Delusion of Pregnancy: An Updated Systematic Review.

Delusion of pregnancy (DoP) is a rare psychopathological symptom characterised by the fixed, delusional conviction of being pregnant in the absence of objective evidence. The systematic review by Bera and Sarkar remains, to this day, the principal quantitative reference on the phenomenon, based on 84 cases published up to August 2014. The aim of the present work is to update this case series by including literature published between September 2014 and August 2026, and to analyse any clinical and aetiological differences that emerge relative to the original sample. Thirty-two new cases were ide

Risk factors for idiopathic granulomatous mastitis: scoping review.

BACKGROUND: Idiopathic granulomatous mastitis (IGM) is a rare, benign, inflammatory breast condition. The diagnostic challenge can cause delays in diagnosis and treatment. The quality of evidence for the diagnosis and management of IGM is low. A scoping review of existing literature was conducted to determine risk factors for IGM, with the aim of informing clinical suspicion, earlier diagnosis, and treatment of this condition. METHODS: A scoping review of the MEDLINE, Embase, and Cochrane Central Register of Controlled Trials (CENTRAL) databases (2003-2023) was performed following PRISMA-ScR

Body Mass Index-Related Changes in Tibial Morphology: Considerations for Intramedullary Nailing.

Glypican-3 (GPC3) mRNA and protein are expressed in >80% of human hepatocellular carcinomas (HCC) but not in normal tissues except for placenta and fetal liver. The oncofetal antigen GPC3 is a glycosylphosphatidyl inositol-anchored membrane protein and may be secreted. It is a novel tumor marker for human HCC: GPC3 protein was present in sera from 40-50% of HCC patients, but was not detected in sera from patients with liver cirrhosis or chronic hepatitis, or in sera from healthy individuals. alpha-Fetoprotein (AFP) and PIVKA-II (protein induced by vitamin K absence or antagonist-II), are well

Radiological and Functional Outcomes of Joshi External Stabilization System (JESS) in Proximal Tibia Fractures: A Case Series.

INTRODUCTION: External fixators have a limited role in the management of tibial plateau fractures (TPF). They are typically used as an initial management strategy. The Joshi External Stabilisation System (JESS) is a Kirschner wire-based fixator and has been used in TPF where plating may not be feasible due to poor skin conditions or delayed due to other soft tissue complications. This retrospective case series aimed to evaluate the radiological and functional outcomes of TPF managed using the JESS fixator. MATERIALS AND METHODS: Records of patients operated on with the JESS fixator for TPF be

Pelvic Stability During Total Hip Arthroplasty: A Comparison of Traction and Standard Surgical Tables Using Augmented Reality Navigation.

Intestinal parasites are known to cause changes in blood count parameters and immunological markers among various populations. Prison inmates often have poor living conditions and limited access to healthcare, thus infections with intestinal parasites could negatively impact on their overall health and wellbeing. This study assessed the effects of intestinal parasitic infections on the haemato-immunological profile of prison inmates in Southern Ghana. A cross-sectional study using stratified sampling technique was conducted among 347 male prison inmates in three major prisons in Southern Ghana

Inverse Vaccine Nanomimics: In situ Antigen-Specific Immune Tolerance Induction for Durable Cardiac Repair Post-Myocardial Infarction.

INTRODUCTION: Uptake of penicillin prophylaxis to prevent recurrent rheumatic fever and its sequela rheumatic heart disease (RHD) is not optimal in the Northern Territory of Australia. The Full Moon Strategy (the Strategy) was introduced in the Central Australian region in June 2006 to improve the uptake of prophylactic penicillin: clients and healthcare workers were encouraged to use the full moon as a cue for the timing of the 4 weekly prophylactic penicillin injection. OBJECTIVE: To determine the impact and effectiveness of the Strategy on knowledge and uptake of benzathine penicillin prop

Cabergoline monotherapy in GH- and PRL-cosecreting pituitary adenomas.

