Altered social-emotional processing and decision-making in Central Diabetes Insipidus: The role of vasopressin deficiency.

BACKGROUND: Arginine vasopressin (AVP) regulates homeostasis and social behavior. Damage to AVP neurons causes central diabetes insipidus (CDI), now termed arginine vasopressin deficiency (AVP-D), a rare disorder potentially linked to social deficits. This study examined how AVP deficiency affects social, emotional, and psychological functioning in AVP-D patients. METHODS: Twelve AVP-D patients and twelve matched controls participated. Blood samples measured copeptin and other hormones. Participants completed behavioral tasks assessing social exclusion (Cyberball paradigm), emotion recognitio

Erdheim-Chester disease mimicking Wilms tumor in a child: a diagnostic challenge.

BACKGROUND: Erdheim–Chester disease (ECD) is a rare non-Langerhans cell histiocytosis driven by activating mutations of the MAPK signaling pathway, most commonly BRAF V600E. Pediatric cases are exceedingly rare and often present with atypical, multisystem manifestations, resulting in significant diagnostic delay. Renal and retroperitoneal involvement may mimic primary renal malignancies, particularly Wilms tumor. CASE PRESENTATION: We report a case of Erdheim–Chester disease in a child treated in Lviv, Ukraine. A male child who initially presented at two years of age with central diabetes insi

Early Experience With Endoscopic Endonasal Transsphenoidal Surgery for Pituitary Adenomas: Clinical Outcomes and Complications.

OBJECTIVE: Endoscopic endonasal transsphenoidal surgery (EETS) has become the preferred method for treating pituitary adenomas; however, it entails a significant learning curve. This study aims to evaluate clinical features, surgical outcomes, and complication patterns during the early institutional experience with EETS and to establish a historical benchmark for tracking the progress of endoscopic pituitary surgery. METHODS: This retrospective, single-center observational study included 46 consecutive patients who underwent EETS for pituitary adenomas between October 2008 and September 2010.

A Double Whammy of Takotsubo Cardiomyopathy and Severe Hypernatremia in a Patient with Recurrent Craniopharyngioma: A Case Report and Literature Review.

BACKGROUND: Craniopharyngioma (CP) is a rare, usually benign tumour of the central nervous system that arises in the sellar or parasellar region. Although histopathologically benign, CP behaves clinically as a locally aggressive tumour, and it can cause significant morbidity due to its proximity to critical structures. Patients may present with headaches, visual disturbances, endocrine dysfunction, and, particularly after surgery, disturbances in sodium balance. Postoperative hypernatremia often results from hypothalamic involvement leading to diabetes insipidus and impaired thirst regulation.

A Case of Arginine Vasopressin Deficiency with Anti-rabphilin-3A Antibody Positivity Following an Influenza Infection.

Lymphocytic infundibulo-neurohypophysitis (LINH) presents with arginine vasopressin deficiency (AVP-D), also known as central diabetes insipidus, caused by autoimmune mechanisms in the infundibulum and posterior pituitary. A 32-year-old man developed polydipsia and polyuria two months after influenza infection. A hypertonic saline test revealed no AVP responses. Magnetic resonance imaging demonstrated the loss of posterior pituitary hyperintensity. The patient tested positive for serum anti-rabphilin-3A antibodies, a diagnostic marker for LINH. This is the first report of a case of probable LI

The effect of centralized care on the management of postoperative fluctuations in plasma sodium concentration after pediatric suprasellar brain tumor surgery.

PURPOSE: Children undergoing neurosurgery for (supra)sellar tumors are at risk of developing arginine vasopressin-deficiency (AVP-D), which can cause severe sodium fluctuations and associated neurological complications, prolonged hospitalization and mortality. A previous Dutch study reported sodium shifts ≥ 10 mmol/L/24 h in 75.3% of patients with early postoperative AVP-D, with a maximum delta of 46 mmol/L/24 h. Since 2018, pediatric oncology care has been centralized in the Netherlands. We evaluated the impact of this centralization on postoperative sodium fluctuations in children with (supr

Severe Hypernatremia Due to Postoperative Diabetic Ketoacidosis and Transient Central Diabetes Insipidus: A Case Report.

We report a case of acute severe hypernatremia secondary to diabetic ketoacidosis (DKA) and transient central diabetes insipidus (CDI) in a postoperative patient. Diagnosis of DKA may be delayed in patients on sodium-glucose cotransporter 2 (SGLT2) inhibitors, particularly in the postoperative state. Hypernatremia was refractory to medical management and corrected with slow low-efficiency dialysis (SLED) and desmopressin supplementation. This case highlights the importance of suspecting diabetes insipidus in refractory hypernatremia and demonstrates the role of SLED for controlled sodium corre

Reversible Nephrogenic Diabetes Insipidus Induced by Lithium: A Case Report.

Lithium is an effective mood stabilizer but may cause nephrogenic diabetes insipidus (NDI) by impairing the renal collecting duct response to arginine vasopressin (AVP). We report a 52-year-old woman on long-term lithium therapy who presented with diarrhea, fatigue, polyuria, and confusion. Initial evaluation showed hypernatremia (serum sodium 156-159 mmol/L), low urine osmolality (101 mOsm/kg) despite serum osmolality of 286 mOsm/kg, daily urine output of 6.5-7.5 L, and a lithium level of 1.65 mmol/L. Renal function was preserved. Intravenous 5% dextrose was administered for free-water replac

Real-world pharmacovigilance insights and clinical safety signal characterization of the classic mood stabilizer‑lithium carbonate: A disproportionality analysis based on FAERS data.

OBJECTIVE: Lithium carbonate is a classic mood stabilizer for bipolar disorder, yet real-world pharmacovigilance of its safety signals (especially underrecognized systems/populations) is lacking. This study detected disproportional ADR signals via FAERS to clarify its multi-system safety profile and gender/age-specific patterns. METHODS: FAERS data (Q1 2004-Q3 2025) for lithium carbonate as the primary suspected drug were extracted. Duplicate reports were processed per FDA guidelines; ADRs were coded by MedDRA (PTs/SOCs). Disproportionality analysis used ROR, PRR, BCPNN and EBGM algorithms, w

Novel Roles of GPCRs in the Renal Collecting Duct.

G protein-coupled receptors (GPCRs) serve to coordinate renal collecting duct function, orchestrating fine-tuned regulation of water, electrolyte, and acid-base homeostasis. Classical GPCRs such as the vasopressin V2 receptor (V2R) have well-established roles: the V2R serves to regulate water reabsorption, and V2R dysregulation can underlie diabetes insipidus. However, transcriptomic and phosphoproteomic advances have revealed a diverse repertoire of understudied GPCRs expressed in the collecting duct whose physiological roles remain largely undefined. Here, we review emerging insights into th

Different Neuroendoscopic Modalities for Management of Suprasellar Arachnoid Cysts: Comparative Study.

BACKGROUND: Arachnoid cysts account for approximately 1% of all intracranial mass lesions, with suprasellar arachnoid cysts (SACs) representing a rare subset. Hydrocephalus is the most common presentation, occurring in nearly 90% of the cases. Due to their rarity, no universal consensus exists regarding classification, optimal surgical management, or outcome assessment. OBJECTIVE: To compare outcomes of ventriculocystostomy (VC) and ventriculocystocisternostomy (VCC) in the endoscopic management of suprasellar arachnoid cysts. METHODS: This retrospective study reviewed patients with SACs who
