Osilodrostat Treatment for Adrenal and Ectopic Cushing Syndrome: Integration of Clinical Studies With Case Presentations.

Although most cases of endogenous Cushing syndrome are caused by a pituitary adenoma (Cushing disease), approximately one-third of patients present with ectopic or adrenal causes. Surgery is the first-line treatment for most patients with Cushing syndrome; however, medical therapy is an important management option for those who are not eligible for, refuse, or do not respond to surgery. Clinical experience demonstrating that osilodrostat, an oral 11β-hydroxylase inhibitor, is effective and well tolerated comes predominantly from phase III trials in patients with Cushing disease. Nonetheless, r

Ectopic ACTH syndrome secondary to small-cell oropharyngeal cancer.

A woman in her 80s with known metastatic small-cell oropharyngeal cancer was admitted after a fall. There was proximal limb weakness with hypertension, metabolic alkalosis, hypokalaemia and hyperglycaemia. CT scans revealed progression of her cancer with enlarging liver and lung metastases. Her admission was complicated by delirium and progressive dysphagia relating to her primary oropharyngeal malignancy. Cortisol and adrenocorticotropic hormone (ACTH) were markedly raised and were not suppressed after overnight and high-dose dexamethasone tests. Typical clinical features of Cushing's syndrom

Primary Renal Neuroendocrine Tumor Presenting as Ectopic ACTH Syndrome.

Ectopic adrenocorticotropin syndrome (EAS) is rarely caused by genitourinary neuroendocrine tumors (NETs). We describe an unusual case of renal NET presenting with severe EAS. A 32-year-old woman had presented to endocrine clinic with a 2-month history of proximal muscle weakness, skin hyperpigmentation, amenorrhea, and weight gain. She was hypertensive (160/90 mm Hg) with facial puffiness, hirsutism, and obesity. Investigations suggested high 8 Am cortisol, 24-hour urine free cortisol, and high adrenocorticotropin level. She required inpatient admission because of worsening symptoms, and work

Survival probabilities in patients with ectopic Cushing's syndrome-a systematic review and a single-arm meta-analysis.

OBJECTIVE: We aimed to estimate 1- and 5-year survival probabilities in patients with different forms of ectopic Cushing's syndrome (ECS) and identify factors influencing survival. METHODS: In this systematic review and meta-analysis, we searched the online databases PubMed, Scopus, and Web of Science up to October 18, 2023, for studies reporting survival in patients with ECS. Data extraction and risk of bias assessment were performed by 3 independent investigators. Primary outcome was survival in patients with ECS and secondary outcome was factors influencing survival. RESULTS: We included

Utility of gel filtration chromatography in evaluating successful resection of ectopic adrenocorticotropic hormone-producing tumor: a case report and literature review.

Ectopic adrenocorticotropic hormone (ACTH) syndrome resulting from ectopically secreting tumors poses a significant clinical challenge. Accurately identifying the tumor source and achieving curative resection are pivotal for patient prognosis; however, achieving these objectives is often complicated by complex ACTH secretion patterns. High-molecular-weight ACTH, frequently secreted by ectopic ACTH-producing tumors, is distinct from the conventional 39-amino-acid ACTH (ACTH1-39) produced by the pituitary gland. We believe that the evaluation of ACTH characteristics using gel filtration chromato

Radiofrequency Ablation: Alternative Treatment of Ectopic Adrenocorticotropic Syndrome Secondary to Lung Carcinoid Tumor.

Ectopic ACTH secretion is a rare, severe condition covering a wide spectrum of tumors of different location and aggressiveness. The usual poor prognosis of patients with ectopic ACTH secretion requires prompt management. We report a case of a patient with high surgical risk and intolerance to cortisol-lowering drug, in which hypercortisolism was successfully treated with radiofrequency ablation of a lung carcinoid tumor. A 78-year-old woman was referred for a rapidly progressive ACTH-dependent Cushing syndrome (CS). Biological tests suggested the presence of CS due to ectopic ACTH production.

Advances in the medical management of hypercortisolism: current strategies, monitoring, and emerging therapies.

