Frailty and pituitary surgery: a systematic review.

BACKGROUND: Frailty is a state of physiological vulnerability rendering patients susceptible to adverse perioperative outcomes after neurosurgery. The effect of frailty on surgical success and complication rates in patients undergoing transsphenoidal pituitary surgery is unclear. METHODS: A systematic review of the literature was performed in accordance with the PRISMA statement. Studies that utilised validated metrics to report the effect of frailty on pituitary surgery were included. RESULTS: A total of 13 studies were included, comprising 124,989 patients. Frailty was exclusively assessed

Postpartum pheochromocytoma crisis associated with cardiogenic shock and Sheehan's syndrome.

We report a case of a woman in her mid-30s who developed severe cardiac shock hours after giving birth to her second child with the need for extracorporeal haemodynamic support. Initially, postpartum cardiomyopathy was suspected, and high-urgency heart transplantation was considered. However, the endocrine work-up and imaging revealed pheochromocytoma as the cause for acute heart failure that was completely reversible. Notably, the patient also developed Sheehan's syndrome with pituitary necrosis and sustained hypopituitarism, most likely as a consequence of the haemodynamic failure during phe

Acute Sheehan's syndrome.

OBJECTIVE: To present a case of a patient who developed acute Sheehan's syndrome, despite adequate estimated blood loss. CASE REPORT: Sheehan's syndrome is a relatively rare disease with various incidences, mainly due to different obstetric care factors in individual countries. Pathogenetic mechanisms are not fully understood. An important factor is reduced blood flow through the pituitary arteries caused by hypotension in the setting of postpartum hemorrhage. Subsequent clinical manifestations depend on the extent of damage to the pituitary gland, and consequently, on the loss of individual

[Abnormal elevation of growth hormone in patients with pituitary adenoma combined with cirrhosis: A case report].

The oral glucose growth hormone suppression test is commonly used in the clinical diagnosis of acromegaly, but its results can be influenced by a variety of factors. This case report discusses a patient with a pituitary tumor and concurrent liver cirrhosis, highlighting the complexities in interpreting test results under such conditions. The patient, a 54-year-old male, presented with blurred vision as his primary complaint. Notably, the physical examination revealed no changes in facial features, no enlargement of hands or feet, and no other symptoms typically associated with acromegaly, whic

Neuroendocrine Sarcoidosis: A Rare Bird.

Sarcoidosis is a systemic auto-inflammatory disease of unknown etiology. It is characterized by granuloma formation and can affect any organ. While it commonly affects the lungs and the lymphatic system, it can also involve the central nervous system, leading to neurosarcoidosis or the endocrine system and subsequent multiple endocrinal pathologies. Neurosarcoidosis can affect any part of the nervous system and therefore manifests with variable symptoms mimicking other neurological pathologies. Any endocrine gland can be infiltrated by sarcoidosis; however, the pituitary gland and the hypothal

In vivo effects of NPFF and NPAF peptides on gene expression in the brain-pituitary-gonadal axis of female half-smooth tongue soles.

Neuropeptide FF (NPFF) is one of the members of RF-amide peptide family, which has been implicated in diverse physiological processes across mammals. However, there is scarce information regarding the biological function of NPFF in fish reproduction. In the current study, npff was identified and characterized in the half-smooth tongue sole. The NPFF precursor consisted of one signal peptide and two mature peptides, designated as NPFF and NPAF. Analysis of tissue distribution indicated that npff exhibited high expression levels in the brain and pituitary, while its mRNA levels were much lower i

Unmasking the pituitary shadow 11 years postpartum: A case report of late-stage Sheehan's syndrome.

Sheehan's syndrome (SS) is a rare but significant condition that occurs as a result of postpartum pituitary gland infarction, typically following severe blood loss or hypotension during or after childbirth. The syndrome is characterized by signs of anterior pituitary insufficiency, including deficiencies in hormones such as cortisol, thyroid hormones, and gonadotropins. The diagnosis of this rare but potentially fatal disease is often delayed due to the vague symptoms and the insidious nature of pituitary dysfunction. It is typically confirmed through hormonal assays and imaging, which reveal

Surgery for the treatment of arterial hypertension in patients with unilateral adrenal incidentalomas and mild autonomous cortisol secretion (CHIRACIC): a multicentre, open-label, superiority randomised controlled trial.

BACKGROUND: Adrenal incidentalomas are found in 3-10% of adults undergoing abdominal imaging. Of these, 30-50% are responsible for mild autonomous cortisol secretion (MACS), which is frequently associated with hypertension. The impact of adrenalectomy on hypertension in patients with unilateral incidentalomas and MACS remains uncertain. The aim of the CHIRACIC study was to prospectively assess the impact of surgical excision of the incidentaloma on blood pressure with a randomised trial combining accurate blood pressure measurement and standardisation of antihypertensive treatment. METHODS: C

Gonadotrophs have a dual origin, with most derived from early postnatal pituitary stem cells.

