High prevalence of adrenal imaging abnormalities in Cushing's disease.

INTRODUCTION: Cushing’s disease (CD) is the most common cause of endogenous Cushing’s syndrome. Adrenocorticotropic hormone (ACTH) has trophic and mitogenic effects on the adrenal cortex that may cause diffuse adrenal enlargement and nodular lesions. AIM: To evaluate the prevalence of adrenal structural abnormalities in patients with CD. METHODS: Retrospective cohort study. We conducted a computerized search in our medical centers databases for the diagnosis of CD recorded between the years 1995–2024. Out of 124 patients with ACTH dependent Cushing’s syndrome, we identified 68 patients with CD

[Dilated cardiomyopathy in a patient with Cushing's disease - clinical presentation, diagnosis and treatment: a case report].

Cortisol-induced dilated cardiomyopathy (CI-DCM) is a rare manifestation of endogenous hypercortisolism (EH). Optimal management of patients with CI-DCM is a major challenge due to the rarity of the pathology and the lack of expert community guidelines. This article describes a case of successful management of a patient with ACTH-secreting pituitary tumor and CI-DCM.A 44-year-old patient was hospitalized with symptoms of chronic heart failure (CHF) and EH. The diagnosis of non-ischemic myocardial damage with phenotype of DCM was verified by echocardiography and coronary angiography. According

Development and validation of clinical screening systems for Cushing disease in the United States.

PURPOSE: Clinical screening systems (CSSs) for Cushing syndrome (CS) validated in Europe have not been evaluated for CS or Cushing disease (CD) in the United States (US). We aimed to evaluate existing CSSs in US patients and develop two new symptom-based CSSs to identify patients with high pre-test probability of disease warranting referral for definitive biochemical workup-one for broad CS screening and one specifically for CD. METHODS: Data were obtained from the Registry of Adenomas of the Pituitary and Related Disorders (RAPID)-comprising 615 patients who underwent transnasal transsphenoi

Prevalence and Associated Risk Factors for Venous Thromboembolism in a Large Cohort of Patients With Cushing Disease.

OBJECTIVE: Endogenous Cushing syndrome is associated with an intrinsic hypercoagulable state and an increased risk of venous thromboembolism (VTE). This study aimed to determine the prevalence and risk factors for VTE in a large cohort of patients with Cushing disease (CD). METHODS: A retrospective study was conducted at a tertiary care center, including 408 patients diagnosed with CD. Clinical, laboratory, hormonal, imaging, and outcome data were analyzed and compared based on the occurrence of VTE events. A control group of 323 patients with clinically nonfunctioning pituitary adenomas, all

Selective Hippocampal Subfield Atrophy Mediates Cognitive Decline in Cushing's Disease.

BACKGROUND: Cushing's disease (CD) provides insight into how prolonged high cortisol exposure affects brain structure. While CD patients show cognitive and emotional symptoms linked to hippocampal function and detailed analysis of hippocampal subfield changes remains limited. METHODS: The study included 91 patients with active CD and 53 matched healthy controls who underwent T1-weighted magnetic resonance imaging and comprehensive neuropsychological assessment. We employed voxel-based morphometry, automated segmentation, and shape analysis to evaluate gray matter volume, subfield volumes, and

Giant Prolactinoma Resistant to High-Dose Cabergoline in a Young Male Lost to Follow-Up.

Pituitary adenomas are neoplasms of the adenohypophysis (anterior pituitary gland) that can cause a multitude of symptoms secondary to the hormones they secrete (functioning adenomas) or mass effect on the optic chiasma (non-functioning adenomas). Functioning adenomas can secrete prolactin, growth hormone, adrenocorticotrophic hormone (ACTH) or thyroid-stimulating hormone, with some adenomas co-secreting more than one. This can lead to symptoms of prolactinoma, acromegaly, Cushing's disease, and TSHoma dependent on the elevated hormone. In non-functioning adenomas, visual changes, and headache

Crooke Cell Adenoma Presenting With Refractory Hypokalemia and Uncontrolled Hypertension.

Crooke cell adenomas (CCAs) are a rare subtype of pituitary corticotroph adenomas with a prevalence of less than 1% of pituitary adenomas. CCAs have variable clinical manifestations and can present as functional adenomas secreting adrenocorticotropic hormone (ACTH), resulting in hypercortisolism, as well as silent invasive macroadenomas. CCAs also exhibit a higher risk for recurrence, less biochemical remission, and normalization of pituitary function after tumor resection compared to other secretory corticotroph adenomas. Refractory hypokalemia and hypertension as manifestations of hypercorti

Resistant hypertension post-transsphenoidal surgery for pituitary Cushing's disease, leading to a diagnosis of primary aldosteronism.

