Gut microbiota composition and functionality are associated with prolactin-secreting pituitary adenoma and serum lipid levels.

BACKGROUND: The changes in gut microbiota composition and their correlations with serum lipid levels in prolactin-secreting pituitary adenoma (PPA) patients remain unknown. This study investigates these alterations and associations and explores microbial markers for PPA diagnosis. METHODS: A total of 101 participants were enrolled, comprising 72 PA patients (31 with prolactin-secreting adenomas and 41 with non-functioning adenomas, i.e., PPA and NFPA groups) and 29 age and sex-matched healthy controls (HC). Utilizing 16 S rRNA gene amplicon sequencing, we examined the gut microbiota community

Metformin suppresses MEN1-associated pancreatic and pituitary neuroendocrine tumors: evidence from mouse models and clinical data.

Pancreatic neuroendocrine tumors (PanNETs) represent a rare subset of pancreatic cancers, comprising approximately 1-2% of all cases. Non-functioning PanNETs (NF-PanNETs), which account for the majority of PanNETs, can be difficult to treat as they show no hormone-related symptoms and are often not diagnosed until more advanced stages. Current therapeutic agents have limited efficacy, highlighting the need for novel treatment strategies. Multiple endocrine neoplasia type 1 is a hereditary syndrome strongly associated with PanNETs and pituitary neuroendocrine tumors (PitNETs), caused by germlin

Utility of standardized T2-weighted MRI intensity ratio for prediction of pituitary macroadenoma consistency.

OBJECTIVE: The physical consistency of pituitary adenomas (PAs) is highly variable, ranging from soft/cystic to firm/calcified. The association between PA consistency and surgical/endocrinological outcomes has been well established, with firm tumors demonstrating poorer outcomes and higher complication rates. However, to date, no reliable means to determine PA consistency preoperatively exist, although T2- and diffusion-weighted imaging show early promise. As such, this study aimed to quantitatively analyze the value of normalized T2-weighted imaging (T2WI) and diffusion-weighted imaging ratio

Contemporary series of transsphenoidal microsurgery in pediatric patients.

Pituitary tumors are rare in the pediatric population, accounting for less than 10% of childhood tumors. Craniopharyngiomas are the most common pediatric sellar lesions, but pituitary adenomas/PitNETs also occur, representing 2-8% of all sellar lesions. This study explores a decade of transsphenoidal microsurgery on pediatric patients, examining perioperative features including complications, clinical outcomes and anatomical peculiarities. This retrospective study included a total of 147 transsphenoidal surgeries performed by a single surgeon on 122 pediatric patients under the age of 18 betwe

Are PTTG1 variants associated with tumor characteristics and p53/Ki-67 expression in pituitary neuroendocrine tumors.

BACKGROUND: Pituitary tumor-transforming gene 1 (PTTG1) is a proto-oncogene implicated in pituitary neuroendocrine tumors (PitNETs) pathogenesis through regulation of the cell cycle, genomic instability, and angiogenesis. Overexpression of PTTG1 promotes tumor growth, but the impact of its genetic variants in PitNETs size is insufficiently defined. OBJECTIVE: To evaluate the impact of PTTG1 gene variants (rs1895320, rs2910200, and rs6882742), circulating PTTG1 levels, and immunohistochemical markers (Ki-67 and p53) on PitNETs susceptibility and clinical features. METHODS: case-control study

Class III Obesity as a Presenting Syndrome in Prolactinoma Patients: Insights Into Obesity and Metabolic Dysfunction in This Unique Group.

The occurrence of patients with lactotroph adenoma presenting with class III obesity is extremely rare. This study reports long-term outcomes of such patients treated with transsphenoidal surgery (TSS) or dopamine agonists (DAs) and integrates findings with existing literature to explore prolactin's (PRL) role in obesity pathophysiology. We analysed BMI and metabolic changes and compared findings with published case series. Four patients presented with class III obesity (BMI range, 40.0-43.1 kg/m2). The diagnostic work-up revealed hyperprolactinemia (PRL range, 56.1-4389 μg/L) and hypogonadism

Developmental and neurobehavioural toxicity of per- and polyfluoroalkyl substances in aquatic organisms: mechanistic insights from molecular disruption to behavioural dysfunction.

Per- and polyfluoroalkyl substances (PFAS) are persistent synthetic contaminants ubiquitously detected in aquatic ecosystems, raising growing concern over their ecological and toxicological impacts. Increasing evidence demonstrates that PFAS pose significant risks to aquatic organisms, particularly during early developmental stages, by disrupting development and neurobehavioural function. This review synthesizes current knowledge on PFAS-induced developmental and neurobehavioural toxicity in aquatic organisms, integrating molecular, physiological, and behavioural evidence. Consensus on the diagnosis of Cushing's disease: a collaborative statement from the Korean Endocrine Society and Japan Endocrine Society.

Cushing's disease (CD) is a rare but serious endocrine disorder caused by excessive cortisol secretion due to adrenocorticotropic hormone-secreting pituitary tumors. Despite recent developments in diagnostic criteria and treatment options, CD remains associated with substantial comorbidities and mortality. Early and accurate diagnosis is thus essential. Both the Korean Endocrine Society (KES) and Japan Endocrine Society (JES) guidelines are intended to standardize diagnostic approaches to CD, and they share common principles; however, notable differences exist, particularly in biochemical test

Unraveling SUDEP: Mechanisms of Seizure-Induced Cardiac and Respiratory Impairment.

