BACKGROUND: Central diabetes insipidus (CDI) is a frequent complication after craniopharyngioma (CP) surgery. Although many patients recover within 2 weeks, a substantial proportion develop persistent CDI that requires long-term management. Systemic inflammatory activation may aggravate hypothalamic-neurohypophyseal injury and delay functional recovery. However, reliable preoperative predictors of early recovery remain limited. This study aimed to develop a multivariable prediction model for persistent CDI at 2 weeks after CP surgery.
METHODS: This retrospective study included patients with p

IgG4-related disease (IgG4-RD) is a rare fibro-inflammatory condition with scarce data on its clinical spectrum and management in the pediatric population. We present a single-center observational study of six children with clinically and morphologically confirmed IgG4-RD. We analyzed the age of onset, time to diagnosis, and comprehensive clinical, imaging, laboratory, and histopathological findings at presentation and during follow-up (range: 3 months to 7 years). The 2020 revised comprehensive diagnostic criteria for IgG4-RD were used. The diagnosis was based on a biopsy of orbital tissue wit

Immune checkpoint inhibitors are increasingly associated with complex immune-related endocrine adverse events that may evolve dynamically over time. We report a rare case of pembrolizumab-induced hypophysitis complicated by central diabetes insipidus, which was unmasked following glucocorticoid replacement. A 68-year-old female patient receiving immunotherapy for lung adenocarcinoma presented with hyponatremia and secondary adrenal insufficiency, followed by abrupt severe hypernatremia after steroid initiation. Subsequent evaluation confirmed central diabetes insipidus, and treatment with desm

BACKGROUND: Osmotic demyelination syndrome (ODS) is an acute neurological disorder resulting from demyelination, typically triggered by rapid osmotic shifts. This syndrome is rare in adults and even more uncommon in children. To date, no reported cases have documented ODS secondary to hyponatremia caused by post-traumatic pituitary stalk injury. We report a pediatric patient with traumatic brain injury who developed ODS subsequent to pituitary stalk disruption and hyponatremia, and discuss the corresponding diagnostic and therapeutic strategies.
CASE PRESENTATION: We report the case of a 13-ye

Arginine vasopressin is primarily recognized as an osmoregulatory hormone; however, its contribution to vascular tone and blood pressure regulation under basal and hypertensive conditions remains debated. This study examined the effects of vasopressin deficiency induced by neurointermediate pituitary lobectomy in Wistar and spontaneously hypertensive rats, compared with sham-operated controls. Water intake, urine output, plasma vasopressin and oxytocin levels, blood pressure, and heart rate were evaluated over time. Neurointermediate pituitary lobectomy produced transient diabetes insipidus fo

Central diabetes insipidus, or arginine vasopressin deficiency (AVP-D), is a rare disorder caused by insufficient release of antidiuretic hormone from the pituitary gland. The disease is characterised by severe polyuria and polydipsia, which, in rare cases, can be caused by variants in the AVP gene. The aim of the present study was to identify the genetic cause of AVP-D in two families and compile and discuss an updated and complete catalogue of variants in AVP associated with AVP-D. Two Danish families with autosomal dominant inheritance of symptoms indicating AVP-D were included, and proband

OBJECTIVE: The aim of this study was to assess whether endonasal endoscopic gross-total resection (GTR) of pediatric craniopharyngioma invading the hypothalamus is associated with increased morbidity compared with subtotal resection (STR).
METHODS: Medical records were reviewed for all pediatric patients (age ≤ 21 years) with craniopharyngioma removed via an endonasal endoscopic approach (EEA) between 2006 and 2024. The Sainte-Rose hypothalamic involvement score (HIS) was used to quantify hypothalamic invasion and to assess its correlation with outcome.
RESULTS: Overall, 23 patients (14 male

OBJECTIVE: Craniopharyngioma is increasingly recognized as a chronic morbidity-driven disease in which hypothalamic and pituitary dysfunction often outweigh purely oncological endpoints. The advent of extended endoscopic endonasal approaches (EEAs) has improved visualization of the infundibulum, yet the optimal management of the pituitary stalk remains controversial. The aim of this study was to evaluate the impact of pituitary stalk preservation versus transection on endocrine, visual, surgical, and early oncological outcomes in patients with suprasellar craniopharyngiomas treated via an exte

BACKGROUND: Cushing's disease (CD) is a rare endocrine disorder associated with significant morbidity and increased mortality if left untreated. Endoscopic transsphenoidal surgery (ETSS) is the preferred first-line treatment; however, reported remission and recurrence rates vary widely due to differences in biochemical remission criteria and follow-up duration. This study aimed to evaluate surgical outcomes, remission rates using standard and strict biochemical criteria, recurrence, and postoperative complications following ETSS for CD at a tertiary referral center.

