Long-term cyclic ectopic adrenocorticotropic hormone syndrome with extreme fluctuations and adrenal insufficiency.

A 77-year-old woman presented with worsening glycemic control and hypokalemia and was found to have adrenocorticotropic hormone (ACTH)-dependent hypercortisolism with loss of diurnal cortisol variation and failure of suppression on low- and high-dose dexamethasone suppression tests. Computed tomography (CT) demonstrated bilateral adrenal enlargement, whereas pituitary magnetic resonance imaging and chest CT failed to identify a source of ACTH secretion, leading to a diagnosis of occult ectopic ACTH syndrome. During 15 years of follow-up, she exhibited cyclic fluctuations in ACTH and cortisol c

Marked hypokalemia at presentation in an adolescent with ectopic ACTH syndrome secondary to a bronchial carcinoid tumor: a case report.

INTRODUCTION: Ectopic ACTH syndrome (EAS) is a rare cause of Cushing's syndrome in children and adolescents, accounting for less than 1% of pediatric cases. Compared with Cushing's disease, EAS follows a more rapid clinical course with more pronounced metabolic disturbances, among which hypokalemia is one of the most distinctive features. CASE PRESENTATION: A 14.7-year-old male presented with a three-month history of rapid weight gain, violaceous striae, hypertension, and proximal muscle weakness. Marked hypokalemia (2.8 mmol/L) prompted endocrine work-up, which revealed ACTH-dependent hyperc

Ectopic ACTH Syndrome Caused by Pheochromocytoma: A Case Report and Systematic Review.

INTRODUCTION: Ectopic adrenocorticotropic hormone (ACTH) syndrome caused by pheochromocytoma is extremely rare, and its highly variable clinical manifestations make diagnosis challenging. CASE PRESENTATION: This report describes a case of a 48-year-old female with typical Cushingoid features who complained of edema in both lower limbs for 20 days. Laboratory testing showed hypokalemia with metabolic alkalosis, high levels of cortisol and ACTH, and the absence of circadian rhythm. Neither low-dose or high-dose dexamethasone suppression testing could inhibit cortisol secretion. Abdominal comput

Disease activity and maternal-fetal outcomes in pregnant women with cushing's syndrome: a systematic review and meta‑analysis.

Pregnancy in women with Cushing’s syndrome (CS) is uncommon due to the detrimental effects of hypercortisolism on fertility and is associated with an increased risk of maternal and fetal complications. This systematic review evaluated the natural course and maternal–fetal outcomes of CS during pregnancy. Searches were conducted in MEDLINE, EMBASE and LILACS for observational studies including at least three pregnant women with CS. Twenty-two studies comprising 381 pregnancies were included. Among 200 women with reported etiologies, 54% had Cushing’s disease, 24% had adrenal adenoma, and 11% ha

[68Ga]Ga-DOTATATE PET/CT and PET/MR enhances the detection of pituitary ACTH-secreting adenomas in cushing's disease.

INTRODUCTION: This study aimed to evaluate the diagnostic performance of [68Ga]Ga-DOTATATE PET/CT or PET/MR in comparison to dynamic MR (dMR) in locating ACTH-secreting pituitary adenomas in Cushing's disease (CD). The utility of PET and MR semi-quantitative parameters was additionally explored as non-invasive biomarkers for tumor characterization. METHODS: Aretrospective analysis was conducted in 14 patients with CD and 16 controls with ectopic ACTH syndrome. Surgical pathology and post-operative follow-up served as reference standard. All patients underwent [68Ga]Ga-DOTATATE PET/CT or PET/M

Is diagnosis of Cushing's disease moving beyond Bilateral inferior petrosal sinus sampling?

BACKGROUND AND OBJECTIVE: Bilateral inferior petrosal sinus sampling (BIPSS) is the gold standard for distinguishing Cushing's Disease (CD) from Ectopic ACTH syndrome (EAS). It is however rarely evaluated in an intention-to diagnose-analysis, even though final diagnosis remains inconclusive in 7 to 39% of patients in published series. It has been suggested that BIPSS use may be reduced in patients explored by modern imaging and hormonal testing. We tested this hypothesis. DESIGN AND METHODS: Evolution of BIPSS use and diagnostic outcomes over time in a single-center retrospective study of 229

Fourteen-year bridge to cure in occult ectopic ACTH syndrome: resection of a 3-mm pulmonary carcinoid.

