Ectopic ACTH secretion is a rare, severe condition covering a wide spectrum of tumors of different location and aggressiveness. The usual poor prognosis of patients with ectopic ACTH secretion requires prompt management. We report a case of a patient with high surgical risk and intolerance to cortisol-lowering drug, in which hypercortisolism was successfully treated with radiofrequency ablation of a lung carcinoid tumor. A 78-year-old woman was referred for a rapidly progressive ACTH-dependent Cushing syndrome (CS). Biological tests suggested the presence of CS due to ectopic ACTH production.

PURPOSE OF REVIEW: This review outlines the indications for medical management of hypercortisolism and summarizes available and emerging pharmacologic agents.
RECENT FINDINGS: Endogenous Cushing syndrome is a complex endocrine disorder resulting from chronic cortisol excess. The underlying etiology of Cushing syndrome is due to either adrenocorticotropic hormone (ACTH)-dependent causes, such as pituitary tumors (Cushing disease) or ectopic ACTH syndrome from nonpituitary neuroendocrine tumors, or ACTH-independent causes, such as autonomous cortisol production by adrenal adenomas, hyperplasia,

Ectopic ACTH secretion-related Cushing's syndrome may manifest as a rapid onset of hypercortisolism, elevated urinary free cortisol (UFC) and blood ACTH concentrations, and hypokalemia. However, for patients whose primary lesions are difficult to remove, bilateral adrenalectomy is the recommended treatment method according to guidelines. Here, we present a case of non-small cell lung cancer with metastasis. Due to significant deconditioning, the patient was unable to undergo bilateral adrenalectomy and instead underwent catheter-based adrenal ablation; subsequently, he achieved clinical remiss

Cancer of unknown primary (CUP) with neuroendocrine features is classified as a favorable subset, and treatment is recommended based on the grade of the neuroendocrine tumor (NET). Although rare, medullary thyroid carcinoma (MTC) can present as CUP with neuroendocrine features and should be carefully considered as a crucial differential diagnosis in such cases. A 65-year-old man with no relevant medical history presented with a gradually enlarging left cervical mass. Laboratory evaluation revealed hypokalemia due to adrenocorticotropic hormone (ACTH)-dependent Cushing's syndrome. Contrast-enha

Episodic severe Cushing syndrome resulting from ectopic ACTH secretion (EAS) from appendiceal neuroendocrine tumor (NET) is extremely rare. Here, we describe a case of a 24-year-old woman with episodic severe EAS resulting from appendiceal NET with extensive metastatic disease. The patient presented with rapid weight gain, violaceous striae, fatigue, edema, and anxiety. Biochemical evaluation showed markedly elevated 24-hour urinary free cortisol greater than 10-fold above the upper limit of normal, and widely fluctuating peaks and troughs of serum cortisol and ACTH concentrations indicating e

BACKGROUND: Ectopic ACTH syndrome (EAS) is caused by non-pituitary neuroendocrine tumor (NET) that produces adrenocorticotropic hormone (ACTH).
OBJECTIVE: To identify survival predictors and to analyze long-term outcomes in patients with EAS.
METHODS: Medical records of patients with verified EAS between 1990 and 2024 were analyzed to obtain the initial clinical and biochemical data along with subsequent interventions and survival outcomes.
RESULTS: The study included 173 patients (107 women and 66 men), with a median (Q25-Q75) age of 42 years (29; 55). The median follow-up period was 54 mo

Cushing's disease (CD), the most common cause of Cushing's syndrome (CS), is ideally treated by resection of the underlying ACTH-producing pituitary adenoma. However, it may be difficult to establish that CD is the underlying etiology of CS. In this article, we outline a suggested diagnostic approach and discuss common pitfalls, from initial screening for hypercortisolism through the evaluation of ACTH-dependence and confirmation of the presence of a pituitary tumor using imaging and biochemical testing. Most diagnostic steps should preferably take place at a center specialized in pituitary di

Undifferentiated pleomorphic sarcoma (UPS) is a rare, high-grade soft tissue sarcoma. Ectopic adrenocorticotropic hormone (ACTH) syndrome is an uncommon paraneoplastic phenomenon and is most frequently associated with neuroendocrine tumors. Herein, we present a unique case of a 55-year-old woman diagnosed with UPS of the left thigh, complicated by paraneoplastic ectopic ACTH syndrome. To our knowledge, this is the first reported case of ectopic ACTH syndrome associated with UPS in the adult population.

CONTEXT: Bilateral inferior petrosal sinus sampling (BIPSS) is widely used to differentially diagnose ectopic ACTH syndrome (EAS) and Cushing's disease (CD) with pituitary lesions less than 6 mm detected on MRI.
PURPOSE: This study aimed to investigate the diagnostic accuracy of BIPSS and prognosis of low-lateralization [lateralization ratio of bilateral inferior petrosal sampling (IPS) ≤ 1.4] CD with microadenoma (≤ 6 mm).
METHODS: This single-centre retrospective study (2011-2019) included 25 EAS and 179 CD (microadenoma ≤ 6 mm) patients whose diagnoses were confirmed by pathology. All CD

CONTEXT: The diagnosis and differential diagnosis of Cushing syndrome (CS) is often challenging.
OBJECTIVE: To evaluate the 4-mg intravenous dexamethasone suppression test (IVDST) to differentiate CS from normal subjects and subjects with low probability of CS (LPC), and define the cortisol responses in pituitary, adrenal, and ectopic adrenocorticotropin (ACTH) CS.
METHODS: Data from 140 patients with surgically confirmed Cushing disease (CD), 5 with ectopic ACTH syndrome (EAS), 26 with adrenal CS (AC), and 97 with LPC, from 4 tertiary hospitals between 1995 and 2024 were retrospectively eva

