Spectrum of Primary Aldosteronism and Risk of Cardiovascular Outcomes: The Atherosclerosis Risk in Communities Study.

IMPORTANCE: Mounting evidence suggests that renin-independent aldosteronism is common and often underrecognized. Yet, whether aldosteronism across this broader spectrum is associated with incident cardiovascular disease (CVD) has not, to the authors' knowledge, been comprehensively evaluated. OBJECTIVE: To determine whether aldosterone measures are associated with incident CVD events in community-dwelling older adults. DESIGN, SETTING, AND PARTICIPANTS: This prospective cohort analysis included participants from the Atherosclerosis Risk in Communities (ARIC) study with serum aldosterone and

One-stop evaluation using [68Ga]Ga-Pentixafor PET integrated with contrast-enhanced CT for visualization and localization of adrenal nodules in patients with primary aldosteronism.

PURPOSE: To evaluate the diagnostic utility of integrating [68Ga]Ga-Pentixafor PET with contrast-enhanced CT (CECT) for the visualization and localization of adrenal lesions in patients with primary aldosteronism (PA). METHODS: Thirty-five patients with clinically suspected PA who underwent [68Ga]Ga-Pentixafor PET and integrated CECT scans were retrospectively enrolled in this study. Adrenal lesions were identified and segmented using non-threshold methods. PET parameters (SUVmax, lesion-to-contralateral adrenal ratio [LCR], lesion-to-liver ratio [LLR]) and CT parameters (attenuation values,

Usefulness of steroid profiling in the diagnosis of primary Aldosteronism a systematic review.

OBJECTIVE: To summarize studies evaluating the utility of steroid profiling and metabolomics in the diagnosis (differentiation from other causes of hypertension) and subtyping (unilateral vs. bilateral) of primary aldosteronism (PA). METHODS: We systematically searched PubMed and Embase databases for studies published up to March 31,2024, that assessed metabolomics-based approaches (serum, plasma and urine), for the diagnosis or subtyping of PA. RESULTS: A total of 32 articles involving 2851 participants with PA were included. Of these, six assessed both diagnosis and subtyping, 13 focused sol

Treatment of Primary Aldosteronism.

Primary aldosteronism (PA) is a common cause of hypertension, characterized by renin-independent aldosterone production that drives inappropriate mineralocorticoid receptor activation, sodium retention, volume expansion, and potassium wasting, ultimately resulting in hypertension and adverse cardiorenal outcomes. Management of PA involves therapies that target these pathophysiologic mechanisms to restore homeostasis and reduce risk, which is usually tailored based on patient preference and whether PA is lateralizing or nonlateralizing. For patients with lateralizing PA, surgical adrenalectomy,

Hypertensive Disorders of Pregnancy and Primary Aldosteronism.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) affect up to 15% of pregnancies and are linked to adverse maternal and fetal outcomes. Primary aldosteronism (PA) affects up to 25% of patients with hypertension. In women with prior HDP and PA risk factors, we examined PA prevalence and its relationship to hypertension trajectory. METHODS: Adults across the US meeting guideline-recommended screening criteria for PA were prospectively tested. Women with a self-reported history of HDP completed a questionnaire examining the relationship between PA and hypertension trajectory. RESULTS: Of 3

The Burden, Outcomes, and Management of Patients with Aldosterone Dysregulation: A Targeted Literature Review.

INTRODUCTION: Aldosterone dysregulation (excess aldosterone production at the source within the adrenal glands) in the absence of primary aldosteronism is a complex physiologic driver of uncontrolled hypertension and cardiovascular and kidney outcomes. Understanding the manifestations and burden of aldosterone dysregulation is critical to optimizing treatment and improving outcomes in patients with hypertension. This review examines the prevalence, patient attributes and burden of aldosterone dysregulation, and its impact on patient outcomes. METHODS: A targeted literature review of articles

The Critical Care Phenotype of Hypokalemic Paralysis: Etiology, Outcomes, and Predictors of Respiratory Failure in a Retrospective Cohort Study.