People with epilepsy have a markedly increased risk of premature mortality, with sudden unexpected death in epilepsy (SUDEP) accounting for approximately half of epilepsy-related deaths. Although rarely witnessed, converging evidence indicates that SUDEP arises from seizure-triggered cardiorespiratory failure mediated by central and peripheral mechanisms. In this article, we explore mechanisms of SUDEP related to respiratory, cardiac, and autonomic control of vital functions. Specifically, we examine how seizure propagation to a discrete amygdala subregion can induce profound ictal and postict

Is Hypothalamic Dysfunction a Cause of Type II Diabetes Mellitus?

<p>Background: A number of factors contribute to the development of diabetes mellitus (DM), but until recently, the role of hypothalamic dysfunction in the development of this disorder has not received sufficient attention. Hypothalamic neurons that project to the autonomic nervous system control overall glucose metabolism and insulin sensitivity. These neurons, in turn, are controlled by specialized astrocytes possessing proteins that enable them to function as nutrient sensors for glucose and fatty acids. An aging-related pathological process (that of mitochondrial degeneration) is present i

Histone modifications and depression: epigenetic mechanisms, therapeutic targets, and translational outlook.

Major depressive disorder (MDD) is a highly prevalent and heterogeneous psychiatric disorder shaped by the interplay of genetic, environmental, and epigenetic factors. Histone modifications, particularly acetylation and methylation, have emerged as critical regulators of chromatin dynamics and gene expression in stress adaptation, neuroplasticity, and emotional regulation. This review synthesizes current evidence linking dysregulated histone deacetylases (HDACs) and histone methyltransferases (HMTs) to impaired neuroplasticity, neuroinflammation, mitochondrial dysfunction, and hypothalamic-pit

Pituitary Neuroendocrine Tumors Extending Primarily Below the Sella and into the Clivus: A Distinct Growth Pattern with Specific Challenges.

Evaluation of pituitary neuroendocrine tumors remains complex depending on the exact growth pattern, involvement of critical neurovascular structures, pituitary function and endocrinological activity of the tumor. A predominant growth into the sphenoid sinus and clivus poses specific challenges. We reviewed 557 surgeries for pituitary neuroendocrine tumors in an endonasal endoscopic technique performed between 1 January 2015 and 31 August 2025 to identify 13 cases (2.3%). Clinical, radiological and surgical data were selected by chart review. Thirteen cases aged from 31 to 68 years with almost

Multi-class classification of brain tumors using optimized CNN and transfer learning techniques.

Brain tumors are abnormal cell growths within the brain or central nervous system that disrupt normal brain function and can be life-threatening. Early detection and precise classification of brain tumors are critical for effective treatment and improved patient prognosis. In this research, a Convolutional Neural Networks model with four convolution blocks is proposed for automated brain tumor detection and classification. The proposed scheme is applied to four types of MRI images, i.e., glioma tumor, meningioma tumor, no tumor, and pituitary tumor. The proposed architecture is optimized using

The synaptic triad in depression: how stress-related pathways converge on BDNF, NMDA receptor, and MMP-9.

Chronic stress is a major precipitating factor in depression, engaging interconnected central and peripheral pathways. In this review, we propose an integrative model that organizes the neurobiological consequences of stress around five major mechanistic domains: neurotrophic dysfunction, disturbances in inhibition-excitation (E/I) balance, dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis, neuroinflammatory activation, and monoaminergic alterations. Although each hypothesis highlights distinct aspects of stress-related pathology, we argue that they ultimately converge on shared m

Mitochondrial Dysfunction: The Cellular Bridge from Emotional Stress to Disease Onset: A Narrative Review.

Severe emotional stress constitutes a significant public-health concern associated with negative health outcomes. Although the clinical effects are well acknowledged, the specific biological mechanisms that translate emotional suffering into systemic disease remain incompletely understood. Psychological stress activates the sympathetic nervous system and hypothalamic-pituitary-adrenal axis, which directly target mitochondria and alter their bioenergetic and redox capacity. For this reason, this narrative review proposes that mitochondria serve as the primary subcellular link in the mind-body c

Understanding the Secular Decline in Testosterone: Mechanisms, Consequences, and Clinical Perspectives.

Testosterone is a key regulator of male and female physiology, influencing reproductive function, muscle and bone anabolism, metabolic homeostasis, and psychological well-being. Growing evidence indicates a secular, age-independent decline in testosterone levels across populations, a trend associated with reduced fertility, metabolic and cardiovascular dysfunction, mood disturbances, and impaired quality of life. While aging and genetic factors play a role, a wide range of modifiable influences-including obesity, physical inactivity, unhealthy dietary patterns, chronic stress, poor sleep, and

Age-Dependent Effects of Heavy Metals on the Hypothalamic-Pituitary-Testicular Axis-Related Hormones in Men.

The effect of heavy metals on male hormonal regulation-particularly the hypothalamic-pituitary-testicular (HPT) axis-remains poorly characterized. We aim to investigate associations between heavy metal exposure and HPT axis-related hormones. We analyzed data, including male participants aged 3-80 years, from a nationally representative survey. Five metals and twelve sex hormones were measured. We used multivariate linear regression and restricted cubic splines to assess associations and dose-response relationships. Mixture effects were quantified using quantile-based g computation. The modifyi