SUMMARY: We describe a case of a 42-year-old gentleman, 5 years post-transsphenoidal surgery (TSS) for pituitary-dependent Cushing's disease, initially presenting with malignant hypertension. Despite an initial improvement in his blood pressure post-TSS, he was found to be persistently hypertensive on follow-up despite no clinical or biochemical evidence of recurrence of hypercortisolism. His blood pressure remained elevated despite five antihypertensive agents. His renin concentration was <5 mIU/L (9-103.5) and aldosterone concentration was 877 pmol/L (0-670). A subsequent CT of the adrenals

Cushing Syndrome in Childhood.

We describe a 15-year-old boy who presented with low back pain due to vertebral compression fractures, growth deceleration, excessive weight gain, rounded facies, dorsocervical fat pad, and hypertension. He was diagnosed as having Cushing syndrome (CS) due to primary pigmented nodular adrenocortical disease resulting in excess cortisol produced by the adrenal glands, leading to disruption of the hypothalamic-pituitary-adrenal axis. The most common cause of CS is exogenous glucocorticoids, with endogenous causes being extremely rare, often leading to delay in diagnosis or misdiagnosis. Herein,

Back to the Future: Dynamic Contrast-Enhanced Photon-Counting Detector CT for the Detection of Pituitary Adenoma in Cushing Disease.

Historically, MR imaging has been unable to detect a pituitary adenoma in up to one-half of patients with Cushing disease. This issue is problematic because the standard-of-care treatment is surgical resection, and its success is correlated with finding the tumor on imaging. Photon-counting detector CT is a recent advancement that has multiple benefits over conventional energy-integrating detector CT. We present the use of dynamic contrast-enhanced imaging using photon-counting detector CT for the detection of pituitary adenomas in patients with Cushing disease.

Recurrent Cushing's Disease Caused by a TPIT-Lineage Densely Granulated Corticotroph Pituitary Neuroendocrine Tumor: A Case Report.

INTRODUCTION: Recurrent Cushing's disease (recurrent CD) is an uncommon and intricate clinical form of Cushing's syndrome. However, the connection between the pathological types of ACTH-secreting PitNETs and the clinical signs of recurrent CD remains uncertain. CASE DESCRIPTION: A 64-year-old woman, previously diagnosed with renal carcinoma, was admitted to our hospital due to recent weight gain. Previous endocrine tests indicated fluctuating hypercortisolemia and a recurrent pituitary tumor over the past six years. She underwent two transsphenoidal hypophysectomies, and histopathological ana

Different patient versus provider perspectives on living with Cushing's disease.

CONTEXT: Patients with Cushing's disease (CD) face challenges living with and receiving appropriate care for this rare, chronic condition. Even with successful treatment, many patients experience ongoing symptoms and impaired quality of life (QoL). Different perspectives and expectations between patients and healthcare providers (HCPs) may also impair well-being. OBJECTIVE: To examine differences in perspectives on living with CD between patients and HCPs, and to compare care goals and unmet needs. DESIGN: Memorial Sloan Kettering Pituitary Center established an annual pituitary symposium fo

Paediatric Cushing's disease: long-term outcome and predictors of recurrence.

Paediatric Cushing's disease (CD) is characterized by excess ACTH secretion from a pituitary adenoma, leading to hypercortisolism. It has approximately 5% of the incidence of adult CD and is a rare disorder in the paediatric age range. The four most specific presenting features of hypercortisolism are: change in facial appearance, weight gain, decreased linear growth and virilisation shown by advanced pubic hair for the stage of breast development or testicular volume. The main diagnostic priority is the demonstration of hypercortisolism followed by distinction between its ACTH-dependent and A

Accuracy of the 10 μg desmopressin test for differential diagnosis of Cushing syndrome: a systematic review and meta-analysis.

UNLABELLED: We evaluated the accuracy of the 10 μg desmopressin test in differentiating Cushing disease (CD) from non-neoplastic hypercortisolism (NNH) and ectopic ACTH syndrome (EAS). A systematic review of studies on diagnostic test accuracy in patients with CD, NNH, or EAS subjected to the desmopressin test obtained from LILACS, PubMed, EMBASE, and CENTRAL databases was performed. Two reviewers independently selected the studies, assessed the risk of bias, and extracted the data. Hierarchical and bivariate models on Stata software were used for meta-analytical summaries.
