Primary hyperaldosteronism is a common and potentially curable cause of hypertension (HTN), affecting 10 to 15% of hypertensive patients. Underdiagnosed, it is associated with a higher cardiovascular risk than essential HTN. Recent recommendations from the Endocrine Society (2025) and the European Society of Cardiology (2024) advocate screening in all patients with newly diagnosed HTN. The diagnosis is based on an increased aldosterone-to-renin ratio. A new classification helps assess disease severity and the likelihood of a unilateral form,. Treatment relies on mineralocorticoid receptor anta

Background/Objectives: The aim of this retrospective study was to characterize the preoperative hormonal, biochemical, and imaging features of adrenal lesions (ALs) and to assess differences in these features between benign and malignant lesions. Methods: This was a retrospective, single-center observational study conducted at the Endocrinology Clinic of Ankara Bilkent City Hospital. We reviewed the medical records of all patients referred to our clinic for the evaluation of an AL between February 2019 and August 2025. Demographic and clinical data and initial imaging were extracted from elect

In young patients with resistant hypertension and hypokalemia, primary hyperaldosteronism should be suspected. Biochemical assessment and adrenal imaging could identify surgically curable aldosterone-producing adenomas, and a normalization of blood pressure, potassium levels, and symptom resolution can be achieved through adrenalectomy.

SUMMARY: Bardet-Biedl syndrome (BBS) is a rare autosomal recessive ciliopathy characterized by marked clinical and genetic heterogeneity, classically presenting with retinal dystrophy, obesity, polydactyly, renal dysfunction, and hypogonadism. Although arterial hypertension is frequently observed in affected individuals, it is not currently included in diagnostic criteria and remains insufficiently studied. We report the case of a 35-year-old man with clinically diagnosed BBS who developed severe, treatment-resistant hypertension and hypokalaemia. A comprehensive endocrine evaluation revealed

In recent years, studies have been carried out that have expanded the understanding of the role of pathophysiological mechanisms underlying impaired regulation of aldosterone and activation of mineralocorticoid receptors in arterial hypertension. The aim of this consensus is to consider the association between aldosterone dysregulation and the development of cardiovascular complications, as well as to assess the effectivity and safety of novel nonsteroidal mineralocorticoid receptor antagonists, to expand the understanding of their potential in ensuring the prevention of target organ damage an

BACKGROUND: Hypokalemic periodic paralysis (HPP) presenting as acute quadriparesis is a neuromuscular emergency. While its etiology is described in general wards, its severe "critical care phenotype" in the intensive care unit (ICU) remains poorly characterized. We aimed to define this phenotype by analyzing the clinical profile, etiological spectrum, and predictors of life-threatening severity.
METHODS: A retrospective study was conducted of 12 patients (nine male, three female; median age: 31.5 years) admitted to a tertiary ICU (2015-2021) with acute quadriparesis and hypokalemia (median po

BACKGROUND: Adrenal vein sampling (AVS) is the gold standard for subtyping primary aldosteronism (PA), but the role of adrenocorticotropic hormone (ACTH) stimulation during this procedure remains debated. AVS is technically challenging and is available at limited centres; strategies that limit its use to selected patient groups are needed.
METHODS: We retrospectively analysed 98 patients with biochemically confirmed PA who underwent AVS between 2018 and 2025. Outcomes were compared between unstimulated (n = 31) and ACTH-stimulated (n = 67) protocols, including bilateral cannulation success, l

BACKGROUND: Primary hyperaldosteronism (PHA) is a common condition with a prevalence of around 11% in the hypertensive population. For unilateral PHA, the preferred treatment approach is adrenalectomy with the objective to normalize aldosterone levels and its associated harmful effects. However, the reported biochemical outcomes after adrenalectomy vary widely within the literature. This study aimed to evaluate the biochemical and clinical remission rates after unilateral adrenalectomy in patients with PHA.
METHODS: We conducted a retrospective study in two academic centers in Switzerland to

Primary aldosteronism (PA), the most common cause of secondary hypertension, is characterized by autonomous adrenal aldosterone overproduction independent of the renin-angiotensin system, resulting in sodium retention, potassium wasting, and cardiovascular sequelae that exceed the risk attributable to blood pressure elevation alone. Although hypertension is the predominant clinical manifestation, PA may also present without hypertension or hypokalemia, making atypical presentations a significant diagnostic challenge. Aldosterone-mediated myocardial fibrosis combined with hypokalemia-driven ele

