Layer Change Concept for Extracapsular Removal of Functional Pituitary Neuroendocrine Tumors: Technical Note.

OBJECTIVE: Surgery for moderately large pituitary neuroendocrine tumors often identifies the normal pituitary gland on either the left or right side, whereas the layer of the normal pituitary gland becomes thinned or disappears on the opposite side (cavernous sinus side). Conventional extracapsular resection dissects the extra-pseudocapsular layer circumferentially to remove the tumor. However, a more reliable and easier approach is to dissect between the outer pituitary capsule and the medial wall of the cavernous sinus on the cavernous sinus side than the extra-pseudocapsular layer (the laye

Perinatal physiology and welfare in water buffalo: calving, mother-calf bonding, lactation, and postpartum disorders.

The peripartum period is a critical biological stage characterized by coordinated endocrine, physiological, and behavioral adaptations that determine successful parturition, neonatal survival, and productive efficiency in water buffaloes. This review aims to discuss the changes that buffalo cows show at calving onset, as well as the implications of the mother-calf bond, lactation, and postpartum pathologies, highlighting their relevance to animal welfare and neonate viability. As in other ruminants, parturition is initiated by the activation of the fetal hypothalamic-pituitary-adrenal axis, tr

A case report on the co-presentation of pituitary abscess and POEMS-like syndrome.

RATIONALE: POEMS syndrome is a rare paraneoplastic syndrome, manifesting with progressive distal polyneuropathy, organomegaly, endocrinopathy, monoclonal protein, and skin changes, which has multisystem manifestations and chronic cytokine overproduction. Pituitary abscess is also a rare life-threatening disease that could lead to panhypopituitarism. We report a case of pituitary abscess with high suspicion of POEMS syndrome, which to our knowledge has not been reported previously. PATIENT CONCERNS: The co-presentation of pituitary abscess and POEMS-like syndrome is rare and its clinical manif

Unconventional Uses of Human Growth Hormone.

Growth hormone (GH), a 191-amino acid peptide produced by the anterior pituitary gland, plays a central role in linear growth, body composition, metabolism, and homeostasis. Exogenous GH has been used to treat GH deficiency (GHD) in children and adults since the early 1960s. GH used to be purified from human pituitaries obtained at autopsy, with the consequent limited availability and risk of Creutzfeld Jacob disease due to prion contamination. Since the mid 1980s all the GH used to treat humans is synthesized by recombinant DNA technologies (rhGH). rhGH is currently used in several conditions

Two Decades of Endocrine Silence: Severe Hyponatremia Presenting as Late-Onset Sheehan Syndrome.

Sheehan syndrome is a rare but potentially fatal complication of postpartum hemorrhage caused by ischemic necrosis of the anterior pituitary gland. Although its incidence has decreased in countries with comprehensive obstetric care systems, it remains a significant cause of panhypopituitarism in regions lacking timely and adequate access to perinatal care. The clinical challenge lies in its insidious presentation: symptoms develop slowly and gradually, and the diagnosis may remain unrecognized for decades. We report the case of a 52-year-old woman who presented with a one-year history of progr

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Overlap syndrome between sarcoidosis and Sjögren's disease presenting as hypopituitarism: case report and review of literature.

Sarcoidosis is a granulomatous disease that affects multiple organs; however, pituitary involvement is extremely rare. Although autoimmune diseases, such as Sjögren's disease (SjD), may coexist, overlapping cases presenting with hypopituitarism are exceedingly rare. A 76-year-old man was admitted with hyponatremia and altered consciousness as chief complaints. Endocrine testing revealed panhypopituitarism, and magnetic resonance imaging showed pituitary gland and pituitary stalk enlargement with homogeneous enhancement. Whole-body imaging revealed extensive lymphadenopathy. Histological examin

Structural basis for Mg²⁺-glycine synergy in the tandem glycine riboswitch revealed by cryo-EM.

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

MAGIS syndrome: phenotypes, pathogenesis, and treatment.

Inborn errors of immunity (IEI) presenting with immunodeficiency and autoimmunity can illuminate pathways essential for immunocompetence and self-tolerance. We recently characterized a new IEI named MAGIS ("Midline malformations of the brain, Anterior pituitary gland dysfunction, Growth retardation, Immunodysregulation/immunodeficiency, and Skeletal defects") caused by heterozygous germline activating mutations in GNAI2 (encoding the heterotrimeric G-protein, Gαi2). This disorder demonstrates the central role of Gαi2 regulation of chemotaxis in humans and a novel pathway by which G-proteins re

Hormone circuit explains why most HPA drugs fail for mood disorders and predicts the few that work.

Elevated cortisol in chronic stress and mood disorders causes morbidity including metabolic and cardiovascular diseases. There is therefore interest in developing drugs that lower cortisol by targeting its endocrine pathway, the hypothalamic-pituitary-adrenal (HPA) axis. However, several promising HPA-modulating drugs have failed to reduce long-term cortisol in mood disorders, despite effectiveness in other hypercortisolism conditions such as Cushing's syndrome. The reasons for these failures remain unclear. Here, we use a mathematical model of the HPA axis to demonstrate that the pituitary an

The association between gestational selective serotonin reuptake inhibitor (SSRI) treatment and newborn thyroid screen: a large-scale cohort study.