PURPOSE OF REVIEW: This review outlines the indications for medical management of hypercortisolism and summarizes available and emerging pharmacologic agents. RECENT FINDINGS: Endogenous Cushing syndrome is a complex endocrine disorder resulting from chronic cortisol excess. The underlying etiology of Cushing syndrome is due to either adrenocorticotropic hormone (ACTH)-dependent causes, such as pituitary tumors (Cushing disease) or ectopic ACTH syndrome from nonpituitary neuroendocrine tumors, or ACTH-independent causes, such as autonomous cortisol production by adrenal adenomas, hyperplasia,

Bilateral Adrenal Ablation for the Treatment of Ectopic ACTH Syndrome With Severe Hypokalemia: A Case Report and a Literature Review.

Ectopic ACTH secretion-related Cushing's syndrome may manifest as a rapid onset of hypercortisolism, elevated urinary free cortisol (UFC) and blood ACTH concentrations, and hypokalemia. However, for patients whose primary lesions are difficult to remove, bilateral adrenalectomy is the recommended treatment method according to guidelines. Here, we present a case of non-small cell lung cancer with metastasis. Due to significant deconditioning, the patient was unable to undergo bilateral adrenalectomy and instead underwent catheter-based adrenal ablation; subsequently, he achieved clinical remiss

A Case of Micro-medullary Thyroid Carcinoma Presenting as Cancer of Unknown Primary.

Cancer of unknown primary (CUP) with neuroendocrine features is classified as a favorable subset, and treatment is recommended based on the grade of the neuroendocrine tumor (NET). Although rare, medullary thyroid carcinoma (MTC) can present as CUP with neuroendocrine features and should be carefully considered as a crucial differential diagnosis in such cases. A 65-year-old man with no relevant medical history presented with a gradually enlarging left cervical mass. Laboratory evaluation revealed hypokalemia due to adrenocorticotropic hormone (ACTH)-dependent Cushing's syndrome. Contrast-enha

Episodic Severe Ectopic Adrenocorticotropic Hormone Syndrome by Metastatic Appendiceal Neuroendocrine Tumor.

Episodic severe Cushing syndrome resulting from ectopic ACTH secretion (EAS) from appendiceal neuroendocrine tumor (NET) is extremely rare. Here, we describe a case of a 24-year-old woman with episodic severe EAS resulting from appendiceal NET with extensive metastatic disease. The patient presented with rapid weight gain, violaceous striae, fatigue, edema, and anxiety. Biochemical evaluation showed markedly elevated 24-hour urinary free cortisol greater than 10-fold above the upper limit of normal, and widely fluctuating peaks and troughs of serum cortisol and ACTH concentrations indicating e

Diagnosis of Cushing's Disease.

Cushing's disease (CD), the most common cause of Cushing's syndrome (CS), is ideally treated by resection of the underlying ACTH-producing pituitary adenoma. However, it may be difficult to establish that CD is the underlying etiology of CS. In this article, we outline a suggested diagnostic approach and discuss common pitfalls, from initial screening for hypercortisolism through the evaluation of ACTH-dependence and confirmation of the presence of a pituitary tumor using imaging and biochemical testing. Most diagnostic steps should preferably take place at a center specialized in pituitary di

Ectopic Adrenocorticotropic Hormone Syndrome Secondary to Undifferentiated Pleomorphic Sarcoma of the Thigh: A Rare Paraneoplastic Manifestation.

Undifferentiated pleomorphic sarcoma (UPS) is a rare, high-grade soft tissue sarcoma. Ectopic adrenocorticotropic hormone (ACTH) syndrome is an uncommon paraneoplastic phenomenon and is most frequently associated with neuroendocrine tumors. Herein, we present a unique case of a 55-year-old woman diagnosed with UPS of the left thigh, complicated by paraneoplastic ectopic ACTH syndrome. To our knowledge, this is the first reported case of ectopic ACTH syndrome associated with UPS in the adult population.

Revamped perspective on conventional interpretation: the foreboding prognostic significance of low-lateralization in inferior petrosal sinus sampling for diagnosis of Cushing's disease.