BACKGROUND: Radical resection of transinfundibular craniopharyngiomas (TCs) while preserving endocrine function remains a major surgical challenge because of their origin within the pituitary stalk. The "vertical infundibular split with subpial excavation" (VISSE) technique in endoscopic endonasal surgery (EES) is an effective approach that may enable gross total resection (GTR) in selected cases while aiming to preserve pituitary stalk integrity and neuroendocrine function. This procedure involves a vertical split of the pia mater of the pituitary stalk at sites with sparse pituitary portal v

BACKGROUND: Pituitary metastases represent approximately 1% of all pituitary tumors and are associated with a poor prognosis. Although rare, early diagnosis and treatment are critical to limit rapid clinical decline. Because of their rarity, published data remain limited. The authors report their institutional experience at The Ottawa Hospital, focusing on clinical and radiological presentation, surgical management, adjuvant therapy, and outcomes.
OBSERVATIONS: The authors retrospectively reviewed the cases of adult patients undergoing endoscopic endonasal skull base surgery for pituitary met

CASE SUMMARY: A 2-year-old castrated male domestic shorthair cat presented with severe lethargy, hyporexia, dehydration and poor body condition. The incidence of polyuria/polydipsia was unknown because of the presence of multiple other cats in the household. Laboratory findings revealed severe hypernatraemia, isosthenuria, haemoconcentration and moderate azotaemia. MRI of the head demonstrated partial hypoplasia/aplasia of the hypothalamus and pituitary gland, confirming congenital central diabetes insipidus; additional endocrine testing excluded other causes. Initial fluid therapy improved th

BACKGROUND: Pituitary stalk interruption syndrome (PSIS) is a clinical syndrome caused by the absence or thinning of the pituitary stalk. This syndrome mainly impairs the transport of hormones from the hypothalamus to the posterior pituitary gland and affects the action of relevant hormones on the anterior pituitary gland, thereby leading to common clinical manifestations such as diabetes insipidus, growth retardation, gonadal dysplasia, hypothyroidism, and adrenocortical hypofunction. In PSIS, cases presenting with infantile cholestasis as the initial and prominent manifestation are relativel

Diabetic ketoacidosis (DKA) is a life-threatening complication of diabetes characterized by hyperglycemia, high-anion-gap metabolic acidosis, and ketonemia. Hypernatremia in DKA is uncommon and typically reflects disproportionate free water loss. Arginine vasopressin resistance (AVP-R), previously known as nephrogenic diabetes insipidus, may occur in the setting of hypokalemia, leading to polyuria, hypotonic urine, and worsening hypernatremia. We report the case of a 14-year-old previously healthy female who presented with new-onset severe DKA complicated by profound acidosis, hyperglycemia, h

Giant aneurysms are rare vascular anomalies that can induce symptoms through compression of neural structures. This report describes a case of a giant sellar aneurysm initially manifesting as diabetes insipidus (DI), challenging conventional diagnostic paradigms for sellar mass lesions. Although DI typically arises from antidiuretic hormone (ADH) deficiency, its presentation as the primary symptom of a giant sellar aneurysm is exceedingly uncommon. The distinctiveness of this case lies in the occurrence of DI, likely due to aneurysmal compression of the pituitary gland, underscoring the impera

BACKGROUND AND OBJECTIVES: Craniopharyngiomas confined to the third ventricle present formidable surgical challenges owing to their deep location and proximity to critical neurovascular structures. This study evaluates the transcavernous sinus transtuber cinereum (TCSTC) approach, a technically advanced route that enables direct access to the third ventricle while preserving surrounding anatomy.
METHODS: Ten cadaveric heads were meticulously prepared with vascular injections, and the right side was dissected under magnification using a surgical microscope. High-resolution imaging and digital

BACKGROUND: Nephrogenic diabetes insipidus (NDI) is a rare, potentially life-threatening renal tubular disorder characterized by a decreased ability to concentrate urine due to antidiuretic hormone (ADH) resistance, leading to significant free water loss. The congenital form, most often caused by X-linked mutations in the AVPR2 gene, typically presents in early infancy. Due to its rarity and nonspecific clinical features in young children, such as failure to thrive, vomiting, and excessive fluid intake, diagnostic delays are common, especially in centers with limited prior experience in managi