BACKGROUND: The diagnosis of depression or anxiety treated by SSRIs has become relatively common in women of childbearing age. However, the impact of gestational SSRI treatment on newborn thyroid function is lacking. We explored the impact of gestational SSRI treatment on newborn thyroid function as measured by the National Newborn Screening (NBS) Program and identified contributory factors. METHODS: An observational large-scale study of mother-infant dyads of liveborn infants delivered between 2011 and 2022. From pituitary cells to prostate gland in health and disease: direct and indirect endocrine connections.

The prostate gland is an endocrine-sensitive organ responding to multiple stimuli. Its development and function are regulated by multiple hormones (i.e. steroids such as androgens, estrogens and glucocorticoids) but also by other key hormonal systems such as those comprised by insulin-like growth factor 1 and insulin, which are sourced by different tissues [e.g. testicles/adrenal-gland/adipose-tissue/liver/pancreas, etc.). Particularly important for the endocrine control of prostatic pathophysiology and anatomy are hormones produced and/or secreted by different cell types of the pituitary glan

Ovarian cancer causing hyperprolactinemia: A case report and narrative review.

The most common cause of hyperprolactinemia is prolactinoma. In addition, it is necessary to exclude potential physiological and pharmacological factors as well as health disorders to determine the cause of hyperprolactinemia. However, few studies have linked elevated prolactin (PRL) levels to ovarian cancer (OC). OC cells can ectopically release PRL, which then attaches to PRL receptors (PRLRs) in ovarian tissue and initiates signaling cascades that induce OC carcinogenesis. Therefore, we can consider PRL as a biomarker or tumorigenesis factor for OC. Furthermore, both PRL and PRLRs are poten

Pituitary adenylyl cyclase-activating polypeptide modulates the stress response: the involvement of different brain areas and microglia.

Stress is necessary for survival. However, chronic unnecessary stress exposure leads to cardiovascular, gastrointestinal and neuropsychiatric disorders. Thus, understanding the mechanisms involved in the initiation and maintenance of the stress response is essential since it may reveal the underpinning pathophysiology of these disorders and may aid in the development of medication to treat stress-mediated diseases. Pituitary adenylyl cyclase activating polypeptide (PACAP) and its receptors (PAC1, VPAC1 and VPAC2) are expressed in the hypothalamus and other brain areas as well as in the adrenal

Expanding the phenotype of multiple endocrine neoplasia type 5 (MEN5): Pituitary gigantism, myelolipoma and familial pheochromocytoma due to a germline pathogenic MAX variant.

PURPOSE: Multiple endocrine neoplasia type 5 (MEN5) is an emerging syndrome of endocrine and non-endocrine tumors caused by germline pathogenic variants or genomic rearrangements of the MAX gene. Although MAX variants are predominantly associated with pheochromocytoma-paraganglioma (PPGL) risk, there are a growing number of associated tumors in other organs, including pituitary adenomas. We characterized the clinical presentation of various tumors in an extensive new kindred with a novel germline pathogenic variant of MAX. METHODS: Clinical, genetic, pathological, radiological and hormonal in

[Effects of acupuncture on the hypothalamic-pituitary-ovarian axis and FSH/cAMP signaling pathway in aged rats].

OBJECTIVE: To explore the mechanism of acupuncture on improving ovarian hypofunction in aged rats from two perspectives: the overall regulation of the hypothalamic-pituitary-ovarian (HPO) axis and the local ovarian follicle stimulating hormone (FSH)/cyclic adenosine monophosphate (cAMP) signaling pathway. METHODS: Six 3-month-old female SPF-grade Sprague-Dawley (SD) rats were selected as the blank group. Another twelve 9-month-old female SD rats were randomly divided into a model group and an acupuncture group, with six rats in each. The acupuncture group received acupuncture at "Baihui" (GV2

Small Cell Lung Cancer with Dual Paraneoplastic Syndromes: A Case Report.

INTRODUCTION: Paraneoplastic syndromes are common in cancers such as lung, breast, and ovarian cancers. Still, the dual paraneoplastic syndromes of ectopic adrenocorticotropic hormone (ACTH) syndrome (EAS) and raised pancreatic enzymes at the same time are rare. EAS is due to the production of ACTH by tumors other than the pituitary gland, which stimulates the hyperplasia of the adrenal cortex to secrete excessive corticosteroids, most commonly in lung cancer. Elevated pancreatic enzymes are associated with ectopic secretion from lung cancer. Clinically, some patients with small cell lung canc

Alpha-synuclein expression in anterior pituitary cells of aged cattle.

The anterior pituitary (AP) gland must function properly for homeostasis and fertility; however, aging suppresses AP function via unclear mechanisms. α-synuclein is a protein encoded by the SNCA gene. Its monomers aggregate to the fibril form, which causes brain disease as age advances. Importantly, fibril α-synuclein propagates among neurons akin to prions. Recently, we discovered α-synuclein expression in oxytocin neurons and gonadotrophin-releasing hormone (GnRH) neurons in bovine hypothalamus. However, whether α-synuclein propagates into gonadotrophs remains unclear. We tested the hypothes

Growth hormone and gonadotropin association: A case report of full-term pregnancy in a patient with panhypopituitarism.

Hypopituitarism is the inability of the anterior pituitary gland to properly supply the hormone levels. When this disease affects all the hormones produced by the anterior pituitary, it is called panhypopituitarism (PHP). Since pituitary-derived hormones directly influence fertility, often the assisted reproduction techniques are the only option to PHP women have a full-term pregnancy. However, not all patients diagnosed with PHP properly respond to ovulation induction. Thus, a poor response may indicate decreased ovarian reserve or reflect a deficiency in other key components of ovarian funct