CONTEXT: Bilateral inferior petrosal sinus sampling (BIPSS) is widely used to differentially diagnose ectopic ACTH syndrome (EAS) and Cushing's disease (CD) with pituitary lesions less than 6 mm detected on MRI. PURPOSE: This study aimed to investigate the diagnostic accuracy of BIPSS and prognosis of low-lateralization [lateralization ratio of bilateral inferior petrosal sampling (IPS) ≤ 1.4] CD with microadenoma (≤ 6 mm). METHODS: This single-centre retrospective study (2011-2019) included 25 EAS and 179 CD (microadenoma ≤ 6 mm) patients whose diagnoses were confirmed by pathology. All CD

A Second Look at Refractory Edema: Delayed Diagnosis of Paraneoplastic Cushing's Syndrome in Small Cell Lung Cancer.

Paraneoplastic Cushing syndrome (PCS) is a rare manifestation of ectopic adrenocorticotropic hormone (ACTH) production, mostly associated with bronchial carcinoid and small cell lung cancer (SCLC). Its clinical manifestations: refractory hypertension, profound hypokalemia, metabolic alkalosis, worsening hyperglycemia, and edema, can easily be misattributed to more common conditions, especially in older adults with multiple comorbidities, leading to diagnostic errors. We present a case of an 84-year-old man with a history of stage IA non-SCLC treated one year earlier, who developed progressive

Hypokalemic metabolic alkalosis as a clinical clue to ectopic ACTH syndrome: two cases of neuroendocrine carcinoma.

Objective. Ectopic adrenocorticotropic hormone (ACTH) syndrome (EAS) is a rare, but potentially life-threatening cause of Cushing's syndrome. Its clinical recognition may be delayed, especially when classical features of hypercortisolism are absent. We present two cases, in which hypokalemic metabolic alkalosis was the initial and main clinical clue leading to the diagnosis of neuroendocrine carcinoma with ectopic ACTH production. Case 1. The first case was a 71-year-old woman admitted with progressive weakness, gait disturbance, and uncontrolled hypertension. Laboratory tests revealed severe

The Modern Presentation, Etiology, and Rate of Occurrence of Cushing Syndrome Differs when Compared to Prior Population-Based Studies.

BACKGROUND/OBJECTIVE: Endogenous Cushing Syndrome (CS) is considered a rare disorder, with European population studies estimating an incidence of 1.5-3.2 cases/million population/year and Cushing disease (CD) being recognized as the most common etiology. The purpose of this study is to characterize the etiologies of endogenous CS, CS severity, and case frequency of CS at a single institution. METHODS: Between May 2017 and December 2022, clinic records from a single tertiary care medical center were reviewed to identify Wisconsin residents who were newly diagnosed and treated for CS. Update in the Management of ACTH-Secreting Gastroenteropancreatic and Thoracic Neuroendocrine Neoplasms.

The adrenocorticotropic hormone (ACTH)-secreting neuroendocrine neoplasms (NENs) represent a rare but frequently severe disease that is associated with poor prognosis, if not adequately managed. Treatment strategies vary according to the characteristics of the tumor and the severity of hypercortisolism. The optimal treatment is surgical resection of the NEN with a curative intent. In unresectable or metastatic tumors, medical control of hypercortisolism with concomitant treatment of NENs based on current guidelines is suggested. Steroidogenesis inhibitors are the principal choice to control hy

A comparison of bilateral internal jugular vein sampling with bilateral inferior petrosal sinus sampling at Mayo Clinic for the subtype evaluation of corticotropin-dependent cushing syndrome.

OBJECTIVE: Distinguishing between pituitary corticotropin (ACTH)-dependent Cushing disease (CD) and ectopic ACTH-dependent Cushing syndrome (EADCS) is diagnostically challenging. This study compared the diagnostic performance of bilateral inferior petrosal sinus sampling (BIPSS) and bilateral internal jugular vein sampling (BIJVS). METHODS: We retrospectively reviewed 109 patients at Mayo Clinic (1998-2018) who underwent simultaneous BIPSS and BIJVS. Eighty-four patients proceeded to pituitary surgery based on BIPSS results, and 81 ultimately received confirmed diagnoses: 74 with CD and 7 wit