Paraneoplastic Cushing syndrome (PCS) is a rare manifestation of ectopic adrenocorticotropic hormone (ACTH) production, mostly associated with bronchial carcinoid and small cell lung cancer (SCLC). Its clinical manifestations: refractory hypertension, profound hypokalemia, metabolic alkalosis, worsening hyperglycemia, and edema, can easily be misattributed to more common conditions, especially in older adults with multiple comorbidities, leading to diagnostic errors. We present a case of an 84-year-old man with a history of stage IA non-SCLC treated one year earlier, who developed progressive

Objective. Ectopic adrenocorticotropic hormone (ACTH) syndrome (EAS) is a rare, but potentially life-threatening cause of Cushing's syndrome. Its clinical recognition may be delayed, especially when classical features of hypercortisolism are absent. We present two cases, in which hypokalemic metabolic alkalosis was the initial and main clinical clue leading to the diagnosis of neuroendocrine carcinoma with ectopic ACTH production. Case 1. The first case was a 71-year-old woman admitted with progressive weakness, gait disturbance, and uncontrolled hypertension. Laboratory tests revealed severe

BACKGROUND/OBJECTIVE: Endogenous Cushing Syndrome (CS) is considered a rare disorder, with European population studies estimating an incidence of 1.5-3.2 cases/million population/year and Cushing disease (CD) being recognized as the most common etiology. The purpose of this study is to characterize the etiologies of endogenous CS, CS severity, and case frequency of CS at a single institution.
METHODS: Between May 2017 and December 2022, clinic records from a single tertiary care medical center were reviewed to identify Wisconsin residents who were newly diagnosed and treated for CS. We collec

The adrenocorticotropic hormone (ACTH)-secreting neuroendocrine neoplasms (NENs) represent a rare but frequently severe disease that is associated with poor prognosis, if not adequately managed. Treatment strategies vary according to the characteristics of the tumor and the severity of hypercortisolism. The optimal treatment is surgical resection of the NEN with a curative intent. In unresectable or metastatic tumors, medical control of hypercortisolism with concomitant treatment of NENs based on current guidelines is suggested. Steroidogenesis inhibitors are the principal choice to control hy

OBJECTIVE: Distinguishing between pituitary corticotropin (ACTH)-dependent Cushing disease (CD) and ectopic ACTH-dependent Cushing syndrome (EADCS) is diagnostically challenging. This study compared the diagnostic performance of bilateral inferior petrosal sinus sampling (BIPSS) and bilateral internal jugular vein sampling (BIJVS).
METHODS: We retrospectively reviewed 109 patients at Mayo Clinic (1998-2018) who underwent simultaneous BIPSS and BIJVS. Eighty-four patients proceeded to pituitary surgery based on BIPSS results, and 81 ultimately received confirmed diagnoses: 74 with CD and 7 wit

The article presents a description of a clinical case of an ectopic focus of adrenocorticotropic hormone (ACTH) hypersecretion located in the adrenal medulla in a 64-year-old obese woman without other characteristic clinical signs of hypercorticism, with complaints of rapidly progressing lower limb edema, severe muscle weakness, loss of appetite, weight loss by 4 kg, as well as stage 2 arterial hypertension and type 2 diabetes mellitus. The patient's blood biochemistry showed severe hypokalemia, up to 2.2 mmol/l, despite infusion and oral therapy with potassium preparations. Taking into accoun

Occult ectopic adrenocorticotropic hormone (ACTH) syndrome (EAS) is a relatively rare form of ACTH-dependent Cushing's Syndrome (CS). We describe a middle-aged female with symptomatic CS, where despite thorough investigations including hormone suppression testing, multiple imaging modalities, and inferior petrosal sinus sampling (IPSS), an ectopic source could not be demonstrated. Our case demonstrates that ketoconazole can be an effective therapeutic to suppress cortisol production even when the source of ectopic ACTH production is unknown. However, long-term evaluation is required.

A 13-year-old girl with severe Cushingoid features had ectopic ACTH-dependent Cushing syndrome. 18 F-FDG PET/CT demonstrated diffusely FDG-avid bilateral adrenal hyperplasia, confirming ectopic ACTH stimulation, and detected a presacral mass with mild uptake. 68 Ga-DOTA-NOC PET/MRI revealed intense somatostatin-receptor activity (SUVmax: 56.1), and MRI identified fat and cystic elements, suggesting teratoma. Surgery proved a mature teratoma harboring an ACTH-secreting NET (G1). This rare case demonstrates that in pediatric patients with aggressive-appearing presacral masses and life-threatenin

BACKGROUND/OBJECTIVE: Ectopic adrenocorticotropic hormone (ACTH)-dependent Cushing syndrome is a rare paraneoplastic disorder caused by excessive cortisol production from nonpituitary tumors. Olfactory neuroblastoma (ONB), a rare neuroendocrine malignancy of the sinonasal cavity, is an exceedingly uncommon source of ectopic ACTH production, with fewer than 25 cases reported worldwide. This report presents a case of ACTH-dependent Cushing syndrome due to ONB, emphasizing the diagnostic complexity, multidisciplinary management, and favorable clinical outcomes.

BACKGROUND: Ectopic adrenocorticotropic hormone (ACTH) syndrome accounts for 15% to 20% of Cushing syndrome cases with unique diagnostic challenges. Tumor localization remains difficult, with approximately 20% of cases having occult sources despite extensive imaging. This report describes a patient whose initially occult tumor was successfully localized through serial imaging enabled by medical stabilization, resulting in curative surgical resection.