BACKGROUND: Hypokalemic periodic paralysis (HPP) presenting as acute quadriparesis is a neuromuscular emergency. While its etiology is described in general wards, its severe "critical care phenotype" in the intensive care unit (ICU) remains poorly characterized. We aimed to define this phenotype by analyzing the clinical profile, etiological spectrum, and predictors of life-threatening severity. METHODS: A retrospective study was conducted of 12 patients (nine male, three female; median age: 31.5 years) admitted to a tertiary ICU (2015-2021) with acute quadriparesis and hypokalemia (median po

Apparent Bilateral Aldosterone Suppression During Adrenal Vein Sampling: An Artefact of the Liaison Chemiluminescence Immunoassay and Other Causes Revealed by Comparisons With Another Commercial Immunoassay and Mass Spectrometry.

OBJECTIVE: Subtyping of primary aldosteronism usually requires adrenal vein sampling (AVS). Interpretation can be compromised by apparent bilateral aldosterone suppression (ABAS), which we hypothesised reflects an artefact of the Liaison immunoassay of aldosterone. We therefore compared measurements of aldosterone by liquid chromatography-tandem mass spectrometry (LC-MS/MS) with those by Liaison and iSYS immunoassays. DESIGN: Observational multicentre study. PATIENTS: The study involved 216 patients who underwent bilaterally selective non-stimulated AVS. MEASUREMENTS: Adrenal and peripheral

Renin-aldosterone testing: The essentials.

Primary aldosteronism (PA) is a common yet significantly underdiagnosed cause of secondary hypertension, estimated to affect around 9.4% of hypertensive adults. This is critical because patients with PA face markedly higher rates of adverse cardiovascular and renal outcomes, risks substantially mitigated by targeted treatment (surgery or medical therapy). Despite this, screening rates remain critically low. This article provides a practical, evidence-based approach to PA screening specifically for generalist settings, focusing on contemporary test interpretation and mitigating common pitfalls.

[How I investigate primary aldosteronism].

Primary aldosteronism (PA) is a common yet still underdiagnosed cause of secondary hypertension (HTN). We report two cases illustrating the variability of its clinical presentation: one revealed by left ventricular hypertrophy, the other by refractory hypertension. These observations highlight the importance of a structured diagnostic approach to determine the PA subtype and tailor management accordingly. Early diagnosis improves blood pressure control, corrects electrolyte disturbances, and prevents aldosterone-related visceral complications. These findings also support the need for more syst

Link of Obesity With Primary Aldosteronism: Causal or Not?

Obesity has been shown to correlate directly with circulating aldosterone levels and urinary aldosterone excretion in normotensive and hypertensive individuals without primary aldosteronism (PA). This relationship may be bi-directional, though it is probably not symmetrical. Activation of the mineralocorticoid receptor in adipocytes may cause expansion of visceral fat mass. Conversely, adipocytes have been shown to augment the adrenal production of aldosterone by their release of aldosterone secretagogues-leptin, C1q/TNF-related proteins 1, and resistin-into the circulation. Patients with PA a

Hyperaldosteronism in Mice Lacking the Distal Polybasic Tract of the γ-Subunit of the Epithelial Sodium Channel During Sodium Restriction.

AIMS: The epithelial sodium channel ENaC consists of the subunits α, β, and γ and is activated at an individual channel level by proteolytic processing. Murine γENaC contains a distal polybasic tract 186RKRK mediating proteolytic ENaC activation by serine proteases in vitro. The relevance of ENaC activation at this cleavage site for sodium homeostasis in vivo is unknown. METHODS: Mice were generated carrying a mutation of the distal polybasic tract (RKRK186QQQQ or γENaCki/ki) using CRISP/Cas9. Sodium homeostasis and proteolytic processing of γENaC were investigated under a low sodium diet, ph

Role of 99mTc-DTPA Captopril Renal Scintigraphy in the Diagnosis of Renal Fibromuscular Dysplasia: A Case Report.

