CONTEXT: Patients with primary aldosteronism (PA) have higher cardiac comorbidities including more pronounced left ventricular hypertrophy than patients with essential hypertension.
OBJECTIVE: Autonomous cortisol cosecretion (ACS) is a common subtype in PA associated with a worse metabolic profile.
HYPOTHESIS: Autonomous cortisol cosecretion may affect myocardial parameters and result in a worse cardiac outcome compared to patients with PA and without ACS.
METHODS: Three hundred and sixty-seven patients with PA undergoing 1 mg dexamethasone suppression test (DST) and echocardiography at bas

Background: Distinguishing unilateral aldosterone-producing adenomas (APA) from idiopathic hyperaldosteronism (IHA), nonfunctional adrenal adenoma (NFA), and pheochromocytoma (PHEO) within primary aldosteronism (PA) presents a significant challenge. Studies have demonstrated high levels of chemokine receptor (CXCR) 4 expression in APA, thereby validating the use of 68Ga-labeled CXCR4 PET/CT for detecting APA. This study evaluates the efficacy of [18F]AlF-NOTA-pentixather PET/CT in distinguishing APA from other PA types. Methods: In the initial experiment, a comparative analysis was conducted t

Hypertension remains a significant global health issue, increasingly affecting younger populations due to lifestyle and dietary changes. This case report presents a 28-year-old male diagnosed with primary hyperaldosteronism, a rare but treatable cause of secondary hypertension, presenting as hypertensive urgency. The patient reported persistent headaches and weakness, with an initial blood pressure of 190/120 mmHg and severe hypokalemia. Through a systematic diagnostic approach, including biochemical and imaging assessments, unilateral adrenal hyperplasia was identified as the cause of primary

Adrenal adenoma, which leads to increased production of the hormone aldosterone, commonly presents as hypertension and hypokalemia. Rhabdomyolysis as a result of hypokalemia secondary to primary hyperaldosteronism is a rare but important complication with only a few reported cases. Low potassium levels can disrupt the regulation of arteriolar musculature, leading to reduced blood flow to skeletal muscles. This hypoperfusion may ultimately result in ischemia and cause rhabdomyolysis. We present the case of a woman with complaints of weakness and fatigue; laboratory reports showed hypokalemia an

BACKGROUND: Paget's disease of bone, the second most common metabolic bone disease in the United States, is characterized by localized areas of excessive bone resorption coupled with accelerated bone formation, resulting in new bone that is less structurally organized and is weaker than normal bone. Complications of Paget's disease can include bone pain, osteoarthritis, skeletal deformity, hearing loss, and fractures. The objective of this review is to provide a comprehensive overview of current standards of treatment in Paget's disease.
SCOPE: A review of literature from 1974 to 2007 was per

Surgical diseases of the adrenal gland include pheochromocytoma/paraganglioma, primary hyperaldosteronism, Cushing syndrome, and adrenocortical carcinoma. These conditions may be associated with familial syndromes, and genetic testing is available and recommended in most. For adrenal surgeons to be familiar with these syndromes and know when to consider referral for genetic counseling and genetic testing is important. Identification of patients with familial syndromes allows for the detection and screening of associated syndromic neoplasms, guides surgical planning and operative approach, infl

Primary hyperaldosteronism (also called Conn's syndrome) is a rare condition of the adrenal glands characterized by excessive secretion of the hormone aldosterone, which regulates the balance of water and electrolytes in the body, and maintains blood volume and pressure. Hyperaldosteronism causes sodium and water retention, hypokalemia, hypertension, and muscle weakness. Common cause of primary hyperaldosteronism is an adrenal adenoma or bilateral adrenal hyperplasia. A 36-year-old female presented with hypertension, hypokalemia and muscle cramps, and on further evaluation by computed tomograp

Heterozygous germline variants in ATP1A1 , the gene encoding the α1 subunit of the Na + /K + -ATPase (NKA), have been linked to diseases including primary hyperaldosteronism and the peripheral neuropathy Charcot-Marie-Tooth disease (CMT). ATP1A1 variants that cause CMT induce loss-of-function of NKA. This heterodimeric (αβ) enzyme hydrolyzes ATP to establish transmembrane electrochemical gradients of Na + and K + that are essential for electrical signaling and cell survival. Of the 4 catalytic subunit isoforms, α1 is ubiquitously expressed and is the predominant paralog in peripheral axons. Hu

Primary hyperaldosteronism (PAH) is an important cause of secondary hypertension (HTN). The study of the same requires a high clinical suspicion in addition to a hormonal study that confirms hormonal hypersecretion. It is important to start the appropriate treatment once the diagnosis is confirmed, and for this is necessary to demonstrate whether the hormonal hypersecretion is unilateral (patients who could be candidates for surgical treatment) or bilateral (patients who are candidates for pharmacological treatment only). At the Hospital del Mar since 2016 there has been a multidisciplinary wo

Approximately 10% of cases of arterial hypertension are due to a secondary cause, being among the most frequent primary hyperaldosteronism, characterized by hypertension, metabolic alkalosis and hypokalemia. However, on rare occasions it can present in an atypical way, in the form of muscle weakness and myalgia secondary to rhabdomyolysis due to severe hypokalemia, as in the case described.

Secondary hypertension occurs in 5% to 10% of all patients with hypertension. Given the majority of patients with hypertension will not have a secondary cause, only select patients with specific characteristics should be screened. The causes include a range of abnormalities, some are quite rare, such as pheochromocytoma, while others are much more common, such as chronic kidney disease. When considering which disorders to test for, it is important to incorporate the clinical history, family history, and prevalence of each disease. Treatment is specific to the underlying cause and includes medi

Primary hyperaldosteronism typically leads to resistant hypertension, hypokalemia, and metabolic alkalosis. Excess aldosterone secretion by the adrenal glands may lead to heart failure with preserved ejection fraction. Potassium-sparing diuretics and aldosterone antagonists directed to lower excess aldosterone levels may help treat the associated heart failure and lead to control of blood pressure, resulting in improved outcomes. We report a case of a 55-year-old male with poorly controlled hypertension and newly symptomatic heart failure with preserved ejection fraction in the setting of exce