UNLABELLED: Functional pituitary tumours, also called pituitary neuroendocrine tumours (PitNETs), typically produce a single hormone, based on the cell lineage from which they arise in the anterior pituitary. Pituitary adenoma dual hypersecretion is less common and may present management dilemmas. Prolactin (PRL) and growth hormone (GH) co-secreting pituitary adenomas (GH and PRL-PA) are the most common form of pituitary adenoma dual hypersecretion, arising from PIT1-lineage PitNETs, most commonly mammosomatotroph and mixed somatotroph-lactotroph adenomas. Pharmacotherapies employed in prolact

Sex- and Age-Specific Metabolic Outcomes of Prolactin Normalisation in Hyperprolactinaemia.

BACKGROUND: Hyperprolactinaemia has been linked to adverse metabolic profiles, but the metabolic consequences of prolactin (PRL) normalisation remain insufficiently characterised. OBJECTIVE: To evaluate changes in anthropometric, blood pressure, and metabolic parameters following PRL normalisation, with analyses stratified by sex and age. METHODS: We retrospectively analysed 445 patients (286 women, 159 men) treated with cabergoline who had paired clinical and biochemical measurements before and after PRL normalisation. Pre-post differences were assessed overall and stratified by sex and age

Genetic Variations in the Dopaminergic Signaling Pathways and Their Implications for Antipsychotics Pharmacogenetics.

The role of neurotransmitter systems in the pathology and management of psychiatric disorders is well documented. Targeting these systems has remained the mainstay of standard pharmacological interventions, with typical and atypical antipsychotics demonstrating effectiveness in reducing both positive and negative symptoms of schizophrenia. However, their clinical efficacy and side effect profiles vary among individuals due to genetic differences. This review aims to examine key genetic variants in the dopaminergic neurotransmitter system that influence antipsychotic response, with particular e

Women and psychosis: a guide to evidence-based, hormone-informed care.

Psychosis is associated with sex differences that are rarely considered in routine care. Oestrogen exerts protective effects, with symptom exacerbation and increased relapse risk occurring during low-oestrogen states, such as perimenstruation, postpartum, and menopause transition. Antipsychotic pharmacokinetics and response vary by sex and hormonal status, with premenopausal women requiring lower doses, and postmenopausal women showing reduced treatment efficacy. Antipsychotic-induced hyperprolactinaemia can suppress endogenous oestrogen, compounding both mental and physical health risks. Desp

Oncogenic driver and therapeutic target: Prolactin signalling axis in retroperitoneal sarcoma.

BACKGROUND: Retroperitoneal sarcoma (RPS) is a type of malignant tumour arising from mesenchymal tissues within the retroperitoneal space. RPSs tend to develop covertly and are often undiscovered when they have already grown significantly and invaded surrounding tissues and organs. These malignancies demonstrate high recurrence rates, present surgical challenges and exhibit limited responsiveness to radiotherapy and chemotherapy. Serum-derived molecules are known to play critical roles in tumourigenesis and tumour progression. However, the serum molecular profile of RPS patients remains unclea

Diagnostic delay of prolactinoma: Focus on clinical manifestations.

Prolactinomas are the most common functioning pituitary adenomas and can present with a broad spectrum of manifestations. As pituitary adenomas grow insidiously and often result in nonspecific symptoms, diagnostic delay is common. The duration of symptoms first reported by the patient to diagnosis can vary greatly, from patients being diagnosed within one year to over 40 years after symptom onset. Early detection of pituitary adenomas is essential to avoid the accumulation of both hyperprolactinemia and mass effect manifestations, such as visual field loss, infertility, and fatigue among other

Brexpiprazole mitigates hyperprolactinaemia induced by paliperidone in a patient with a history of skin reaction to aripiprazole: case report introducing a new therapeutic approach.

Hyperprolactinaemia is a common adverse effect of antipsychotic medication, primarily resulting from dopamine D2 receptor blockade. It is characterised by menstrual irregularities, gynaecomastia, galactorrhoea and, with prolonged exposure, an increased risk of osteoporosis and breast cancer. When switching towards a prolactin-sparing antipsychotic is not feasible, adjunctive use of aripiprazole - a dopamine D2 partial agonist - has emerged as a validated strategy to mitigate hyperprolactinaemia without compromising antipsychotic efficacy. However, alternative options are needed for patients wh

Impulse control disorders and their relationship with psychopathology in patients treated with cabergoline for hyperprolactinaemia.

