Clinical characteristics associated with somatic GNAS mutations in acromegaly: a systematic review and institutional experience.

INTRODUCTION: Acromegaly is a rare, insidious disease associated with significant morbidity and mortality usually caused by a growth hormone (GH)-secreting pituitary tumor. Somatic mutations in GNAS are common in these tumors, yet their diagnostic, prognostic, and therapeutic implications are less clear. METHODS: We conducted a structured review of the literature and meta-analysis to investigate the association of GNAS mutation status with clinical characteristics and treatment outcomes in adult patients with acromegaly. This was complemented by an analysis comparing patients with acromegaly

Decoy Lesion in Functional Multiple Pituitary Adenomas: Literature Review and Illustrative Cases on Diagnostic Pitfalls and Surgical Strategies.

BACKGROUND: Multiple pituitary adenomas (MPA), particularly those involving functioning adenoma, represent a special subtype of pituitary adenomas that pose dual challenges in diagnosis and treatment. In this setting, the coexistence of hormonally active and silent lesions may result in a diagnostic pitfall when a radiologically dominant tumor does not correspond to the hormonally responsible lesion. This misleading lesion, often referred to as decoy lesion, can adversely influence surgical decision-making in patients with MPA. METHODS: We systematically reviewed English-language case reports

Disorders of Sodium.

Sodium imbalances are a common occurrence in the emergency department. Although recognition and diagnosis are relatively straightforward, discovering the cause and management should be approached systematically. The most important history items to ascertain is if the patient has symptoms and how long this imbalance has taken to develop. Treatment rapidity depends on severity of symptoms with the most rapid treatment occurring in only the severely symptomatic. Overcorrection has dire consequences and must be approached in a careful and systematic fashion in order to prevent these devastating co

Fever and syndrome of inappropriate antidiuretic hormone secretion in a patient with multiple system atrophy after treatment with duloxetine.

BACKGROUND: Multiple system atrophy (MSA) is a neurodegenerative disease that affects the extrapyramidal, cerebellar, and autonomic nervous systems. In some patients, hypothalamic involvement can lead to impaired thermoregulation and hyponatremia. Duloxetine, a serotonin-norepinephrine reuptake inhibitor used for chronic pain, has rarely been linked to fever and syndrome of inappropriate antidiuretic hormone secretion (SIADH). CASE PRESENTATION: A 66-year-old man with clinically probable MSA and baseline hyponatremia (serum sodium 121.6 mmol/L) started duloxetine 30 mg/day for migrating pain.

Early postoperative LDDST enhances remission prediction beyond morning cortisol in cushing's disease.

PURPOSE: Transsphenoidal surgery (TSS) is the recommended treatment for Cushing's disease (CD). While low morning serum cortisol (MSeC) indicates early remission, the predictive value of an early postoperative overnight 2 mg low-dose dexamethasone suppression test (LDDST) remains unclear. This study compares the accuracy of early postoperative MSeC and LDDST for predicting early and long-term remission after TSS. METHODS: We retrospectively analysed 342 CD surgical cases, who underwent TSS in our department between 2004 and 2024. Morning cortisol measurements were obtained postoperatively. An

Individualized gamma knife radiosurgery prescription dosing for pituitary adenomas: development and internal validation of a feedforward neural network model.

BACKGROUND: Gamma Knife radiosurgery (GKRS) is an established treatment for pituitary adenomas yet prescription dose selection is often guided by clinician experience. Data-driven models may help standardize dose selection using routinely available clinical and imaging features. OBJECTIVE: To develop and internally validate a feedforward neural network (FNN) to predict GKRS prescription dose for pituitary adenomas. METHODS: We retrospectively analyzed 102 pituitary adenomas treated with GKRS at a single center. Model inputs included age at treatment, sex, race, pre-treatment Karnofsky Perfor

Anterior scleral thickness in acromegaly: a prospective multimeridional optical coherence tomography analysis.

PURPOSE: Acromegaly is a chronic endocrine disorder characterized by prolonged exposure to excess growth hormone (GH) and insulin-like growth factor-1 (IGF-1), leading to systemic connective tissue remodeling. Although corneal and choroidal structural changes have been extensively reported, scleral alterations remain poorly understood. This prospective case–control study aimed to quantitatively assess anterior scleral thickness (ST) across multiple meridians in patients with acromegaly compared with healthy controls using anterior segment optical coherence tomography (AS-OCT). METHODS: Fifty-f

Surgical outcomes of endoscopic endonasal surgery for nonfunctioning pituitary adenoma in elderly patients: a comprehensive analysis beyond age : Surgery for pituitary adenoma among elderly patients.

BACKGROUND: Due to the rapid aging of the global population, it is challenging to identify elderly individuals who are optimal candidates for the surgical treatment of pituitary neuroendocrine tumors. While chronological age has traditionally influenced surgical decision-making, the independent effect of age on outcomes requires a comprehensive evaluation that controls for comorbidities and tumor characteristics. METHODS: This retrospective analysis included 305 patients (≥18 years) who underwent endoscopic endonasal surgery for clinically nonfunctioning pituitary neuroendocrine tumors between

Biological targets and therapeutic strategies in the treatment of non-functioning pituitary tumors: state-of-the-art.

