Venous Drainage Patterns of the Cavernous Sinus: A Retrospective Angiographic Analysis.

BACKGROUND AND PURPOSE: Cavernous sinus (CS) venous drainage patterns may affect the accuracy of inferior petrosal sinus (IPS) sampling, but descriptions of the normal venographic CS outflow patterns are scarce. We investigated CS drainage patterns using the venous phases of normal cerebral angiograms. MATERIALS AND METHODS: A retrospective review of elective cerebral angiograms was performed. Patients were included if they had no cerebrovascular abnormalities that could influence the cerebral venous flow. Patients with prior intracranial surgery, ruptured aneurysms, arteriovenous malformatio

Safety and effectiveness of rivaroxaban thromboprophylaxis in adrenocorticotropic hormone-dependent Cushing syndrome.

CONTEXT: Adrenocorticotropic hormone (ACTH)-dependent Cushing syndrome (CS) is associated with a markedly increased risk of venous thromboembolism (VTE), yet thromboprophylaxis strategies remain inconsistent across clinical practice. In 2019, our center introduced routine oral rivaroxaban prophylaxis (10 mg once daily) for all patients with ACTH-dependent CS. OBJECTIVE: To evaluate the safety and effectiveness of routine rivaroxaban prophylaxis in ACTH-dependent CS. METHODS: We retrospectively reviewed 70 adults with ACTH-dependent CS managed between 2012 and 2025 (29 pre-2019 and 41 post-20

Impact of sex on mortality in patients with pituitary adenomas.

PURPOSE: Pituitary adenomas are increasingly diagnosed, with prolactinomas and non-functioning pituitary adenomas (NFPA) representing the large majority. Though most pituitary adenomas are indolent and very few are aggressive overall, mortality rates are higher than in general population. This varies with the type of pituitary adenoma, with the highest standardized mortality ratios in patients with Cushing’s disease (CD). Notably, sex differences in mortality remain controversial for all pituitary adenomas. METHODS: A scoping PubMed literature review with studies published in English from 1970

Sex differences in the clinical presentation, management and long-term outcomes of pituitary tumors in children and adolescents.

Pituitary adenomas (PAs) are rare in children and adolescents. Despite their typically benign histology, these tumors raise significant clinical challenges because they occur during critical developmental windows, impacting growth, puberty, and long-term quality of life. This review aims to focus on the sex-specific patterns in the epidemiology, clinical presentation, and outcomes of pediatric PAs. Prolactinomas are the most frequent subtype, showing a marked post-pubertal female predominance. Adolescent girls typically present early with endocrine symptoms like amenorrhea, whereas boys are of

Crooke Cell Adenoma: Is Its Aggressiveness Overestimated? Evidence From a Single-Centre Experience.

OBJECTIVE: Crooke cell adenomas (CCAs) are considered rare and potentially aggressive variants of corticotroph adenomas. However, data on their clinical behaviour remain limited. This study aimed to compare the clinical, biochemical, radiological, and surgical outcomes of patients with CCAs and non-CCA Cushing's disease (CD). MATERIAL AND METHODS: We retrospectively analyzed patients diagnosed with CD at our tertiary Pituitary Centre. Clinical presentation, baseline biochemical characteristics, tumour size, Ki-67 index, remission rates, recurrence, and presence of residual tumour on follow-up

Age and hematologic parameters can predict prolonged glucocorticoid replacement after remission of cushing disease and adrenal Cushing's syndrome: A nationwide cohort study.

PURPOSE: Duration of glucocorticoid (GC) replacement following curative treatment of endogenous Cushing’s syndrome (CS) is highly variable, with no validated markers to guide tapering or predict prolonged dependency. We evaluated clinical and hematologic predictors of GC duration, stratified by CS etiology and GC formulation. METHODS: This nationwide retrospective cohort included patients with endogenous CS diagnosed over 20 years (2000–2023) in the Clalit Health Services database from Israel. Patients with adrenal carcinoma or ectopic CS were excluded. Duration of GC therapy following curativ

Growth Retardation in an Adolescent Secondary to Cushing's Disease Caused by a Pituitary Microadenoma: A Case Report.