Gonadotrophs are the essential pituitary endocrine cells for reproduction. They produce both luteinizing (LH) and follicle-stimulating (FSH) hormones that act on the gonads to promote germ cell maturation and steroidogenesis. Their secretion is controlled by the hypothalamic gonadotrophin-releasing hormone (GnRH), and gonadal steroid feedback. Gonadotrophs first appear in the embryonic pituitary, along with other endocrine cell types, and all expand after birth. While gonadotrophs may display heterogeneity in their response to GnRH, they appear, at least transcriptionally, as a homogenous popu

Extensive Nail Changes Are a Possible Clue Indicating Multisystem Involvement in Childhood Langerhans Cell Histiocytosis: A Case Report and Literature Review.

INTRODUCTION: Langerhans cell histiocytosis (LCH) is a rare myeloid neoplasm that can involve nearly any organ, leading to multisystem damage. Nail involvement in LCH is particularly uncommon. Here we report a case of a young boy with multisystem LCH initially presenting with nail changes. CASE PRESENTATION: We described a boy who presented with a 2-year history of asymptomatic changes characterized by onycholysis, subungual hyperkeratosis, and purpuric striae affecting most fingernails and toenails, initially attributed to onychomycosis. Two years later, he developed multisystem involvement

Pituitary apoplexy in the setting of severe headache and unconsciousness.

Pituitary apoplexy is a rare but life-threatening condition caused by acute ischaemia or haemorrhage within the pituitary gland, often occurring in pre-existing adenomas. We present a case of an unconscious patient with acute headache, initially managed empirically for suspected meningitis. Investigations, such as cerebrospinal fluid analysis, revealed initially xanthochromia, prompting evaluation for subarachnoid haemorrhage. Magnetic resonance imaging and pituitary function testing identified a pituitary macroadenoma with optic chiasm compression and hypopituitarism, leading to successful tr

Case Report: A novel likely pathogenetic variant of the MEN1 gene in multiple endocrine neoplasia type 1.

BACKGROUND: Multiple endocrine neoplasia type 1 (MEN1) is a rare disease caused by mutations in the oncosuppressor gene MEN1 and characterized by co-occurrence of tumors of the parathyroid gland, pancreas, and pituitary gland. The clinical manifestations of MEN1 are varied, and misdiagnosis is common. The life expectancy of patients with untreated MEN1 is short. Here, we report a case of a 50-year-old patient with recurrent urinary calculi for more than 10 years who had a pancreatic neuroendocrine tumor and parathyroid adenoma. The patient received a definitive diagnosis of MEN1. We analyze hi

Novel STAG2 variant expands Mullegama-Klein-Martinez Syndrome phenotype.

We present a novel case of a female patient with a de novo heterozygous splice site pathogenic variant STAG2 (NM_001042749.3):c.1196 + 4_1196+7del. The STAG2 gene encodes "Stromal Antigen 2" (STAG2), a fundamental subunit of the cohesin complex, which regulates the separation of sister chromatids during cell division. Pathogenic STAG2 gene variants are associated with Mullegama-Klein-Martinez Syndrome (MKMS), a rare X-linked cohesinopathy. This patient exhibits symptoms not previously associated with MKMS, thereby expanding the known clinical phenotype of this rare disease. The patient has sev

Pituitary surgery in New Zealand: A single neurosurgeon case series of 1,224 operations.

Pituitary neuroendocrine tumours (PitNETs) are among the most common tumours of the central nervous system, yet data regarding surgical outcomes is limited for New Zealand (NZ). This study presents a 23-year single-surgeon case series of pituitary surgery. A retrospective analysis of 1,224 pituitary operations performed between August 1999 and July 2023 was conducted. Patient demographics, tumour characteristics, complications, and long-term outcomes were analysed. Statistical analyses included univariate and multivariate models. Pituitary neuroendocrine tumours (PitNETs) accounted for 88 % of

Pituitary Photon-Counting Detector CT for Cushing Disease: Preoperative Lesion Localization, Intraoperative Findings, and Postoperative Outcomes.

BACKGROUND AND PURPOSE: MRI fails to localize a tumor in up to 40% of cases of adrenocorticotropic hormone-dependent Cushing disease (CD), impacting patient outcomes and creating uncertainty in the diagnosis. Photon-counting detector CT (PCD-CT) is a new CT technology with better imaging performance than conventional CT. PCD-CT is a novel imaging technique for CD, especially for MR-negative cases, however, its application to CD remains unknown. MATERIALS AND METHODS: We present a cohort of consecutive patients with a preoperative PCD-CT undergoing transsphenoidal surgery for CD at a single in
