Accurate segmentation and classification of brain tumors from Magnetic Resonance Imaging (MRI) remain key challenges in medical image analysis, primarily due to the lack of high-quality, balanced, and diverse datasets with expert annotations. In this work, we address this gap by introducing BRISC, a dataset designed for brain tumor segmentation and classification tasks, featuring high-resolution segmentation masks. The dataset comprises 6,000 contrast-enhanced T1-weighted MRI scans, which were collated from multiple public datasets that lacked segmentation labels. Our primary contribution is t

Microglia play essential roles in maintaining energy homeostasis, and their dysfunction contributes to metabolic disease. Although high-fat diet (HFD) exposure induces microglial activation, the underlying mechanisms remain poorly defined. Here, we identified a previously unrecognized role for THIK-1 channel in mediating glucose sensing of microglia in arcuate nucleus of the hypothalamus (ARH), during HFD-induced obesity. Pharmacological inhibition of THIK-1 channel with tetrapentylammonium (TPA) suppresses feeding and attenuates body-weight gain in diet induced obese mice. Mechanistically, in

INTRODUCTION: Acromegaly is a rare, insidious disease associated with significant morbidity and mortality usually caused by a growth hormone (GH)-secreting pituitary tumor. Somatic mutations in GNAS are common in these tumors, yet their diagnostic, prognostic, and therapeutic implications are less clear.
METHODS: We conducted a structured review of the literature and meta-analysis to investigate the association of GNAS mutation status with clinical characteristics and treatment outcomes in adult patients with acromegaly. This was complemented by an analysis comparing patients with acromegaly

BACKGROUND: Multiple pituitary adenomas (MPA), particularly those involving functioning adenoma, represent a special subtype of pituitary adenomas that pose dual challenges in diagnosis and treatment. In this setting, the coexistence of hormonally active and silent lesions may result in a diagnostic pitfall when a radiologically dominant tumor does not correspond to the hormonally responsible lesion. This misleading lesion, often referred to as decoy lesion, can adversely influence surgical decision-making in patients with MPA.
METHODS: We systematically reviewed English-language case reports

Sodium imbalances are a common occurrence in the emergency department. Although recognition and diagnosis are relatively straightforward, discovering the cause and management should be approached systematically. The most important history items to ascertain is if the patient has symptoms and how long this imbalance has taken to develop. Treatment rapidity depends on severity of symptoms with the most rapid treatment occurring in only the severely symptomatic. Overcorrection has dire consequences and must be approached in a careful and systematic fashion in order to prevent these devastating co

BACKGROUND: Multiple system atrophy (MSA) is a neurodegenerative disease that affects the extrapyramidal, cerebellar, and autonomic nervous systems. In some patients, hypothalamic involvement can lead to impaired thermoregulation and hyponatremia. Duloxetine, a serotonin-norepinephrine reuptake inhibitor used for chronic pain, has rarely been linked to fever and syndrome of inappropriate antidiuretic hormone secretion (SIADH).
CASE PRESENTATION: A 66-year-old man with clinically probable MSA and baseline hyponatremia (serum sodium 121.6 mmol/L) started duloxetine 30 mg/day for migrating pain.

PURPOSE: Transsphenoidal surgery (TSS) is the recommended treatment for Cushing's disease (CD). While low morning serum cortisol (MSeC) indicates early remission, the predictive value of an early postoperative overnight 2 mg low-dose dexamethasone suppression test (LDDST) remains unclear. This study compares the accuracy of early postoperative MSeC and LDDST for predicting early and long-term remission after TSS.
METHODS: We retrospectively analysed 342 CD surgical cases, who underwent TSS in our department between 2004 and 2024. Morning cortisol measurements were obtained postoperatively. An

BACKGROUND: Gamma Knife radiosurgery (GKRS) is an established treatment for pituitary adenomas yet prescription dose selection is often guided by clinician experience. Data-driven models may help standardize dose selection using routinely available clinical and imaging features.
OBJECTIVE: To develop and internally validate a feedforward neural network (FNN) to predict GKRS prescription dose for pituitary adenomas.
METHODS: We retrospectively analyzed 102 pituitary adenomas treated with GKRS at a single center. Model inputs included age at treatment, sex, race, pre-treatment Karnofsky Perfor

The purpose of this study was to assess the effects of a 60% ethanolic extract of Aster yomena (EAY) on chronic unpredictable mild stress (CUMS)-induced depressive cognitive dysfunction. The results showed that EAY mitigated CUMS-induced depressive-like behaviors, as confirmed by the sucrose preference test (SPT), open field test (OFT), tail suspension test (TST), and forced swimming test (FST). In addition, EAY showed protective effects on cognitive function in the Y-maze and the Morris water maze (MWM) tests. In this regard, EAY alleviated hyperactivation of the hypothalamic-pituitary-adrena