Ectopic adrenocorticotropic hormone (ACTH) syndrome (EAS) often involves occult microtumors, making localization challenging. We report a 68-year-old woman with severe ACTH-dependent hypercortisolism in whom the primary tumor remained occult despite extensive imaging, including 68Ga-tetraazacyclododecanetetraacetic acid-D-Phe(1)-Tyr(3)-octreotide positron emission tomography/computed tomography. Based on a positive octreotide challenge test, she received long-acting release octreotide for over a decade, achieving sustained biochemical stability. In 2020, she developed breast cancer, with the t

Ectopic Adrenocorticotropic Hormone Syndrome in a Patient Diagnosed With Lung Adenocarcinoma and Hospitalized Due to Diabetic Ketonuria: A Case Report.

Ectopic adrenocorticotropic hormone (ACTH) syndrome (EAS) is most often a paraneoplastic manifestation of small-cell lung cancer and is rare in lung adenocarcinoma. Its features include a cushingoid appearance, skin hyperpigmentation, muscle weakness, hypertension, glucose intolerance, hypokalemia, and metabolic alkalosis resulting from nonsuppressible, ACTH-driven hypercortisolism. We report a 70-year-old man admitted with pneumonia and diabetic ketonuria without acidosis, in whom EAS secondary to a poorly differentiated lung adenocarcinoma was diagnosed. Key biochemical findings were a marke

A case report of small-cell carcinoma of the prostate with ectopic adrenocorticotropic-hormone (ACTH) syndrome and hypokalemia.

BACKGROUND: Small-cell prostate cancer is the most common type of treatment-related neuroendocrine prostate cancer. It typically originates from prostate adenocarcinoma following prolonged endocrine therapy, presenting as desmoplasia-resistant. At this stage, indicators such as prostate-specific antigen (PSA) and androgen receptor (AR) remain within normal ranges, conflicting with widespread metastatic lesions. Simultaneously, neuroendocrine indicators exhibit a significant elevation. Small-cell prostate cancer can secrete hormones, leading to ectopic endocrine syndrome. CASE DESCRIPTION: In

Single-Cell Transcriptomic Profiling of Ectopic ACTH-Secreting Pheochromocytoma Reveals the Chromaffin Cell Origin of Ectopic Hormone Production.

Ectopic ACTH-secreting pheochromocytomas are rare and life-threatening endocrine tumors responsible for hypertension, paroxysmal symptoms, and Cushing's syndrome. The cellular origin of ACTH and the tumor's molecular characteristics remain poorly understood. Single-cell RNA sequencing was performed on tumor specimens and adjacent adrenal tissues from three patients with ectopic ACTH-secreting pheochromocytomas. Integrated bioinformatic analyses, including differential expression, functional enrichment, cell-cell communication, and pseudotemporal trajectory inference, were conducted. Key findin

Hidden in the Mediastinum: A Grade 1 Neuroendocrine Tumor Revealed by Refractory Hypokalemia and Ectopic Adrenocorticotropic Hormone (ACTH)-Dependent Cushing's Syndrome.

Ectopic adrenocorticotropic hormone (ACTH)-dependent Cushing's syndrome (CS) is a rare, fulminant variant of endogenous hypercortisolism that demands rapid localization of the ACTH source to achieve a cure. A 51-year-old woman presented with progressive proximal muscle weakness, centripetal obesity, new-onset hypertension with a blood pressure of 150/90 mmHg at arrival, and persistent hypokalemia (2.2 mmol/L). Biochemical testing confirmed severe hypercortisolism that failed to suppress after an overnight 1-mg dexamethasone test. Plasma ACTH was markedly elevated (798 pg/mL), indicating ACTH-d