Fibromuscular dysplasia (FMD) represents the most common non-inflammatory and non-atherosclerotic cause of renal artery stenosis (RAS), frequently manifesting as resistant renovascular hypertension (RVH). While anatomical imaging can identify structural abnormalities such as the classic "string-of-beads" pattern, 99mTc-DTPA captopril renal scintigraphy provides essential physiological data by demonstrating a measurable drop in the glomerular filtration rate (GFR) when the compensatory renin-angiotensin-aldosterone system is disrupted. This case report details a 33-year-old male with severe hyp

Impact of continuous positive airway pressure therapy on renin-angiotensin-aldosterone system in obstructive sleep apnea: an updated systematic review and meta-analysis.

PURPOSE: While continuous positive airway pressure (CPAP) reduces cardiovascular risk in obstructive sleep apnea (OSA), its effects on renin–angiotensin–aldosterone system (RAAS) modulation remain controversial. This meta-analysis investigates CPAP's differential impacts on RAAS components and identifies responsive patient subgroups. METHODS: Sixteen studies were included in the systematic review and eight (231 patients) in the meta-analysis. Primary outcomes included plasma aldosterone concentration (PAC), plasma renin activity (PRA), and angiotensin II (AngII) changes. Subgroup analyses exam

[Diagnostic yield of hypertension evaluation in a nephrology day hospital].

INTRODUCTION: Secondary hypertension (HTN) accounts for 5–10% of HTN. The actual diagnostic yield of a comprehensive etiological work-up in nephrology-specialized centers remains poorly documented. MATERIALS AND METHODS: We conducted a single-center retrospective study including all patients evaluated in the day hospital program of the ESH-accredited Hypertension and Nephrology Unit at Rouen University Hospital for suspected secondary HTN, between 2018 and 2019. The standardized protocol combined clinical assessment, laboratory testing, imaging, and multidisciplinary discussion. RESULTS: One

Aldosterone synthase expression vs cross-sectional imaging in lateralized primary aldosteronism.

CONTEXT: Adrenal vein sampling is the standard of care for guiding surgery in primary aldosteronism (PA). Because of its technical complexity and limited availability, however, many centers still use cross-sectional imaging for surgical guidance. DESIGN: Single referral-center retrospective cohort study of patients with PA who underwent unilateral adrenalectomy between 2012 and 2024. Blinded cross-sectional imaging interpretation was corroborated with aldosterone synthase (CYP11B2) immunohistochemistry (IHC) of formalin-fixed paraffin-embedded adrenal tissue. RESULTS: Of 173 patients, age 52

Aldosterone increases the expression and subcellular localization of SERCA2a and SERCA2b in the rat mesenteric artery.

Aldosterone (Aldo), a mineralocorticoid hormone, modulates cardiovascular function by regulating the expression of intracellular Ca2+ handling proteins, among other effects. In rat resistance-sized mesenteric arteries (MA), Aldo treatment (10 nM, 24 h) upregulates both the L-type voltage-gated Ca2+ channel α1C subunit (CaV1.2) and the Sarco/Endoplasmic Reticulum Ca2+ ATPase (SERCA pump), thereby increasing Sarcoplasmic Reticulum (SR) Ca2+ load. Two SERCA isoforms, SERCA2a and SERCA2b, are expressed in rat MA, but their specific physiological contributions to distinct intracellular Ca2+ signals

Case Report: Challenges in the diagnosis of pseudohypoaldosteronism: insights from a resource-limited setting.

BACKGROUND: Pseudo-hypoaldosteronism type 1 (PHA1) is a hereditary condition characterized by end-organ resistance to aldosterone, resulting in electrolyte imbalance, metabolic acidosis, and hyperaldosteronism. It is inherited in an autosomal dominant renal or an autosomal recessive systemic form. Here, we report a case series of PHA1, highlighting its challenging diagnosis and underscoring the complexities involved in reaching an accurate diagnosis in a resource-limited setting. CASE REPORT: Seven patients from seven unrelated Sudanese families were admitted to pediatric endocrinology units