Bisphenol S (BPS), increasingly used as a substitute for Bisphenol A, has raised toxicological concern due to its environmental persistence and widespread exposure. Zebrafish (Danio rerio) provides a valuable model for assessing BPS toxicity because of conserved endocrine and neural pathways, rapid development, and well-established behavioural assays. This review synthesizes current evidence on the effects of BPS exposure in zebrafish. Available studies indicate that BPS is associated with alterations in multiple endocrine axes, including the hypothalamic-pituitary-gonadal, thyroid, and interr

Glucagon-like peptide-1 (GLP-1), an incretin secreted by intestinal L-cells in response to nutrients, regulates glucose homeostasis by enhancing insulin secretion, suppressing glucagon release, delaying gastric emptying, and reducing appetite via hypothalamic signaling. Beyond these canonical actions, emerging evidence reveals GLP-1's pleiotropic functions across multiple systems, with relevance to metabolic disorders, chronic inflammation, and aging-related pathologies. This review summarizes molecular mechanisms underlying GLP-1's protective roles, highlighting its contributions to metabolic

Hypothalamic obesity (HO) is a rare, complex disorder characterized by disruption of brain pathways regulating energy intake, expenditure, autonomic function, and hormonal signaling. It occurs in rare monogenic obesity syndromes affecting central leptin-melanocortin pathways or can be acquired (aHO) as a consequence of hypothalamic injury due to a tumor (e.g., craniopharyngioma), its treatment, or trauma. In this narrative review, we focus on aHO. Damage to specific hypothalamic nuclei leads to hyperphagia, central insulin and leptin resistance, decreased sympathetic activity, reduced energy e

Dysfunctioning of stress systems, i.e., the autonomic nervous system (ANS) and hypothalamic-pituitary-adrenal (HPA) axis, has been implicated in chronic pain. However, the exact interplay between (re)activity and recovery of stress and pain systems in chronic pain remains unclear. A systematic review and meta-analysis was pre-registered on PROSPERO (CRD42024495934). Six databases were searched to identify relevant literature. Risk of bias (RoB) was evaluated with the Newcastle-Ottawa Scale, and certainty of evidence (CoE) with GRADE. Clusters of interactions between physiological markers of st

Maternal infection during pregnancy may contribute to autism spectrum disorder (ASD) through dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis, however, the precise mechanisms disrupting fetal neurodevelopment remain unclear. By integrating human cohort analyses with experimental models, this study identifies prenatal infection as a significant maternal risk factor associated with elevated glucocorticoid (GC) exposure in offspring. In animal model, maternal lipopolysaccharide (LPS) exposure induced fetal GC surges, leading to GC receptor (GR)-dependent upregulation of inhibitor of

PURPOSE: Acromegaly is a chronic endocrine disorder characterized by prolonged exposure to excess growth hormone (GH) and insulin-like growth factor-1 (IGF-1), leading to systemic connective tissue remodeling. Although corneal and choroidal structural changes have been extensively reported, scleral alterations remain poorly understood. This prospective case–control study aimed to quantitatively assess anterior scleral thickness (ST) across multiple meridians in patients with acromegaly compared with healthy controls using anterior segment optical coherence tomography (AS-OCT). BACKGROUND: Due to the rapid aging of the global population, it is challenging to identify elderly individuals who are optimal candidates for the surgical treatment of pituitary neuroendocrine tumors. While chronological age has traditionally influenced surgical decision-making, the independent effect of age on outcomes requires a comprehensive evaluation that controls for comorbidities and tumor characteristics. METHODS: This retrospective analysis included 305 patients (≥18 years) who underwent endoscopic endonasal surgery for clinically nonfunctioning pituitary neuroendocrine tumors between

There is increasing evidence that exposure to environmental toxicants may impact fertility, especially during critical windows of reproductive axis development. Hypothalamic gonadotropin-releasing hormone (GnRH) neurons, essential for puberty onset and fertility, originate from the olfactory placode and migrate toward the hypothalamus during development, making them particularly vulnerable to environmental insults. Cadmium (Cd), a widespread heavy metal, is well known for its gonadotoxicity, but its impact on human hypothalamic neuron development remains unclear. Using human fetal GnRH neurobl

Depression represents a leading cause of global disability, yet its pathogenesis remains incompletely understood. This review synthesizes emerging evidence highlighting the multifaceted role of Aquaporin-4 (AQP4), the central nervous system's predominant water channel, in the pathophysiology of depression. Preclinical studies frequently report AQP4 dysregulation in depression models, characterized by reduced perivascular expression and impaired polarization in mood-relevant brain circuits. We delineate how AQP4 impairment is implicated in depression through several interconnected mechanistic p

Resilience is commonly framed as a psychological trait, yet clinical and experimental evidence demonstrates that resilience failures emerge concurrently across metabolic, endocrine, immune, and cognitive domains. This review examines resilience as a bioenergetic property constrained by how organisms allocate finite metabolic resources under stress. We synthesize evidence from endocrinology, mitochondrial biology, immunometabolism, and stress physiology to propose a parsimonious, hypothesis-driven Energy Allocation System (EAS) in which the hypothalamic-pituitary-adrenal (HPA), thyroid (HPT), a
