalised patients, with syndrome of inappropriate antidiuretic hormone secretion (SIADH) representing a major underlying cause. Clinical manifestations are variable, with confusion being a common presenting feature. While hyponatraemia is often readily identified and managed, recurrent or complex presentations may reveal multiple coexisting aetiologies, including malignancy, medication-related causes, substance misuse, and neuropsychiatric disorders. Pain and Proximal Leg Weakness: An Atypical Presentation of Guillan-Barré Syndrome.

Guillain-Barré syndrome (GBS) is an acute, rapidly progressive immune-mediated polyradiculoneuropathy that is a neurologic emergency given the risk of mortality associated with respiratory failure. This report adds to the growing literature on atypical early presentations of GBS.  A 63-year-old man initially presented to the ED with a two-day history of diffuse myalgias and intermittent 'sharp zingers' lasting a few seconds. These started in his hands and quickly spread to his upper and lower extremities. He had recently recovered from an upper respiratory infection. Lab workup was notable for

Cabergoline monotherapy in GH- and PRL-cosecreting pituitary adenomas.

UNLABELLED: Functional pituitary tumours, also called pituitary neuroendocrine tumours (PitNETs), typically produce a single hormone, based on the cell lineage from which they arise in the anterior pituitary. Pituitary adenoma dual hypersecretion is less common and may present management dilemmas. Prolactin (PRL) and growth hormone (GH) co-secreting pituitary adenomas (GH and PRL-PA) are the most common form of pituitary adenoma dual hypersecretion, arising from PIT1-lineage PitNETs, most commonly mammosomatotroph and mixed somatotroph-lactotroph adenomas. Pharmacotherapies employed in prolact

Sex- and Age-Specific Metabolic Outcomes of Prolactin Normalisation in Hyperprolactinaemia.

BACKGROUND: Hyperprolactinaemia has been linked to adverse metabolic profiles, but the metabolic consequences of prolactin (PRL) normalisation remain insufficiently characterised. OBJECTIVE: To evaluate changes in anthropometric, blood pressure, and metabolic parameters following PRL normalisation, with analyses stratified by sex and age. METHODS: We retrospectively analysed 445 patients (286 women, 159 men) treated with cabergoline who had paired clinical and biochemical measurements before and after PRL normalisation. Pre-post differences were assessed overall and stratified by sex and age

Genetic Variations in the Dopaminergic Signaling Pathways and Their Implications for Antipsychotics Pharmacogenetics.

The role of neurotransmitter systems in the pathology and management of psychiatric disorders is well documented. Targeting these systems has remained the mainstay of standard pharmacological interventions, with typical and atypical antipsychotics demonstrating effectiveness in reducing both positive and negative symptoms of schizophrenia. However, their clinical efficacy and side effect profiles vary among individuals due to genetic differences. This review aims to examine key genetic variants in the dopaminergic neurotransmitter system that influence antipsychotic response, with particular e

Women and psychosis: a guide to evidence-based, hormone-informed care.

Psychosis is associated with sex differences that are rarely considered in routine care. Oestrogen exerts protective effects, with symptom exacerbation and increased relapse risk occurring during low-oestrogen states, such as perimenstruation, postpartum, and menopause transition. Antipsychotic pharmacokinetics and response vary by sex and hormonal status, with premenopausal women requiring lower doses, and postmenopausal women showing reduced treatment efficacy. Antipsychotic-induced hyperprolactinaemia can suppress endogenous oestrogen, compounding both mental and physical health risks. Desp

Oncogenic driver and therapeutic target: Prolactin signalling axis in retroperitoneal sarcoma.

BACKGROUND: Retroperitoneal sarcoma (RPS) is a type of malignant tumour arising from mesenchymal tissues within the retroperitoneal space. RPSs tend to develop covertly and are often undiscovered when they have already grown significantly and invaded surrounding tissues and organs. These malignancies demonstrate high recurrence rates, present surgical challenges and exhibit limited responsiveness to radiotherapy and chemotherapy. Serum-derived molecules are known to play critical roles in tumourigenesis and tumour progression. However, the serum molecular profile of RPS patients remains unclea

Diagnostic delay of prolactinoma: Focus on clinical manifestations.

Prolactinomas are the most common functioning pituitary adenomas and can present with a broad spectrum of manifestations. As pituitary adenomas grow insidiously and often result in nonspecific symptoms, diagnostic delay is common. The duration of symptoms first reported by the patient to diagnosis can vary greatly, from patients being diagnosed within one year to over 40 years after symptom onset. Early detection of pituitary adenomas is essential to avoid the accumulation of both hyperprolactinemia and mass effect manifestations, such as visual field loss, infertility, and fatigue among other