Introduction and Clinical Significance: Cushing's disease is a rare but clinically consequential cause of endogenous hypercortisolism in children, most commonly resulting from ACTH-secreting pituitary microadenomas. In contrast to adults, growth deceleration accompanied by disproportionate weight gain represents the earliest and most sensitive clinical marker in pediatric cases. Sustained hypercortisolism during critical periods of somatic maturation may compromise final height, disrupt pubertal progression, and induce persistent metabolic and neurocognitive sequelae, even after biochemical re

Unprovoked Deep Vein Thrombosis as a Manifestation of Cushing's Disease.

Cushing's disease is a rare condition, and patients have a higher risk of thromboembolism, specifically venous events. We present a case of a middle-aged woman who underwent sleeve surgery many years ago and slowly regained weight. She had two episodes of unprovoked deep vein thrombosis (DVT), and no risk factors were identified at the time of these events. She presented 10 years later with easy bruising, myopathy, gradual weight gain, and insomnia, and was found to have ACTH-dependent pituitary Cushing's. She successfully underwent endoscopic resection of her pituitary macroadenoma. She had w

Symptom management with osilodrostat in multiple endocrine neoplasia type 1 with a Cushing syndrome presentation.

Multiple endocrine neoplasia type 1 (MEN1) syndrome is a rare autosomal dominant disorder characterized by predisposition to a multitude of endocrine neoplasms. Cushing syndrome (CS) within MEN1 presents complex diagnostic and therapeutic challenges. We report a case of a 36-year-old male with adrenocorticotropic hormone (ACTH)-dependent Cushing disease and MEN1 syndrome with thymic and pancreatic neuroendocrine tumors. The patient presented with weight gain, weakness and proximal myopathy. Biochemical testing confirmed ACTH-dependent hypercortisolism. Imaging revealed a pituitary microadenoma

Management of Cushing's Disease in Pregnancy: A Case Report.

Cushing's syndrome (CS) is a rare condition encountered in pregnancy, and Cushing's disease (pituitary-dependent CS) represents an even smaller subset. The occurrence of pregnancy in women with CS is uncommon because hypercortisolism suppresses the hypothalamic-pituitary-gonadal axis, frequently leading to anovulation and infertility. When conception does occur, Cushing's disease can significantly complicate the course of pregnancy, posing serious risks to both the mother and fetus. Reported maternal complications include hypertension, gestational diabetes mellitus, pre-eclampsia, heart failur

Unsupervised clustering analysis reveals distinct postoperative cortisol trajectories following pituitary adenoma resection in Cushing's disease.

PURPOSE: Cushing’s disease (CD), caused by ACTH-secreting pituitary adenomas, results in hypercortisolism. Transsphenoidal surgery (TSS) is the first-line treatment. Early postoperative cortisol trends are frequently monitored, though standard interpretation is lacking. We sought to determine whether early postoperative cortisol trajectories are associated with biochemical remission at 1 year using unsupervised machine learning. METHODS: We retrospectively reviewed 94 adult patients with CD who underwent TSS between 2011 and 2021 at two academic centers. Serum cortisol was measured every 6 h,

Distinct plasma protein profiles after long-term remission of Cushing's disease.

OBJECTIVE: Cortisol excess in Cushing's disease (CD) induces widespread plasma protein alterations, affecting various pathways including coagulation and lipid metabolism. The extent to which these abnormalities normalize during long-term remission remains unclear. DESIGN: Cohort study investigating plasma protein profiles in CD patients before, and during long-term remission. METHODS: EDTA plasma samples of 26 CD patients were collected during active disease and during long-term remission (median 4.4 years, interquartile range 3.3-5.1). Plasma protein profiles were generated using quantitati

Carney complex with adrenocorticotropic hormone-independent Cushing syndrome with PRKAR1A gene variant: a case report.

BACKGROUND: Primary pigmented nodular adrenal disease (PPNAD) is an autosomal dominant condition and a rare cause of Cushing syndrome, independent of pituitary origin, and is adrenocorticotropin (ACTH) independent. Carney complex PPNAD (cPPNAD) is associated with inactivating mutations of the regulatory subunit type 1A of the cAMP-dependent protein kinase (PRKAR1A) gene which is found in more than 70% of Carney complex cases. PRKAR1A is a tumor suppressor gene located in the 17q23-24 region of the human chromosome and plays an important role in regulating cellular metabolism, differentiation,

Differential diagnosis of ACTH-dependent Cushing's syndrome: Biochemical, imaging, and predictive approaches.