PURPOSE: Dopamine agonists (DAs), such as cabergoline (CAB), are first-line treatments for hyperprolactinaemia but have been associated with impulse control disorders (ICD). This observational study aimed to investigate the prevalence, severity, and clinical characteristics of ICD in CAB-treated patients compared with controls, using a combined clinician-administered and multimethod self-report assessment. METHODS: A total of 126 participants were included: 63 patients with prolactin-secreting pituitary adenomas receiving cabergoline (CAB) and 63 controls (CG). ICD were evaluated using the Pa

Long-term endocrine trajectories and reoperation risk after surgery for Rathke's cleft cysts: a single-center cohort study of 177 patients.

PURPOSE: To characterise endocrine trajectories after surgery for Rathke's cleft cysts (RCCs) and identify factors associated with pituitary hormonal dysfunction and reoperation. METHODS: Retrospective single-centre cohort of 177 patients undergoing RCC surgery (2004-2023). Preoperative MRI volumetry, cyst location, histopathology, and longitudinal endocrine assessments were analysed. Early postoperative MRI defined residual cyst status. Reoperation-free survival was assessed by Kaplan-Meier and multivariable Cox regression. RESULTS: Patients were 67.8% women (mean age 44.8 ± 17.0 years); me

Pituitary adenoma-associated with Schöpf-Schulz-Passarge syndrome.

Hidrocystomas are benign cystic skin tumours of sweat gland origin which often involve the eyelids but are usually solitary. We report the case of a middle-aged man with multiple bilateral eyelid hidrocystomas, palmoplantar keratoderma, hypodontia and nail dystrophy, in keeping with a diagnosis of Schöpf-Schulz-Passarge syndrome, an exceedingly rare ectodermal dysplasia. Hyperprolactinaemia was found on testing, and radiology revealed an incidental prolactinoma. Raised serum prolactin has not been previously associated with Schöpf-Schulz-Passarge syndrome. The significance of raised serum prol

Association Between Ergot-Derived Dopamine Agonists and Valvular Heart Disease: A Systematic Review and Meta-Analysis.

Dopamine agonists (DAs) have been linked to valvular heart disease (VHD). Literature suggests worsening valvular regurgitation with usage of these drugs, although some reports have found no significant association. This systematic review and meta-analysis aimed to determine whether treatment with ergot-derived DAs is associated with an increased incidence of VHD or worsening of preexisting valvular regurgitation. A systematic review and meta-analysis was conducted by synthesising data from multiple observational studies and clinical trials, focusing on the effects of both ergot-derived and non

Hyperprolactinaemia masked by the hook effect in a patient with a giant prolactinoma with growth hormone co-secretion.

We report a woman in her 50s with a giant co-secreting pituitary macroadenoma producing both prolactin and growth hormone. She presented with headache, proptosis, chemosis and secondary amenorrhoea since her 20s. Workup showed hyperprolactinaemia (251 μg/L; normal ≤23 μg/L). A brain MRI showed a giant mass at the base of the skull. Repeat prolactin assay after serum dilution showed marked hyperprolactinaemia (386 020 μg/L), consistent with a giant prolactin-secreting tumour. Additional tests established the presence of growth hormone excess. She was treated with cabergoline, leading to substan

Persistent Rhinosinusitis-Like Symptoms Revealing a Lactotroph Pituitary Neuroendocrine Tumour: A Case Report.

Pituitary neuroendocrine tumours (PitNETs) commonly present with symptoms related to mass effect or hormonal dysfunction, including headache, visual disturbances, and endocrine abnormalities. However, atypical presentations without neurological or hormonal "red flag" symptoms may delay diagnosis, particularly in primary care settings where benign upper respiratory conditions are more frequently encountered. We report the case of a 56-year-old postmenopausal woman who presented with a five-month history of persistent postnasal drip, chronic non-productive cough and a sensation of nasal congesti
