The stem bark decoction of Myrianthus arboreus P. Beauv. (Cecropiaceae) shows anti-uterine leiomyoma effects in Wistar rat.

ABSTRACT: Over the past 30 years, the number of new cases of uterine leiomyomas (UL) in women of reproductive age has increased by 67.14% worldwide. The limitations of the current therapeutic options have led to the search for alternatives. Myrianthus arboreus P. Beauv., used for infertility and tumors, has never been tested for UL. In the present study, the decoction of its stem bark was evaluated in a model of UL induced in female Wistar rats. Animals were treated once daily by gavage for 30 days. Normal control and model groups received distilled water, positive groups received mifepristone

A Novel Gene Expression Profile and Combined Effects in the Pathogenesis of Adrenocorticotropic Hormone-Secreting Pituitary Neuroendocrine Tumors.

Cushing's disease is a rare endocrine disorder characterized by excessive endogenous glucocorticoid production, primarily resulting from adrenocorticotropic hormone-secreting pituitary neuroendocrine tumors (ACTH-PitNETs). This study investigated the expression of several genes implicated in the development of ACTH-PitNETs, including EGFR, USP8, CABLES1, USP2, STAM2, VPS28, HDAC2, IL-6, SMARCA4, WEE1, CDKN2A, CCND1, NR4A1, NEUROD1, and RIPK1. The methylation levels of the USP8 and CDKN2A genes were also assessed for insights into their regulatory mechanisms.Formalin-fixed paraffin-embedded pit

Successful treatment of Cushing's disease in a pediatric patient with Gamma Knife radiosurgery: a case report.

INTRODUCTION: Cushing's disease in young-onset cases is a rare and diagnostically challenging endocrinopathy, most commonly caused by an adrenocorticotropic hormone (ACTH)-secreting pituitary adenoma. Early recognition and management are crucial to preventing long-term complications associated with hypercortisolism. This report highlights the case of a pediatric patient with Cushing's disease, emphasizing diagnostic challenges and treatment considerations. CASE PRESENTATION: We present the case of a 14-year-old male from Kashmore with a 4-year history of progressive truncal obesity, growth fa

Prospective Multicenter Evaluation of [11C]Methionine PET/MRI Sensitivity Compared with MRI for Localizing Small Pituitary Neuroendocrine Tumor or Pituitary Adenoma in Cushing Disease.

Functional small pituitary neuroendocrine tumors (PitNETs) of the corticotroph type lead to Cushing disease (CD), a condition associated with significant morbidity and increased mortality. Pituitary MRI is the primary imaging modality used to localize the tumor, but it is inconclusive in up to 30% of cases. Accumulating retrospective evidence suggests that [11C]methionine ([11C]MET) PET could address this diagnostic gap. This prospective study evaluates the sensitivity of [11C]MET PET/MRI compared with MRI for small PitNET localization in CD. Methods: This prospective multicenter study (Clinic

High prevalence of adrenal imaging abnormalities in Cushing's disease.

INTRODUCTION: Cushing’s disease (CD) is the most common cause of endogenous Cushing’s syndrome. Adrenocorticotropic hormone (ACTH) has trophic and mitogenic effects on the adrenal cortex that may cause diffuse adrenal enlargement and nodular lesions. AIM: To evaluate the prevalence of adrenal structural abnormalities in patients with CD. METHODS: Retrospective cohort study. We conducted a computerized search in our medical centers databases for the diagnosis of CD recorded between the years 1995–2024. Out of 124 patients with ACTH dependent Cushing’s syndrome, we identified 68 patients with CD

[Dilated cardiomyopathy in a patient with Cushing's disease - clinical presentation, diagnosis and treatment: a case report].

Cortisol-induced dilated cardiomyopathy (CI-DCM) is a rare manifestation of endogenous hypercortisolism (EH). Optimal management of patients with CI-DCM is a major challenge due to the rarity of the pathology and the lack of expert community guidelines. This article describes a case of successful management of a patient with ACTH-secreting pituitary tumor and CI-DCM.A 44-year-old patient was hospitalized with symptoms of chronic heart failure (CHF) and EH. The diagnosis of non-ischemic myocardial damage with phenotype of DCM was verified by echocardiography and coronary angiography. According

Development and validation of clinical screening systems for Cushing disease in the United States.

PURPOSE: Clinical screening systems (CSSs) for Cushing syndrome (CS) validated in Europe have not been evaluated for CS or Cushing disease (CD) in the United States (US). We aimed to evaluate existing CSSs in US patients and develop two new symptom-based CSSs to identify patients with high pre-test probability of disease warranting referral for definitive biochemical workup-one for broad CS screening and one specifically for CD. METHODS: Data were obtained from the Registry of Adenomas of the Pituitary and Related Disorders (RAPID)-comprising 615 patients who underwent transnasal transsphenoi

Selective Hippocampal Subfield Atrophy Mediates Cognitive Decline in Cushing's Disease.