Levetiracetam (LEV) is frequently prioritized in epilepsy management due to its minimal drug-drug interactions and perceived metabolic neutrality. However, its potential involvement in severe electrolyte disturbances remains scarcely documented. We describe a 36-year-old male admitted for status epilepticus and initiated on a dose-escalated LEV regimen (2000 mg/day). The clinical course was complicated by the development of severe, recalcitrant hypokalemia (nadir 2.8 mmol/L). This electrolyte depletion proved remarkably refractory to intensive parenteral and enteral potassium chloride suppleme

BACKGROUND: Aldosterone excess in primary aldosteronism (PA) has been implicated in vascular remodeling and aortic pathology. However, the prevalence of abdominal aortic aneurysm (AAA) in PA patients remains largely unexplored. This study aimed to evaluate AAA prevalence among patients with biochemically confirmed PA and to explore potential aldosterone-mediated vascular effects. METHODS: In this single-center, case-control study, 73 PA patients and 74 age- and sex-matched individuals with essential hypertension were included. Patients with smoking history, connective tissue disorders, vasculi

BACKGROUND: Shared mechanical ventilation has been proposed during ventilator shortages, but unequal tidal volume delivery remains a major limitation when lung compliance differs. Professional societies have cautioned against this practice, yet the intervention has never been validated clinically, and ethical constraints preclude direct patient testing. Translational simulation-simulation applied beyond education to probe clinical systems, test crisis-driven innovations, and generate actionable safety data-offers a methodological pathway for evaluating interventions that cannot be studied in v

BACKGROUND: Individuals with anorexia nervosa frequently feel ambivalent about treatment and weight restoration, and drop out and relapse rates in treatment are high. Increased insight into the function of the eating disorder is considered essential for achieving long-lasting, meaningful change. However, research investigating the functions of anorexia nervosa tends to focus on the role of the disease per se. Distinctions are rarely made across features. In particular, the subjective experience, understanding and sense making of the engagement in compulsive exercise in individuals with anorexi

CASE SUMMARY: This report describes a 14-year-and-9-month-old, male neutered cat with a primary adrenal tumour secreting aldosterone with pulmonary metastases, alongside concurrent hyperthyroidism and congestive heart failure. The cat presented with hyporexia and muscular weakness, including marked cervical ventroflexion. Hypokalaemia, hypertension, marked hyperaldosteronaemia and presence of a left-sided adrenal mass were consistent with primary hyperaldosteronism, and pulmonary nodules were detectable radiographically. Congestive heart failure and poor quality of life resulted in euthanasia.

Peroxisome proliferator-activated receptors (PPARs) are members of the nuclear receptor superfamily that regulate lipid, glucose, and amino acid metabolism. More recently, PPARs and corresponding ligands have been shown in skin and other organs to regulate important cellular functions, including cell proliferation and differentiation, as well as inflammatory responses. These new functions identify PPARs and corresponding ligands as potential targets for the treatment of various skin diseases and other disorders. It has been shown that in inflammatory skin disorders, including hyperproliferativ

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Diesel exhaust particles (DEPs), a major component of air pollution, are well-known to induce inflammation and vascular dysfunction. However, the molecular mechanisms linking DEP exposure to the disruption of the blood–retina barrier (BRB) remain poorly understood. Toll-like receptors (TLRs), particularly TLR2 and TLR4, play critical roles in inflammatory signaling and may contribute to DEP-induced retinal endothelial dysfunction. This study investigates the involvement of TLR2 and TLR4 in mediating DEP-induced disruption of the BRB and evaluates the protective effects of TLR inhibition using

OBJECTIVE: Recent studies have demonstrated that C-peptide exerts beneficial effects on the diabetic state, including improvements in kidney and nerve function. Thus, we investigated the effect of residual pancreatic C-peptide secretion on the cardiac autonomic nervous system in well- and poorly controlled type II diabetic patients.
DESIGN: Randomised cross-sectional study.
PATIENTS: Forty type II diabetic patients free from diabetic neuropathy, with similar anthropometric parameters, volunteered for our study.
MEASUREMENTS: Insulin action, residual pancreatic C-peptide secretion and the ca

This case report describes an adult man in his 50s with a history of type 2 diabetes and previously well-controlled hypertension, who presented with uncontrolled hypertension, muscle weakness and fatigue. Biochemical testing revealed hypokalaemia. There was no evidence of renal/renovascular disease. Evaluation for primary hyperaldosteronism revealed suppressed renin concentration with low aldosterone levels. Detailed history taking revealed the daily consumption of three to four cups of liquorice root tea for 3 months. Glycyrrhetinic acid, a metabolite of glycyrrhizic acid found in liquorice,