Endogenous hypercortisolism remains a high-stakes diagnostic challenge in which delays or misclassification drive excess morbidity and mortality. This chapter synthesizes an evidence-based, physiology-anchored approach to the differential diagnosis of ACTH-dependent Cushing's syndrome, integrating clinical phenotyping, first-line biochemistry, dynamic testing, imaging, and invasive confirmation. We first outline bedside features that raise or lower the pre-test probability of Cushing disease (CD) versus ectopic ACTH syndrome (EAS). Then, the initial laboratory tests (nocturnal salivary cortiso

Molecular profile of Cushing's syndrome.

Cushing's syndrome comprises a heterogeneous group of disorders characterized by chronic excess of glucocorticoid release from adrenal glands, caused by altered adrenal function or by ACTH hypersecretion from pituitary or ectopic sources. Advances in high-throughput molecular profiling have substantially refined our understanding of the mechanisms underlying endogenous hypercortisolism. This chapter provides an integrated overview of the genetic, transcriptomic, and epigenetic alterations driving both ACTH-dependent and ACTH-independent forms of the disease. A retrospective study of severity-dependent HPG Axis suppression by glucocorticoids in Chinese women with Cushing syndrome.

BACKGROUND: Cushing syndrome (CS) is a severe endocrine disorder caused by prolonged exposure to glucocorticoid excess. Among its diverse complications, reproductive and sexual disorders arehighly prevalent, impacting patients' quality of life. Despite this clinical significance, clinicalstudies specifically investigating the hypothalamic-pituitary-gonadal (HPG) axis status in femalepatients with CS remain insufficient, highlighting the urgent need for thorough evaluation. METHOD: This retrospective cross-sectional study evaluated the HPG axis status in 137 women diagnosed with CS between 200

Diagnostic and Operative Utility of Bilateral Inferior Petrosal Sinus Sampling in ACTH-Dependent Cushing Syndrome using Vasopressin.

BACKGROUND: Accurate localization of adrenocorticotropic hormone (ACTH) secretion is crucial for the surgical management of ACTH-dependent Cushing's syndrome. While magnetic resonance imaging (MRI) and biochemical testing aid diagnosis, bilateral inferior petrosal sinus sampling (BIPSS) remains a reference standard in diagnostically challenging cases. In the absence of corticotropin-releasing hormone (CRH), vasopressin may serve as an effective alternative corticotroph stimulant during BIPSS; however, its additional benefit in lateralizing corticotroph adenomas remains uncertain. OBJECTIVE: T

Predictors of long-term remission following trans-sphenoidal surgery in cushing's disease: a systematic review and meta-analysis based on different remission criteria.

Cushing's Disease (CD) is caused by pituitary adenomas, leading to hypercortisolism. This condition burdens patients socioeconomically, with a reduced quality of life and higher mortality. Transsphenoidal surgery (TSS) is the first-line treatment, offering a 78% remission rate. This study aims to identify factors influencing TSS outcomes to find the predictors of remission for improved treatment planning. A systematic search of electronic databases was performed based on the PRISMA guideline. Studies were selected if they reported data on remission and non-remission groups. A comparative meta-

Persistent Adrenocortical Insufficiency After Long-Term Metyrapone Treatment for Cushing's Disease.

INTRODUCTION: Cushing's disease is caused by pituitary tumors that secrete excess adrenocorticotropic hormone (ACTH). Metyrapone is widely used for medical management as a bridge to transsphenoidal pituitary surgery. CASE REPORT: An 80-year-old man presented with severe hypercortisolemia (101 μg/dL [reference range, 5-23 μg/dL]) and elevated plasma ACTH, 228 pg/mL [reference range, 7.2-63.3 pg/mL]). Initial high-dose dexamethasone suppression and corticotropin-releasing hormone (CRH) tests suggested ectopic ACTH syndrome; however, extensive imaging failed to identify a source. Metyrapone was
