Liver-specific sustained activation of the LPS/TLR4/NF-κB pathway: from a gut-liver axis hub to cascading extensions in reproductive and neuroinflammation.

The chronic activation of the hepatic lipopolysaccharide (LPS)/toll-like receptor 4 (TLR4)/nuclear factor-κB (NF-κB) pathway, which links intestinal dysbiosis, chronic hepatic inflammation, and injury to other organs, is a crucial mechanism of pathogenesis in the gut-liver axis. Intestinal barrier disruption facilitates the transport of LPS to the liver, where it activates the TLR4/NF-κB pathway in hepatic stellate cells and Kupffer cells. As a result, a number of positive feedback loops produce a "chronic inflammatory microenvironment," which fuels liver damage, including that caused by alcoh

Arginine vasopressin deficiency revealing a disseminated human chorionic gonadotropin-secreting germinoma.

Arginine vasopressin deficiency (AVP-D) is an uncommon disorder caused by deficient vasopressin production due to hypothalamic or pituitary gland dysfunction resulting in hypotonic polyuria, whose etiology oftentimes remains elusive. We present the case of a 20-year-old male patient evaluated for polyuria and polydipsia. Laboratory evaluation demonstrated hypernatremia, elevated serum osmolality, and inappropriately dilute urine with symptomatic improvement following initiation of desmopressin, consistent with AVP-D. Initial pituitary magnetic resonance imaging (MRI) showed a small enhancing l

Safety and effectiveness of setmelanotide in young patients with severe obesity due to craniopharyngioma: a real world monocentric experience.

BACKGROUND: Hypothalamic obesity (HO) is a severe, often treatment-refractory complication after craniopharyngioma (CP) surgery. Setmelanotide, a melanocortin-4 receptor agonist (MC4R-A), offers a targeted therapeutic approach, but real-world data are limited. METHODS: We performed a retrospective review of patients with post-CP HO treated with setmelanotide at our center, collecting clinical, metabolic, and treatment-related data. RESULTS: Three young adults with HO after surgery for CP were included. Treatment responses were variable: one patient discontinued setmelanotide after continued

Tryptanthrin Nanoliposomal Lotion as a Corticosteroid-Free Strategy for Safe and Effective Treatment of Atopic Dermatitis.

Consuming a high-fat diet (HFD) is widely recognized to cause obesity and result in chronic brain inflammation that impairs cognitive function. Repetitive transcranial magnetic stimulation (rTMS) has shown effectiveness in both weight loss and cognitive improvement, although the exact mechanism is still unknown. Our study examined the effects of rTMS on the brain and intestinal microecological dysfunction. rTMS successfully reduced cognitive decline caused by an HFD in behavioral assessments involving the Y maze and novel object recognition. This was accompanied by an increase in the number of

Bidirectional Pharmacology of GIP Receptor Targeting in Obesity: Historical Foundations, Mechanistic Divergence, and Translational Boundaries.

The glucose-dependent insulinotropic polypeptide receptor (GIPR) has emerged as a central target in next-generation anti-obesity drug development. Its pharmacology, however, presents an apparent paradox: sustained GIPR agonism and GIPR antagonism can both lower body weight, and both can augment weight loss produced by glucagon-like peptide-1 receptor (GLP-1R) agonism. For this structured narrative review, PubMed/MEDLINE and ClinicalTrials.gov were searched from database inception through 1 August 2026 using prespecified terms for receptors, interventions, obesity, and bariatric surgery to iden

Epigenetics of Chronic Stress: Mechanisms and Health Impacts.

Chronic stress is a major public health concern, contributing to a spectrum of physiological and psychological disorders, including anxiety, depression, cardiovascular diseases, and metabolic syndromes. This review examines the epigenetic mechanisms mediating the impact of chronic stress on human health, focusing on DNA methylation, histone modifications, and non-coding RNAs. These epigenetic alterations act as dynamic interfaces between environmental stressors and genomic responses, modifying gene expression in critical stress-related pathways, such as the hypothalamic-pituitary-adrenal (HPA)

Machine Learning-Driven PCDI Classifier for Invasive PitNETs.

INTRODUCTION: Aggressive Pituitary Neuroendocrine Tumors (PitNETs) pose significant therapeutic challenges due to their invasive behavior and resistance to conventional therapies. Current prognostic markers lack the ability to capture molecular heterogeneity, necessitating novel biomarkers. Dysregulated Programmed Cell Death (PCD) pathways are implicated in tumorigenesis, but their prognostic relevance in invasive PitNETs remains unexplored. METHODS: GEO datasets (GSE51618, GSE169498, GSE260487) were analyzed to identify differential gene expression between noninvasive and invasive PitNETs. A

Bifidobacterium longum supplementation prevents intestinal inflammation and cognitive impairment in sleep-deprived mice by modulating clock gene expression.

Probiotics have shown potential for alleviating circadian disruption, but efficient screening models for identifying effective strains are lacking, and the mechanisms by which probiotics modulate circadian disruption and related diseases require further investigation. In this research, we developed an in vitro screening model targeting clock gene expression and identified Bifidobacterium longum CCFM1238 as an effective strain for alleviating circadian disruption in sleep-deprived mice. B. longum CCFM1238 exhibited a rhythm-regulating effect similar to melatonin in vitro and alleviated both int

The Incidence of Calcified Pituitary Macroadenomas: Challenging the Craniopharyngioma Assumption.

BACKGROUND AND PURPOSE: Large sellar/suprasellar masses containing calcifications are classically associated with craniopharyngiomas. In our practice, however, we have noticed that some giant pituitary macroadenomas have calcifications. The purpose of our study was to evaluate the incidence and characteristics of calcifications in pituitary macroadenomas. MATERIALS AND METHODS: We reviewed an internal database for pathology-proven giant macroadenomas (≥4 cm). Basic patient demographics were collected, including age and sex. Adenoma subtype and maximum adenoma dimension were recorded. Preopera

Clinical and Pathological Spectrum of Acromegaly: Distinguishing GH PitNETs, Mammosomatotrophs, and Mixed Tumors.

INTRODUCTION: Acromegaly is a rare disease usually caused by a pituitary neuroendocrine tumor (PitNET) that produces GH PitNET. PitNETs secreting GH and prolactin (GH&PRL PitNETs) contribute up to 30% to the spectrum of acromegaly and have been attributed a more aggressive behavior. GH&PRL PitNETs can be classified into 2 predominant phenotypes: mammosomatotroph arising from a single-cell population of Pit-1 lineage and mixed somatotroph-lactotroph PitNETs (mixed SL PitNETs). PURPOSE: To evaluate the clinical and molecular differences between GH PitNETs, mammosomatotroph, and mixed SL PitNETs

Transcriptomic evidence of hypothalamic neuroinflammation in mice due to aspartame exposure through the TLR4/NF-κB/NLRP3 pathway.

The use of aspartame (ASP) instead of sugar is increasing. ASP exposure has been shown to affect the hypothalamus. However, whether it causes hypothalamic inflammation in mice and the specific molecular mechanisms are unknown. Here, we established an ASP exposure model with a body-weight-adjusted dose of 50 mg kg-1 and employed Oxford Nanopore Technologies to investigate the differences in genes in the mice's hypothalamus. 491 differentially expressed genes were identified in the ASP group when compared with the control group. Functional enrichment analysis found that several terms and pathway