Non-functioning pituitary neuroendocrine tumors (NF-PitNETs) represent one of the most common and heterogenous sellar pathology, and are nowadays still orphan of an effective pharmacological therapy that could substitute surgery or support in case of persistency/recurrence. In fact, despite NF-PitNETs are generally benign tumors, they can be characterized by supra- or extra-sellar growth with invasion of the cavernous sinus and/or involvement of the optical chiasm. For these reasons, lots of studies tried to unveil the molecular mechanisms underlying resistance and tumoral cells growth, so as

Surgical outcomes and longitudinal quality of life after endoscopic endonasal resection of craniopharyngioma.

BACKGROUND: Craniopharyngiomas present a complex challenge for the neurosurgeon. The imperative is finding the balance between optimizing the extent of resection, decompressing critical structures and limiting the morbidity to the patient. The endoscopic endonasal approach is increasingly utilised for the management of craniopharyngioma, offering improved access to the suprasellar region while minimizing morbidity to the optic apparatus and neurovascular structures. Data regarding post operative quality of life outcomes remains limited in the literature. This mutli-centered prospective study e

Diffusion-based virtual multi-stain staining for pituitary adenoma histopathology.

BACKGROUND AND OBJECTIVE: Immunohistochemistry (IHC) for lineage-defining transcription factors-such as Pit1, Tpit, and SF1-together with markers like Ki67 and ACTH, is central to classifying pituitary neuroendocrine tumors and guiding management. Yet preparing multiple IHC slides beyond hematoxylin-eosin (H&E) is costly and inconsistent across sites. We aim to produce clinically reliable virtual IHC stains from a single H&E image under unpaired supervision. METHODS: We develop a diffusion-based one-to-many virtual staining framework that transforms a single H&E image into five key IHC stains

Real-time intraoperative depth estimation in transsphenoidal surgery using deep learning: A feasibility study.

PURPOSE: Endoscopic endonasal and transcranial approaches are used for the resection of various pathological lesions in neurosurgery, especially pituitary adenomas, craniopharyngiomas, chordomas, or meningiomas. The video feed provided by endoscopes is generally two-dimensional, which can hinder depth perception. Thus, generating three-dimensional imaging without the need for special endoscopes using deep learning might be beneficial for enhanced intraoperative orientation. METHODS: DINOv2 is a pre-trained deep-learning model published by Meta in 2023. One of its capabilities is to estimate t

Dopamine agonist withdrawal in patients with prolactinoma.

Dopamine agonists (DA) are an effective first-line treatment for prolactinomas, achieving high rates of prolactin normalization, gonadal recovery and tumor shrinkage. However, chronic DA therapy also has many downsides, including common short-term side effects such as nausea and dizziness, the risk of impulse control or emotional disorders, and the need for cardiac valvular monitoring at high doses. This duality of beneficial and adverse DA effects has led clinicians to more frequently attempt treatment reduction and withdrawal. In this review, we will summarize the evidence and determine the

Hyponatremia: Pathophysiology of different etiologies.

Hyponatremia is the most frequently encountered electrolyte disorder in clinical practice. Understanding the pathophysiology of the many different etiologies of hyponatremia is essential for appropriate diagnosis and therapy of hyponatremic disorders. Hyponatremia can occur by an increase in total body water (TBW), a decrease in body Na+ or K+, or any combination of these. In many cases, more than one of these mechanisms is operant. Therefore, a classification system to separate the various etiologies of hyponatremia should be based on factors other than the level of serum [Na+] itself. An ass

The psychological impact of adult-onset craniopharyngioma: A follow-up survey.

PURPOSE: Adult-onset craniopharyngioma (AoC) is a rare benign tumour of the sellar/parasellar region associated with significant physical morbidity and a poorer quality of life. Limited medical studies have documented the psychological impact of AoC, but psychological studies may be better placed to investigate these effects. This study used an exploratory sequential mixed-methods design to develop and administer a new patient-informed survey to quantitatively assess the psychosocial consequences of AoC. METHODS: A 30-item questionnaire was developed based on previous qualitative interviews wi

Non-thyroidal illness (euthyroid sick) syndrome: Laboratory aspects and clinical significance in critically ill patients and other diseases - A narrative review.

Formerly termed euthyroid sick syndrome, non-thyroidal sickness syndrome (NTIS) is a disorder that frequently occurs in acute or chronic illnesses that alter the levels of thyroid hormone and patterns, even in the absence of hypothalamic-pituitary-thyroid axis problems or diseases. The primary findings on the thyroid hormone panel in NTIS are elevated reverse T3 (rT3) and decreased triiodothyronine (T3) levels, which may be followed by other thyroid hormone abnormalities, such as thyroid-stimulating hormone (TSH) and thyroxine (T4). The incidence of NTIS increases among hospitalized patients w