BACKGROUND: Cushing's disease (CD) provides insight into how prolonged high cortisol exposure affects brain structure. While CD patients show cognitive and emotional symptoms linked to hippocampal function and detailed analysis of hippocampal subfield changes remains limited. METHODS: The study included 91 patients with active CD and 53 matched healthy controls who underwent T1-weighted magnetic resonance imaging and comprehensive neuropsychological assessment. We employed voxel-based morphometry, automated segmentation, and shape analysis to evaluate gray matter volume, subfield volumes, and

Giant Prolactinoma Resistant to High-Dose Cabergoline in a Young Male Lost to Follow-Up.

Pituitary adenomas are neoplasms of the adenohypophysis (anterior pituitary gland) that can cause a multitude of symptoms secondary to the hormones they secrete (functioning adenomas) or mass effect on the optic chiasma (non-functioning adenomas). Functioning adenomas can secrete prolactin, growth hormone, adrenocorticotrophic hormone (ACTH) or thyroid-stimulating hormone, with some adenomas co-secreting more than one. This can lead to symptoms of prolactinoma, acromegaly, Cushing's disease, and TSHoma dependent on the elevated hormone. In non-functioning adenomas, visual changes, and headache

Crooke Cell Adenoma Presenting With Refractory Hypokalemia and Uncontrolled Hypertension.

Crooke cell adenomas (CCAs) are a rare subtype of pituitary corticotroph adenomas with a prevalence of less than 1% of pituitary adenomas. CCAs have variable clinical manifestations and can present as functional adenomas secreting adrenocorticotropic hormone (ACTH), resulting in hypercortisolism, as well as silent invasive macroadenomas. CCAs also exhibit a higher risk for recurrence, less biochemical remission, and normalization of pituitary function after tumor resection compared to other secretory corticotroph adenomas. Refractory hypokalemia and hypertension as manifestations of hypercorti

Resistant hypertension post-transsphenoidal surgery for pituitary Cushing's disease, leading to a diagnosis of primary aldosteronism.

SUMMARY: We describe a case of a 42-year-old gentleman, 5 years post-transsphenoidal surgery (TSS) for pituitary-dependent Cushing's disease, initially presenting with malignant hypertension. Despite an initial improvement in his blood pressure post-TSS, he was found to be persistently hypertensive on follow-up despite no clinical or biochemical evidence of recurrence of hypercortisolism. His blood pressure remained elevated despite five antihypertensive agents. His renin concentration was <5 mIU/L (9-103.5) and aldosterone concentration was 877 pmol/L (0-670). A subsequent CT of the adrenals

Occult ectopic ACTH syndrome presenting with pulmonary nocardiosis and cognitive decline in a vulnerable patient.

A woman in her early 60s with a remote history of lung cancer presented with severe hypercortisolism, bilateral pulmonary consolidations and progressive cognitive decline. Extensive imaging and biopsies failed to identify a definitive adrenocorticotropic hormone (ACTH)-secreting tumour, but pulmonary nocardiosis was diagnosed, reflecting profound cortisol-induced immunosuppression. Management was further complicated by limited social supports and a history of unstable housing, raising concerns about access to advanced imaging and reliable follow-up. Despite medical therapy, hypercortisolism pe

Secondary Meniere's Syndrome Following Excessive Noise Exposure.

BACKGROUND: The effects of noise on the vestibular and balance function are under-studied. Studies have demonstrated that like the cochlea, the otolith organs are also sensitive to sound but the exact mechanism is unclear. Both impulse and continuous noise exposure may damage the cochlear and/or vestibular organ and lead to the formation of endolymphatic hydrops. We describe 9 patients who had Meniere's syndrome following excessive noise exposure. MATERIALS AND METHODS: Study design: retrospective study.Setting: Tertiary care centre in South India.Subject and methods: Medical records of patie

Prolonged adrenal insufficiency following discontinuation of osilodrostat treatment for intense hypercortisolism.

The cases of 3 patients with Cushing's disease who developed long-term adrenal insufficiency after discontinuation of prolonged osilodrostat therapy were recently described for the first time. We report 2 additional cases of persistent prolonged adrenal insufficiency after discontinuation of osilodrostat treatment for intense hypercortisolism due to Cushing's disease and ectopic ACTH syndrome. In addition, we show for that adrenal insufficiency in these patients was associated with low/normal 11-deoxycortisol concentrations despite high plasma ACTH concentrations. These results suggest that CY

Two cases of pancreatic neuroendocrine tumors with ectopic ACTH syndrome during their disease course.

Pancreatic neuroendocrine tumors (PanNETs) are rare malignant tumors that occur in the pancreas. They are divided into functioning and non-functioning tumors based on the presence or absence of their specific hormonal hyper-expression symptoms. Adrenocorticotropic hormone (ACTH)-producing PanNETs are rare, functional tumors, and their clinical characteristics and outcomes have not been well reported. Here, we report the cases of two patients with PanNETs who presented with ectopic ACTH syndrome (EAS) during the course of their disease. Case 1 involved a non-functioning PanNET at the time of sur
