Newly Diagnosed Graves' Disease Followed by the Discovery of a Cavernous Internal Carotid Artery Aneurysm Causing Panhypopituitarism: A Case Report.

Sellar and parasellar vascular lesions are rare causes of pituitary dysfunction and may mimic pituitary adenomas or other sellar masses. Cavernous internal carotid artery (ICA) aneurysms can cause hypopituitarism through compression of the pituitary gland or stalk, but diagnosis may be delayed when symptoms are initially attributed to a more common endocrine disorder. We report a 37-year-old man who presented with six weeks of unintentional weight loss, fatigue, heat intolerance, excessive sweating, anxiety, tremor, and tachycardia. Initial evaluation showed suppressed thyroid-stimulating horm

Multiply recurrent pituitary apoplexy treated nonsurgically: illustrative case.

BACKGROUND: Pituitary apoplexy is rare clinical entity resulting from hemorrhage or infarction of a pituitary tumor, typically presenting with headaches, endocrinopathy, and variable neurological manifestations. Recurrent apoplexy, particularly multiply recurrent cases, is exceedingly rare and associated with clinical equipoise in many cases. OBSERVATIONS: The authors report the case of an 85-year-old female who presented with three distinct episodes of pituitary apoplexy over 6 years, each presenting with acute headache in the absence of neurological deficits. Given the patient's age, lack o

MRI Features in Children with Pituitary Stalk Interruption Syndrome: Associations with Hormonal Phenotype and Intracranial Anomalies.

RATIONALE AND OBJECTIVES: Pituitary stalk interruption syndrome (PSIS) is a rare congenital disorder with variable endocrine and magnetic resonance imaging (MRI) phenotypes, and the relationship between MRI features, hormonal phenotype, and associated intracranial anomalies remains incompletely understood. This study evaluated MRI features associated with hormonal phenotype and intracranial anomalies in pediatric PSIS. MATERIALS AND METHODS: This retrospective study included 51 pediatric patients with biochemically confirmed pituitary hormone deficiency and MRI findings of PSIS. Pituitary sta

Diagnostic challenges of masked arginine vasopressin deficiency in neurosarcoidosis complicated by sodium-glucose cotransporter 2 inhibitor use.

A wide variety of endocrine disorders caused by sarcoidosis have been reported to date. We report the case of an 82-year-old male patient who presented with hypercalcemia and anterior pituitary hormone deficiency at the time of his sarcoidosis diagnosis and was subsequently diagnosed with arginine vasopressin deficiency (AVP-D) 6 months after the initiation of glucocorticoid therapy. Magnetic resonance imaging revealed an enlarged pituitary gland, thickened pituitary stalk, and loss of the posterior pituitary bright spot, raising the suspicion of hypothalamic-pituitary sarcoidosis. Despite imp

Endocrine therapies and hormonal dysfunction after traumatic brain injury: A scoping review.

BACKGROUND: Traumatic brain injury (TBI) is a leading cause of morbidity and mortality worldwide, with current management strategies primarily focused on preventing secondary injury rather than modifying disease progression. Post-traumatic endocrine dysfunction has emerged as a significant and potentially treatable contributor to outcomes after TBI. METHODS: A scoping review was conducted following PRISMA-ScR guidelines. PubMed, Embase, Scopus, and Web of Science were searched from inception to September 2025 for clinical studies evaluating endocrine dysfunction and endocrine-targeted therapi

Predictive Factors for Diabetes Insipidus and Syndrome of Inappropriate Secretion of Antidiuretic Hormone After Transsphenoidal Surgery for Pituitary Adenoma.

Objective The objective of this study is to determine preoperative predictors of diabetes insipidus (DI) and syndrome of inappropriate secretion of antidiuretic hormone (SIADH) after transsphenoidal surgery (TSS) for pituitary adenomas. DI and SIADH are common complications associated with TSS. Methods This small unicentric retrospective study was conducted in a tertiary referral hospital in Saudi Arabia. The study enrolled 42 patients. We reviewed the medical records of all patients who underwent surgical resection of pituitary adenoma at our hospital between January 2010 and December 2022. T

Reframing traumatic brain injury and spinal cord injury as chronic multisystem disorders: toward integrated, long-term care.

Traumatic brain injury (TBI) and spinal cord injury (SCI) affect more than 220 million people worldwide and are associated with cardiometabolic, endocrine, neurological, and psychiatric sequelae for years to decades after an acute injury. However, most health systems still treat neurotrauma (TBI and SCI, collectively) as a time-limited event, leaving long-term surveillance of these patients fragmented or absent, which leads to preventable mortality and avoidable readmissions. TBI and SCI in adults should be reclassified as acute-onset multisystem chronic-risk conditions. Existing cardiovascula

Refractory Pericardial Effusion Secondary to Central Hypothyroidism due to Empty Sella Syndrome: A Rare Case Report.

Refractory pericardial effusion with a low free T4 but inappropriately normal TSH should prompt evaluation for central hypothyroidism, including pituitary MRI to identify treatable causes such as empty sella syndrome. Glucocorticoid cover must precede levothyroxine to prevent adrenal crisis in patients with possible hypopituitarism.

Case Report: Pituitary apoplexy temporally associated with acute alcohol intoxication, presenting as complete oculomotor nerve palsy in an SF-1 lineage gonadotroph PitNET.

Pituitary apoplexy (PA) is an acute, potentially life-threatening neuroendocrine emergency caused by hemorrhage or infarction within the pituitary gland, most often arising in a previously unrecognized pituitary neuroendocrine tumor (PitNET). Reported precipitants include surgery, anticoagulation, dynamic pituitary testing, hypertension, and head trauma; alcohol is rarely implicated. We report a 61-year-old man with long-standing hypertension, type 2 diabetes mellitus and coronary artery disease (on aspirin after coronary stenting) who developed sudden thunderclap headache and complete right o

Acinetobacter baumannii Nosocomial Meningitis Secondary to Neurosurgical Intervention: A Case Report.

Nosocomial meningitis is a serious complication of neurosurgical procedures, associated with high morbidity and mortality, as well as significant diagnostic and therapeutic challenges. Among the causative pathogens, Acinetobacter baumannii (A. baumannii) is particularly concerning due to its ability to survive for prolonged periods in the hospital environment, its opportunistic virulence, and, especially, its tendency to develop multidrug resistance to antimicrobial agents. Diagnosis is often difficult in the postoperative setting, particularly when the cerebrospinal fluid (CSF) is altered as

Arginine vasopressin deficiency revealing a disseminated human chorionic gonadotropin-secreting germinoma.

Arginine vasopressin deficiency (AVP-D) is an uncommon disorder caused by deficient vasopressin production due to hypothalamic or pituitary gland dysfunction resulting in hypotonic polyuria, whose etiology oftentimes remains elusive. We present the case of a 20-year-old male patient evaluated for polyuria and polydipsia. Laboratory evaluation demonstrated hypernatremia, elevated serum osmolality, and inappropriately dilute urine with symptomatic improvement following initiation of desmopressin, consistent with AVP-D. Initial pituitary magnetic resonance imaging (MRI) showed a small enhancing l

Parallel Venovenous and Venoarterial Extracorporeal Membrane Oxygenation for Differential Titration of Cardiopulmonary Support in Combined Severe Cardiac and Respiratory Failure: A Case Report.

Parallel venovenous (VV) and venoarterial (VA) extracorporeal membrane oxygenation (ECMO) can allow separate titration of respiratory and circulatory support in patients with concurrent severe cardiopulmonary failure. A 64-year-old woman with central hypopituitarism developed cytomegalovirus infection with hemophagocytic lymphohistiocytosis, followed by severe Pseudomonas aeruginosa pneumonia and septic cardiomyopathy. Despite VV-ECMO and intra-aortic balloon pump support, refractory cardiogenic shock required the addition of VA-ECMO. After parallel ECMO was initiated, hemodynamics improved ra

Functional analysis of adrenocorticotropic-hormone-producing pituitary cells derived from human pluripotent stem cells in murine and primate models of hypopituitarism.

BACKGROUND: Hypopituitarism has a prevalence of approximately 45.5 cases per 100,000 population, is therefore not a rare condition. Hypopituitarism does not resolve spontaneously and is designated as an intractable disease. In particular, deficiency of adrenocorticotropic hormone (ACTH) can result in adrenal insufficiency and life-threatening crises. The current standard of care is hormone replacement therapy; however, precise adjustment to match the physiological variations of hormone demand, such as circadian rhythm and stress responses, is often difficult. Consequently, hypopituitarism pati

Pituitary metastases: a real-world single-center surgical cohort.

BACKGROUND: The pituitary is a rare site for metastatic spread. Pituitary metastases (PMs) account for about 1% of lesions removed by pituitary surgery. Data on epidemiology, symptoms, pituitary hormone assessment, and radiological findings are limited. This paper aimed to describe our retrospective cohort of histologically diagnosed PMs. METHODS: A single-center, retrospective, observational cohort study was conducted on patients diagnosed with PM who underwent surgical resection between 2000 to 2025. We analyzed epidemiological, clinical, radiological, and surgical data about these lesions.

Patient-reported measures in hypothalamic-pituitary diseases.

Modern medicine centres its focus on the central stakeholder, the patient, to deliver a more patient-centred and holistic care and improve quality of care, emphasizing the importance of communication between healthcare providers and patients. To learn how the patient experiences and evaluates outcome of any health intervention, different tools have been developed, known as PREMs (Patient-Reported Experience Measures) and PROMs (Patient-Reported Outcome Measures). PREMs reflect the patient's personal experience when being cared for, like whether the professionals explained things clearly, or if

Predictors of overall mortality in pituitary adenomas.

PURPOSE: Various predictors of overall mortality like hypopituitarism or treatment have been demonstrated in pituitary adenomas (PA). Whether these predictors are specific to tumor subtype was not studied. We aimed to directly compare predictors of mortality in clinically significant PA subtypes (acromegaly, Cushing's disease (CD), macroprolactinomas (MacroPRM) and non-functioning pituitary macroadenomas (MacroNFPA). METHODS: We retrospectively analyzed all patients with acromegaly (n=224), CD (n=42), MacroPRM (n=123) and MacroNFPA (n=310) admitted in our department between 2011 and 2025. The

Chronic Central Adrenal Insufficiency in a Patient With Remote Traumatic Brain Injury and an Apparently Adequate Standard-Dose ACTH Response: A Case Report.

In patients with a remote history of traumatic brain injury, central adrenal insufficiency may remain unrecognized for decades. An apparently adequate response to standard-dose ACTH stimulation should not preclude further evaluation of the hypothalamic-pituitary-adrenal axis when clinical and biochemical findings continue to suggest adrenal insufficiency.

A Trench Plate-Based Approach for the Selective Isolation of Metal-Cycling Bacteria.

OBJECTIVE: Several reports have suggested that IGF-I levels in patients with childhood-onset (CO) GH deficiency are lower than those observed in patients with adult-onset (AO) GH deficiency. However, these reports are unsatisfactory as there are several differences between the cohorts studied other than the timing of onset of GH deficiency; in particular, the patients were not matched for equal severity of GH deficiency. We have pursued this question further by examining the IGF-I standard deviation score (SDS) in patients with CO and AO GH deficiency, with equal degrees of severity of GH defi
